Anterior implant case report using digital guided implant template (임플란트 가이드를 활용한 전치부 수복증례)

  • Guided implant surgery is useful in anterior implant case in terms of fixture installation and temporary crown delivery. For the aesthetic prosthetics in anterior implant, the position of the implant fixture is crucial. Guided surgery is the top-down procedure and we designed prosthetics first and then determine the position of the fixture. Guided surgery can reduce the stress of dentist with difficult anterior implant case.

  • Dental trauma is common in active children and adolescents. Among them, in the case of ankylosis due to avulsion, diagnosis through clinical examination is important, and the treatment is important for esthetic prosthetic restoration after adulthood. This case is a case of esthetic prosthetic restoration of maxillary anterior teeth through decoronation and implant. After that, space was maintained for prosthetic restoration using a decoronated crown. After becoming an adult, precise implant placement and esthetic prosthetic restoration were possible using guide surgery. For soft tissue support, the temporary crown were replaced by changing the contours of the temporary crown, and the final esthetic prosthesis was fabricated by digital wax-up.

  • During the past several years, significant advances have occurred in the utilization of osseointegrated implants for the treatment of partially edentulous patients. One of the biggest purposes for treating of these patients is the high demand for improved esthetics, especially in the anterior region. For this esthetics, the new trend in dental implants is the immediate placement and immediate superstructure fabrication. The refined surgical technique, the skillful soft tissue management, and the proper prosthetic coordination are the main factors to achieve natural looking of implant supported prosthesis. The customized provisional restoration and the customized impression coping are recommended for the optimal peri-implant soft tissue contour. The basic concept of Frialit 2 system was the immediate replacement of a tooth with root-analog fixture after extraction. This system guarantees an ideal result in function and esthetics. The ceramic abutment system offers improved quality in the respect of esthetics, fitness, translucency, and biocompatibility. In this clinical report, the final restoration made with IPS Empress 2 crown on the CeraBase abutmen of Frialit 2 system allowed the reproduction of the natural vitality of tooth and adjacent gingiva.

  • Transitional implants were developed to support provisional restorations and to allow for load-free osseointegration of conventional implants while a patient was provided with immediate esthetics and function and are usually placed simultaneously at the time of definitive implant placement. Transitional implants are placed in a non-submerged fashion in a single-stage surgery and are designed to be immediately loaded. They generally are made of commercially pure titanium or titanium alloy and are designed as 1-piece implants composed of root and crown replacement segments. Transitional implants can be used in a wide range of indications, such as basic use as temporary implant, to support and protect the primary implants during the healing phase, single crown in the edentulous anterior region of mandibular, anchorage for orthodontic treatment, support a surgical and radiographic template, and primary implant to extremely atrophied alveolar crests of the mandible and maxilla. This article describes the clinical use of transitional implants to support the provisional complete denture and single crown in the restricted edenturous central incisor region of mandible.

Reestablishing the occlusal plane in full mouth rehabilitation patient, using Shilla system (전악수복환자에서 Shilla system을 이용한 교합평면 재구성 증례)

  • Occlusal plane is a sagittal expression of dental arch form, and it composes the shape of occlusion, which is one of the most important elements of Maxillo-oral system. In this case, vertical, horizontal coordinates of bionic-median-sagittal plane was produced in articulator, and to achieve relation of left and right position of upper, lower teeth and deficits in alveola, Shilla system was used to reconstruct occlusal plane. In this case, a 41 year-old male patient visited for fracture of 10 unit metal-ceramic fixed partial denture of upper anterior teeth and for overall treatment. Clinical, radiographical, model examination was held, full mouth rehabilitation was achieved by placing dental implant. Maxillo-oral relation was recorded using Gothic arch Tracer complex and were mounted. And for the next step, we estimated original occlusal plane using Shilla system. After analysis we produced diagnosis wax pattern. On the basis of this, radiography stent was manufactured and dental implant was placed, and temporary prosthesis was made by using diagnosis wax pattern. Cross mounting and anterior guiding table were performed in order to reproduce temporary restoration morphology and bite pattern, followed by final restoration made of all ceramic crown with zirconia coping. As stated above, appropriately esthetic and functional results can be seen in using Shilla system in diagnosis and treatment procedure of full mouth rehabilitation patient.
