Backgroud: We have performed the CT-guided celiac plexus block (CPB) using anterior approach to evaluate the safety and efficacy of the procedure and to determine the role of CT. Methods: CPB were done in 10 patients (5 men and 5 women: mean age, 58.1 years) with intractable upper abdominal pain due to terminal malignancy of the stomach (n=3), pancreas (n=4), gallbladder (n=2), and liver (n=1). To permit an anterior approach, patients lay supine on the CT scan table during the procedure. One 21-guage Chiba needle was placed just anterior to the diaphragmatic crus between the celiac and superior mesenteric arteries and 10~12 ml of dehydrated alcohol was injected. Degree of pain relief following the procedure was assessed and pain was graded on a numeric rating scale (NRS) from 0 to 10. Results: The results suggest a direct relation between the degree of celiac invasion and the response to the CPB. With CT guidance, it is possible for us to direct the needle into more accurate region, allowing alcohol to be deposited in specific ganglion area. Conclusions: CT-guided CPB using an anterior approach was an easy and effective way of reducing intractable upper abdominal pain due to terminal malignancies. CT-guidance allowed precise needle placement and safe procedure. Careful classification of cases is important to predict the degree of pain relief using the grading system based on the degree of involvement of the celiac plexus.

Extensive dental erosion and wear will cause serious loss of function and aesthetics in the mouth. In order to recover this condition, careful analysis of the patient's bite relationship is required. In particular, a treatment plan should be established considering the possibility of reproduction of the vertical dimension and centric relation, and appropriateness of the occlusal plane and anterior guidance. Also, the choice of prosthetic materials is an important consideration in patients with severe wear. In this case, patients with overall wear and erosion on tooth was established anterior guidance by orthodontic treatment and fully restored with monolithic zirconia, without increasing vertical dimension.
Patients with $Sj{\ddot{o}}gren^{\prime}s$ syndrome usually suffer from teeth discoloration and attrition due to xerostomia. If the anterior teeth are badly worn, problems such as loss of anterior guidance, occlusal disharmony, and limited space for restoration may occur. However, ideal occlusion is obtained in both centric and eccentric relation by regaining the disocclusion of the posterior teeth through the anterior and lateral guidance using twin-stage method. In this case, rehabilitation was performed for a $Sj{\ddot{o}}gren^{\prime}s$ syndrome patient with maxillary and mandibular incisor's severe attrition and teeth discoloration by using twin-stage method.

Inadequate or unstable posterior support cause extrusion of antagonistic teeth and overloading of remaining teeth. Extrusion of antagonistic teeth result in collapsed occlusal plane and decreased prosthetic space. Also, overloading of remaining teeth increase occlusal wear or anterior extrusion of anterior teeth, which is resulting in loss of anterior guidance, disharmony of occlusion and decreased prosthetic space. In this case, careful evaluation of OVD(occlusal vertical dimension) is the general first trial to regain appropriate function and esthetics. Simultaneously, adequate anterior guidance and harmonious occlusal plane should be established in the repeatable reference position of the mandible. We report this case because severely worn dentition achieved functional and esthetic results from full mouth rehabilitation with increased vertical dimension and use of centric relation.
Oligodontia is defined as a congenital tooth agenesis with the absence of six or more permanent teeth. This clinical report describes a patient with non-syndromic partial oligodontia, with retained deciduous teeth and the absence of 16 permanent teeth. Anterior esthetic problems were caused by interarch tooth size discrepancy, interdental space, aberrant tooth dimensions, and the absence of centric contacts of the anterior teeth. Prosthetic restoration after orthodontic and implant treatment was performed with a multi-disciplinary team approach. Favorable functional and esthetic results were obtained using a definitive prosthesis.
Excessive tooth wear can cause irreversible damage to the occlusal surface and can alter the anterior occlusal relationship by destroying the structure of the anterior teeth needed for esthetics and proper anterior guidance. The anterior deep bite is not a morbid occlusion by itself, but it may cause problems such as soft tissue trauma, opposing tooth eruption, tooth wear, and occlusal trauma if there are no stable occlusal contacts between the lower incisal edge against its upper lingual surface. The most important goal of treatment is to form stable occlusal contact in centric relation. In this case report, patients with decrease in vertical dimension and anterior deep bite due to maxillary posterior tooth loss and excessive tooth wear were treated full mouth rehabilitation with increased vertical dimension to regain the space for restoration and improve anterior occlusal relationship and esthetics. The functional and aesthetic problems of the patient could be solved by the equal intensity contact of all the teeth in centic relation (CR), anterior guidance in harmony with the functional movement, and restoration of the wear surface beyond the enamel range.

In children, large odontogenic cysts affect adjacent anatomical structures as well as displace developing permanent teeth. Odontogenic cysts are treated via enucleation or marsupialization. This case reports a 5-year-old boy and a 10-year-old boy who have not only displaced 3 or more permanent teeth but also elevated the maxillary sinus floor due to the large size of the cyst in the maxillary anterior region. In both cases, marsupialization was selected to minimize complications. After marsupialization, a customized acrylic obturator, window opening, and orthodontic traction for eruption guidance were gradually attempted, and it showed a good prognosis, so we report these cases.
The discrepancy between centric relation and centric occlusion have a great role on the successful prosthodontic and occlusal treatment. This study was performed to investigate the effect of the discrepancy between centric relation and centric occlusion on condylar guidance inclination and Bennett shift (immediate and progressive side shift). Sixteen adults who have physiologically normal occlusion and are free of TMJ dysfunction were selected. The amount of the sagittal CR-CO discrepancy in lower anterior incisor was obtained by Saphon Visi-Trainer. The amount of the CR-CO discrepancy in condylar level was measured on the individualized corrected tomography. Pantronic survey was performed by using a arbitrary hinge axis according to manufacturer's direction. All subjects were divided into two groups, group I (small) and group II (large), according to the amount of CR-CO discrepancy. At first the amount of the CR-CO discrepancy in condylar level between two groups was compared and then the condylar guidance inclination and Bennett shift between two groups were compared and analyzed. The results were as follows; 1. The average CR-CO discrepancy in lower anterior incisors was 0.7mm superoinferiorly, 0.49mm anteroposteriorly, and 0.88 mm in total. The average CR-CO discrepancy in condylar level was 0.43mm. 2. The CR-CO discrepancy measured on teeth level and condylar level were highly correlated (p<0.01). 3. The correlation of the condylar position in the glenoid fossa between two groups was not statistically significant. 4. The large CR-CO discrepancy group showed greater amount of Bennett shift and condylar guidance inclination, but there is no statistical significancy. 5. It seems that the CR-CO discrepancy have greater effect on progressive side shift than other elements of mandibular movements.
It is reported that the causes of unaesthetic proportion of anterior teeth vary widely. Especially, when the unaesthetic tooth proportion of the mandibular incisors arises due to the wear of the anterior teeth accompanied by the compensation of the alveolar bone, it may cause serious functional and aesthetic problems. In such case, it should be considered that the evaluation of vertical dimension and tooth proportion as well as smile line, soft tissue and hard tissue morphology. And, increase of vertical dimension or clinical crown lengthening followed by prosthodontic restorations is needed to improve the interdental mesial/distal, width/length ratio considering the anterior guidance. This case report demonstrates functional and aesthetic improvements through systematic diagnosis and treatment procedures in a 48-year-old male patient with unaesthetic anterior teeth proportion because of tooth wear accompanied by the compensation of alveolar bone and defect of several central incisors due to chronic periodontitis.

Proper management of provisional prosthesis is key to success in prosthodontics. Provisional restoration on maxillary anterior missing area frequently come across a incident of falling off especially in patients with long span pontics and oval arch shape. This is because maxillary anterior teeth are more exposed to horizontal force than the posterior teeth and additional anterior cantilever effect will negatively affect to the retention of provisional prosthesis. Beside that maxillary anterior provisional prosthesis should provide proper incisal guidance during the mandibular functional movements. However occlusal contacts on the prosthesis in maximum intercuspal position are located on opposite side of fulcrum line of prosthesis which will cause removing force against the provisional prosthesis. This case report present that provisional implant prevent pre-described harmful effect on maxillary anterior fixed provisional prosthesis and provide comfort and satisfactory result during post-extraction healing period.
