한국 성인에서 당뇨병이 빈혈의 발생에 미치는 영향 - 2005년 국민건강영양조사 자료를 중심으로 (The Presence of Diabetes Mellitus and Anemia in Korean Adults-based on Data from 2005 Korean National Health and Nutrition Examination Survey(KNHANES III))

  • Anemia is common in Diabetes Mellitus(DM) with chronic kidney disease. Recent research suggests that DM itself also may be a risk factor of anemia even though kidney failure causes anemia. However, it has not been reported that the impact of DM on anemia in representative data of Korean population. A total of 5,417 Korean adults aged 20 years and older(2,328 men, 3,089 women) were selected from the participants of the 2005 Korean National Health and Nutrition Examination Survey(KNHANES) for this study. Anemia was defined as hemoglobin(Hb) < 13 g/dL and hematocrit(Hct) < 39% for men or Hb < 12 g/dL and Hct < 36% for women. DM was defined as a fasting blood glucose $\geq$ 126 mg/dL. Korean adults with anemia had a higher prevalence of DM than in normal adults(11.4 vs 7.5%; p<0.0001). The unadjusted odds ratio(OR) for anemia was greater in Korean men with DM than in normal men(OR=4.25; 95% CI: 2.48-7.29). This results did not differ after adjustment for the putative risk factors for anemia including chronic disease(OR=2.64; 95% CI: 1.45-4.83). However, the presence of DM was not related with anemia in Korean women. In conclusion, this study revealed that DM might be an independent risk factor for anemia in Korean men. Identification and management for anemia are needed in Korean population with DM as well.

Prognostic Significance of Preoperative Anemia, Leukocytosis and Thrombocytosis in Chinese Women with Epithelial Ovarian Cancer

  • Malignant tumors are often accompanied by increased risk of hematological abnormalities. However, few studies have reported any prognostic impact of preoperative thrombocytosis, leukocytosis and anemia in epithelia ovarian cancer (EOC). This study aimed to investigate preoperative hematological parameters for anemia, leukocytosis and thombocytosis in relation to established prognostic factors and survival in EOC cases. A total of 816 Chinese women treated for EOC were retrospectively included in the study focusing on the relationship between preoperative hemoglobin, leukocyte and platelet counts, and a panel of clinicopathologic characteristics and outcome. Preoperative anemia was present in 13.4%, leukocytosis in 16.7% and thrombocytosis in 22.8%. Additionally, EOC patients with low differentiation grade, advanced stage, lymph node (LN) metastasis, residual disease ${\geq}1cm$, ascites volume >1,000ml, serum cancer antigen 125 (CA125) >675U/ml, and disease recurrence had the higher prevalence of preoperative anemia, leukocytosis and thrombocytosis (all p<0.05). Moreover, EOC patients with older age or postmenopausal EOC patients had the higher prevalence of thrombocytosis (28.7% vs 17.3% or 26.0% vs 17.7%). Furthermore, in a Cox proportional hazard model, thrombocytosis was an independent factor for progression-free survival (PFS) and overall survival (OS) (p<0.001). Conclusively, preoperative anemia, leukocytosis or thrombocytosis in EOC patients is closely associated with more malignant disease phenotype and poorer prognosis. Significantly, thrombocytosis may independently predict the disease-specific survival for EOC patients.

청소년기 여학생에서 빈혈 및 철분 영양 상태 (Anemia and Serum Iron Status in Adolescent Female)

  • 목 적 : 빈혈은 아직도 전세계적으로 가장 흔히 볼 수 있는 영양결핍으로 특히 청소년기 여아에 많이 발생하는데, 그 원인은 철분의 불충분한 섭취, 빠른 성장에 따른 철분 필요량 증가 및 월경으로 인한 혈액 손실을 들 수 있다. 본 연구는 건강하게 보이는 중학생 및 고교의 여학생에 있어서 지난 10년간에 걸친 빈혈 및 철분에 관한 영양상태의 발표를 종합하였다. 방 법: 1990년 대구지역에서 도시와 농촌의 여학생에 대해, 1997년 부천지역 여학생에 대해, 1999년 울산지역에서 도시와 농촌지역 여학생에 대해, 2000년 인천에서 도시와 농촌의 여학생에 대해 혈액학적 조사와 신체 계측 그리고 설문지를 통한 조사가 이루어졌다. 빈혈은 혈색소 11.5 g/dL 미만으로 정의하였다. 철 결핍은 혈청 ferritin이 10 ng/mL 미만인 경우로 정의하였고, IDA는 혈색소가 최저 기준치 이하이면서 MCV 78 fL 미만, ferritin 10 ng/mL 미만, 또는 transferrin 포화도 10% 미만인 경우로 정의하였다. 결 과 : 1) 도시와 농촌간의 혈색소 평균치에는 유의할 만한 차이가 없었고, 연령이 증가함에 따라 평균 혈색소의 감소를 볼 수 있었다. 2) 전체적인 빈혈의 유병율은 1990년 13.4%, 1997년 6.9%, 1999년 6.0%, 2000년 5.7%로 감소하는 소견을 보이고 있고, 각각의 조사에서는 연령이 증가함에 따라 유병율도 증가하였다. 특히 여고생에서의 빈혈의 유병율은 1997년 10.1%, 2000년 12.6%로 높게 나타났다. 3) 철 결핍 유병율은 1990년 36.1%에 비해 1997년과 1999년은 13.9%, 13.3%로 감소하는 소견을 보이나 2000년 연구에서는 23.2%로 증가되었다. 4) 연도별 IDA의 유병율은 1990년에는 10.0%, 1997년 4.6%, 1999년 8.3%, 2000년 6.1%로 일정 하지는 않으나 감소하는 소견을 보이고 있고, 연령이 증가하면서 유병율이 증가하는 것을 볼 수 있다. 결 론 : 중, 고교 여학생에 있어서 10년 전에 비하여 철분 영양은 개선되었지만, 농촌과 도시에 관계없이 빈혈 및 철 결핍이 빈발하였고, 청소년기 연령이 올라갈수록 더욱 현저하였다. 더욱이 최근의 다이어트 풍조와 영양섭취의 불균형을 고려할 때, 다른 영양소의 부족 역시 우려할만한 것으로 사료된다. 따라서 중학교 3학년 이상의 여학생과 빈혈의 위험군에 대해서는 선별검사가 요구되며, 아울러 지속적이며 효과적인 영양 교육과 철분지표와 영양지식에 대한 평가가 필요할 것으로 사료된다.

임신 시기별 생화학적 철분 분석 및 철분 결핍상태에 대한 횡적 조사 연구(II) (A Cross-sectional Study of Biochemical Analysis and Assessment of Iron Deficiency by Gestational Age(II))

  • The purpose of this research is to assess hematological and biochemical status and the prevalence of iron deficiency of pregnant women by gestational age to provide the primary data about iron nutritional status of pregnant women. Pregnant women visiting public health centers in Ulsan participated in study and were divided into 3 trimester by last menstrual period(LMP). Hemoglobin (Hgb), hematocrit(Hct)and mean corpuscular volume(MCV) among iron status indices were not statistically different from normal distribution, however total iron binding capacity(TIBC) and serum ferritin were skewed to left and serum iron and transferrin saturation(TS) were skewed to right. Hgb was positively correlated with Hct(r=0.93, p<0.001) but TIBC was negatively correlated with all indices. Serum ferritin was also correlated with all indices, especially in 3rd trimester but not reached to 1st trimester level. Mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), Red cell distribution width(RDW), serum iron and TS were not significantly different by trimester, however when serum serum iron was adjusted with hematocrit to correct the hemodilution, it significantly decreased in 2nd trimester. MCV increased in 2nd trimester and was maintained until late pregnancy, TIBC continued to increase throughout the trimester. The prevalence of anemic by CDC(Centers for Disease Control) Hgb criteria(Hgb <11.0g/dl in 1st and 3nd trimester, Hgb<10.5g/dl in 2nd trimester) was 2.8% in 1st trimester, 22.5% in 2nd trimester, 27.1% in 3rd trimester and was similar with prevalence by CDC Hct criteria(Hct < 33% in 1st and 3rd, Hct < 32% in 2nd). The prevalence of anemic of total subjects was 32.7% by WHO criteria(Hgb < 11.0g/dl). Although almost iron status indices increased in 3rd trimester, the prevalence of anemia by different criteria of all indices increased throughout the trimester, so iron nutritional status was considered as serious during late pregnancy. However, since factors other than iron deficiency, such as infection, infection, inflammation, other nutrient deficiency may also play a significant role, to differentiate the anemia due to mainly iron deficiency from the anemia due to other factors, serum ferritin is among the more useful indices in distinguishing the two conditions because it is depressed only in iron deficiency. Hgb<11.0g/dl and serum ferritin<12.0ug/L as the criteria of iron deficiency was suggested by CDC. 17.8% of all subjects were classified as iron deficient anemia, 14.9% as anemic from other reasons, 21.2% as iron deficiency any only 46.2% were in normal iron status.

Congenital Malaria in Newborns Selected for Low Birth-Weight, Anemia, and Other Possible Symptoms in Maumere, Indonesia

  • Congenital malaria is assumed to be a risk factor for infant morbidity and mortality in endemic areas like Maumere, Indonesia. Infected infants are susceptible to its impact such as premature labor, low birth weight, anemia, and other unspecified symptoms. The aim of this study was to investigate the prevalence of congenital malaria and the influence of mother-infant paired parasite densities on the clinical outcome of the newborns at TC Hillers Hospital, Maumere. An analytical cross sectional study was carried out in newborns which showed criteria associated with congenital malaria. A thick and thin blood smear confirmed by nested PCR was performed in both mothers and infants. The association of congenital malaria with the newborn's health status was then assessed. From 112 mother-infant pairs included in this study, 92 were evaluated further. Thirty-nine infants (42.4%) were found to be infected and half of them were asymptomatic. Infected newborns had a 4.7 times higher risk in developing anemia compared to uninfected newborns (95% CI, 1.3-17.1). The hemoglobin level, erythrocyte amount, and hematocrit level were affected by the infants' parasite densities (P<0.05). Focusing on newborns at risk of congenital malaria, the prevalence is almost 3 times higher than in an unselected collective. Low birth weight, anemia, and pre-term birth were the most common features. Anemia seems to be significantly influenced by infant parasite densities but not by maternal parasitemia.

MCV와 RDW를 이용한 복합유기용제 취급 근로자의 빈혈 유형에 관한 조사 (The Type of Anemia in Organic Solvent Workers by MCV and RDW)

  • The hematologic effect by low-concentration and repeated exposure to mixed organic solvent remains obscure. This study was performed to evaluate the hematologic effect by mixed organic solvent exposure, especially on the type of anemia by mean corpuscular volume(MCV) and red blood cell distribution width(RDW). The subject were 64 organic solvent workers(male 4, female 60) and 78 general workers(male 18, female 60) who showed anemic tendency in worker's health examination which carried out by Pusan Paik Hospital from January to December, 1988. The author gained some hematologic findings(hemoglobin, hematocrit, RBC, WBC, MCV, MCHC, platelet count, ESR, RDW) from auto-analysis method by coulter counter S plus IV, and compared the type of anemia by MCV and RDW. In the pilot study for estimating the prevalence of anemia according to the type of b, the prevalence rate was higher in organic solvent workers than in public officials as 10.9% and 4.1% respectively. There were statistical significance in the value of hemoglobin, hematocrit, MCV, MCHC, platelet count, ESR, RDW between the two study groups with more severe anemic tendency in organic solvent workers. The type of anemia in organic solvent workers was microcytic and anisocytic with the mean value of $68.28{\pm}8.3fL$ of MCV and $19.1{\pm}4.0%$ of RDW.

대도시 여고생의 빈혈 실태와 이와 영향을 미치는 요인 분석 (The status of anemia of high school girls in the large cities and an analysis of the related factors)

  • This study was attempted to investigate the prevalence of anemia and to obtain nutritional education data for high school girls by analyzing the factors influencing on anemia. Total fo 113 subjects(21 female high school students in Seoul and 92 female students in Inchon) were participated in this study from July 1 to July 15, 1990. Hb and Hct values were measured to evaluate the iron status. Duncan's multiple range test were performed for the comparison between the mean values of the various group. Correlations among various factors were analyzed by Pearson's correlation coefficient. The results of this study are summarized as follows. 1. The maen Hb and Hct values were 13.2g/dl and 41.3%. The rate of anemia of the subjects was 5.3% by the criteria of Hb concentration and 2.7% by the Hct value according to World Health Organization. 2. Subjects who had an early menarche had significantly higher weight and triceps than the ones ofr the girls with a late menarche. 3. There were positive relationships among Hb and Hct values, ages of memarche and cycle of menstruation. Hb, Hct values were significantly higher in the subjects who had frequent bed time snack, high intakes of meats and fishes group and vitamin A, and low calcium intake. In conculsion, the prevalence of anemia of the subjects was much lower than the previous years. There were positive relationships among Hb and Hct values, age of menarche, night snack, and intakes of meats and fishes and fruits. Therefore, it could be concluded that nutrition education should be emphasized for the high school girls to improve the quality of diet and prevent anemia.

우리나라 임신부의 혈액학적 철분 영양상태 평가 지표의 비교 분석 및 판정 (III) (Comparison and Evaluation of Hematological Indices for Assessment of Iron Nutritional Status in Korean Pregnant Women(III))

  • The purpose of this research is to assess th iron nutritional status of pregnant women and to evaluate the appropriateness of the present cut off levels of hemoglobin(Hgb), hematocrit(Hct) and total iron binding capacity(TIBC) for assessing iron deficiency status. Pregnant women who were visiting public helath centers in Ulsan were interviewed and agreed to attend the study. Blood sample was taken and biochemical analysis of blood was performed. The collected data were classified into 3 trimesters by gestational age and then statistical analysis was performed. The prevalence of anemia in all subjects was 32.3% by WHO criteria(Hgb < 11.0g/dl) and 17.8% of all subjects was iron deficient anemia by CDC criteria(Hgb < 11.0/dl and serum ferritin < 12.0ug/l). Since the iron deficient anemia generally occures at the last stage of iron deficiency, it is not efficient to diagnose and prevent the iron deficient anemia in pregnant women by using the present cut off level of Hgb. Therefore, the new cut off level of iron status indices is necessary for assessing iron deficiency in early pregnancy before manifestation of anemia and for reducing the prevalence of anemia in later pregnancy. For this reason, the present cut off levels of iron status indices were estimated and compared by assessing the iron deficiency judged by serum ferritin level (<12.0ug/l)as true iron deficiency. It follows from the results of this research that present cut off levels of Hgb, Hct and TIBC were very insensitive in identifying the subjection with iron deficiency. The appropriate cut off levels of Hgb were 11.5g/dl for total period of pregnancy, 12.0g/dl for 1st and 3rd trimester, and 11.5g/dl for 2nd trimester. The cut off level of Hct was 34.0% for total period for pregnancy, 35.0% for 1st trimester, and 34.0% for 2nd and 3rd trimester. The cut off level of TIBC was 400ug/dl for total period, 360ug/dl for 1st 2nd trimester, and 450ug/dl for 3rd trimester.

Prevalence and risk factors for vitamin D deficiency in children with iron deficiency anemia

  • Purpose: The increasing prevalence of breast feeding has led to concerns about vitamin D deficiency (VDD) and iron deficiency anemia (IDA) in children. We evaluated the prevalence of VDD in a population of Korean children with IDA and assessed the risk factors for VDD in these children. Methods: A total of 79 children who were diagnosed with IDA were prospectively surveyed from April 2010 to March 2011. Data were collected by questionnaire, medical assessment, and laboratory tests, including measurement of 25-hydroxyvitamin D (25OHD), hemoglobin, and wrist radiography. Results: The median age was 22 months and 30% of the subjects were female. Over a half of subjects (58%) had subnormal vitamin D level (25OHD<30 ng/mL), and VDD (25OHD<20 ng/mL) was present in 39% of children. There was no difference in serum hemoglobin level between IDA patients with VDD and those without VDD. Most subjects (89%) were currently or had recently been breastfed and almost all subjects (97%) who had VDD received breastfeeding. Children with VDD were more likely to be younger than 2 years, to have been breastfed, and to have been tested in winter or spring. Multivariable analysis indicated seasonal variation was a significant independent risk factor for VDD in our IDA patients. Conclusion: Our results demonstrated that VDD has a high prevalence in Korean children with IDA. Primary care physicians should be aware of the possibility of VDD in children with IDA and should supplement the vitamin D as well as iron.

Comparison of nutrient intakes by nutritional anemia and the association between nutritional anemia and chronic diseases in Korean elderly: Based on the 2013-2015 Korea National Health and Nutrition Examination Survey Data

  • BACKGROUND/OBJECTIVES: The elderly are reported to have a high prevalence of nutritional anemia when they have lower intakes of nutrients or chronic diseases. This study was conducted to compare nutritional status according to nutritional anemia and to determine associations between nutritional anemia and chronic diseases in Korean elderly. SUBJECTS/METHODS: This study utilized data on 3,258 elderly aged ${\geq}65$ years gathered during the $6^{th}$ Korea National Health and Nutrition Examination Survey 2013-2015. Subjects were divided into nutritional anemia (NA) group (n = 415) and non-NA group (n = 2,843) by hemoglobin concentration. Nutrient intakes were assessed using dietary intake data obtained using the 24-hour recall method. The odds ratios (ORs) for nutritional anemia by chronic diseases were determined. Statistical analysis was performed using SPSS Ver. 23.0. RESULTS: Of 3,258 subjects, 12.7% had nutritional anemia. Intakes of potatoes, pulses, and mushrooms by males and potatoes, fruits, meats, eggs, and seafood by females were significantly lower in NA group than in non-NA group. The proportion of the subjects whose intakes of protein, vitamin A, vitamin $B_1$, vitamin $B_2$, niacin, and iron less than estimated average requirement (EAR) were significantly higher in NA group compared to non-NA group. After adjusting for age, the number of family members, energy intake, and alcohol drinking, ORs for nutritional anemia in the subjects with diabetes and myocardial infarction or angina pectoris were significantly higher by 1.74 times and 1.59 times as compared to the subjects without those diseases, respectively. However, ORs for nutritional anemia in the subjects with obesity, abdominal obesity, and hypertriglyceridemia were significantly lower by 0.64 times, 0.60 times, and 0.59 times as compared to the subjects without those diseases, respectively. CONCLUSIONS: The results of this study suggested that nutritional management should be done to enable the Korean elderly to consume foods with high hematopoietic nutrients density to prevent nutritional anemia. Korean elderly need to make regular efforts to check for nutritional anemia.
