Analysis of Vestibuloautonomic Reflex by Heart Rate Variability (심박수 변이도를 이용한 전정자율신경반사의 분석)

  • There is substantial evidence that anatomical connections and functional interactions exist between vestibular and autonomic systems. Heart rate variability (HRV) including mean, standard deviation, coefficient of variation (CV), power spectrum was analyzed for evaluation of the physiological role of the vestibular system on control of heart rate in rabbits. In anesthetized rabbits, electrical stimulation of the vagus nerve decreased heart rate and decreased LF/HF by increasing HF. On the cervical sympathetic nerve increased heart rate and increased LF/HF by increasing LF. Atropine, cholinergic blocker, increased heart rate and increased LF/HF by reducing HF, and propranolol, ${\beta}$-adrenergic blocker, decreased heart rate and decreased LF/HF by reducing LF> In unanesthetized rabbits, stimulation of the vestibular system induced by rotation or caloric increased heart rate and increased LF/HF by increasing LF> Also electrical stimulation of the vestibular nerve produced the same of effects as rotation or caloric in anesthetized rabbits. These results suggest that Stimulation of the vestibular system increased heart rate not by inhibiting the parasympathetic nerve but by activating the sympathetic nerve.

Coarctation of Aorta: A Report of 4 Cases (대동맥교약증 수술치험 4예)

  • Recent advances in the surgical treatment of congenital disorders of the heart have necessitated an accurate preoperative diagnosis. Right heart catheterization has become widely accepted as a research tool and diagnostic test to detect the heart diseases, especially in the congenital heart anomalies. Right heart catheterizations were carried out in 50 cases of congenital heart diseases at Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital, during the period of June, 1975 through September 1978. In age distribution, 27cases were below 10 years of age, 18 cases between 11 and 20, and 5 cases above 20 male to female ratio was 2.8:1. The distribution of congenital heart diseases was VSD [42%], TOF [36%], PDA [10%], ASD [8%], and PS[4%]. Of these, 44 cases [88%], were compatible with the clinical impressions that were made preliminarily before cardiac catheterization, and all the cases except 1 case of VSD was correlated well with the postoperative diagnosis. The right heart catheterization is considered to be reliable and accurate toll in the preoperative diagnosis of congenital heart diseases. These procedures caused complications such as left side hemiplegia [lcase], occlusion of the femoral artery [lcase], and transient ventricular tachycardia [1case], and so the complication rate of right heart catheterization was 6% [3 cases]. None of patients who have undergone right heart catheterization was died.

Pediatric heart transplantation: how to manage problems affecting long-term outcomes?

  • Since the initial International Society of Heart Lung Transplantation registry was published in 1982, the number of pediatric heart transplantations has increased markedly, reaching a steady state of 500-550 transplantation annually and occupying up to 10% of total heart transplantations. Heart transplantation is considered an established therapeutic option for patients with end-stage heart disease. The long-term outcomes of pediatric heart transplantations were comparable to those of adults. Issues affecting long-term outcomes include acute cellular rejection, antibody-mediated rejection, cardiac allograft vasculopathy, infection, prolonged renal dysfunction, and malignancies such as posttransplant lymphoproliferative disorder. This article focuses on medical issues before pediatric heart transplantation, according to the Korean Network of Organ Sharing registry and as well as major problems such as graft rejection and cardiac allograft vasculopathy. To reduce graft failure rate and improve long-term outcomes, meticulous monitoring for rejection and medication compliance are also important, especially in adolescents.

Reoperation after Open Heart Surgery -Clinical analysis of 27 cases- (개심술후 재수술에 대한 임상적 고찰)

  • The emergence and expansion of cardiac surgery over the past decade has resulted in an increasing number of patients undergoing cardiac operations but many kinds of heart surgery was realized only palliative, resulting in increasing numbers of secondary cardiac procedures. From 1978 to 1988, 10 cases of various congenital heart diseases and 17 cases of acquired heart diseases were reoperated at Hanyang University Hospital. The leading indication of second operation was residual shunt or valvular malfunction due to technical failure in congenital heart disease and primary valve failure, endocarditis, paravalvular leakage were for acquired heart disease. The mortality of reoperation was 0% for congenital heart disease and 11.7%[2 death among the 17 patients] for acquired heart disease. The leading causes of death were myocardial failure, sepsis with endocarditis, acute renal failure and congestive heart failure.

Change Pattern of Heart Age in Korean Population Using Heart Age Predictor of Framingham Heart Study (Framingham Heart Study의 Heart Age Predictor를 활용한 한국인 심장나이 추이분석)

  • The purpose of this study is to observe the trends of heart age of Koreans by using the predictor of heart age of the Framingham Heart Study. The subjects were 20,012 adults aged 30~74 years who were enrolled in the Korean National Health and Nutrition Examination Survey from 2005~2013. They filled in the determinants data and they had no history of cardiovascular disease (CVD). The heart age was calculated using a non-laboratory based model of prediction. The difference of heart age and chronological age, and the rate of excessive heart age over 10 years were calculated. The annual trend, the difference according to gender, the age bracket and geographic region, the heart age were all evaluated. Data analysis performed using the SAS program (version 9.3). Complex designed analysis was done. The heart age showed differences according to gender, age bracket and geographic region. The heart age is a useful comprehensive indicator for predicting the CVD events in the near future. So, it could be used for the purposes of exercising caution and guidance on CVD for administering medical care. It is strongly recommended to use heart age as an indicator for customized medical management to focus efforts on relatively vulnerable subjects and their factors for CVD. Further study on Koreans' customized heart age is needed.

Influence of Physical Conditions of Mountain Trails on Heart Rate Changes (등산로의 물리적 조건이 심장박동수에 미치는 영향)

  • To investigate the heart rate changes under the influence of physical conditions on mountain trails, heart rates of 3 subjects were recorded on 9 routes in Kyeryongsan National Park and Odaesan National Park. Maximal heart rate during uphill walking recorded between 84% and 96% to individual maximal heart rate using ergometer, thus uphill walking on mountain trail was an exhaustive exercise. The ratio of maximal heart rate during downhill walking indicated between 78% and 93% to maximal heart rate during uphill walking. And gradient of trail influenced on heart rate, but width of trail didn't. The heart rate on wooden- or stony-step-trail were rapidly increased in comparison with the heart rates on soil-trails.

  • The structural defects of a heart often reflects the sounds that the heart produces. This paper describes heart sound analysis method using Wavelet transform and average Shannon energy. This can extract the features of heart sounds in various disease identify the heart sounds. Experimental results show that the presented method has potential application in detecting various heart diseases.

  • The objective of this paper is to find an easier and non-invasive a way of diagnosing heart diseases based on the heart sound, rigidly heart murmurs, recordings from subjects. Although most of the heart sounds can be easily heard, analysis of the findings by auscultation strongly depends on skills and experience of the physician. Therefore, the heart murmur is require quantitative analysis for automatic diagnosis equipment. For a good sound analysis, the noisy component ware filtered. This can be done using Wiener filter. Once the signal is filtered, it can be segmented into its basic components by signal energy using FFT. After segment the heart sound signal, the relative positions of the different heart sound components will be identified and will be used for quantification purposes. We are using murmur energy ratio. The experimental results are fairly good in relation to automatic diagnosis.

  • At least 88 percents of ventricular aneurysms result from anterior infarction, while the remainder follow inferior infarction. Posterior infarction that produce a distinct left ventricular aneurysm is unusual. We report two operative cases of postinfarction posterobasal left ventricular aneurysms, one with a true aneurysm and the other with a false one.
