요각통 및 요추간판탈출증 침 치료에 다용되는 혈위 조사 및 협척혈의 해부학적 위치에 대한 문헌적 고찰 (The Trend Review of Acupoints for Lumbar HIVD Treatment and the Literature Review of Anatomical Location of Hwatahyeopcheock)

  • Objectives : The purpose of this study is to explore the trends of acupoints used for lumbar HIVD(Herniated Intervertebral Disc) and treatment and anatomical location of Hwatahyeopcheock. Methods : We searched the latest clinical studies on acupuncture treatment for lumbar HIVD(Herniated Intervertebral Disc) through domestic studies search. To analyze the data, we categorized them by year and article types of literatures and investigated often-used acupoints and acupuncture types for treatment by reading treatment part of every paper searched. Domestic and Chinese literatures related to Hwatahyeopcheock were also studied for its anatomical location. Results : Total 50 articles are searched and local points are more used than distant points for lumbar HIVD(Herniated Intervertebral Disc) and most of them showed curative effects. Back-su points were used the most and Hwatahyeopcheock use accounted for one-third of the articles. In the articles of Hwatahyeopcheock use, except for one, patients complained of both low back pain and radiating pain, and all of them showed positive results after treatment. Conclusions : Back su point is considered to have its meaning as physical region where patients feel pain including herniated disc level rather than the effect meridian system makes. Through document research and meridian muscle theory, we found that Hwatahyeopcheock means vertebral facet joint, intervertebral foramen and surrounding muscle, nerve, blood vessel and related spinal nerves.

Clinicopathological Features of Colon Adenocarcinoma in Qazvin, Iran: A 16 Year Study

  • Background: Colorectal cancer (CRC) was the fourth most commonly diagnosed cancer in Iran between 2000 and 2009, with adenocarcinoma (AC) as the most common histological type. Demographic, topographic and histological variables are important in the epidemiology and biology of cancer. The aim of this study was to investigate clinicopathological features of colon adenocarcinomas in Qazvin, Iran. Materials and Methods: With a retrospective design, patient records of two pathology wards from March 1997 to March 2013 were studied with regard to anatomical location and histological classification. A broader anatomical grouping was also used including distal vs proximal regions and right sided vs left sided tumors. Data were analyzed using T-test and chi-square test. Results: 118 (50.9%) male and 114 (49.1%) female patients were included in the study. Mean age was $57.3{\pm}14.7$ years, with 29.2% under 50 years. There was no significant gender difference for age at diagnosis. The rectum (56%) and sigmoid colon (25%) were the most frequent anatomical locations. Proximal cases accounted for 18.6% in males and 8.8% in females (p=0.02). AC was more prevalent than other usual types in younger patients. The proportion of proximal cancer was 1.7% in first eight years of the study period vs 12.1% in the second one (p=0.005). A similar trend was also seen in right sided colon cancers (p=0.018). Conclusions: Young people are also at risk for the cancer with poor prognosis. Screening programs and weight loss in obese individuals can reduce incidence and complications of CRC.

Improved Nipple Location in Young Korean Patients

  • Background Gynecomastia is benign enlargement of breast tissue in males and is fairly common. Mastectomy not only helps in improving the shape of anterior chest, but can also improve the location of nipple. Therefore, a principle element of mastectomy design is defining the normal location of nipple based on major anatomical reference points. Here, the nipple location was compared for before and after gynecomastia surgery. In addition, the same was also compared between male patients undergoing gynecomastia surgery and control group of subjects without gynecomastia. Methods We retrospectively analyzed gynecomastia patients who underwent conventional subcutaneous mastectomy. Preoperative and postoperative anatomical landmark distances and chest circumferences were measured and compared to the same anthropometric data from 20 healthy adult male controls. Results Nipple locations were compared among 13 patients and 20 controls. The mean weight of resected breast tissue was 246 g, and overall patient satisfaction grade was 4.3 out of 5. In the patient group, the slopes for the height-distance from the sternal notch to the nipple and chest circumference-distance between the mid-line of the sternum and the nipple were 0.175 and 0.125 postoperatively, respectively. The slopes of the control group were 0.122 and 0.177, respectively; these differences were statistically significant (P<0.05). Conclusions Nipple positions were considerably lower in patients with gynecomastia than in control subjects. Subcutaneous mastectomy was associated with mild elevations, but postoperative locations were still lower compared to controls. Further efforts are needed to improve the location of postoperative nipple-areola complex in patients with gynecomastia.

Analysis of location and prevalence of maxillary sinus septa

  • Purpose: The sinus lift procedure requires detailed knowledge of maxillary sinus anatomy and the possible anatomical variations. This study evaluated the location and prevalence of maxillary sinus septa using computed tomography (CT). Methods: This study was based on the analysis of CT images for posterior maxilla which were obtained from patients who visited Chonbuk National University Dental Hospital during the period of June 2007 to December 2008. With the exclusion of cases presenting any pathological changes, 236 maxillary sinuses in 204 patients were retrospectively analyzed. The average age of the patients was 50.9. The cases were divided into two groups, an atrophy/edentulous segment and a non-atrophy/dentate segment, and maxillary sinus septa of less than 2.5 mm were not taken in-to consideration. The location of septa was also divided for analysis into 3 regions: the anterior (1st and 2nd premolar), middle (1st and 2nd molar) and posterior (behind 2ndmolar) regions. Results: In 54 (20.9%) of the 204 patients there were pathologic findings, and those patients were excluded from the analysis. Sinus septa were present in 58 (24.6%) of the 236 maxillary sinuses and in 55 (27%) of the 204 total patients. In the atrophy/ edentulous ridge group (148 maxillary sinuses), 41 cases (27.7%) were found, and 17 cases (19.3%) were found in the non-atrophy/ dentulous ridge group (88 maxillary sinuses). In terms of location, septa were found in 18 cases (27.3%) in the anterior, in 33 cases (50%) in the middle and in 15 cases (22.7%) in the posterior regions. Conclusions: In the posterior maxilla, regardless of type of ridge (atrophy/edentulous or non-atrophy/dentate), the anatomical variation of sinus septa is diverse in its prevalence and location. Thus, accurate information on the maxillary sinus of thepatient is essential and should be clearly understood by the surgeon to prevent possible complications during sinus lifting.

교신(KI8)혈 혈위에 관한 연구 (Study on the Acupoint Location of Kyoshin (KI8))

  • Objectives : The objective of this study was to examine the acupoint location of Kyoshin (KI8) from classic literatures. Methods : A review of KI8 location along the meridian route from 18 classics of acupuncture and moxibustion - 『Huangdineijing·Lingshu』, 『Huangdimingtangjingjixiao』, 『Zhenjiujiayijing』, 『Huangdineijingtaisu』, 『Huangdimingtangjiujing』, 『Beijiqianjinyaofang』, 『Waitaimiyaofang』, 『Ishimpo』, 『Taipingshenghuifang』, 『Tongrenshuxuezhenjiutujing』, 『Shengjizonglu』, 『Zhenjiuzishengjing』, 『Shisijingfahui』, 『Shenyingjing』, 『Zhenjiujuying』, 『Yixuerumen』, 『Zhenjiudacheng』, and 『Yizongjinjian』 - was performed. Then, KI7 location on classics and current standard KI8 location were compared. Results : Based on modern standard acupoint location system, the acupoint of KI8 is located on the medial aspect of the leg, in the depression posterior to the medial border of the tibia and it is on the route of spleen meridian. But no classics of acupuncture and moxibustion said KI8 was located on the route of spleen meridian. In addition, KI8 location on classics was largely described as being located in front of KI7, but only in 『Yizongjinjian』 was it written that KI8 was located posterior to KI7. Conclusions : Through a classic literature review, it is possible to explain that KI8 is located posterior to spleen meridian. The acupoint of KI8 seems to be located between medial border of flexor pollicis longus and flexor digitorum longus based on anatomical location.

뒤안쪽넙다리 관통가지피판의 해부와 임상적 적용 (Posteromedial thigh perforator flap : An Anatomical Study and Clinical Applications)

  • Purpose: An anatomical study concerning the location of the perforators to the posteromedial thigh perforator flap was done for the purpose of clinical application, and reconstruction using this flap was undergone for 3 patients with ischial sores. Methods: The authors dissected 6 cadavers, measuring the distance between the perforator of the posteromedial thigh perforator flap and the extended line of the inguinal crease. The location of the perforator was studied using this data. In the 3 cases mentioned above, perforators were traced using Doppler ultrasonography and the sores were reconstructed with posteromedial thigh perforator flaps. Results: Anatomical study results revealed that posteromedial thigh perforators were found $77{\pm}18.9mm$ below the extended line of the inguinal crease. Application of the flap in the 3 patients was successful. Conclusion: The perforator to the posteromedial thigh perforator flap was found to be located in a relatively consistent position. Since this flap is also comparatively easy to elevate and mobilize, and shows low donor site morbidity, it is thought to be very useful in the treatment of ischial sores.

A numerical method for improving the reliability of knee translation measurement in skin marker-based motion analysis

  • In skin-marker based motion analysis, knee translation measurement is highly dependent on a pre-selected reference point (functional center) on each segment determined by the location of anatomical landmarks. However, the placement of skin markers on palpable anatomical landmarks (i.e., femoral epicondyles) has limited reproducibility. Thus, it produces large variances in knee translation measurement among different subjects, as well as across studies. In order improve the repeatability of knee translation measurement, in this study an optimization method was introduced, by which the femoral functional center was numerically determined. At that point the knee anteroposterior translation during the stance phase of walking was minimized. This new method was tested on 30 healthy subjects during walking in gait lab with motion capture system. Using this new method, the impact of skin marker position (at anatomical landmarks) on the knee translation measurement has been minimized. In addition, the ranges of anteroposterior knee translations during stance phase were significantly (p<0.001) smaller than those measured by conventional method which relies on a pre-selected functional center ($11.1{\pm}3.5mm$ vs. $19.9{\pm}5.5mm$). The results of anteroposterior translation using this new method were very close to a previously reported knee translation (12.4 mm) from dual fluoroscopic imaging technique. Moreover, this new method increased the reproducibility of knee translation measurement by 50%.

Computed tomography-guided 3D printed patient-specific regional anesthesia

  • Classic anesthetic techniques for the inferior alveolar nerve, lingual nerve, and long buccal nerve blockade are achieved by estimating the intended location for anesthetic deposition based on palpation, inspection, and subsequent correlation for oral anatomical structures. The present article utilizes computed tomography (CT) data to 3D print a guide for repeatable and accurate deposition of a local anesthetic at the ideal location. This technical report aims to anatomically define the ideal location for local anesthetic deposition. This process has the potential to reduce patient discomfort, risk of nerve damage, and failed mandibular anesthesia, as well as to reduce the total anesthetic dose. Lastly, as robotic-based interventions improve, this provides the initial framework for robot-guided regional anesthesia administration in the oral cavity.

종양핵의학에서 PET/CT의 역할 (Combined PET/CT in Oncology)

  • Presently, PET is widely used in oncology, but suffers from limitations of poor anatomical information. To compensate for this weakness, a combined PET/CT has been developed by Professor Townsend at the University of Pittsburgh Medical Center. The prototype was designed as PET and CT components combined serially in a gantry. The CT images provide not only accurate anatomical location of the lesions but also transmission map for attenuation correction. More than 300 cancer patients have been studied with the prototype of PET/CT since July, 1998. The PET/CT studies affected the managements in about $20{\sim}30%$ of cancer patients. These changes are a consequence of the more accurate localization of functional abnormalities, and the distinction of pathological from normal physiological uptake. Now a variety of combined PET/CT scanners with high-end PET and high-end CT components are commercially available. With the high speed of multi-slice helical CT, throughput of patient's increases compared to conventional PET. Although some problems (such as a discrepancy in breathing state between the two modalities) still remain, the role of PET/CT in oncology is very promising.
