Anti-Vascular Endothelial Growth Factor Treatment of Retinopathy of Prematurity: Efficacy, Safety, and Anatomical Outcomes

  • Purpose: To investigate the efficacy, safety, and anatomical outcomes associated with intravitreal anti-vascular endothelial growth factor (VEGF) treatment of retinopathy of prematurity (ROP). Methods: We performed a retrospective review of intravitreal anti-VEGF (bevacizumab or ranibizumab) treatment of 153 eyes (83 infants) diagnosed with ROP at two tertiary hospitals from June 2011 to January 2017. The primary outcome was the rate of recurrence requiring additional treatment; secondary outcomes included incidence of major complications and final refractive error. Results: A total of 101 eyes were treated with bevacizumab, and 52 with ranibizumab. The bevacizumab and ranibizumab groups were characterized by mean birthweights of $941.8{\pm}296.1$ and $1,257.7{\pm}514.5g$, gestational ages at birth of $26.9{\pm}1.9$ and $28.1{\pm}3.2$ weeks, and postmenstrual ages at treatment of $40.4{\pm}2.4$ and $39.2{\pm}2.3$ weeks, respectively. The two groups differed significantly in birthweights and gestational ages at birth, but not in postmenstrual ages at treatment. The mean follow-up duration was $30.9{\pm}18.4$ months for the bevacizumab group, and $13.9{\pm}12.5$ months for ranibizumab. More cases were classified as zone 1 ROP in the ranibizumab group (44.2% vs. 11.9%, p < 0.001). Major surgical interventions included scleral encircling and vitrectomy (one and two eyes, respectively, both in the bevacizumab group). Retinal detachment was noted in one eye treated with bevacizumab. There was no significant difference in the most recent spherical equivalence for the two groups ($+0.10{\pm}3.66$ and $+0.22{\pm}3.00$ diopters for bevacizumab and ranibizumab, respectively). Univariable analysis revealed that only ROP stage influenced the occurrence of major complications (odds ratio, 9.046; p = 0.012). Conclusions: Intravitreal anti-VEGF treatment of ROP with both bevacizumab and ranibizumab achieved stable retinal vascularization with a low rate of complications and recurrence. Ranibizumab achieved similar anatomical outcomes as bevacizumab, without additional risk for major complications.

Dendro-anatomical Study for Identification of Pine at Korea (국내산 소나무 식별을 위한 연륜해부학적 연구)

  • This study identified Korean red pine (Pinus densiflora) and exotic pines (Pinus resinosa, Pinus sylvestris) with a similar anatomical structure using a dendro-anatomical method that applied dendrochronology, tracheid length, and uniseriate ray cell size. Korean red pine samples were collected from 13 national parks, while exotic pine samples were secured from two wood importers. Tracheid length was measured by distinguishing earlywood from latewood, and uniseriate ray height and cell number were determined. As the exotic pine tree-ring chronology was consistent with the foreign standard tree-ring chronology and displayed high statistical significance, the country and region where the pine samples had been felled and the exact felling date were confirmed. According to the results, which compared tracheid length and uniseriate ray size, no difference was observed between the Korean red and Russian pines. However, the tracheid length of the Russian pines turned out to be slightly longer than the length of the Korean red pine. Additional research securing a larger number of exotic pines (P.resinosa, P.sylvestris) is required to yield more accurate results in the future.

Pure laparoscopic versus open left lateral sectionectomy for hepatocellular carcinoma: A propensity score matching analysis

  • Backgrounds/Aims: Anatomical resection has superior oncologic outcomes over non-anatomical resection in hepatocellular carcinoma, and left lateral sectionectomy is the simplest and easiest perform anatomical resection procedure among liver resections. The purpose of this study was to find out the safety and feasibility of pure laparoscopic left lateral sectionectomy (PLLLS) for hepatocellular carcinoma. Methods: Patients who underwent left lateral sectionectomy at a tertiary referral hospital, from August 2007 to April 2019 were enrolled in this retrospective study. After matching the 1 : 3 propensity score, 17 open and 51 pure laparoscopic cases were selected out of 102 cases of total left lateral resection for hepatocellular carcinoma. The group was analyzed in terms of patient demographics, preoperative data, and postoperative outcomes. Results: During the study period, there was no open conversion case. The mean operative time and complication were not statistically significant different between the two groups. There was no statistically significant difference in disease-free survival and overall survival had no statistical between the two groups. There were no mortality cases, and postoperative hospital stay was significantly shorter in the PLLLS group than in the open left lateral sectionectomy (OLLS) group. Conclusions: The oncologic outcomes and complication rate were the same in the PLLLS and OLLS groups. However, the hospital stay was shorter in the PLLLS group than in the OLLS group. The present study findings demonstrate that the PLLLS is a safe and feasible procedure for hepatocellular carcinoma.

Clinical Results of Anatomical Single Bundle Anterior Cruciate Ligament Reconstruction Using Autogenous Quadriceps Tendon (자가 대퇴사두건을 이용한 해부학적 단일 다발 전방십자인대 재건술의 임상적 결과)

  • Purpose: The purpose of this study was to evaluate the clinical outcomes and stability of anatomical single bundle anterior cruciate ligament reconstruction (SBACLR) with quadriceps tendon comparable to double bundle anterior cruciate ligament reconstruction (DBACLR). Materials and Methods: We retrospectively reviewed 28 consecutive patients (16 male, 12 female) who underwent SBACLR using quadriceps tendon from March 2009 (Group 1) and compared its clinical results to whom DBACLR with semitendinosus tendon for 53 patients (51 male, 2 female) from August. 2006 (Group 2). Mean age were 34.9 (range, 16-52) in Group 1 and 21.6 (range, 17-55) in Group 2. Mean follow up periods were 13.9 months (range, 12-20) in Group 1 and 36.2 months (range, 20-52) in Group 2. Lysholm score, International Knee Documentation Committee (IKDC) evaluation form and Tegner score were performed for evaluating the clinical outcome. Lachman test, pivot shift test and KT-1000 arthrometer (MEDmetric Corp., San Diego, CA, USA) were performed for stability. Results: There was no statistical significant difference between the two groups in terms of Lysholm score (Group 1: $85.9{\pm}2.6$, Group 2: $90.9{\pm}1.0$, P=0.226), IKDC score (P=0.345) and Tegner score (Group 1: $6.9{\pm}1.4$, Group 2: $7.1{\pm}1.3$, P=0.523). Nor was there statistical significance between the two groups in terms of KT-1000 arthrometer (Group 1: $1.5{\pm}1.1\;mm$, Group 2: $1.5{\pm}1.6\;mm$, P=0.457), Lachman test (P=0.547) and pivot shift test (P=0.073). Conclusion: Anatomical SBACLR with quadriceps tendon shows similar clinical outcomes and stability comparable to anatomical DBACLR with hamstring tendon.

A STUDY ON THE ACCURACY OF DENTAL CAST AND DIE MATERIALS USING PHOTO-SCANNING (사진 주사(走査)를 이용한 치과용 모형재의 정확도에 관한 연구)

  • Dental cast and die materials are essential material using in almost dental prsthodontic procedure and it's most important requirement is accuracy for reqorducing the oral anatomical structures. In this study, 5 abutments A, B, C, D, E were fabricated on the metal master model to simulate the arch form and specimens were poured with 4 cast materials. Inter-abutment distances, A-B, A-C, A-D, A-E, B-C, B-D were calculated using the photo-scanning and the deviations from the metal master model were also evaluated. The results were as follows; 1. The distance between A-B, A-C, A-D, A-E, B-C, B-D of the abutments A, B, C, D, E of each cast material was calculated. And after comparing the deviations between the metal master model. $Fujirock^{(R)}$ showed the lowest value with $0.20{\pm}0.22mm$, and the deviation increased in the order of $Suprastone^{(R)}$, Epoxy $Die^{(R)}$, Die $Keen^{(R)}$. There was significant difference between $Fujirock^{(R)}$ and Epoxy $Die^{(R)}$, Die $Keen^{(R)}$. 2. In each calculation area, the difference in measurements between cast material and metal master model showed singificant difference between A-B and Cross arch measure-ments of A-D, B-D, A-E(p<0.05). 3. The difference in measurements between cast material and metal master model in the A-B area showed $Fujirock^{(R)}$ to be the lowest with $0.05{\pm}0.04$mm and increased in the order of Die $Keen^{(R)}$, $Suprastone^{(R)}$, Epoxy $Dies^{(R)}$. There was significant difference between $Fujirock^{(R)}$ and $Suprastone^{(R)}$, Epoxy $Die^{(R)}$ (p<0.05). 4. The difference in measurements between cast material and metal master model in the B-C area showed $Fujirock^{(R)}$ to bo the lowest with $0.17{\pm}0.11$mm and increased in the order of $Suprastone^{(R)}$, Die $Keen^{(R)}$, Epoxy $Dies^{(R)}$. There was significant difference between $Fujirock^{(R)}$ and Die $Keen^{(R)}$, Epoxy $Die^{(R)}$(p<0.05). 5. The difference in measurements between cast material and metal master model in the B-D area showed $Fujirock^{(R)}$ to bo the lowest with $0.13{\pm}0.07$mm, Epoxy $Dies^{(R)}$and increased in the order of $Suprastone^{(R)}$, Die $Keen^{(R)}$. There was significant difference between $Fuji-rock^{(R)}$ and Die Keen(p<0.05). 6. In this experiment, Epoxy $Dies^{(R)}$ showed mean contraction in every calculation area. And when reconstruction cross arch restorations it is thought that distortion should be considered in every cast material.

Assessment of the proximity between the mandibular third molar and inferior alveolar canal using preoperative 3D-CT to prevent inferior alveolar nerve damage

  • Background: The inferior alveolar nerve (IAN) may be injured during extraction of the mandibular third molar, causing severe postoperative complications. Many methods have been described for evaluating the relative position between the mandibular third molar and the inferior alveolar canal (IAC) on panoramic radiography and computed tomography, but conventional radiography provides limited information on the proximity of these two structures. The present study assessed the benefits of three-dimensional computed tomography (3D-CT) prior to surgical extraction of the mandibular third molar, to prevent IAN damage. Methods: This retrospective study included 4917 extractions in 3555 patients who presented for extraction of the mandibular third molars. The cases were classified into three groups, according to anatomical relationship between the mandibular third molars and the IAC on panoramic radiography and whether 3D-CT was performed. Symptoms of IAN damage were assessed using the touch-recognition test. Data were compared using the chi-square test and Fisher's exact test. Results: Among the 32 cases of IAN damage, 6 cases were included in group I (0.35 %, n = 1735 cases), 23 cases in group II (1.1 %, n = 2063 cases), and 3 cases in group III (0.27 %, n = 1119 cases). The chi-square test showed a significant difference in the incidence of IAN damage between groups I and II. No significant difference was observed between groups I and III using Fisher's exact test. In the 6 cases of IAN damage in group I, the mandibular third molar roots were located lingual relative to the IAC in 3 cases and middle relative to the IAC in 3 cases. The overlap was ${\geq}2mm$ in 3 of 6 cases and 0-2 mm in the remaining 3 cases. The mean distance between the mandibular third molar and IAC was 2.2 mm, the maximum distance 12 mm, and the minimum distance 0.5 mm. Greater than 80 % recovery was observed in 15 of 32 (46.8 %) cases of IAN damage. Conclusions: 3D-CT may be a useful tool for assessing the three-dimensional anatomical relationship and proximity between the mandibular third molar and IAC in order to prevent IAN damage during extraction of mandibular third molars.

A prospective clinical trial to compare the performance of four initial orthodontic archwires (교정치료 초기에 사용되는 4가지 호선의 초기 치료효과를 비교하기 위한 전향적 임상 실험 연구)

  • The aim of this study was to compare the clinical performance of 4 types of orthodontic wires, indicated for initial tooth alignment: stainless steel, multistranded steel, superelastic and thermoactivated nickel-titanium. A prospective randomized clinical trial was conducted on a sample of 45 patients, at the Dental School of the State University of Rio do Janeiro, Brazil. Fixed appliances were fitted and study casts were obtained from each patient. Randomly, the wires were allocated as follows: 26 dental arches for superelastic NiTi wires, 22 for stainless steel, 22 for multistranded and 20 for thermoactivated archwires. After 8 weeks, the archwires were removed and impressions for study casts were taken again. Using a 3D digitization technique of defined anatomical points on the study cast crowns, a Dental Irregularity Index (DII) was created for each study cast. The difference between DII before and after the archwire insertion expressed the aligning effect of the wires. ANOVA tests were employed to evaluate the anatomical point approximation (positive DII) and separation (negative DII), for each area of the dental arches: upper and lower whole arch and anterior arch. Results showed no significant difference between the different archwires.

  • Objectives This study is a preliminary study for safe and accurate cervical Hyeopcheok acupuncture by comparing the depth of cervical Hyeopcheok acupoint with cervical spine rotation. Methods Researchers took ultrasonographic images of cervical Hyeopcheok acupoint during full rotation of cervical spine, 45 degree rotation and neutral condition in a prone position. 4 healthy volunteers were recruited. Results No significant difference in needling depth of cervical Hyeopcheok acupoint was observed. However, there were a differences in anatomical structures' shape such as muscles. Conclusions There are no significant difference in needling depth, but anatomical structures' shape were changed. This result mean that if patient rotate cervical spine during acupuncture treatment, it can occur complications. Thus, when acupuncture treatment on cervical Hyeopcheok acupoint, practioner should make subject's cervical spine alignment corrected and be careful not to change posture during the procedure.

A Study of Bodice′Basic Pattern by the Clothing Pressure (의복압에 의한 Bodice 원형에 관한 연구)

  • Even though it has been to research on the variance of clothing pressure and on its effect of human body, it has not been available to evaluate the clothing pressure as an objective standard ta for the determination of an extra quantity of bodice' basic pattern. In this paper the basic pattern(the close adherence original shape) is determined by a drawing of plane figure after detaching an unweaving clothing from the inside of the gypsum, and the extra quantity is figured by a extension quantity when is formatted with 0.5, 1.0, 1.5 cm extra quantity longitudinal-cross section figures. With our experimental method, review the variance of clothing pressure according to difference of the extra quantity which was figured with subjects wearing experimental clothing including the extra quantity. The difference of the extra quantity was able to calculate with data form the subjects when anatomical position and five movements, then compare with subjects wearing non-extra quantity experimental clothing. The results of experiment as the follows : 1. There is only few body portions with the significant variance according to the increasing of he extra quantity at he body portion and the sleeve portion. 2. The clothing pressure of the sleeve portion was higher than the clothing pressure of the body portion. The difference of clothing pressure according to the variance of the extra quantity at the sleeve portion is more significant than the body portion. 3. Consider several important pressure points which wil be the No. 1 at the front of body portion. No. 17, 18 at the back of body portion and No. 21 at the sleeve portion. 4. It is important to have plane figure of gypsum when format an basis pattern.

Evaluation of Gustatory Function in Patients with Sleep Disordered Breathing

  • Purpose: The aim of this study is to evaluate the difference between gustatory functions in a sleep disordered breathing (SDB) group and a control group. The pathogenesis of SDB has not been fully understood. Though the precise contributions of neuromuscular and anatomical factors on SDB pathogenesis are still debated, we hypothesized that the gustatory dysfunction could be predisposed to SDB. Methods: All patients were diagnosed as SDB by polysomnography (PSG). On the basis of PSG results, patients were divided into 3 groups: snoring, mixed, and obstructive sleep apnea (OSA). The control group comprised healthy volunteers who were the same age as those of the SDB group and whose breathing was verified as normal using a portable sleep monitor device. The patient group and the control group were evaluated for gustatory functions with an electrogustometry (EGM). The electrical taste thresholds were measured in the anterior, midlateral, and posterior sides of the tongue and soft palatal regions, both sides. To find out the difference in EGM scores, statistical analysis was performed using the Kruskal-wallis and Mann-Whitney U test with 95% confidence interval and p<0.05 significance level. Results: The patients with SDB had higher EGM scores than the control group at all spots tested, except for the right midlateral of the tongue, and there was a statistical significance in the comparison between the control group and the divided SDB groups, respectively. Among the divided SDB groups, the snoring group had the most significant differences in the number of the measured spots, but there was no difference among the snoring, mixed, and OSA groups. Conclusions: These results may suggest that neurologic alterations with sleep disordered breathing could be associated with gustatory dysfunction. In the future, further systemic studies will be needed to confirm this study.
