Factors affecting the Organizational Commitment of Industrial Accident Hospital Employees by Job Category (병원 구성원들의 직종별 조직몰입의 영향요인에 관한 연구)

  • The purpose of this study was to analyze the characteristics of socio-demographic, organizational culture, organizational conflict and organizational commitment, and to examine the interrelation of influential factors in the organizational commitment. The data for this study were collected through a self-administrated survey with a structured questionnaire to 1,027 subjects from several medical doctor staff members, nursing staff members, administration staff members, pharmacist, and technical expert of eleven hospitals. The data were collected by self-reporting questionnaire from July 29 to September 7, 2002. In this analysis frequency test, t-test, ANOVA, multiple regression were used. The main results of this research is as following; 1. According to socio-demographic characteristics the respondents' level of the organizational commitment was higher in a administration staff members than others, for males than females, and for employees aged more than forty, as working for longer time, and as higher level of the working position. 2. According to the organizational culture characteristics the progressive culture, affiliative culture, and task-focused culture among many types of organizational culture were moderately and positively correlated with the level of the organizational commitment while the hierarchical culture was negatively correlated. 3. According to the organizational conflict characteristics as the respondents who got less conflict experience in the organization, their level of the organizational commitment was higher. And, technical conflict experiences were expressed greater than hierarchical conflict experiences. 4. According to the job satisfaction characteristics as his or her satisfaction that is about the promotion, working itself, salary, and fellowship in the organizational was higher, the level of the organizational commitment was higher. The most important factor of the satisfactions was the fellowship among the respondents while the level of job satisfaction for the promotion and salary was average. 5. Overall, according to the result of Multiple Regression as older age and longer working hours, the level of the organizational commitment was higher and as a higher level of the satisfaction for the promotion, working itself, salary, and fellowship in the organization, it caused more effective factors for the organizational commitment. 6. According to the result of Multiple Regression for the doctor staff members in special hospitals rather than general hospitals the hierarchical culture and task focused culture was positively correlated with the satisfaction of working itself while hierarchical conflict factors in the organizational conflict characteristics was negatively correlated with the organizational commitment. For the nursing staff members the affiliative culture and the job satisfaction for the promotion, working itself, salary, and fellowship were positively correlated with the organizational commitment. For the administration staff members as the job satisfaction for the fellowship was positively correlated with the organizational commitment. For medical and pharmacy staff members as more working experience, correlation with the organizational commitment was positive. Besides, as he or she has a high perception of the affiliative culture, it caused statistically more effective factors for the organizational commitment. For the skill and technicians, male worker expressed greater organizational commitment in the organization than female worker. And also older age along with higher education also showed higher organizational commitment. Moreover, the job satisfaction for the fellowship was positively correlated with the organizational commitment. This study identified the major effective factors of the organizational commitment and analyzed the differences among the job category. In that respect, it is significant for the study to be able to provide a reference for managing hospital of industrial accident and organizational development. However, this study has a problem, which is not to identify a valuable model for examining the relationship between organizational factors such as organizational culture, conflict, satisfaction, and commitment. Therefore, further study is needed and strengthened in the field of organizational commitment for hospital for industrial accident.

A Study on the Change of household-Economy incidental to the Family Life Cycle. (가정생활 주기에 따른 가계변동에 관한 연구)

  • This study aims to present basic data for a reasonable home management through investigating the change of home economy conditions incidental to the family life cycle, also through analyzing the effect of socio-anthropological variables on the family-life cycle. The data investigation using the questionnaires method was conducted on housewives in Seoul as the central census tract. Housewives as the subject of investigation were chosen by the method of the purpose-sampling in consideration of the regional differences and the socio-economical strata. Nine hundred and forty questionnaires were distributed to housewives but seven hundred questionnaires were collected. Only five hundred and ten questionnaires of them were analyzed in this study. The frequency and the percentage of collected data, first of all, were founded in order to grasp the general characteristics of the subject of investigation. To classify the stage of family life cycle, the correlations of the classifying factors among each group were examined x2 Test and One-Way ANOVA were applied to explore the differences among each stage of the change of household-economy. And the effect of socio-anthropological variables on the change of household- economy was analyzed by Two-way ANOVA. the conclusions derived from the results of this study are as follows; 1) The marriage-period and the educational conditions of the first child were selected as the classifying factors through analyzing correlation among the age, the marriage-period and the educational conditions of their children. As a result of this analysis, the family life cycle were classified into seven stages: Stage 1; Establishment Stage 2; Preschool family Stage 3; Elementary school family stage 6; Adult period family Stage7; Marriage period family . 2) The change of household economy incidental to the progress of family life cycle has a significant differences in all of variables (except the other member of family's income) Stage 1; Though the husband's income and the income from property are on a low level, the total income is on a high level due to the housewife's income. Stage 2; The total income is on a low level owing to the decrease of house wife's income, though the husband's income keep growing. Stage 3; Owing to the increase of husband's income, the cost of living as well as the total income keeps growing but the savings are on the decrease. Stage 4; Compared with Stage 3, the total income tends to be on a low level but the living expenses are on the increase. Stage 5; The husband's income is on the highest level in the stages of family life cycle. The total income and the living expenses are on a high level. Stage 6; The income of husband and housewife are on the remarkable decrease but the children's income is on the increase. Stage 7; Owing to the increase of the children's income and the income from property, the total income is on the highest level in the stages of family life cycle. 3) Considering the effect of socio-anthropological variables on the conditions of household-economy, family system has an significant effect on children's income. the husband's occupation exerts a significant effect upon the housewife's and children's income. The husband's schooling exercises an effect upon the children's income. S.E.S has a important effect on the income of husband, housewife and children. From the above results, it is found that the change of household-economy conditions is incidental to the progress of family life cycle. Therefore, a suitable measure to cope with the desire of family and the conditions of household-economy should be prepared, in order to carry on a reasonable home management.

Oxygen Toxicity of Superoxide Dismutase-Deficient Saccharomyces cerevisiae by Paraquat (Paraquat에 의해 유도된 Superoxide Dismutase 결핍 효모의 산소 독성)

  • Using superoxide dismutase (SOD)-deficient mutants of Saccharomyces cerevisiae, the oxygen toxicity induced by paraquat was studied. In aerobic culture condition, yeasts lacking MnSOD (milochondrial SOD) showed more significant growth retardation than CuZnSOD (cytoplasmic SOD)-deficient yeasts. However, not so big differences in growth pattern of those mutants compared with wild type were observed under anaerobic condition. When exposed to paraquat, the growth of yeasts lacking CuZnSOD was severely affected by higher than 0.01mM of paraquat in culture medium. By the analysis of several cellular components ivolved in free radical generating and scavenging system, it was found that, under aerobic condition, the content of lipid peroxides in cell membrane as well as cellular activity of glutathion peroxidase of CuZnSOD-deficient mutants was increased in the presence of paraquat, although significant decrease of catalase activity was observed in those stratns. In MnSOD-deficient yeast, however, increment in cellular activity of glutathion peroxldase and catalase by paraquat was observed without any deterioration of membrane lipid. It implies that the lack of mitochondrial SOD could be compensated by both of glutathion peroxldase and catalase, but that only glutathion peroxidase might act for CuZnSOD in cytoplasm. In contrast, all of SOD-deficient mutants showed a significant decrease in catalase activity, but slight increase in the activities of glutathion peroxidase, when cultivated anaerobically in the medium containing paraquat. Nevertheless, any significant changes of lipid peroxides in cell membranes were not observed during anaerobic cultivation of SOD-deficient mutants. It suggests that a little amount of free radicals generated by paraquat under anaerobic condition could be sufficiently overcome by glutathion peroxidase but not by catalase.

Factors Predicting the Development of Radiation Pneumonitis in the Patients Receiving Radiation Therapy for Lung Cancer (방사선 치료를 시행 받은 폐암 환자에서 방사선 폐렴의 발생에 관한 예측 인자)

  • Background : Radiation pneumonitis(RP) is the major serious complication of thoracic irradiation treatment. In this study, we attempted to retrospectively evaluate the long-term prognosis of patients who experienced acute RP and to identify factor that might allow prediction of RP. Methods : Of the 114 lung cancer patients who underwent thoracic radiotherapy between December 2000 and December 2002, We performed analysis using a database of 90 patients who were capable of being evaluated. Results : Of the 44 patients(48.9%) who experienced clinical RP in this study, the RP was mild in 33(36.6%) and severe in 11(12.3%). All of severe RP were treated with corticosteroids. The median starting corticosteroids dose was 34 mg(30~40) and median treatment duration was 68 days(8~97). The median survival time of the 11 patients who experienced severe RP was significantly poorer than the mild RP group. (p=0.046) The higher total radiation dose(${\geq}60Gy$) was significantly associated with developing in RP.(p=0.001) The incidence of RP did not correlate with any of the ECOG performance, pulmonary function test, age, cell type, history of smoking, radiotherapy combined with chemotherapy, once-daily radiotherapy dose fraction. Also, serum albumin level, uric acid level at onset of RP did not influence the risk of severe RP in our study. Conclusion : Only the higher total radiation dose(${\geq}60Gy$) was a significant risk factor predictive of RP. Also severe RP was an adverse prognostic factor.

  • Background : Synthetic glucocorticoids are widely used in many chronic inflammatory diseases because of their excellent anti-inflammatory activity. Enhancing the transcription of $I{\kappa}B$ and preventing activated NF-${\kappa}B$ from binding to ${\kappa}B$ sites are thought to be the underlying mechanisms. But these data are largely derived from in vitro studies using cell lines. In this study, after administrating a steroid to volunteers, we evaluated the effect on the NF-${\kappa}B$ system. Methods : Prednisolone(0.5mg/kg/d) was orally administered to 5 healthy volunteers for 7 days. Before and after the administration, we sampled their peripheral blood monocytes, and performed western blot analysis both with stimulation, using IL-$1{\beta}$, LPS, TNF, and without stimulation(baseline). We also performed EMSA after stimulation with LPS. Results : After ingestion of the steroid, baseline expressions of $I{\kappa}B{\alpha}$ were increased in two of the subjects, while suppressed degradations of $I{\kappa}B{\alpha}$ to stimulations were observed in all five. In addition, the binding capacity of NF-${\kappa}B$ after the administration was decreased. Conclusion : Steroid plays such roles as enhancing the transcription of $I{\kappa}B{\alpha}$, suppressing the DNA binding capacity of NF-${\kappa}B$, and suppressing the degradation of $I{\kappa}B{\alpha}$.

The Effect of Self-Efficiency and Instrumental Activities of Daily Living of Local Community Stroke Patients on Quality of Life in Caregiver (지역사회 거주 뇌졸중환자의 수단적 일상생활동작과 자기 효능감이 보호자의 삶의 질에 미치는 영향)

  • Objective : This study is aimed to examine the degree of independence in stroke patients residing in a community based on their self-efficacy and Instrumental Activities of Daily Living and also correlation between community-residing stroke patients and their guardians by figuring out the life quality of their guardians. Methods : For total two weeks between May 12th and 23rd, 2014, this researcher collected data by using a survey (K-IADL, a scale to measure self-efficacy, WHOQOL-BREF) targeting 138 stroke patients who received over 24 points in MMSE-K among the outpatients of three hospitals located in Busan Metropolitan City and two hospitals located in Geoje-si, Gyeongsangnam-do, and total 125 sheets were finally used for analysis. Results : Among the 125 patients, 70 were males, and 55 were females, and their average age was 58.72 (16 to 82). The subjects' general self-efficacy (p<0.01) and detailed self-efficacy (p<0.01) indicated significant correlation with their guardian's life quality, and there was no significant correlation found between their Instrumental Activities of Daily Living and their guardian's life quality. Conclusion : Their general self-efficacy and detailed self-efficacy indicated correlation with their guardian's life quality, and among their general characteristics, the factor of their financial situation indicated correlation with their guardian's life quality. In conclusion, when stroke patients' self-efficacy is higher, their guardian's life quality increases, too, and if their financial situation is more favorable, their guardian's life quality gets higher. However, stroke patients' Instrumental Activities of Daily Living did not indicate correlation with their guardian's life quality. When looking at the chronic trend of outpatients, therapeutical plan must be established about the self-efficient recovery for the development of those who are not satisfied.

A Research Study on the Sexual Awareness of Teenagers. Their Parents and Teachers in an Urban Area (일부 시 지역 고교생.학부모.교사들의 성의식 조사)

  • The specific purpose of this study was as follows: 1) To identify the level of sexual awareness and demand for sex education of teenagers, their parents and teachers. 2) To investigate the attitude toward and acceptance of heterosexual friendships by teenagers, their parents and teachers. 3) To identify the experience and direction of sex education of parents and teachers. For this purpose, this study selected 341 male and female teenagers at three high schools located in a city, 119 parents with high school students and 243 elementary/middle/high school teachers. The structured questionnaire was distributed to each of them and returned by them. This study was conducted over the period between June 1. 2001 and Aug 31. 2001. A total of 720 questionnaires were distributed to them and 703 were returned. Data analysis was conducted to obtain percentage and frequency using the SAS program. As a result, the following findings were obtained: Parents and teachers showed a similar attitude about sexual awareness, but teenagers showed a different attitude. In terms of the level of feeling of and interest in 'sex', teenagers, parents and teachers were comfortable in accepting it. As for sexual curiosity in puberty, 84.8% of the parents and 95.5% of the teachers responded that it was natural, whereas only 14.7% of teenagers responded that it was natural. But As for the item that 'sex is natural as one of human needs', 3.6% of the teenagers, 16.8% of the parents and only 1.2% of the teachers responded that it was not. As for the item that 'A man has a stronger sexual impulse than a woman', 6.1% of the teenagers, 16.8% of the parents and 17.3% of the teachers responded that it was not. 64.2% of the teenagers and 90.2% of the teachers responded that it was impossible to overcome the sexual impulse, whereas only 8.4% of the parents responded that it was possible. As for the item of masturbation, 64.5% of the teenagers responded that it was possible for male students and not possible for female students. 67.2% of the parents and 91.1% of the teachers responded that it was possible regardless of sex. As for the item of an attitude toward virginal purity, both the parents and teachers showed that virginal purity was important for both man and woman, but teenagers showed a weak awareness of virginal purity. As for the item, 'It is possible to have sexual intercourse with a lover during adolescence', 20.2% of the teenagers, 79% of the parents and 87.7% of the teachers responded that it was impossible. As for the item 'It is necessary to keep premarital virginal purity for a happy married life', 21.1% of the teenagers, 74.8% of the parents and 55.1% of the teachers responded that it was necessary, which shows that teenagers had a different opinion than parents and teachers. In case of pregnancy in adolescence, most of the responding teenagers, parents and teachers chose abortion. As for the item 'female refusal on male sexual approach', respondents accepted female refusal as they did for orders of teachers, parents and adolescents. As for item 'An assaulter is entirely to blame for sexual assault', all of three groups responded in the affirmative.

Effect of Sex Education on Middle School Students' Access to the Obscene Online Computer and Video Film Contents (성교육이 중학생의 컴퓨터와 비디오 음란물 접촉에 미치는 효과)

  • To evaluate the effect of sex education on middle school students' access to the obscene online computer and video film contents. 154 students were selected as experimental group. and 154 students were selected as control group, sampled randomly from Andong. Kyungbook, Korea. An analysis was performed. A non-equivalent control group pre test-post test research design was used. The data were collected from April 2nd to April 19th. 2001. A pre-survey was done on general characteristics and the condition of accessing obscene online computer and video film contents on both experimental and control group. From the survey results information. sex education contents were put together. The researcher organized 3 ready-made sex education program and explained to the four school health nurses about the ready-made sex education program step by step and they educated their selected students with three classes of 45 minutes lecture. Two weeks after the last lecture, a post-test was conducted. Four weeks from the last lecture, another post-test was conducted. The existing studies by Choi Yongseon(1998) and Kim Hyeok(1998) were reviewed and two professors in the department of community health nursing advised on the study questionnaire writing. An SPSS Win 10.0 was used. The data of respondents' general characteristics were analyzed using frequency and percentage. $X^2$ test was used to verify the homogeneity of the experimental group and the control group. Repeated Measures ANOVA was used to find out whether sex education had an effect on the awareness of obscene online computer and video film contents and under-age prostitution through the online computer networks. and time and frequency of access to the obscene online computer and video film contents. The results of the study are as follow. 1. The results of the verification of homogeneity between the experimental group and the control group showed that there was no significant difference between the experimental group and the control group. 2. The first hypothesis, 'the experimental group which received sex education would have a higher level of awareness of accessing obscene contents than the control group which did not receive the education' was supported at p<0.0001. 3. The second hypothesis. 'the experimental group which received sex education would have a higher level of awareness of underage prostitution on computer networks than the control group which did not receive the education' was supported at p<0.05. 4. The third hypothesis, 'the experimental group which received sex education would spend time less accessing obscene video and computer contents than the control group which did not receive the education' was rejected at p>.05. 5. The 4-1 hypothesis. 'the experimental group which received sex education would access obscene computer contents less frequently than the control group which did not receive the education' was supported at p<0.0001. 6. The 4-2 hypothesis, 'the experimental group which received sex education would access obscene video contents less frequently than the control group which did not receive the education' was supported at p<0.0001. In conclusion, a systematic step-by-step sex education program should be developed to protect middle school students from the harmful online computer and video film access. An effective teaching material for sex education should be prepared to decrease middle school students' access to obscene online computer and video film contents.

WATER CHLOROFORM LEVELS IN INDOOR SWIMMING POOLS IN A CITY OF KOREA AND IN A CITY OF NEW JERSEY IN THE UNITED STATES (국내 및 미국 뉴저지주의 수영장 물에서의 클로로포름)

  • Chlorinated water in swimming pools contains chloroform at elevated levels compared to chlorinated drinking water Chloroform levels in four indoor swimming pools(swimming pools A, B and C in a city of Korea and swimming pool D in a city of New Jersey in the United States) were examined. The chloroform levels in the water of swimming pool C (city-managed) weve shown to be significantly(p=0.0001) different from those of private swimming pools A and B: the mean chloroform levels in the pools A, B, and C are 22.8, 17.8, and 31.1 $\mu\textrm{g}$/l, respectively. Furthermore, all of these chloroform levels are significantly(P=0.0001) different from those of New Jersey: chloroform concentration of the Korean pools ranged from 10.9 $\mu\textrm{g}$.l to 47.9 $\mu\textrm{g}$/l with a mean of 23.2 $\mu\textrm{g}$/l, while it ranged from 27 $\mu\textrm{g}$/l to 96$\mu\textrm{g}$/l with a mean of 64.4 $\mu\textrm{g}$/l in the New Jersey pool. The disinfection processes would cause part of this difference since the swimming pools in Korea applied both chlorination and ozonation method, while the swimming pool in New Jersey used chlorination method only. It was implied that swimming parameters inconsistently vary, resulting in fluctuation of and no constant accumulation of chloroform in the water with the change of time for the day. A regression analysis showed no relationships between sampling time and chloroform concentrations for the sampling day in the swimming pools of Korea. A F-test indicated no significant difference of chloroform concentrations in the morning and afternoon samples collected in the swimming pools. Ingestion dose was estimated to be 0.58$\mu\textrm{g}$ from an hour swimming in a city of Korea, taking into accounting an average of 23.2 $\mu\textrm{g}$/l in swimming pools in the city In extreme situation, the ingestion dose was estimated to be 12.0 $\mu\textrm{g}$ from an hour swimming in a city of Korea.

Environmental Adaptability of Eupatorium rugosum : Relationship between Accumulation of Heavy Metals and Phenolic Compounds (서양등골나물의 환경적응력 : 중금속 축적과 Phenolic Compounds의 관계)

  • Seed germination rate and seedling growth were measured on 6 different species(Phytolacca americana, Eupatorium rugosum, Rumex acetocella, Echinochloa crusgalli, Cassia mimosoides var. nomame, Setaria viridis) treated with leaf extract of E. rugosum. Total phenolic compound and heavy metal were analyzed on leaf and soil with and without E. rugosum. The growth of P. americana seedlings were stimulated by 10% and 25% of E. rugosum water extract treatment. The content of total phenolic compounds in soil was lower than that of leaf extract, and 25% was confirmed as threshold concentration in natural systems because the total phenolic compounds were not significantly different between the control soils and the soil treated with 10%, and 25% extract. Total phenolic compound concentrations of the leaf extracts were highest (1.66 mg/l) with E. rugosum grown under the Quercus forest canopy and lowest (1.09 mg/l) for the plant grown in the mixed forest edge. Leaf extracts of plants selected in different sampling sites (Forest interior, Forest edge, under Pinus Canopy and Quercus Canopy) were significant, while soil extracts were not. Seed germination of R. acetocella and S. viridis were significantly inhibited at over 50% concentrations of E. rugosum, but C. mimosoides var. nomame was not affected at any concentration. The radicle and shoot growth of the native species group were reduced two times more than those of the exotic species group by the treatment of extracts. Especially, the seed germination percentage and dry weight of E. rugosum were greater than those of the control group by treatments with extracts of 10% and 25%. Analysis of aqueous extracts from E. rugosum by HPLC identified 6 phenolic compounds: caffeic acid (460.9 mg/l), benzoic acid (109.7 mg/l), protocatechuic acid (7.3 mg/l), ρ-hydroquinone (6.0 mg/l), cinnamic acid (2.7 mg/l) and hydroquinone (0.23 mg/l). The seed germination of P. americana was also inhibited dramatically by protocatechuic acid and cinnamic acid even though the content of caffeic acid (460.9 mg/l) was the highest among analyzed phenolic compounds. The heavy metal content of soil without A. altissima was higher than that of soil with E. rugosum. Particularly, Al, Fe and Mn was considerably high and most of the heavy metal were accumulated in leaves where a high level of total phenolic compounds was found.
