CASE REPORT OF NECROTIZING FASCITIS ON THE CERVICOFACIAL AREA (경부에 발생한 의인성 괴사성 근막염의 증례)

  • Necrotizing fascitis is a severe soft tissue infection characterized by extensive necrosis of superficial fascia, suppurative fascitis, vascular thrombosis, widespread undermining of surrounding tissues. Associated systemic problems are widespread undermining of surrounding tissues, Associated systemic problems are common, with chronic alcoholism and diabetes being most prominent. Most commonly this disease presents in the extremities, trunk, and perineum. Necrotizing fascitis of dental origing is rare and its fulminating clinical course is not well documented in the dental literature. The present report is a case of necrotizing fascitis following vital extirpation of the pulp in a patient with uncontrolled diabetes mellitus and liver cirrhosis. Originally throught to be caused by hemolytic streptococcus organism or stphylococcus aureus, advances in anaerobic culturing have shown it to be a synergistic bacterial infection involving aerobic and ovligate anaerobes. it is relatively rare in relatively rare in haea and neck regions. If it was not diagnosed and treated in early stages, necrotizing fascitis can be potentially fetal, with a mortality rate approaching 40%. It's treatment requires early recognition, prompt and aggressive surgical debriment and proper supportive cares, such as, antibiotic therapy, fluid resuscitation and correction of metabolic and electrolyte disorder, resolving of the underlying systemic disease. Recently, we experienced two cases of necrotizing fascitis in cervicofacial region, One patient was 60 years old male with uncontrolled Diabetes Mellitus and other patient was 48 years old with steroid therapy during 30 years. Local surgical wound healing was successful but, patients were died after admission, because of lung abscess, gastrointestinal bleeding, septic shock and respiration hold.

Inhibition of Micobial Growth by Paraquat (제초제 Paraquat의 미생물 생육저해 작용)

  • This study was carried out to investigate the toxic action of herbicide, paraquat(1,1'-dimethyl-4,4'-bipyridylium-dichloride), against microorganisms. The toxic effect of paraquat was observed mainly using Escherichia coli(KCTC 1039), as follows; Growth of aerobic microorganisms which comprise 4 strains of bacteria and 2 strains of yeast and 4 strains of mold was inhibited drastically in the presence of 1.0mM paraquat But the growth of anaerobic bacteria was not affected by the chemical. When actively growing cells of E. coli were exposed to the paraquat at the concentration higher than 1.0 mM, they rapidly lost their ability to form colony and clearly formed inhibitory zone by well test More than 50% of the cells were killed by 1.0 mM paraquat treatment, even at immediate addition of paraquat to the medium.

Nitrogen Removal using Autotrophic Microorganism in Membrane-Attached Biofilm Reactor (MABR) (Membrane-Attached Biofilm Reactor(MABR)에서의 독립영양 미생물을 이용한 질소 제거)

  • The purpose of this study is to investigate the performance of nitrogen removal using autotrophic microorganism in the Membrane-Attached Biofilm Reactor (MABR). The treatment system consists of an aerobic MABR (R1) for nitrification and an anaerobic MABR (R2) for hydrogenotrophic denitrification. Oxygen and hydrogen were supplied through the lumen of hollow-fiber membranes as electron acceptor and donor, respectively. In phase Ι, simultaneous organic carbon removal and nitrification were carried out successfully in R1. In phase II, to develop the biofilm on the hollow-fiber membrane surface and to acclimate the microbial community to autotrophic condition, R1 and R2 were operated independently. The MABRs, R1 and R2 were connected in series continuously in phase III and operated at HRT of 8 hr or 4 hr with $NH_4{^+}-N$ concentration of influent, from 150 to 200 mgN/L. The total nitrogen removal efficiency reached the maximum value of 99% at the volumetric nitrogen loading rate of $1.20kgN/m^3{\cdot}d$ in the combined MABR system with R1 and R2. The results in this study demonstrated that the combined MABR system could operate effectively for the removal of nitrogen in wastewater not containing organic materials and can be used stably as a high rate nitrogen removal technology.

Comparison of Appropriate Piperacillin/Tazobactam Doses in Korean Obese Patients with Cancer Based on Different Body Size Descriptor Equations in a Tertiary Care Hospital (국내 3차 병원의 비만 암환자에서 각각 다른 체중 측정 공식들을 적용한 piperacillin/tazobactam의 용량 적절성 비교 연구)

  • Background: Piperacillin/tazobactam (TZP) is an antibiotic against a broad spectrum of gram-positive, gram-negative, and aerobic and anaerobic strains of bacteria. Due to changes in its pharmacokinetic and pharmacodynamic parameters by TZP-treated patients' renal functions and obesity, it is important to administrate and monitor TZP based on their renal functions and Body Mass Index (BMI) levels. The purpose of this study was to determine the appropriateness of administration doses of TZP based on renal functions of obese cancer patients in a tertiary hospital. Methods: This study was retrospectively conducted with obese cancer patients with $BMI{\geq}30kg/m^2$ in a tertiary hospital, Korea from September 2004 to August 2014. Data were collected through Electronic Medical Record (EMR) which contained laboratory data and TZP dosing of each patient. Results: Among 7,058 patients during the study period, 102 prescriptions were selected based on inclusion and exclusion criteria and classified by their renal functions. Although TZP should be used based on patients' renal functions to adjust its dose, its initial dose and dosing interval were consistently used without considering patients' renal functions on a regular basis. Especially, in the comparison with FDA dosing standard of TZP, approximately twice patients with $20mL/min{\leq}CrCl{\leq}40mL/min$ received domestically 4.5 g instead of 2.25 g as the TZP starting dose. Conclusion: The appropriate doses of TZP were administered to almost all of obese cancer patients; however, the recommended TZP dose was different between Korea and other countries by twice the amount. Further related studies are necessary to clearly determine the results, to optimize TZP treatment for obese patients with cancer in clinical practice, and to design and develop new TZP formulations for them in pharmaceutical industry.

Secular Trends of Species and Antimicrobial Resistance of Blood Isolates in a Tertiary Medical Center for Ten Years: 2003~2012

  • Periodic analysis of local epidemiologic data of prevalent pathogens of blood culture can provide clinicians with relevant information to guide empirical antibiotic therapy. In this study, we analyzed a pattern of change of causative microorganisms and antimicrobial resistance at a tertiary medical center in Chungcheong province from 2003 to 2012, retrospectively. Of 70,258 blood specimens cultured, 6,063 (8.6%) were positive. Among the positive isolates, 95.9% were aerobic or facultative anaerobic bacteria, 0.1% were anaerobes, and 3.9% were fungi. Coagulase-negative Staphylococci (CoNS) (32.9%), Escherichia coli (16.7%), Staphylococcus aureus (9.1%), Klebsiella pneumoniae (6.4%), and ${\alpha}$-hemolytic Streptococcus (5.9%) were commonly isolated bacteria, and Candida albicans (1.4%) was the most commonly isolated fungi. Enterococcus faecium progressively increased but Streptococcus pneumoniae, Acinetobacter baumannii and Proteus species gradually decreased over a period of 10 years. The multidrug-resistant microorganisms such as methicillin-resistant S. aureus (MRSA), vancomycin-resistant enterococci (VRE), cefotaxime-resistant E. coli, imipenem-resistant Pseudomonas aeruginosa (IRPA) and imipenem-resistant A. baumannii (IRAB), were significantly increased. Therefore, there is a need for a more strict control of antibiotics and a more updated guideline for the treatment of bloodstream infection.

A Study on Removal of Organics, Nitrogen and Phoschorus of Domestic Wastewater in Pilot-Scale Upflow Packed Bed Column Reactor (Pilot 규모의 상향류식 충전탑 반응기를 이용한 생활오수의 유기물 및 질소, 인 처리에 관한 연구)

  • This study used biofilm process, which needs simple operation, maintenance and smaller facility area than conventional activated sludge process with the small plant operation, in the treatment of increasing sewage with the rapid industrial growth. The reactor used in this study consists of one anaerobic and one aerobic chamber filled with waste ceramic and waste vinyl as media and the treated sewage was from restaurant source. The experiment was scaled up from lab. to pilot scale and lasted for about 100 days. We focused on the removal efficiency of organics, nitrogen and phosphorus with constant HRT and continuous aeration. The removal efficiency of $BOD_5$ and SS were 94.33% and 87.77% respectively, which was a satisfaction level. However the removal efficiency of $COD_{Cr}$ was 81.46% somewhat below the desired level of 90%, and that of T-N and T-P showed 71.92% and 21.10% respectively, that was below the expected value. The removal efficiency of $COD_{Cr}$ and T-N in the pilot scale was about 10% low compared with the lab.-scale.

A Study on the Treatment of Parathion Pesticide Using Marsh and Pond Type Constructed Wetlands (Marsh와 Pond 형태의 인공 습지를 이용한 Parathion 농약의 처리에 관한 연구)

  • The microcosm type wetland systems were constructed in order to treat wastewater contaminated with parathion. The microcosm reactor consisted of marsh and pond type. The experiment was carried out using batch (marsh or pond) and continuous (marsh-pond and pond-marsh type) systems. In the batch reactor, marsh-type wetland completely removed parathion in water within 8 days, while pond reactor removed 97% of parathion during the same period. During parathion degradation, the amount of 4-nitrophenol production, one of the metabolites from parathion degradation, was higher in marsh-type batch reactor. In the continuous systems, both marsh-pond and pond-marsh combination systems effectively removed parathion from water, and the production of 4-nitrophenol was also minimal. In the extraction experiment, the parathion and its metabolite were not found in the wetland soil and the plant. In order to achieve both aerobic and anaerobic conditions, the continuous wetland system combining marsh and pond type can be the alternative for the non-point source pollutants such as parathion pesticide.

Optimization of Pretreatment of Persimmon Peel for Ethanol Production by Yeast Fermentation (효모를 이용한 에탄올 생산을 위한 감껍질 전처리조건의 최적화)

  • A response surface method based on a central composite design experiment was used to determine the optimum conditions for pretreatment of persimmon peel. It was mathematically predicted that the maximum amount of reducing sugars would be obtained at an $H_2SO_4$ concentration of 1.77% (w/v) and a heat treatment time of 26.4 min. A reducing sugar concentration of 63.23 g/l was obtained under the optimum pretreatment conditions determined by RSM. Under anaerobic growth conditions, Saccharomyces cerevisiae NK28 produced 15.52 g/l of ethanol with a yield of 0.34 g ethanol/g glucose from pretreated persimmon peel, which corresponded to 14% and 26% enhancements in ethanol productivity and ethanol yield, respectively, compared with those obtained in aerobic growth conditions. This study suggests that persimmon peel might be a useful substrate for bioethanol production by yeast fermentation.

Thermal-Induced Osteonecrosis of Adjacent Vertebra after Intradiscal Electrothermal Therapy

  • A 42-year-old man was admitted to our hospital with complaints of low back pain and intermittent right thigh pain. Twelve weeks before admission, the patient received intradiscal electrothermal therapy (IDET) at a local hospital. The patient still reported low back pain after the procedure that was managed with narcotic analgesics. Follow-up magnetic resonance imaging (MRI) was performed, and his referring physician thought the likely diagnosis was spondylodiscitis at the L4-5 spinal segment with a small epidural abscess. At admission to our department, the patient reported aggravated low back pain. Blood test results, including the erythrocyte sedimentation rate and C-reactive protein levels, were slightly elevated. Biopsy samples of the L4, L5 vertebral bodies and disk were obtained. The material underwent aerobic, anaerobic, fungal, mycobacterial cultures and histologic examination. Results of all cultures were negative. Histologically, necrosis of the bone was evident from the number of empty osteocyte lacunae. In addition, there was no evidence of infection based on biopsy results. No antibiotic treatment was administered on discharge. Repeat computed tomography and MRI performed 12 months after IDET showed a bony defect in the L4 and L5 vertebral bodies, and a decrease in the size of the L4-5 intervertebral disc lesion. We report a case of lumbar vertebral osteonecrosis induced by IDET and discuss etiology and radiologic features.

The Analysis of the Effects of Physical Activity on Impaired Fasting Glucose in Adults Over 20 Years of Age

  • Purpose : The purpose of this study was to investigate the effects of physical activity on impaired fasting glucose in adults aged 20 years or older. Methods : This study utilized raw data from the 8th National Health and Nutrition Examination survey (2019~2021). The subjects of this study were 5,344 adults aged 20 years or older who were confirmed to be free of diabetes. The control variables in this study model are health behavior characteristics (subjective health status, smoking, drinking), anthropometric characteristics (body mass index), and personal background characteristics (gender, age, income level, education level, marital status). As for the analysis method, the degree of physical activity was made into a dummy variable, and a probit model was used. Results : As a result of this study, compared to quartile 1 of the relative grip strength value obtained by dividing the grip strength by the body mass index (body mass index, kg, m2), fasting blood glucose levels were significantly higher in quartile 2 (.05, p<.01), quartile 3 (.04, p<.01), and quartile 4 (.04, p<.01). It was found that the probability of belonging to the normal category was higher than that of impaired fasting glucose. In addition, in the group of adults aged 20 or older who had a lot of aerobic and anaerobic physical activity, fasting blood sugar was more likely to be in the normal category. Conclusion : Based on the results of this study, it was suggested that diabetes should be managed through physical activity in the pre-diabetic stage, as prevention is important as well as treatment. From a practical point of view, muscle strength, such as grip strength, can be identified as a reliable indicator for identifying impaired fasting glucose.
