건강보험 청구자료를 이용한 진료 연속성이 당뇨 관련 예방 가능 입원에 미치는 영향 분석: 중·고령군을 중심으로 (Effects of Continuity of Care on Diabetes-Related Avoidable Hospitalizations among Middle- and Old-Aged Patients: Analysis of National Health Insurance Claims Data)

  • Background: Diabetes is known as one of the most important ambulatory care sensitive conditions. This study purposed to assess the status of continuity of care (COC) and diabetes-related avoidable hospitalizations (DRAHs) of a group of middle- and old-aged patients and to observe the relationship of the two elements by the two age groups. Methods: This study utilized the National Health Insurance Service's National Sample Cohort data and the subjects are diabetes patients of 45 and over, classified into two groups of 'middle-aged'(45-64 years) and 'old-aged'(${\geq}65years$) patients. The dependent variable was DRAHs, which was defined in accordance with the definition of the Organization for Economic Cooperation and Development "Health Care Quality Indicators" project. COC, as an independent variable, is measured by the COC index in this study. Two-part model (multi-variate and multi-level analyses) was utilized. Results: Factors associated with the status and the number of DRAHs differed by each age group. Meanwhile, the two-part model showed that higher COC was associated with a lower risk of preventable hospitalizations in both middle- and old-aged groups. Conclusion: Study findings can provide health policy insights and implications in order to strengthen the primary care system for further improvement of diabetes management, especially for middle- and old-aged groups.

Simultaneous monitoring of motion ECG of two subjects using Bluetooth Piconet and baseline drift

  • Uninterrupted monitoring of multiple subjects is required for mass causality events, in hospital environment or for sports by medical technicians or physicians. Movement of subjects under monitoring requires such system to be wireless, sometimes demands multiple transmitters and a receiver as a base station and monitored parameter must not be corrupted by any noise before further diagnosis. A Bluetooth Piconet network is visualized, where each subject carries a Bluetooth transmitter module that acquires vital sign continuously and relays to Bluetooth enabled device where, further signal processing is done. In this paper, a wireless network is realized to capture ECG of two subjects performing different activities like cycling, jogging, staircase climbing at 100 Hz frequency using prototyped Bluetooth module. The paper demonstrates removal of baseline drift using Fast Fourier Transform and Inverse Fast Fourier Transform and removal of high frequency noise using moving average and S-Golay algorithm. Experimental results highlight the efficacy of the proposed work to monitor any vital sign parameters of multiple subjects simultaneously. The importance of removing baseline drift before high frequency noise removal is shown using experimental results. It is possible to use Bluetooth Piconet frame work to capture ECG simultaneously for more than two subjects. For the applications where there will be larger body movement, baseline drift removal is a major concern and hence along with wireless transmission issues, baseline drift removal before high frequency noise removal is necessary for further feature extraction.

Environmental Factors Influencing the Prevalence of Falls in South Korea

  • Objective: This study is a systematic review conducted to analyze the environmental factors that cause falls in the older adult. Design: Systematic review Methods: The study was conducted by searching the Research Information Sharing Service (RISS), Korean studies Information Service System (KISS), and Data Base Periodical Information Academic (DBpia) databases for literature published in South Korea up to July 2020. A total of 12 studies were selected for analysis based on the inclusion and exclusion criteria. Results: The results of the analysis revealed that all the selected literature were surveys and the study subjects were 65 years of age and above. The sample size ranged from a minimum of 95 subjects to a maximum of 3,278. A total of eight tools were used to measure the environmental factors associated with falls. The prevalence and recurrence of falls increase with age and deterioration of health. Older adult individuals who experience falls encounter difficulties in recovering from impaired physical function and disability; moreover, in severe cases, falls may lead to death. Falls are largely associated with a combination of intrinsic and extrinsic (i.e., environmental) factors. The purpose of this study was to assess potential extrinsic risk factors for falls. Falls occur in indoor environments, such as washrooms, bathrooms and living rooms, and outdoor environments, including roads and stairways, depending on the season, time of day, and use of ambulatory aids. In such environments, falls are mainly caused by slipping and stuttering. Conclusions: Therefore, as the rate of fall is influenced by several factors, extrinsic factors should be improved by developing comprehensive accident prevention programs that address the improvement of environmental risk factors around places of residence to reduce risk factors among the older adult, who, especially, are at a high risk for falls.

ICF 모델 기반 신경계 환자 물리치료 평가 도구 사용 조사 (Evaluation Tools for Patients with Neurologic Disorders Based on the ICF Model: A Survey of Korean Physical Therapists)

  • Purpose: Physical therapists are required to properly choose the most appropriate treatment for each patient within the framework of the International Classification of Functioning, Disability, and Health (ICF model). The aims of this study were to determine whether neurological physical therapists in clinical settings in South Korea know about the ICF model and to investigate the current trends of outcome measures (OMs) used by them. Methods: Two hundred and one physical therapists who worked with patients with neurological disorders participated in this study. The survey was conducted via e-mail and asked about commonly used OMs and the considerations for selecting OMs. Results: All physical therapists involved in this study responded completely, and 45.8% of participants learned about the ICF model, while 37.3% understood the detailed information related to the ICF model. The rest of the participants did not know or just heard about the ICF model. The most frequently used tools at the body function/structure level were the Range of Motion (98%), Manual Muscle Test (97%), Berg Balance Scale (83.1%), and Modified Ashworth Scale (70.6%) when allowing repetition. At the activity level, the 10-meter walk test (71.1%), 6-minute walk test (54.2%), and Functional Ambulatory Category (43.3%) were used, while the Activity-Specific Balance Confidence Scale (23.9%) was used at the participation level. There was a positive relationship between the number of tools used and years of work, as well as the level of understanding of the ICF model. Conclusion: The results of this study suggest that it is necessary to learn the ICF model in a clinical setting. In addition, the medical system needs to be modified to encourage physical therapists in South Korea to use proper OMs within the ICF model.

여성의 건강을 위한 간호전달체계 모형개발 - 조산원 중심으로 - (Model Development of Nursing Care System for Women's Health : Based on Nurse-Midwifery Clinic)

  • The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.

의료정보 표준에 기반한 EHR 플랫폼의 설계 및 개발 (Design and Development of an EHR Platform Based on Medical Informatics Standards)

  • 최근 미국의 경기부양 법안이 통과됨에 따라 전 세계적으로 의료 산업분야에서의 EHR 시스템에 대한 관심이 증대되고 있다. 이 법안은 다양한 의료표준을 통해 상호 운용성을 보장하는 EHR 시스템을 도입하는 의원이나 병원에 인센티브를 제공하는 프로그램을 제시하고 있다. 이러한 인센티브 프로그램으로 인해 많은 EHR 시스템이 개발되었고, 또한 많은 의원이나 병원이 CCHIT에서 인증한 EHR 시스템을 도입하고 있다. 미국 의료 산업의 변화에 발맞추어 국내의 일부 기업에서도 국내병원에 EMR 시스템을 구축한 경험을 바탕으로 미국 의료 시장에 진출하려고 노력하고 있다. 그러나 미국과 한국의 의료 환경이 상이하여 개발한 시스템의 개선이 불가피하다. 따라서 본 논문에서는 의료표준화 기술에 기반하여 서로 다른 의료정보 시스템 간의 상호 운용성을 보장하는 통합형 EHR 플랫폼을 설계하고 개발한다. 개발한 플랫폼에서는 전송 표준 및 방법, 용어 표준 및 활용, 의사 결정을 위한 지식 관리 등의 다양한 기반 기술을 통합하여 완성된 하나의 시스템을 구현하였고, 의료정보의 표준화를 위한 표준용어체계와 의료정보의 교환을 위한 HL7 인터페이스 엔진을 탑재하여 의료 데이터의 전자적 처리가 가능하도록 하였다. 개발한 플랫폼에 기반하여 미국 개원의의 외래 진료를 지원하기 위한 EHR 시스템인 SeniCare를 개발하고, CMS에서 제시한 "의미 있는 사용"의 조건의 부합 여부를 확인함으로써 개발한 플랫폼의 효용성을 검증한다.

경증 전신성 홍반성 루프스 환자의 인지기능장애 (Cognitive Impairment in the Patients with Mildly Active Systemic Lupus Erythematosus)

  • SLE 환자들에서 인지기능의 장애가 있는지를 알아보기 위해서 신경정신과적 병력이 없는 내과 외래 SLE 환자 20명과 정상 대조군 20명을 대상으로 전산화 신경인지기능 검사인 Vienna test system을 시행하고 이를 인지기능에 영향을 미칠 수 있는 임상 변인들과의 연관성을 분석하여 다음과 같은 결과를 얻었다. 1) SLE 환자군과 정상 대조군의 신경인지기능 검사의 각 항목 비교 인식력 검사항목에서 SLE 환자군은 정상 대조군에 비해 정확하게 응답한 반응수가 적었으며 '예'와 '아니오' 중 '아니오'를 정확하게 반응한 수도 적었다. 또한 '예'와 '아니오'에 대해 각각 정확한 반응을 하는 평균 반응시간이 길었으며 검사소요 시간도 길었다. 주의력 검사항목에서는 SLE 환자군이 정상 대조군에 비해 정확하게 응답한 반응수가 적었고, 평균 반응시간이 길었다. 그러나, corsi단기 기억력 검사항목에서는 시각적 단기 기억력 범위와 정확하게 맞춘 총 응답수에서 두군 간에 유의한 차이가 없었다. 표준도형 지능검사항목에서 SLE 환자군은 정상 대조군에 비해 정확하게 응답한 반응수가 적었다. 신경행동학적 인지상태 검사중 기억력 항목에서 SLE 환자군은 정상 대조군에 비해 유의하게 낮은 점수를 보였다. 2) 신경인지기능의 각 항목과 환자군의 연령, 교육연한, SLE 질환 활성도(SLE Disease Activity Index), 우울 증상의 정도, 항 ds-DNA항체. 보체 C3/C4, 스테로이드의 용량 등의 임상 변인들과의 상관관계를 비교하였을 때 통계적으로 유의하지 않았다. 이상의 결과에서 질환 활성도가 낮은 SLE환자들은 과거 신경정신과적 증상의 병력이 없고 병의 이환 기간이 비교적 짧음에도 불구하고 인지기능의 장애를 나타내었다. 그리고 이는 기타 장기의 침범으로 인한 비특이적 영향이 아닌 중추신경계의 침범에 의한 것으로 보이며 우울정도나 스테로이드 용량에 영향을 받지 않는 것으로 생각된다.

  • Background: From November 2006, The national health insurance system in the Republic of Korea began to cover prescribed long-term oxygen therapy (LTOT) in patients with chronic respiratory insufficiency. This study examined the current status of LTOT after national health insurance coverage. Methods: Between November 1, 2006 and June 30, 2008, the medical records of patients who were prescribed LTOT by chest physicians were reviewed. The data was collected from 13 university hospitals. Results: 197 patients (131 male and 66 female) were prescribed LTOT. The mean age was 64.3${\pm}$13.0 years. The most common underlying disease was chronic obstructive pulmonary disease (n=103, 52.3%). Chest physicians prescribed LTOT using arterial blood gas analysis or a pulse oxymeter (74.6%), symptoms (14%), or a pulmonary function test (11.2%). The mean oxygen flow rate was 1.56${\pm}$0.68 L/min at rest, 2.08${\pm}$0.91 L/min during exercise or 1.51${\pm}$0.75 L/min during sleep. Most patients (98.3%) used oxygen concentrators. Only 19% of patients used ambulatory oxygen supplies. The oxygen saturation before and after LTOT was 83.18${\pm}$10.48% and 91.64${\pm}$7.1%, respectively. After LTOT, dyspnea improved in 81.2% of patients. The mean duration of LTOT was 16.85${\pm}$6.71 hours/day. The rental cost for the oxygen concentrator and related electricity charges were 48,414${\pm}$15,618 won/month and 40,352${\pm}$36,815 won/month, respectively. Approximately 75% of patients had a regular visit by the company. 5.8% of patients had personal pulse oxymetry. 54.9% of patients had their oxygen saturation checked on each visit hospital. 8% of patients were current smokers. The most common complaint with LTOT was the limitation of daily activity (53%). The most common complaint with oxygen concentrators was noise (41%). Conclusion: The patients showed good compliance with LTOT. However, only a few patients used an ambulatory oxygen device or had their oxygen saturation measured.

수족냉증 환자의 실태 및 HRV(Heart Rate Variability)를 이용한 자율신경계 기능 평가 연구 (A clinical study on patients with Cold Hypersensitivity on hands and feet and their autonomic nervous system function using of Heart Rate Variability(HRV))

  • Objectives : This study was designed to analyze basic data for cold hypersensitivity patients with a questionnaire and investigate correlation between cold hypersensitivity and Heart Rate Variability(HRV). Methods : 49 patients who complain of cold hypersensitivity on hands and feet in ambulatory care were investigated in Oriental Gynecology, Kyunghee Oriental Medical Center and Women medical center, Kangnam Koreana hospital from May 1, 2006 to October 20, 2006. All patients were asked to answer a questionnaire. After careful I examination to rule out other disease which may affect Digital Infrared Thermal Imaging(DITI) and HRV data, patients were taken thermography for the diagnosis of cold hypersensitivity Based on the result of thermography, the patients were divided into two groups(Objective cold hypersensitivity and Subjective cold hypersensitivity). Then, these two groups were compared using HRV data which was measured in the supine position for 5 minutes. Results: 1. For most patients, cold hypersensitivity first developed during puberty. 2. The parts of the body that felt cold first time were hands and feet. 3. The cold sensation was increased in winter or at bedtime Showing that the sensation can be changed according to the level of coldness. 4. Among women who sufferfrom cold hypersensitivity, very few of them were treated. 5. More than half of cold hypersensitivity patients's family member also had a cold hypersensitivity. 6. The decrease in mean values of LF/HF ratio was observed in objective cold hypersensitivity group than subjective cold hypersensitivity group and the decrease was significant(P=0.014) when examined by Student t-test. Conclusions : It is necessary that the cold hypersensitivity patients should be treated carefully considering the facts mentioned above. And it can be suggested that dysautinomia be related with cold hypersensitivity and be evaluated by HRV.

완고한 보행장애를 주소로 하는 만성기 뇌출혈 환자의 한의 치료 및 재활 운동 증례보고 1례 : GAITRite®를 이용하여 (A Case Report of a Chronic Intracerebral Hemorrhage Patient with Obstinate Gait Disturbance Treated with Korean Medicine and Rehabilitation Exercise)

  • Objective: The aim of this study was to report the effectiveness of traditional Korean medicine treatment with rehabilitation exercise on a chronic intracerebral hemorrhage patient with a severe gait disturbance. Method: The patient was treated with Korean herbal medicine (Gami-sibjeondaebo-tang) along with acupuncture, moxibustion, and rehabilitation exercise. The treatment effect was evaluated through the Manual Muscle Test (MMT) and the Korean version of the Modified Barthel Index (K-MBI). The gait of the patient was evaluated by Functional Ambulatory Category (FAC) and the Timed Up and Go (TUG) test. The spatiotemporal parameters were evaluated by a walkway system (GAITRite(R) system) and gait symmetry was evaluated by Symmetry Criterion (SC). Results: After 111 days of traditional Korean medicine treatment, the spatiotemporal parameters and symmetry of the patient's gait improved. Conclusion: This study suggested that traditional Korean medicine treatment with rehabilitation exercise could be effective for gait disturbance in patients with chronic intracerebral hemorrhage.
