Immunogloboulin E-Mediated Food Sensitization in Children with Celiac Disease: A Single-Center Experience

  • Purpose: Celiac disease (CD) is an autoimmune disorder of the small intestine caused by an abnormal immune response to gluten proteins and is often characterized by gastrointestinal symptoms. Food allergy (FA) is an adverse immune sensitivity to ingested food proteins leading to inflammation in various organs including the gastrointestinal tract. The relationship between CD and FA remains unclear. This study aimed to assess the prevalence and clinical relevance of immunoglobulin E (IgE)-mediated food sensitization in children with CD. Methods: Fifty-nine children diagnosed with CD were reviewed for clinical symptoms and evidence of IgE-sensitization to food and airborne allergens using the PolyCheck method. Results: IgE-mediated sensitization has been diagnosed in 20.3% of children with CD (CD/A). In the CD/A group, 58.3% of children were sensitized to food and 66.7% to airborne allergens. Further, 41.7% of patients with CD and allergy reported gastrointestinal tract symptoms associated with the ingestion of sensitizing foods. Analysis of the clinical status revealed that the incidence of other allergic disorders in the CD/A group was as follows: atopic dermatitis (33.3%), asthma (25.0%), and allergic rhinitis (16.7%). The percentage of eosinophils was significantly higher in the CD/A group than in the CD group (0.33±0.25 vs. 0.11±0.09; p=0.006). Conclusion: The diagnosis of CD does not exclude FA. The gastrointestinal symptoms in children with CD may be the result of both CD and FA; therefore, children with CD should be evaluated for the presence of FA regardless of age.

Association between Subjective Distress Symptoms and Argon Welding among Shipyard Workers in Gyeongnam Province (경남소재 일개조선소 근로자의 건강이상소견과 아르곤 용접과의 관련성)

  • Objective: This study was conducted to investigate the association between subjective distress symptoms and argon welding among workers in Gyeongnam Province shipyard. Method: 31 argon and 29 non-argon welding workers were selected as study subjects in order to measure concentrations of personal dust, welding fumes and other hazardous materials such as ZnO, Pb, Cr, FeO, MnO, Cu, Ni, $TiO_2$, MgO, NO, $NO_2$, $O_3$, $O_2$, $CO_2$, CO and Ar. An interviewer-administered questionnaire survey was also performed on the same subjects. The items queried were as follows: age, height, weight, working duration, welding time, welding rod amounts used, drinking, smoking, and rate of subjective distress symptoms including headache and other symptoms such as fever, vomiting and nausea, metal fume fever, dizziness, tingling sensations, difficulty in breathing, memory loss, sleep disorders, emotional disturbance, hearing loss, hand tremors, visual impairment, neural abnormality, allergic reaction, runny nose and stuffiness, rhinitis, and suffocation. Statistical analysis was performed using SPSS software, version 18. Data are expressed as the mean ${\pm}SD$. An ${\chi}^2$-test and a normality test using a Shapiro wilk test were performed for the above variables. Logistic regression analysis was also conducted to identify the factors that affect the total score for subjective distress symptoms. Result: An association was shown between welding type (argon or non-argon welding) and the total score for subjective distress symptoms. Among the rate of complaining of subjective distress symptoms, vomiting and nausea, difficulty breathing, and allergic reactions were all significantly higher in the argon welding group. Only the concentration of dust and welding fumes was shown to be distributed normally after natural log transformation. According to logistic regression analysis, the correlations of working duration and welding type (argon or non-argon) between the total score of subjective distress symptoms were found to be statistically significant (p=0.041, p=0.049, respectively). Conclusion: Our results suggest that argon welding could cause subjective distress symptoms in shipyard workers.

Developing Pattern Identification Questionnaire of Allergic Rhinitis (알레르기 비염의 변증을 위한 설문문항 개발)

  • Objectives : The prevalence of allergic rhinitis (AR) is 10-40% worldwide and there are many demands for Traditionl Medicine (TM) treatment for AR. Pattern identification (PI) is essential process in diagnosis and treatment of TM, however, objective assessment measurement for PI of AR in TM is lacked. Methods : PI questionnaire for AR was developed in 2008 by specialists in the department of Otolaryngology of TM, based on systemic and nasal symptoms and signs. However, that questionnaire had many problems in application to clinical practice and has since been revised based on several studies and delphi method. Consequently, the PI questionnaire for AR version 3.0 has been developed. Results & Conclusions : This questionnaire classifies patients with AR as possessing lung-heat, lung-cold, or spleen qi deficiency based on nasal symptoms and general conditions of AR patients. This is first questionnaire for pattern identification of AR. We plan to conduct a validation and reliability study, and revise the questionnaire based on the results of this study.

Intermittent Fasting Modulates Immune Response by Generating Tregs via TGF-β Dependent Mechanisms in Obese Mice with Allergic Contact Dermatitis

  • People with obesity maintain low levels of inflammation; therefore, their exposure to foreign antigens can trigger an excessive immune response. In people with obesity or allergic contact dermatitis (ACD), symptoms are exacerbated by a reduction in the number of regulatory T cells (Tregs) and IL-10/TGF-β-modified macrophages (M2 macrophages) at the inflammatory site. Benefits of intermittent fasting (IF) have been demonstrated for many diseases; however, the immune responses regulated by macrophages and CD4+T cells in obese ACD animal models are poorly understood. Therefore, we investigated whether IF suppresses inflammatory responses and upregulates the generation of Tregs and M2 macrophages in experimental ACD animal models of obese mice. The IF regimen relieved various ACD symptoms in inflamed and adipose tissues. We showed that the IF regimen upregulates Treg generation in a TGF-β-dependent manner and induces CD4+T cell hypo-responsiveness. IF-M2 macrophages, which strongly express TGF-β and inhibit CD4+T cell proliferation, directly regulated Treg differentiation from CD4+T cells. These results indicate that the IF regimen enhances the TGF-β-producing ability of M2 macrophages and that the development of Tregs keeps mice healthy against ACD exacerbated by obesity. Therefore, the IF regimen may ameliorate inflammatory immune disorders caused by obesity.

Anti-inflammatory and Anti-allergic Effects of Herbal Extracts on Atopic Dermatitis(Part I) (약용식물추출물의 아토피성피부염에 대한 항염증 및 항알레르기효과(제 1보))

  • Atopic dermatitis is a chronic inflammatory skin disease associated with cutaneous hyper-reactivity to environmental triggers. In order to develop effective therapeutic herbal extracts for atopic dermatitis, cytotoxicity, antioxidant, anti-inflammatory and anti-allergic activities were investigated for various herbal extracts. Among candidate extracts, we selected Aloe vera L. (AV), Viola mandshurica W. Becker (VM), Punica granatum L. (PG), Dendrobium nobile L. (DN) and mixture of the above extracts (MX) for further investigations. All of them did not show cytotoxic activities to macrophage RAW264.7 cells below the concentration of 100 ppm. All showed antioxidant, anti-inflammatory and anti-allergic effects, although to various extents. In antioxidant effects, AV showed the highest effect, followed by PG and VM, while DN did the lowest. In evaluation for anti-inflammatory activities in macrophage RAW264.7 cells, AV and DN inhibited almost completely the LPS-induced production of nitric oxide, while AV, DN and VM showed strong inhibitory activities on the LPS-induced production of TNF-${\alpha}$. In anti-allergy effect in mast cell HMC-1, DN showed the highest effect, followed by AV and PG, while VM did the lowest. In the topical allergy reaction induced by compound 48/80 in Sprague-Dawley rat, DN exhibited significant anti-allergic effect, while PG, VM and AV did slight effect. These results suggest that AV, VM, PG and DN have antioxidant, anti-inflammatory and anti-allergic activities, and thus have the potential to reduce and alleviate the symptoms of atopic dermatitis.

A Determination of the Salicylate Content of Ingredients Commonly Used in Korean foods Suggested for Allergic Patients

  • Salicylate from plant foods and histamine from animal foods have been suggested to cause various food allergic symptoms. Salicylate regarded as one of the inducing agents f3r chronic urticaria and asthma in allergic patients was studied. There is much interest about the effects of salicylate in food allergy. It it recently suggested that the intake of salicylate from foods may have contributed to chronic urticaria and asthma. The purpose of this study is to analyze the salicylate content in various foods. 153 ingredients commonly used in Korean foods were analyzed by HPLC. Among the vegetables, chard(8.22$\mu\textrm{g}$/g), soybean sprout(5.13$\mu\textrm{g}$/g), perilla lean3.03$\mu\textrm{g}$/g), Chinese cabbage(2.25$\mu\textrm{g}$/g), carrot(2.16$\mu\textrm{g}$/g), and squash(1.33$\mu\textrm{g}$/g) con-rained higher amounts of salicylate as compared to other vegetables. Among fruits, oranges(5.13$\mu\textrm{g}$/g), grapefruits(2.90$\mu\textrm{g}$/g), and plums(1.65$\mu\textrm{g}$/g) contained the most. In animal foods, almost no salicylate was found except in chocolate milk(0.32$\mu\textrm{g}$/g). It was also found in black pepper(8.2$\mu\textrm{g}$/g), coffee(can)(5.00$\mu\textrm{g}$/g), mustard(4.16$\mu\textrm{g}$/g), curry powder(3.24$\mu\textrm{g}$/g), kochujang(2.37$\mu\textrm{g}$/g), cream powder(1.19$\mu\textrm{g}$/g), brown rice(1.04$\mu\textrm{g}$/g) and sesame oil(1.00$\mu\textrm{g}$/g). All information found in this study can be used as nutritional education materials in potentially allergic people for a prudent diet plan and can also provide proper dietary guidelines for these allergic patients. External Application of Fermented Olive Flounder (Paralichthys olivaceus) Oil Alleviates Inflammatory Responses in 2,4-Dinitrochlorobenzene-induced Atopic Dermatitis Mouse Model

  • Allergic skin inflammation such as atopic dermatitis (AD) is characterized by edema and infiltration with various inflammatory cells such as mast cells, basophils, eosinophils and T cells. Thymic stromal lymphopoietin (TSLP) is produced mainly by epidermal keratinocytes, as well as dermal fibroblasts and mast cells in the skin lesions of AD. Omega-3 polyunsaturated fatty acids in fish oil can reduce inflammation in allergic patients. Fermentation has a tremendous capacity to transform chemical structures. The antiinflammatory effects of fish oil have been described in many diseases, but the beneficial effects by which fermented olive flounder oil (FOF) modulates the allergic response is poorly understood. In this study, we produced FOF and tested its ability to suppress the various allergic inflammatory responses. The ability of FOF to modulate the immune system was investigated using a mouse model of AD. The FOF-treated group showed significantly decreased immunoglobulin E (IgE) and histamine in serum. Also, the increased TSLP expression was significantly inhibited in the FOF group; the FOF-treated group was not appreciably different from the hydrocort cream treatment group. In addition, FOF treatment resulted in a smaller spleen size with reduced the thickness and length compared to the induction group. Splenocytes from mice treated with FOF produced significantly less IFN-${\gamma}$, IL-4, T-box transcription factor (T-bet) and GATA binding protein 3 (GATA3) expression compared with the induction group. These results suggest that FOF may be effective in treating the allergic symptoms of AD. 5.

AT9283, 1-Cyclopropyl-3-(3-(5-(Morpholinomethyl)-1H-Benzo[d] Imidazole-2-yl)-1H-Pyrazol-4-yl) Urea, Inhibits Syk to Suppress Mast Cell-Mediated Allergic Response

  • Mast cells are an effector cell that plays a pivotal role in type I hypersensitive immune responses. Mast cells exist in connective tissues, such as skin and mucosal tissue, and contain granules which contain bioactive substances such as histamine and heparin in cells. The granules of mast cells are secreted by antigen stimulation to cause the type I allergic hypersensitivity. In addition, stimulated by antigen, mast cells synthesize and secrete various eicosanoids and cytokines. While AT9283 is known to have anticancer effects, the therapeutic effect of AT9283 on allergic disorders is completely unknown. In this study, it was found that AT9283 reversibly inhibited antigen-IgE binding-induced degranulation in mast cells (IC50, approx. 0.58 μM) and suppressed the secretion of the inflammatory cytokines IL-4 (IC50, approx. 0.09 μM) and TNF-α (IC50, approx. 0.19 μM). For a mechanism of mast cell inhibition, while not inhibiting Syk phosphorylation, AT9283 suppressed the activation of LAT, a downstream substrate protein of Syk, in a dose-dependent manner. As expected, AT9283 also inhibited the activation of PLCγ1 and Akt, downstream signaling molecules of Syk/LAT, and MAP kinases such as JNK, Erk1/2, and P38. In an in vitro protein tyrosine kinase assay, AT9283 directly inhibited Syk activity. Next, AT9283 dose-dependently inhibited passive cutaneous anaphylaxis (PCA), an IgE-mediated allergic acute response, in mice (ED50, approx. 34 mg/kg, p.o.). These findings suggest that AT9283 has potential to use as a new drug for alleviating the symptoms of IgE-mediated allergic disorders.

Efficacy and Safety of Codonopsis lanceolata (S. et Z.) Trautv. Extract on the Improvement of the Hypersensitivity Reaction in Allergic Rhinitis Patients (알레르기 비염 질환자를 대상으로 한 더덕(Codonopsis lanceolata (S. et Z.) Trautv.) 추출물의 과민면역반응 개선에 대한 유효성 및 안전성 평가)

  • Objectives : This clinical trial is performed to investigate the effect of improving the hypersensitivity reaction and the safety of powdered ethanol extract Codonopsis lanceolata (S. et Z.) Trautv. for allergic patients. Methods : This study was randomized, double blinded, placebo-controlled clinical trial. 60 allergic rhinitis subjects were randomly assigned to Treatment group(n=30) and Control group(n=30). They were received the test substance or placebo in their $1^{st}$ and $2^{nd}$ visit, and took it once a day, 1 powder bag per 1 time, for 8 weeks. The freeze dried powder of Codonopsis lanceolata (S. et Z.) Trautv. was mixed with 30% ethanol and the filtrate was collected thrice with constant stirring of the mixture. The placebo was prepared in the same form as the test substance by replacing with corn starch. Total Nasal Symptom Score(TNSS) questionnaires and laboratory tests were performed at the screening and last visit to evaluate the efficacy. And to assess safety, vital signs and adverse events were confirmed at every visit. Statistical analysis about the result was performed using SAS 9.4. Results : In treatment group, there was a significant decrease in B cell after taking the test substance(p<0.005). And there was a statistically significant difference between groups(p<0.005). Changes in nasal symptoms were not statistically significant between groups, and both groups showed statistically significant differences before and after the test(p<0.0001). Also there were no adverse events associated with the clinical trial product. Conclusions : This clinical trial showed that Codonopsis lanceolata (S. et Z.) Trautv. extract was helpful in improving the hypersensitivity reaction in allergic patients without significant side effects.

Experimental sutdy on the anti-allergic effects of Onpyetang and Onpyetang-Gechongbaek (溫肺湯과 溫肺湯去총白의 抗알레르기效果 및 鎭痛, 解熱作用에 대한 實驗的 硏究)

  • Major symptoms of al1ergic rhinitis are nasal obstruction, sneezing and watery rhinorrhea. Onpyetang has been used to treat for nasal obstruction, which is one of the symptoms of allergic rhinitis. This Experimental study was done to research effects of Onpyetang and transformed Onpyetang(Allii Radix is deducted from Onpyetang) on the anti-allergic effects. We have studied the vascular permeability response induced by serotonin and histamine, the contact dermatitis response induced by picryl chloride, the delayed type hypersensitivity response to SRC, the mice paw edema induced by carrageenin, the writhing syndrome induced by $0.7\%$ acetic acid, and the rectal temperature in febrile rats induced by yeast. The results were as follows : 1. In the vascular permeability response to intradermal serotonin and histamine. Onpyetang proved significant inhibitory effect(P<0.05, p<0.001) But transformed Onpyetang proved significant inhibitory effect only to histamine(P<0.05) 2. In the contact dermatitis response induced by picryl chloride, Onpyetang and transformed Onpyetang proved significant inhibitory effect.(P<0.05, P<0.05) 3. In the delayed type hypersensitivity response induced by SRC, Onpyetang and transformed Onpyetang proved significant inhibitory effect on mice paw edema.. (P<0.05, P<0.05) 4. In the delayed type hypersensitivity response induced by SRC, Onpyetang proved significant inhibitory effect to serum IgE.(P<0.01) But transformed Onpyetang proved insignificant inhibitory effect on serum Ig E. 5. In the mice paw edema induced by carrageenin, Onpyetang and transformed Onpyetang proved significant anti-inflammatory effect. (P<0.01, P<0.01) 6. In the writhing syndrome induced $0.7\%$ acetic acid, Onpyetang and transformed Onpyetang proved significant analgestic effect. (P<0.01, P<0.01) 7. In the rectal temperature in febrile rats induced by yeast, Onpyetang and transformed Onpyetang proved significant anti-pyretic effect.(P<0.001, P<0.01) according to this result Onpyetang was conclude to be effective on anti-allergic, anti-pyretic, anti-inflammatory and analgestic action. but transformed Onpyetang(Allii Radix is deducted from Onpyetang) was not effective on the vascular permeability response to intradermal serotonin and increasing Ig E of delayed type hypersensitivity response induced by SRC. In addition, transformed Onpyetang is not effective as Onpyetang. More study should be done about the role of Allii Radix.
