  • 전북의대 부속병원 통증치료실에 의뢰된 4명의 삼차신경통 환자를 95% alcohol을 이용하여 해당분자를 화학적으로 차단하고 필요에 따라 carbamazepine이나 amitriptyline을 병용하거나 별도로 사용하여 좋은 효과를 얻었으므로 삼차신경통의 치료방법을 포괄적으로 고찰하였다.

  • Radiofrequency neurolysis (RFN) of the genicular nerves has recently become accepted as an effective technique to alleviate knee pain particularly in patients with knee osteoarthritis (OA) or postoperative pain. However, genicular nerve RFN can produce high procedure and equipment costs, longer procedural times, procedure-related pain, and failure rate of over 25%. We are presenting two cases of alcohol neurolysis of the genicular nerve using fluoroscopy and ultrasonography in patients with knee OA or persistent postsurgical pain of the knee. Alcohol neurolysis of the genicular nerve with dual imaging modality can be a cheap, safe and effective method in patients with chronic knee pain.

  • Obturator nerve block has been commonly used for pain management to prevent involuntary reflex of the adductor thigh muscles. One of several options for this block is chemical neurolysis. Neurolysis is done with chemical agents. Chemical agents used in the neurolysis of the obturator nerve have been alcohol, phenol, and botulinum toxin. In the current case, a patient with spasticity of the adductor thigh muscle due to cervical cord injury had obturator nerve neurolysis done with botulinum toxin type B (BoNT-B). Most of the previous studies have used BoNT-A with only a few reports that have used BoNT-B. BoNT-B has several advantages and disadvantages over BoNT-A. Thus, we report herein a patient who successfully received obturator nerve neurolysis using BoNT-B to treat adductor thigh muscle spasm.

알코올을 이용한 흉부교감신경절차단술에 대한 임상경험 및 합병증 -증례 보고- (The Clinical Experiences and Complications of Percutaneous Neurolysis of Upper Thoracic Sympathetic Ganglion by Using Ethylalcohol -A report of three cases-)

  • Percutaneous neurolysis of upper thoracic sympathetic ganglion was performed by simultaneously injecting 3 ml of pure alcohol into the $T_2$ and $T_3$ levels after testng with same amount of local anesthetics on the same sites. We experienced poor sympatholytic effect or intercostal neuritis and Horner's Syndrome as the result of complication of thoracic sympathetic ganglion block. In Case 1, in spite of the good testing result, neurolytic block effect was poor. In Case 2, intercostal neuritis occurred, but neuralgia subsided within 3 weeks. In Case 3, Horner's Syndrome occurred for 1 day. To increase the success rate of block and decrease the incidence of complications, good radio-opaque dye appearance and good test block effect should be obtained.

Paraplegia Following Intercostal Nerve Neurolysis with Alcohol and Thoracic Epidural Injection in Lung Cancer Patient

  • The goal of cancer treatment is generally pain reduction and function recovery. However, drug therapy does not treat pain adequately in approximately 43% of patients, and the latter may have to undergo a nerve block or neurolysis. In the case reported here, a 42-year-old female patient with lung cancer (adenocarcinoma) developed paraplegia after receiving T8-10 and $11^{th}$ intercostal nerve neurolysis and T9-10 interlaminar epidural steroid injections. An MRI results revealed extensive swelling of the spinal cord between the T4 spinal cord and conus medullaris, and T5, 7-11, and L1 bone metastasis. Although steroid therapy was administered, the paraplegia did not improve.

상복부(上腹部) 통증완화(痛症緩和)를 위한 복강신경총차단(腹腔神經叢遮斷) (Alcohol Neurolysis of the Celiac Plexus of Upper Abdominal Pain Relief)

  • 상북부 통증을 호소하는 암성통증 환자 18예, 만성 췌장염환자 4 예에서 통증제거 목적으로 60% dehydrated ethanol 30~50ml을 방척추법으로써 복강신경총의 지각전도를 차단한 후 얻은바 결론을 다음과 같이 요약한다. 1. 위암, 간암, 췌장암등으로 인한 상복부 통증에 대해서 86%의 우수한 진통효과가 있었다. 2. 만성췌장염 환자에서도 양호한 효과를 얻을 수 있었다. 3. 진통효과의 지속시간은 55%에서 약 4개월 내지 7개월간 지속되었다. 4. 시술후 일시적인 기립성 혈압하강이 16%에서 나타났다. 5. 시술후 합병증은 경미한 정도에서 안면홍조 혈압하강 요통등이 있었다. 6. 시술전 마약성 약물 사용이 습관화된 경우는 시술후 마약성 약물 투여금단 효과를 억제하기 어려웠다.

다한증 환자에서 흉부 교감신경절 차단과 인지 체온 변화와의 관계 (Changes of Index Finger Temperature as Indices of Success of Thoracic Sympathetic Ganglion Block)

  • Percutaneous neurolysis of upper thoracic sympathetic ganglion was performed in 40 patients by simultaneously injecting 3 ml of pure alcohol into the T2 and T3 levels after 3 ml of injection of local anesthetic agent on the same sites. Using a skin temperature probe, finger tip temperatures were measured on the index finger ipsilateral to the nerve block before block, 15 and 30 minutes after test block, and 30 minutes after alcohol block. Alcohol block was performed immediately after 30 minutes test block. Finger tip temperatures obtained at 30 minutes post alcohol block and test block and the differences in the temperatures measured before and 30 minutes after alcohol block were shown to be statistically important as potential indicators for prediciting long term outcome of therapy for palmar hyperhidrosis using this technique. These results demonstrate that the palmar temperature monitoring method is sufficiently sensitive to predict the outcome of nerve block during and after thoracic sympathetic ganglion block.

CT를 이용한 복강신경총 차단 (CT Guided Celiac plexus Block)

  • 1986년 3월 통증치료실을 개설한 이래 상복부의 불인통을 호소하는 12명의 암환자에서 복강신경총 차단을 시행하였다. 처음 5예는 fluoroscopy의 도움만으로, 나머지 7예는 CT를 이용하여 피부에 천자부위를 정하고 재차 수술실로 옮겨서 시행한 결과 다음과 같은 결론을 얻었다. 1) CT를 이용한 복강신경총 차단을 시행할 경우, 평균 시술시간을 훨씬 단축시킬 뿐 아니라(약 1시간) 초보자에게는 시술이 훨씬 용이하다. 2) Fluoroscopy만 이용한 5예 중 3예에서, CT 및 fluoroscopy틀 동시에 이용한 경우는 7예중 5예에서 양호한 결과를 얻었다. 3) 정확한 위치에서 천자하면 신경파괴제의 양을 줄 일 수 있다. 4) 거대 간장, 심한 복수 등에서는 그 효과가 의문시 된다.

Presentation of Neurolytic Effect of 10% Lidocaine after Perineural Ultrasound Guided Injection of a Canine Sciatic Nerve: A Pilot Study

  • Background: Phenol and alcohol have been used to ablate nerves to treat pain but are not specific for nerves and can damage surrounding soft tissue. Lidocaine at concentrations > 8% injected intrathecal in the animal model has been shown to be neurotoxic. Tests the hypothesis that 10% lidocaine is neurolytic after a peri-neural blockade in an ex vivo experiment on the canine sciatic nerve. Methods: Under ultrasound, one canine sciatic nerve was injected peri-neurally with 10 cc saline and another with 10 cc of 10% lidocaine. After 20 minutes, the sciatic nerve was dissected with gross inspection. A 3 cm segment was excised and preserved in 10% buffered formalin fixative solution. Both samples underwent progressive dehydration and infusion of paraffin after which they were placed on paraffin blocks. The sections were cut at $4{\mu}m$ and stained with hemoxylin and eosin. Microscopic review was performed by a pathologist from Henry Ford Hospital who was blinded to which experimental group each sample was in. Results: The lidocaine injected nerve demonstrated loss of gross architecture on visual inspection while the saline injected nerve did not. No gross changes were seen in the surrounding soft tissue seen in either group. The lidocaine injected sample showed basophilic degeneration with marked cytoplasmic vacuolation in the nerve fibers with separation of individual fibers and endoneurial edema. The saline injected sample showed normal neural tissue. Conclusions: Ten percent lidocaine causes rapid neurolytic changes with ultrasound guided peri-neural injection. The study was limited by only a single nerve being tested with acute exposure.

신경병증성 통증 환자에서 지속적 진통을 위한 5% 리도카인의 사용 (The Use of 5% Lidocaine for Prolonged Analgesia in Neuropathic Pain Patients)

  • Background: The use of neurolytic agents to control neuropathic pain has been described from the last century Phenol and ethyl alcohol have been widely used as neurolytic agents, however, their neurolytic effect is variable in efficacy and duration of action, and infrequently accompanied with grave complications. It has been found that 5% lidocaine causes irreversible conduction blockade in animal studies. The goal of this study was to evaluate the neurolytic effect of 5%o lidocaine on various neuropathic pain syndromes for prolonged analgesia. Methods: Twenty-five patients with a diagnosis of neuropathic pain including trigeminal neuralgia (n = 7), postherpetic neuralgia (n = 10), and postsurgical neuralgia (n = 8) were selected after failure of routine therapeutic regimens. After performing a diagnostic nerve block with 1% lidocaine and 5% lidocaine was injected. The patients were followed for 6 months. Visual analog scale (VAS) scores and side effects were recorded for each patients. Results: A significant decrease in pain scores after neurolytic blockade with 5% lidocaine was seen in all of three pain groups. All the patients reported immediate and prolonged pain relief lasting from 4 weeks to 6 months. None of patients exhibited any appreciable side effects or complications. Conclusions: We suggest that 5% lidocaine may be used safely and effectively for the purpose of prolonged analgesia in selected patients with intractable neuropathic pain syndromes.
