Purpose: There is no recommended bag-squeezing technique for emergency medical providers to maintain correct tidal volume during mechanical ventilation. This study compared the tidal volume of two different bag-squeezing techniques during mechanical ventilation. Methods: The subjects were 38 paramedic students who were trained in airway management techniques. Two different bag-squeezing techniques were used with a bag valve mask on an intubated manikin: a conventional technique and a finger-marked, in which the bag is squeezed until the thumb and the middle finger come into contact. Hand size and grip strength were measured and analyzed statistically. Results: The mean tidal volumes for the finger-marked were significantly higher than those for the conventional technique ($542.6{\pm}35.92mL$ versus $338.0{\pm}111.15 mL$, p<.001). There was a correlation between the two techniques (Pearson $x^2=1.160$, p<.001). The subject's characteristics, including sex, hand size, and grip strength, showed no correlation with tidal volume. Conclusion: A finger-marked bag-squeezing technique provides adequate and correct tidal volumes during mechanical ventilation.
Background: Nasotracheal intubation for general anesthesia is preferred for oral and maxillofacial procedures because it provides improved access to the operative site. Fiberopic nasotracheal intubation is a useful technique when airway management seems difficult. But, intaoral bleeding is considered as the important factor that makes fiberopic nasotracheal intubation difficult. The purpose of our study was to elucidate the effect of laryngeal view and bleeding on intubation difficulty during fiberopic intubation. Methods: We studied 461 patients undergoing nasotracheal intubation with permission. Laryngeal view grades were examined with laryngoscope and were recorded. Then, intubation time and the amounts of bleeding were measured during fibroptic nasotracheal intubation under general anesthesia. Results: There was no significant difference between laryngeal view grade and intubation difficulty (P > 0.05). But severity of bleeding increased intubation difficulty (P < 0.05). Conclusions: In this study, the significant amounts of bleeding had an effect on intubation difficulty.
Acquired tracheal dilatation is a state of abnormal tracheal dilatation developing from various causes. Tracheomalacia and tracheal dilatation can develop in respiratory distress patients with prolonged endotracheal intubation with assisted positive-pressure ventilation due to positive airway pressure and high cuff pressure. The authors have recently experienced one case of respiratory failure, cardiac arrest, and whole body emphysema after tracheostomy and portex tube insertion were performed to patient with the endotracheal intubation with assisted positive-pressure ventilation for two weeks in the septic shock resulted from colon perforation, who developed tracheal dilatation. We summarize diagnostic and therapeutic strategies of acquired tracheal dilatation for the prevention of emergency status and the management for that patients.

In the initial evaluation of patients with facial trauma, airway maintenance and respiratory maintenance are the most important. The principles of treatment include adequate irrigation and debridement, primary closure, or secondary wound healing. Postoperative care such as taping, silicone gel sheeting, and sun screening is important to prevent scarring. The scalp and forehead are abundant in blood and can cause severe bleeding. The eyelid is very thin and has a multi-layered structure, requiring accurate suturing and reconstruction of the layers. It is advisable to determine the presence of hematoma in the ear and treat it. When the cheek area is damaged, it is necessary to identify and treat the damage of the parotid gland and the facial nerve branch. The lips should be sewn with the white roll of lip and vermillion.
Lateral pharyngoplasty is a surgical option for treatment of obstructive sleep apnea (OSA). Here, we present a case involving a 40-year-old healthy man who underwent surgery, including lateral pharyngoplasty and robotic tongue base resection, for OSA. There were no intraoperative or immediate postoperative complications. However, on postoperative day 3, the patient presented with swelling in the temporal and buccal areas and was diagnosed with subcutaneous emphysema, later confirmed by computed tomography. The patient was carefully monitored under conservative care and discharged without complications. Although subcutaneous emphysema following tonsillectomy is a rare complication and usually resolves with conservative management, in certain cases, it might require surgical intervention. Lateral pharyngoplasty involves tonsillectomy and additional incision along the tonsillar fossa, which makes it susceptible to pharyngeal wall defects and, consequently, subcutaneous emphysema. Additionally, lateral pharyngoplasty and robotic tongue base resection cause pain and might thus contribute to the increase in intrapharyngeal pressure, which might aggravate subcutaneous emphysema. Lateral pharyngoplasty should be performed with meticulous dissection of the superior pharyngeal constrictor muscle. Healthcare providers should be aware of these complications and, upon suspicion of the same, place the patient under close observation to prevent life-threatening situations.
The video laryngoscope is a novel instrument for intubation that enables indirect visualization of the upper airway. It is recognized for its ability to enhance Cormack-Lehane grades in the management of difficult airways. Notably, video laryngoscopy is associated with equal or higher rates of intubation success within a shorter time frame than direct laryngoscopy. Video laryngoscopy facilitates faster and easier visualization of the glottis and reduces the need for Magill forceps, thereby shortening the intubation time. Despite the advanced glottic visualization afforded by video laryngoscopy, nasotracheal tube insertion and advancement occasionally fail. This is particularly evident during nasotracheal intubation, where oropharyngeal blood or secretions may obstruct the visual field on the monitor, thereby complicating video laryngoscopy. Moreover, the use of Magill forceps is markedly challenging or nearly unfeasible in this context, especially in pediatric cases. Furthermore, the substantial blade size of video laryngoscopes may restrict their applicability in individuals with limited oral apertures. This study aimed to review the literature on video laryngoscopy, discuss its clinical role in nasotracheal intubation, and address the challenges that anesthesiologists may encounter during the intubation process.
Nasotracheal intubation (NTI) plays an important role in pediatric airway management, offering advantages in specific situations, such as oral and maxillofacial surgery and situations requiring stable tube positioning. However, compared to adults, NTI in children presents unique challenges owing to anatomical differences and limited space. This limited space, in combination with a large tongue and short mandible, along with large tonsils and adenoids, can complicate intubation. Owing to the short tracheal length in pediatric patients, it is crucial to place the tube at the correct depth to prevent it from being displaced due to neck movements, and causing injury to the glottis. The equipment used for NTI includes different tube types, direct laryngoscopy vs. video laryngoscopy, and fiberoptic bronchoscopy. Considering pediatric anatomy, the advantages of video laryngoscopy have been questioned. Studies comparing different techniques have provided insights into their efficacy. Determining the appropriate size and depth of nasotracheal tubes for pediatric patients remains a challenge. Various formulas based on age, weight, and height have been explored, including the recommendation of depth-mark-based NTI. This review provides a comprehensive overview of NTI in pediatric patients, including the relevant anatomy, equipment, clinical judgment, and possible complications.
purpose : The purpose of this study was to analyze the effect of basic life support performance by video simulation training. Methods : The subjects in this study consisted of 24 persons of experimental group and 24 persons of control group in freshmen and sophomore EMT students. The data were collected from September 1 to 30, 2010. Results : 1. Hypothesis one: experimental group is meaningful higher than control group at the operation point (p<0.05) of adult/infant's basic life support for one person. In subcategory that identifying breath, artificial respiration, pulse rate, 30 compressions, Ability to perform all the processes, the experimental group showed statistically higher score (p<0.05) than control group, and sequence from checking reaction to keeping airway management isn't statistically significant difference between experimental group and control. 2. Hypothesis two: In the hypothesis, the experimental group's point of adult basic life support by two persons and use of Automated External Defibrillator is good in experimental group than control group's point(p<0.05). As the result of researching 11 contents of check list about adult basic life support by two persons and Automated External Defibrillator(AED), by dividing into 7 subcategories, every subcategory shows that the experimental group is significant higher than control group(p<0.05). 3. Hypothesis three: In the hypothesis, the experimental group gets higher point of infant basic life support by one or two persons than the control group (p<0.05). As the results of researching 15 contents of check list about infant basic life support by one or two persons, by dividing into 8 subcategories, the experimental group is statistically meaningful higher (p<0.05) than the control group in process of keeping airway, indentifying breathing, identifying pulse, 30 compressions, Ability to perform all the processes. There isn't statistically significant difference between the groups in process of checking reaction, reporting 119, and artificial respiration by giving 2 breaths(p<0.05). Conclusion : As summarizing the results, the group, receiving using video, gets higher points of knowledge of basic life support and operating skill than the general educated group. It is found that the Video simulation training could be effective, because of these positive effects to improve clinical working performance of students, who participate in the department of Emergency Medical Technology.
Although endobronchial hamartoma is a rare benign tumor, most patients with endobronchial hamartoma have respiratory symptoms such as obstructive pneumonia, hemoptysis, cough, or dyspnea due to bronchial obstruction. It can cause irreversible post-obstructive pulmonary destruction, thus early diagnosis and treatment is very important. Recently, there have been cases of neodymium-doped yttrium aluminum garnet (Nd:YAG) laser and electrocautery procedures for bronchoscopic treatment of malignant or benign central airway obstruction with comparable therapeutic efficacy and few complications. Bronchoscopic cryotherapy is a newly developed technique for management of central airway obstruction. Moreover, it provides diagnostic methods with improving diagnostic yield and safety. We report two cases of endobronchial hamartoma, each diagnosed and definitively treated with bronchoscopic techniques. Endobronchial biopsy and removal was successfully performed by cryotherapy via flexible bronchoscopy without notable complications. Follow-up bronchoscopic examinations excluded residual or recurrent disease.

OVA로 유도된 천식 생쥐 모델을 정상군, 대조군, 양성대조군 그리고 실험군으로 구성된 4집단으로 분류하였다. 0.3%의 레몬오일을 실험군에 3주간, 1주에 3회, 1회 30분간 흡입시켰다. 최초 실험일로부터 6주 후 기도 과민성의 정도, 혈액학적 변화, 혈청 내 사이토카인의 변화, 기도조직의 변화를 평가하였다. 실험 결과 레몬 오일은 효과적으로 기도과민성 억제 및 호산구의 세포증식 억제, IgE의 생성 억제를 통하여 기도의 염증 반응 및 과민반응을 유의성 있게 억제하는 것으로 확인되었다.
