  • The objective of the study is to measure the changes in measles infection and measles vaccination rates for the past 10 years in a rural area, Kang Wha. The study population were the entire children who were born between 1971 and 1950 in three townships (Sunwon, Naegae, Buleun) in Kangwha County. Two interview surveys were carried out during the 10 years of period, one in 1977 and the other in 1981. The data were collected by Family Health Workers through interview with structured questionnaires. The diagnosis of measles was mainly based on histories, symptoms and sighs of the disease. If a mother had reported measles history of her child, a public physician reviewed and decided the final diagnosis of the reported case. A retrospective cohort observation was done in order to see the trends of measles infections and measles vaccinations. The major findings were as follows; 1. The 5 year prevalence rate of measles vaccinations was 51.3% between 1971 and 1975 and 71.9% between 1976 and 1980 respectively. The difference between two periods was statistically significant (P<0.05). The secular trend of measles vaccinations showed increasing tendency from 1971 to 1978 and since then kept maintained. 2. In the birth cohort analysis of measles vaccinations, the vaccination rates, in general, were higher in the later cohort groups than that of earlier cohort groups. 9. The 5-year experience rates for measles infections were 24.3% between 1971 and 1975 and 17.2% between 1976 and 1980 respectively. This difference was statistically significant(P<0.05). The secular trend of experience rates for measles infections showed decreasing tendency from 1971 to 1980 except an outbreak in 1976. 4. The birth cohort analysis of experience rates for measles infections showed that the rate was higher in the later cohort groups than that of the earlier cohort groups. This decreasing tendency was prominent between $1973{\sim}1974$ and $1976{\sim}1977$. 5. The distribution of age specific incidence rates for measles infections showed unimodal curve with the peak at the age of 12 to 18 months. This findings were same in both two surveys. 6. Seasonal variations of the measles infections showed two peaks, one major peak in March through May and the another minor peak in September through December. 7. The 5-year reduction rate for measles infections among those vaccinated was 90.4% between 1971 and 1975 and 88% between 1976 and 1980.

  • Dental Implants have been proved to be successful prosthetic modality in edentulous patients for 10 years. However, there are few reports on the survival of implant according to location in molar regions. The purpose of this study was to evaluate the $4{\sim}5$ years' cumulative survival rate and the cause of failure of dental implants in different locations for maxillary and mandibular molars. Among the implants placed in molar regions in Gwangju Mir Dental Hospital from Jan. 2001 to Jun. 2002, 473 implants from 166 patients(age range; $26{\sim}75$) were followed and evaluated retrospectively for the causes of failure. We included 417 implants in 126 periodontally compromised patients, 56 implants in 40 periodontal healthy patients, and 205 maxillary and 268 mandibular molar implants. Implant survival rates by various subject factors, surgical factors, fixture factors, and prosthetic factors at each location were compared using Chi-square test and Kaplan-Meier cumulative survival analysis was done for follow-up(FU) periods. The overall failure rate at 5 years was 1O.2%(subject level) and 5.5%(implant level). The overall survival rates of implants during the FU periods were 94.5% with 91.3% in maxillary first molar, 91.1% in maxillary second molar, 99.2% in mandibular first molar and 94,8% in mandibular second molar regions. The survival rates differed significantly between both jaws and among different implant locations(p<0.05), whereas the survival rates of functionally loaded implants were similar in different locations. The survival rates were not different according to gender, age, previous periodontal status, surgery stage, bone graft type, or the prosthetic type. Epidemiologic Impact of Rapid Industrialization on Head Injury Based on Traffic Accident Statistics in Korea

  • Objective : The aim of the present study is to estimate the incidence trend of head injury and the mortality based on traffic accident statistics and to investigate the impacts of rapid industrialization and economic growth on epidemiology of head injury in Korea over the period 1970-2012 including both pre-industrialized and post-industrialized stages. Methods : We collected data of head injury estimated from traffic accident statistics and seven hospital based reports to see incidence trends between 1970 and 2012. We also investigated the population structure and Gross National Income (GNI) per capita of Korea over the same period. The age specific data were investigated from 1992 to 2012. Results : The incidence of head injury gradually rose in the 1970s and the 1980s but stabilized until the 1990s with transient rise and then started to decline slowly in the 2000s. The mortality grew until 1991 but gradually declined ever since. However, the old age groups showed rather slight increase in both rates. The degree of decrease in the mortality has been more rapid than the incidence on head injury. Conclusion : In Korea during the low income stage, rapid industrialization cause considerable increase in the mortality and the incidence of head injury. During the high income stage, the incidence of head injury gradually declined and the mortality dropped more rapidly than the incidence due to preventive measures and satisfactory medical care. Nevertheless, the old age groups revealed rather slight increase in both rates owing to the large population structure and the declining birth rate.

일부 농촌지역 손상 및 중독 발생에 관한 역학적 연구 (Epidemiologic study of injury and poisoning occurrence in a rural area)

  • The mortality and morbidity caused by injury and poisoning has been major public health problem in Korea. This study was carried out to get information indispensable in developing prevention srategies peculiar to korean rural area. In this study 1.499 people of 496 households. who are living in five rural villages of Chooncheon City. Kang-Won Province. were interviewed in accordance with structured questionaire in 1996. The results obtained are summarized as follows: 1. The age-standardized annual incidence rates of injury and poisoning per 100 persons surveyed were 4.2 in males and 1.9 in females. The difference between sex was statistically significant. 2. The places where most of injuries and poisonings occurred were road 66.7% among males. road 31.6% among females. 3. The occurrence of the injuries and poisonings were clustered in intensive farming season in both males and females. 4. The laceration and stab wound were the most frequent nature of injuries in males and the fracture was the most frequent nature of injuries in females. The fracture. sprain were more frequent among males but sprain and contusion were more frequent among females. 5. Incidence rates of injuries and poisoning by cause were higher in traffic accidents among males. though falls among females. The cause specific incidence rate by age was high in traffic accident for 30-59 year age group. and for the aged people older than 60 years. 6. In the management pattern, 78.8% of the injuries and poisoning were received medical care in hospitals and clinics, and the duration of the treatment over 4 weeks in 37.0%. The results obtained indicate that organized community effort is urgently required to prevent injuries and poisonings in rural area.

Increasing Frequency of Soft Tissue Sarcomas in Vojvodina - Comparison with the Literature

  • Background: Soft tissue sarcomas (STS) represent 1% of all malignant lesions. In this study the authors analyzed the incidence of STS in Vojvodina (the north region of Serbia) in the period from 1985 to 2009. A number of studies conducted worldwide indicate that STS incidence rates are tending to increase. Materials and Methods: On the basis of data from the Cancer Registry of Vojvodina, age standardized STS incidence rates were established as well as their linear trend, with data on histological structure, age, gender and STS distribution at specific locations. Results: The total number of registered patients was 1,308. Average age standardized rate was 1.90/100,000 per year. The investigated period showed a slight increase in the incidence rate (average annual percent increase=0.77%). The most frequent histological type was sarcoma not otherwise specified-NOS (27%), followed by leiomyosarcoma (21%), liposarcoma (14%), rhabdomyosarcoma (11%) and malignant fibrous histiocytoma (9%). The male/female ratio was 0.73:1. Every fifth patient was younger than 39. Conclusions: Comparison among eight international STS epidemiology studies show that the incidence rate range is between 1.4/100,000-5.0/100,000, though our finding is closer to the lower limit. Furthermore, the incidence rate increase was lower than that characteristic for the half of the analyzed studies. A partial explanation for that should be looked for among changes in diagnostic criteria and STS classifications.

Epidemiology of Breast Cancer among Females in Basrah

  • Breast cancer is the most frequent cancer in females. Its incidence is higher in developed countries than in developing ones partly due to variation in risk exposure and partly due to better detection methods. Scattered evidence in Basrah, Iraq, suggests that breast cancer has been increasing at a significant pace in recent years. This study aimed to measure the current level of risk of breast cancer among females in Basrah and to describe the time trend over almost a decade of years. Data on breast cancer cases from all sources of cancer registration in Basrah governorate were compiled for the years 2005-2012. The data for each year were first checked separately for duplicate reporting of cases among various sources. Then the eight files were pooled together and checked again for any duplicate cases among years of registration. The final set of data contained 2,284 cases of breast cancer (2,213 female cases and 71 male cases). All patients were inhabitants of Basrah governorate at the time of diagnosis. Figures on the Basrah population were obtained from various sources including the Ministry of Health, Ministry of Planning and Developmental Collaboration and local household surveys. It was possible to have total population estimates for each year and by age and sex. The data were imported into SPSS (version 17) software. Age specific and year specific incidence rates were calculated. The age standardized incidence rate was also calculated using world population as the standard population to be 34.9 per 100,000 females. Age-wise, no case was reported among children aged less than 15 years and the incidence increased with advancing age reaching a peak of 123.8/100,000 females at the age range of 50-54 years. The time trend of the crude incidence rate showed only modest increased risk with passage of years and no age shift could be documented in this study. Breast cancer in females in Basrah is a significant health problem. The current incidence rate (crude, 23.7/100,000, age-standardized, 34.9/100,000) is high and justifies intensive efforts to improve early detection of cases, provide better treatment amenities and introduce long term preventive measures. Using the age standardized incidence rate as reported in this paper, it is possible to put the risk in Basrah within a regional and international context.

Suicide Mortality Across Broad Occupational Groups in Greece: A Descriptive Study

  • Background: Several studies have investigated the relationship between specific occupations and suicide mortality, as suicide rates differ by profession. The aim of this study was to investigate suicide mortality ratios across broad occupational groups in Greece for both sexes in the period 2000-2009. Methods: Data of suicide deaths were retrieved from the Hellenic Statistical Authority and comparative mortality ratios were calculated. Occupational classification was based on the International Classification of Occupations (ISCO-88) and the coding for Intentional self-harm (X60-X84) was based on the international classification of diseases (ICD-10). Results: Male dominant occupations, mainly armed forces, skilled farmers and elementary workers, and female high-skilled occupations were seen as high risk groups for suicide in a period of 10 years. The age-productive group of 30-39 years in Greek male elementary workers and the 50-59 age-productive group of Greek professional women proved to have the most elevated number of suicide deaths. Conclusion: Further research is needed into the work-related stressors of occupations with high suicide mortality risk and focused suicide prevention strategies should be applied within vulnerable working age populations.

A retrospective study of age-specific disease incidence in major popular breed dogs in Republic of Korea

  • Dogs exhibit patterns of health issues that vary by life stage. An understanding of disease incidence with respect to breed and age/life stage could be an important component of canine health management and welfare. This study aimed to describe the age-specific disease incidence of 3 small dog breeds that attended veterinary clinics in the Republic of Korea, based on data from electronic veterinary medical records (EVMRs). A total of 40,785 EVMRs from Maltese (n = 21,355), Miniature Poodle (n = 11,658) and Shih Tzu dogs (n = 7,772) were analyzed. Common health problems in 3 small dog breeds were 'diseases of the skin' and 'diseases of the ear,' respectively. Among dogs aged ≤ 3 years, 'preventive medicine' was the most common cause cited for veterinary clinic visits. Among dogs aged 4 to 8 years, the most frequent health problems were 'diseases of the skin.' Among dogs aged 9 to 13 years, 'heart diseases,' 'kidney diseases,' 'mammary gland tumor,' and 'neoplasia (unspecified)' were considerably more frequent, compared to the rates in dogs ≤3 years. Among dogs aged ≥ 14 years, 'heart diseases' and 'sneezing/cough' were the main health problems. In all breeds, the frequencies of 'diseases of circulatory system,' 'diseases of respiratory system,' 'diseases of the nervous system,' 'endocrine' and 'neoplasia' increased rapidly with aging. This surveillance could inform strategies for disease screening tests and management based on life stage in these dog breeds and enable more effective health management.

시도의 사망원인별 사망력 (Cause-Specific Mortality at the Provincial Level)

  • 시도의 사망원인별 사망력 분석은 정책수립에 필수적인 정보를 제공하고, 각종 질병 및 사망 원인에 대한 가설을 설정하게 한다. 사회경제적, 문화적, 의료적, 생태학적 이유 등 다양한 원인이 시도의 사망원인별 사망수준에 복합적으로 영향을 주지만, 이 연구에서는 시도의 사망원인별 사망력에 대한 설명보다는 공통점과 차이점 파악을 주 목적으로 하였다. 이를 위하여 1998년 기준 사망신고 및 주민등록인구 자료를 활용하여, 시도별로 지연신고와 영아사망 신고누락을 보완하고, 연령표준화사망률과 생명표를 작성하였다. 모든 사인에 의한 사망수준 관련 주요 결과는 다음과 같다: (1) 남녀전체를 합하여 서울이 가장 낮은 사망수준을 전남은 가장 높은 사망수준을 보였다: (2) 시도간 사망수준의 차이가 여자보다 남자에게서, 65세 이상보다 604세 이하 연령층에서 더 컸다. 사망원인별 사망력 관련 남녀별 및 남녀 전체를 합하여 연령표준화 사망률이나 출생시 사망확률이라는 지표 모두에서 일관된 유형을 보이는 주요 결과는 다음과 같다: (1) 심장질환에 의한 사망수준은 부산에서 최고, 강원도에서 최저를 나타냈고: (2) 간질환에 의한 사망수준은 전남에서 최고를; (3) 운수사고에 의한 사망수준은 충남에서 최고 인천에서 최저로 나타났다. 시도의 사망수준 차이에는 다양한 요인이 관련되어 있으므로 사회경제적 변수를 포함한 25개의 설명 변수와 총90개의 사망력 변수에 대한 탐색적 통계분석을 실시하였다. 모든 사인에 의한 사망력은 사회경제적 변수와 밀접한 관련이 있으며, 사망원인별로는 간질환 및 운수사고에 의한 사망력이 사회경제적 변수와 관련이 있는 것으로 나타났다. 끝으로 사망신고 자료의 질 개선 필요성을 논의하고 있다.

평균초혼연령 측정방법에 관한 소고 (An Analytical Review of the Methods Computing Age at First Marriage)

  • 이 논문은 평균초혼연령을 계산하는 여러 방법들의 특성과 차이점을 논의하고, 1970-1990년 기간 한국 여자의 자료를 이용하여 이들 계산 방법상의 차이가 평균초혼연령 측정치에 미치는 영향을 구체적으로 제시하였다. 또한 연구의 기초자료인 인구센서스와 표본조사결과 자료의 비교성에 대한 통계적 검정결과를 제시하고 있다. 주요 결과를 보면, 한국의 혼인행태에 관한 연구에서 주로 사용되는 지표인 Hajnal의 SMAM은 한 특정기간의 혼인행태에 근거한 측정치가 아니다. 이 때문에 생기는 편의(Bias)는 1970-1990년 기간 한국 여자의 경우 0.3세 미만으로 나타나, 이 기간 혼인형태의 변화를 고려할 때 예상보다 작은 것으로 생각할 수 있다. 그러나 더 큰 편의를 가져올 가능성을 배제할 수 없다. 또한 편의의 방향은 혼인연령이 상승하는 추세에 있을 때 평균연령을 높이는 영향을 주는 것으로 보인다. 한국의 자료사정에서는 단순평균방법과 Agarwala 방법에 의한 평균초혼연령 측정치(ASMAM)를 목적에 따라 이용하는 것이 바람직하다. ASMAM은 두 센서스 자료에서 계산된 연령별 혼인율 추정치를 근거로 함으로 같은 자료를 이용하여 얻은 조혼인표에서 계산할 수 있는 평균초혼연령과 일치한다. 인구센서스와 표본조사에 의한 연령별 미혼인구구성비의 시계열 자료들은 각기 일관성 있는 추세를 보이고 있으나, 같은 시점에서 두 결과자료를 비교했을 때 주요 연령계층에서 통계적으로 유의한 차이(\alpha=0.05)가 있는 것으로 검증되었다. 또한 이러한 차이는 표본틀이나 조사표 설계상의 문제가 아니며 그 원인에 대한 별도의 연구가 필요하다.
