White matter injury following rotavirus infection in neonates: new aspects to a forgotten entity, 'fifth day fits'?

  • That rotavirus infection can cause neurological symptoms in young children has been well established. However, it is surprising why rotavirus infection has been overlooked as a cause of neonatal seizures for many years, despite significant research interest in neonatal rotavirus infection. Neonates are the age group most vulnerable to seizures, which are typically attributed to a wide range of causes. By contrast, because rotavirus infection is usually asymptomatic, it has been difficult to identify an association between this virus and neonatal seizures. The conventional wisdom has been that, although neonates are commonly infected with rotavirus, neurological complications are rare in this age. However, recent studies using diffusion-weighted imaging (DWI) have suggested a connection between rotavirus infection and neonatal seizures and that rotavirus infection can induce diffuse white matter injury without direct invasion of the central nervous system. The clinical features of white matter injury in rotavirus-infected neonates include the onset of seizures at days 4-6 of life in apparently healthy term infants. The recent findings seem to contradict the conventional wisdom. However, white matter injury might not be a completely new aspect of rotavirus infection in neonates, considering the forgotten clinical entity of neonatal seizures, 'fifth day fits'. With increased use of DWI in neonatal seizures, we are just starting to understand connection between viral infection and white matter injury in neonates. In this review, we discuss the historical aspects of rotavirus infection and neonatal seizures. We also present the clinical features of white matter injury in neonatal rotavirus infection.

Seroprevalence of Antibody to Hepatitis A Virus in North-West Jeonbuk Province

  • Hepatitis A (HA) is an acute infectious disease of the liver caused by the Hepatitis A virus (HAV). In acute HA, the presence of anti-HAV IgM is detectable and about 3 weeks after exposure, its titre increases over 4 to 6 weeks. Anti-HAV IgG is detectable within a few days of the onset of symptoms. IgG antibodies continue to last for years after infection and provide lifelong immunity to the host. This study was performed to investigate the current seroprevalence of anti-HAV antibodies in Jeonbuk province, South Korea. A total of 591 (male 322, female 269) serum samples were collected in July 2011 to June 2012. We tested the antibodies of anti-HAV IgG and IgM using a Modular E170 (Roche Diagnostics, Germany), and analysed the serum alanine aminotransferase (ALT) levels by HITACH 7600-100 (HITACH, Japan). The overall seroprevalence of anti-HAV IgG was 84.6% (500/591), and the rate of females (85.9%) was higher than males (83.5%). According to the decade of age, seroprevalence of anti-HAV IgG were as follows; 68.8% (11/16) in the under 10 years old category, 100% (19/19) in the 10~19 category, 96% (48/50) in the 20~29 category, 83.6% (56/67) in the 30~39 category, 84.3% (123/146) in the 40~49 category, 83.3% (135/162) in the 50~59 category, 83.1% (54/65) in the 60~69 category, 78.1% (32/41) in the 70~79 category, and 88% (22/25) in the over 80 category. Total seroprevalence of anti-HAV IgM was 3.4% (20/591), and according to gender, the seroprevalence of male (3.1%) was very similar to that of female (3.7%). Through this study, we know that the seroprevalence of anti-HAV antibody in north-west Jeonbuk province, South Korea, was high. Only children under the age of 10 remain susceptible to HAV infection. Vaccination against HAV is not needed at the present time for the people of Jeonbuk province, South Korea, but a vaccination should be recommended and the improvement in sanitary conditions and personal hygiene should be highlighted.

The Cross Sectional Study of Autonomic Nerve Balance And Stress In Patient Who Visted Dept. of Oriental Medical Dermatology (HRV 검사를 통한 피부과 내원환자의 자율신경 균형 및 스트레스 단면 연구)

  • Objective : Etiologically, stress is a important factor to immune system and skin disease. Human body is controled to keep his homeostasis from stress by autonomic nerve system. So we investigated autonomic nerve balance and stress level of patients with skin disease by heart rate variability(HRV) test. Research Methods and Procedures : Fifty two patient(from 15 years old to 35 years old) who visited Dept. of Oriental medical Dermatology, Kyunghee university at first and had an examination by HRV test, were selected as the subject of our study. We checked items of HRV test(such as SDNN, RMSSD, VLF, LF, HF, TP, norm-LF, norm-HF, LF/HF ratio, LnHF and LnHF), and analyzed time and frequency domain by sex, age, onset, color on end of patient's tongue, sweating and sleep condition. Results and Conclusion : Men showed significant higher SDNN, RMSSD, LF, norm-LF, norm-HF, LF/HF ratio, LnLF and LnHF than women(p <0.05). Patients who were attacked by skin disease within 6 months, showed significant higher TP and VLF than over 6 months(p <0.05). Patients with normal color on the end of their tongue showed significant higher TP and VLF than with flare color. We analyzed correlation to color on the end of tongue and RMSSD. Prevalence risk(RMSSD <20ms) in the case of normal color on the end of tongue was 5 times as high as in the case of flare color(p <0.1). And we analyzed correlation to age and LF/HF ratio. Prevalence risk(LF/HF ratio <1) of patients from 15 years old to 25 years old was 1.467 times as high as from 26 years old to 35 years old(p <0.1).

Clinical Results of Tarsal Tunnel Decompression in Case of Known Etiology (원인이 밝혀진 족근관 증후군의 수술적 치료의 결과)

  • Purpose: The purpose of this study is to present our clinical results after surgical treatment in tarsal tunnel syndrome due to space occupying lesions. Material and Methods: We performed surgical decompression for tarsal tunnel syndrome in 20 patients from July 2004 to February 2007. Out of them, thirteen cases were due to space occupying lesions around the tarsal tunnel. The average age at operation was 51.3 years old and the duration from symptom onset to surgery was 16.5 months. The operation included removal of space occupying lesions and tarsal tunnel decompression. The clinical parameters were pain visual analogue scale (VAS), AOFAS scale, and subjective satisfaction. Results: The ganglion cysts were the most frequent causes (ten cases) and synovial chondromatosis in 1 case, neurofibroma in 1 case, talocalcaneal coalition in 1 case. The average follow-up duration was 14.5 months. The AOFAS scale showed significant improvement from 77.8 to 92.7. The average VAS decreased from 6.4 to 2.2. Seven out of thirteen patients were satisfied with the results. The excellent results were shown in six patients, the good results in one patient, the fair result in three patients and the unsatisfactory results in three patients. Conclusion: Favorable results could be obtained in patients with known etiology. But not all cases with surgical decompression of space occupying lesions showed satisfactory results. We assume that the clinical results were related to the multiple factors, not only well performed surgery but also age, size lesions and duration of symptoms, ect.

Chronic Pain and Its Determinants: A Population-based Study in Southern Iran

  • Background: Prevalence of chronic pain and its association with demographic characteristics have been reported by different studies from different geographical regions in the world. However, data from many Middle East countries including Iran (especially southern Iran) are scare. The aim of the present study was to demonstrate the prevalence of chronic pain and its association with demographic, psychological and socioeconomic factors in an Iranian population. Methods: In this population-based survey, the target population was comprised of subjects aged 20 to 85 years residing in Jahrom, southern Iran during 2009-2011. All eligible subjects were invited to participate in the study. Before a detailed questionnaire was given; face to face interviews were done for each individual. Results: There were 719 men and 874 women with an average age of 40.5 years at the onset of the study. Among the study population, 38.9% (620/1,593) complained of chronic pain, of whom 40.8% (253/620) were men and 59.2% (367/620) were women. Foot and joint pain were observed in 31.9%. Hip and spine pain, migraine and tension headaches, heart pain, and abdomen pain were observed in 21.5%, 15.5%, 9.5%, and 8.0% of chronic pain cases, respectively. There was a significant association among the covariables age, sex, overweight, educational level, income, and type of employment with chronic pain as the dependent variable (P < 0.0001). Conclusions: Our findings show the prevalence of chronic pain and its association with demographic, psychological and socioeconomic factors. Individuals with low incomes and less education became accustomed to pain due to a lack of knowledge.

A Clinical Review of Surgically Treated 115 Cases of Lung Abscess (외과적 치료를 가한 폐농양 115례에 대한 임상적 관찰)

  • Prior to availability of antibiotics, lung abscess has been recognized as one of the most serious pulmonary disease, and despite of diminished morbidity and mortality rate with the advent of antibiotics and development of better technique in the fields of surgery on pulmonary disease it continues to be a virulent form of pulmonary supuration. It is the purpose of this paper to present a clinical review of 115 lung abscess treated by surgically at "The National Medical Center" during past 9 years. 1. The most common age of occurrence is between the age of 30 and 50, and 105 cases were male and 31 cases occurred in female. 2. Numerous etiological factors may play a role in the formation of a pulmonary abscess; aspiration in 36 patients of which 2 patients had diabetes, 54 patients had preceding URI or pneumonia, and 5 patients followed liver abscess, one of which in hematogenous route. In 20 patients, no predisposing factor could be determined- 3. Clubbing digits were presented in 26 of 1315 patients suggesting chronicity of the disease, and gastric ulcer were combined in 3 of 136 patints. 4. Apparently the onset was insidious, and the progress was masked by indiscriminate administration of antibiotics for URI and TB remedies under impression of pul. tbc. Previous TB treatments were done in 56.%[76 cases] for one week to two years and administration of antibiotics in 51 cases. 5. 89 cases were on tbe right and 2t5 cascs were on the left side. 51 cases of pneumonectomy and 8 of open drainage, of which 4 of Monaldi procedure, were made and operative mortality rate was 5.2%. Numerous complications such as 8 of empyema, 4 of each bleeding and atelectasis, 2 of BPF, 1 of esophageal fistula and one of pneumothorax which were responded well to prolonged treatment with good results, were obtained. 6. The incidence of Klebsiella and Pseudomonas were increased recently. In these results, we submit the surgery should be performed if the conservative therapy had failed. The resection was most favorable operation for pulmonary abscess though drainage procedure may be of value in certain patients with grave systemic menifestations. In chronic long standing abscess, more complications and high mortality rate were assumable although the prognosis has been improved with the advent of antibiotics, the development of improved surgical technique and complete preoperative care.

Association between the Weight Gain and Treatment Response to Atypical Antipsychotics in Korean Patients with Schizophrenia (한국인 정신분열병 환자에서 비정형 항정신병 약물의 치료반응과 체중증가의 관련성에 대한 연구)

  • Objectives Atypical antipsychotics show better treatment efficacy, safety and tolerability than typical antipsychotics. Among the adverse events observed during treatment with antipsychotics, extrapyramidal syndrome (EPS) and negative symptoms has been greatly reduced. But still, weight gain is receiving growing attention. The aim of this study was to investigate the association between therapeutic response to atypical antipsychotics and weight gain in admission status. Methods The study was conducted for Korean inpatients with schizophrenia in a university hospital in Seoul, between Jan 2006 and Dec 2010. Data was collected by reviewing the medical record of 39 consecutively hospitalized patients with Schizophrenia (DSM-IV) at a university hospital. Positive and Negative Symptom Scale (PANSS), Body Mass Index (BMI) and body weights were measured. Results No significant difference was observed for sex, age, illness onset age, family history of schizophrenia, numbers of hospitalization before treatment, educational years, marriage status, occupational status and subtype of schizophrenia between weight gainers and non-weight gainers. Regarding treatment response to atypical antipsychotics, weight gainers show significantly more PANSS decrease than non-weight gainers during admission period. Conclusions Our findings suggest that it appears to be more likely to respond to atypical antipsychotics in weight gainers than non-weight gainers (that weght gainers appear to be more likely to respond to atypical antipsychotics than non-weight gainers). These results show that the antipsychotic-induced body weight gain is associated with therapeutic response of antipsychotics in Korean inpatients with schizophrenia.

Assessment of Elementary Teachers' Knowledge about ADHD, Attitude, Stress Level and Intention for Inclusion Education (초등교사의 ADHD 관련 지식, 태도, 스트레스 수준 및 통합교육의지에 대한 인식)

  • Attention deficit hyperactivity disorder (ADHD) is a relatively common condition of childhood onset and is of significant educational and social concern in elementary school. Investigating the determinants of teachers' attitudes and behavior and their relative importance is crucial for improving teaching practices and initial teacher education for effective inclusion of ADHD children. Based on two hundred and twenty seven survey responses from elementary school teachers, having experienced in dealing with ADHD students, this paper examines the age, gender, career and training-hours effects on the knowledge, attitude, stress and inclusive education involving ADHD children. The regression analysis shows that the gender influences the level of stress and that the training hours has some impact on the knowledge about ADHD at 5% significance level. However, the number of ADHD children experienced, age, gender, career and training-hours were identified as the non-significant predictors of teaching behavior in relation to ADHD children. As a conclusion, a brief discussion concerning the critical needs of elementary school teachers is also presented.

Anisakiasis: Report of 15 Gastric Cases Caused by Anisakis Type I Larvae and a Brief Review of Korean Anisakiasis Cases

  • The present study was performed to report 15 anisakiasis cases in Korea and to review the Korean cases reported in the literature. Total 32 Anisakis type I larvae were detected in the stomach of 15 patients by the endoscopy. Single worm was detected from 12 cases, and even 9 larvae were found from 2 cases. Epigastric pain was most commonly manifested in almost all cases, and hemoptysis and hematemesis were seen in 1 case each. Symptom manifestations began at 10-12 hr after eating fish in 73.3% cases. Endoscopy was performed 1-2 days after the symptom onset in most cases. The common conger, Conger myriaster, was the probable infection source in 7 cases. In the review of Korean anisakiasis cases, thus far, total 645 cases have been reported in 64 articles. Anisakis type I larva was the most frequently detected (81.3%). The favorable infection site of larvae was the stomach (82.4%). The common conger was the most probable source of human infections (38.6%). Among the total 404 cases which revealed the age and sex of patients, 185 (45.8%) were males, and the remaining 219 (54.2%) were female patients. The age prevalence was the highest in forties (34.7%). The seasonal prevalence was highest in winter (38.8%). By the present study, 15 cases of gastric anisakiasis are added as Korean cases, and some epidemiological characteristics of Korean anisakiasis were clarified.

A Clinical Study of 19 Cases of Deep Neck Infection (경부 심부 감염 19예에 대한 임상적 고찰)

  • Deep neck infections, which affect soft tissue and facial compartment of head and neck, may cause life-threatening complications despite the advent of antibiotics. The authors recently experienced 19 cases of these infections at Daegu Catholic Hospital and the following results were obtained. 1.The age of onset was from two to sixty one with the mean age of thirty nine, and 13 cases(68%) were male and 6 cases(32%) were female. 2.The sites of infections were submandibular space 11 cases(58%), parapharyngeal space 7 cases(37%), and retropharyngeal space 1 case(5%). 3.The isolated organisms of 12 cases were 6 cases(50%) of hemolytic streptococcus, 3 cases(25 %) of staphylococcus aureus, 3 cases(25 %) of no growth. 4.Without any remarkable complications, all cases were treated with a) surgical drainage with antibiotic therapy 13 cases(68%) b) conservative management 6 cases(32%)
