A Study on the Improvement of Employment Rate of Dental Technology School Graduates in Daegu and the Supply and Demand of Dental Technician Work Forces (대구지역 치기공과 학생의 취업률 향상과 치과기공사의 인력수급에 관한 연구)

  • The purpose of this thesis is to address the imbalance between supply and demand of dental lab technicians and to propose its solutions by analyzing the improvement of employment rate of graduates of dental technology schools in Daegu area and the supply and demand of the technician workforce. Preliminary and full-scale surveys were performed from Aug. 10 to Oct. 10 in 2008 on 150 junior students of dental technology schools who finished their on-the-job-training and 150 dental technicians who were working for dental laboratory in Daegu by self-assessment questionnaire. Followings are the findings of this study. First, regarding the gender of the dental technicians, 32.7% of respondents were female 67.3% were male. Concerning age, respondents younger than 29 years-old occupied the highest percentage(39.3%). With respect to the lab technology school they graduated, DaeguHealthCollege took the largest proportion of 87.3%. With regard to interpersonal relationship at the school, 58.0% of respondents felt their relationship 'smooth'. Second, about the motive of choosing to be a lab technician, the biggest proportion of the respondents(34.7%) replied that 'because it is professional'. Regarding the degree of satisfaction about the profession, 'so-so' was the answer with the biggest percentage(51.3%) followed by 'satisfied'(32.0%) and 'unsatisfied'(16.7%). It showed that respondents were generally satisfied with their major. Third, regarding relevance of on-the-job-training with 'preparation for the employment', highest percentage of both students and lab technicians(50.0%) replied 'so-so'. With respect to 'necessity of practical education', highest percentage of the students and lab technicians replied 'relating didactic education in college to the real world' with 52.7% and 40.7% respectively. With regard to 'the aim of on-the-sport-training', 'practical training of didactic education' was chosen by 54.0% and 47.3% respectively. Fourth, with regard to the 'types of college education advantageous for the employment', both students and lab technicians groups chose 'practical training' with the highest percentage. Concerning 'helpfulness of clinical training on employment', students answered 'so-so' and 'helpful' by 50.0% and 42.7% respectively and dental technicians replied similarly by 50.0% and 46.0% respectively. Fifth, with regard to the influence of financial factors on the supply and demand of dental technician(question #1), 68.0% of respondents agreed that 'salary plays key role in the supply and demand of the work force'. Concerning the question about working condition, respondents requested better working environment. With reference to interpersonal relationship at the laboratory, respondents replied that interpersonal relationship affects the duration of the employment it was found out that there were many conflicts with colleagues or dental office. Sixth, regarding the satisfaction about their profession according to the characteristics of the respondents more respondents were 'satisfied'(10.30 points) with their profession and 'unsatisfied' were 9.47 points and the difference was statistically significant(p<0.05). According to the findings of this study, practical education on campus and on-the-job-training are as important as didactic education at class due to the characteristics of dental technology major. It was found that more practical training is related to higher employment rate. Therefore, efficiency of on-the-job-training by college education should be increased raise employment rate of the graduates and reduce the unemployment. Up-to-date information needs to be promptly delivered and future vision should be presented to the students to raise their satisfaction about the profession. In addition, to reduce dental technician's frequent change of their workplace after facing with the reality of the profession after graduation, related organizations should make an effort to present better future than now.

Effect of a New Spirometric Reference Equation on the Interpretation of Spirometric Patterns and Disease Severity (폐활량측정법의 새로운 정상예측식이 폐활량측정법 장애 양상 및 질병 중증도 해석에 미치는 영향)

  • Background : A spirometric reference equation was recently developed for the general population in Korea. The applicability of the new Korean equation to clinical practice was examined by comparing it with the Morris equation, which is one of the most popular reference equations used for interpreting the spirometric patterns and for grading the disease severity in Korea. Methods : Spirometry was performed on 926 men and 694 women, aged 20 years or older, in November 2004 at the Asan Medical Center, Seoul, Korea. The subjects' age, gender, height, weight, and spirometric values ($FEV_1$ [forced expiratory volume in one second], FVC [forced vital capacity], and $FEV_1/FVC$) were obtained. The spirometric patterns and disease severity were evaluated using both equations, and the results of the Korean equation were compared with the Morris equation. The spirometric patterns were defined as normal, restrictive, obstructive, and undetermined according to the level of $FEV_1/FVC$ and FVC. The disease severity was defined according to the level of $FEV_1$ level for subjects with an airflow limitation, and according to the FVC level for those subjects without an airflow limitation. Results : Spirometric patterns were differently interpreted in 22.5% (208/926) of the men and 24.8% (172/694) of the women after the application of the Korean equation compared with the Morris equation. Of the subjects with airflow limitation, disease severity was differently graded in 30.2% (114/378) of the men and 39.4% (37/94) of the women after the application of the Korean equation. Of the subjects without airflow limitation, disease severity was differently graded in 27.9% (153/548) of the men and 30.2% (181/600) of the women after the application of the Korean equation. Conclusion : Achange in the reference equation for spirometry could have an effect on the interpretation of spirometric patterns and on the grading of disease severity.

Food Habits and Nutrition Knowledge of College Students Residing in the Dormitory in Ulsan Area (울산지역 기숙사 거주 대학생의 식생활 습관과 영양지식)

  • This study has been carried out to obtain information regarding to the food habits including health-related behaviors, the nutrition knowledge and the satisfaction on the dormitory food service. The subjects of this study were 758 college students (580 male and 178 female) residing in the dormitory in Ulsan. The data were obtained through questionnaire and interview, and were analyzed using the SPSS package program. the results of this study are as follows: The average age of the subjects was $20.7\pm2.3$ years old. The average height and weight of male were $175.2\pm5.0\;cm\;and\;69.4\pm9.6\;kg$, respectively. Those of female were $162.6\pm4.1\;cm\;and\;51.8\pm5.9\;kg$, respectively. The BMI value of male was $22.6\pm0.1$ which ranged within normal level, but that of female was $19.6\pm0.2$ which ranged underweight level. In the case of food habits, $26.0\%$ of total subjects always skipped a meal (especially breakfast). The main reasons for skipping meals were no time to eat, no appetite, and weight control. Most of subjects ($69.3\%$) have usually eaten snack after dinner, their most favorite food was noodle (ramen), and the next was pizza. With regard to the health care, there was a significant difference between male and female respect to smoking (p<0.001), drinking (p<0.01) and exercise level (p< 0.001). The mean food habit score of the subjects was $59.9\pm11.1$. Male students had a higher food habit score than female students (male: $60.6\pm10.9$, female: $57.7\pm11.4$), and the group residing longer period had a higher food habit score than the one residing shorter period. Average nutrition knowledge score of subjects was $7.8\pm1.8$. Gender, residing periods, monthly pocket money influenced on the nutrition knowledge score. Female had a higher score than that of male (P<0.001), and the group residing longer period had a higher score than the one residing shorter period (p<0.01), and the group who had more monthly pocket money had a higher score than the one who had less. No significant correlation was found between food habit score and nutrition knowledge score. In the case of dormitory food service, the total subjects showed high satisfaction scores for the sanitation and the atmosphere, while low scores for the menu and the quality of food service. This study may provide basic information on the eating habits and health-related behaviors of college students in dormitory. However, further studies and nutrition counseling are needed to improve food habits and nutrition knowledge.

Analysis of Characteristics and Prognostic Factors in Adult Patients Receiving Mechanical Ventilation in the Medical Intensive Care Unit of a University Hospital (한 대학병원 내과계 중환자실의 기계환기 시행 환자의 현황 및 예후인자의 분석)

  • Background: Respiratory failure is a common condition that requires intensive care, and has a high mortality rate despite the recent improvements in respiratory care. Previous reports of patients with respiratory failure focused on the specific disease or included a large proportion of surgical patients. This study evaluated the clinical characteristics, outcomes and prognostic factors of adult patients receiving mechanical ventilation in a medical intensive care unit. Methods: Retrospective chart review was performed on 479 adult patients, who received mechanical ventilation for more than 48 hours in the medical ICU of one tertiary referral hospital. Results: The mean age of the patients was $60.3{\pm}15.6$ years and 34.0% were female. The initial mean APACHE III score was $72.3{\pm}25$. The cause of MV included acute respiratory failure (71.8%), acute exacerbation of chronic pulmonary disease (20.9%), coma (5.6%), and neuromuscular disorders (1.7%). Pressure controlled ventilation was used as the initial ventilator mode in 67.8% of patients, and pressure support ventilation was used as the initial weaning mode in 83.6% of the patients. The overall mortality rate in the ICU and hospital was 49.3% and 55.4%, respectively. The main cause of death in hospital was septic shock (32.5%), respiratory failure (11.7%), and multiorgan failure (10.2%). Males, an APACHE III score >70, the cause of respiratory failure (interstitial lung disease, coma, aspiration, pneumonia, sepsis and hemoptysis), the total ventilation time, and length of stay in hospital were independently associated with mortality. Conclusion: The cause of respiratory failure, severity of the patients, and gender appears to be significantly associated with the outcome of mechanical ventilatory support in patients with respiratory failure.

The Effectiveness of Sympathetic Skin Response Studies for Patients with Primary Palmar Hyperhidrosis and Who Undergo Thoracic Sympathicotomy (일차성 수부 다한증에서 교감신경절제술 후 교감신경 피부반응 검사의 효용성)

  • Background: There is no standardized tool and parameter that can accurately assess the sympathetic function before and after performing sympathectomy in patients with primary palmar hyperhidrosis. We examined the effective-ness of the sympathetic skin response (SSR) study for documenting the change of sympathetic denervation before and after performing selective thoracic sympathicotomy. Material and Method: We prospectively investigated the SSR from 12 healthy subjects who were diagnosed with primary hyperhidrosis. Each SSR was recorded on the right palm or sole with electrical stimuli applied to the skin at the left wrist and foot and vice versa for the controlateral side. This test was performed before, 2 weeks and 1 year after selective thoracic sympathicotomy. The data was corrected for the onset latency and the amplitude of the SSR (n=24). Result: The mean age of the 12 patients was $24.6{\pm}0.4$ years (range: 19~36) and the gender ratio was 1 : 0.7. The mean values of the preoperative, postoperative 2 weeks and postoperative 1 year onset latency and amplitude of the palmar side (n=24) were $1.46{\pm}0.24$ msec and $6,043{\pm}2,339{\mu}V$, $1.63{\pm}0.42$ msec and $823{\pm}638{\mu}V$, and $1.44{\pm}0.39$ msec and $2,412{\pm}1,546{\mu}V$, respectively. The mean values of the plantar side (n=38) were $1.83{\pm}0.42$ msec and $2,816{\pm}1,694{\mu}V$, $2.16{\pm}0.39$ msec and $1,445{\pm}1,281{\mu}V$ and $1.95{\pm}0.25$ msec and $1,622{\pm}865{\mu}V$, respectively. Among the documented parameters, only the palmar amplitude (p=0.002) showed statistical significance in recording the change of the sympathetic system within the same individual for the pre and postoperative period. Conclusion: The SSR amplitude ratio may be a useful parameter for documenting the efficacy of sympathetic denervation after selective sympathicotomy.

College Students' Dietary Behavior, Health-Related Lifestyles and Nutrient Intake Status by Physical Activity Levels using International Physical Activity Questionnaire (IPAQ) in Incheon Area (국제신체활동설문(IPAQ)을 이용한 신체활동도에 따른 인천지역 대학생의 식행동, 건강관련 생활습관 및 식이섭취 조사)

  • The purpose of this study was to investigate the dietary behavior, health-related lifestyles and nutrient intake status of college students according to physical activity levels. The physical activity levels were measured using the Korean version of the International Physical Activity Questionnaire (IPAQ) short form. The subjects of this study were 438 college students (243 males and 195 females) in Incheon area and were divided by gender and by physical activity levels, which included low, moderate and high physical activity groups. The average age of male and female students were 23.7 and 21.0 years old, respectively. The prevalence of the low physical activity group was 11.5% for males and 30.8% for females and that of the high physical activity group was 38.7% for males and 22.6% for females. According to the criteria for obesity in Asia-Pacific region, 39.1% of the males and 64.1% of the female students investigated were in the normal range of BMI and 30.9% of the males and 4.6% of the female students were in the obesity category. However, percent of body fat in the female students was significantly higher than that of the male students. The soft lean mass of the high physical activity group was higher compared to the other groups. In the low physical activity group, frequency of “eating out” was lower compared to the other groups. The average consumption of energy was 74.4% for males and 80.1% for females of the estimated energy requirement (EER). The mean intakes of vitamin C, vitamin B2, folic acid and Ca were 67.3%, 84%, 52.7% and 36.7% for males and 64.8%, 99.9%, 51.1% and 40.1% for females of recommended intake (RI), respectively. Therefore, it was necessary for obesity prevention as well as health promotion to increase physical activity and to eat an adequate, balanced diet in female college students.

  • The relevance of this scientific research is determined by the negative impact of the COVID-19 pandemic on the current trends and dynamics of world tourism development. This article aims to identify patterns of development of the modern tourist market, analysis of problems and prospects of development in the context of the COVID-19 pandemic. Materials and methods. General scientific methods and methods of research are used in the work: analysis, synthesis, comparison, analysis of statistical data. The analysis of the viewpoints of foreign and domestic authors on the research of the international tourist market allowed us to substantiate the actual directions of tourism development due to the influence of negative factors connected with the spread of a new coronavirus infection COVID-19. Economic-statistical, abstract-logical, and economic-mathematical methods of research were used during the process of study and data processing. Results. The analysis of the current state of the tourist market by world regions was carried out. It was found that tourism is one of the most affected sectors from COVID-19, as, by the end of 2020, the total number of tourist arrivals in the world decreased by 74% compared to the same period in 2019. The consequence of this decline was a loss of total global tourism revenues by the end of 2020, which equaled $1.3 trillion. 27% of all destinations are completely closed to international tourism. At the end of 2020, the economy of international tourism has shrunk by about 80%. In 2020 the world traveled 98 million fewer people (-83%) relative to the same period last year. Tourism was hit hardest by the pandemic in the Asia-Pacific region, where travel restrictions are as strict as possible. International arrivals in this region fell by 84% (300 million). The Middle East and Africa recorded declines of 75 and 70 percent. Despite a small and short-lived recovery in the summer of 2020, Europe lost 71% of the tourist flow, with the European continent recording the largest drop in absolute terms compared with 2019, 500 million. In North and South America, foreign arrivals declined. It is revealed that a significant decrease in tourist flows leads to a massive loss of jobs, a sharp decline in foreign exchange earnings and taxes, which limits the ability of states to support the tourism industry. Three possible scenarios of exit of the tourist industry from the crisis, reflecting the most probable changes of monthly tourist flows, are considered. The characteristics of respondents from Ukraine, Germany, and the USA and their attitude to travel depending on gender, age, education level, professional status, and monthly income are presented. About 57% of respondents from Ukraine, Poland, and the United States were planning a tourist trip in 2021. Note that people with higher or secondary education were more willing to plan such a trip. The results of the empirical study confirm that interest in domestic tourism has increased significantly in 2021. The regression model of dependence of the number of domestic tourist trips on the example of Ukraine with time tendency (t) and seasonal variations (Turˆt = 7288,498 - 20,58t - 410,88∑5) it forecast for 2020, which allows stabilizing the process of tourist trips after the pandemic to use this model to forecast for any country. Discussion. We should emphasize the seriousness of the COVID-19 pandemic and the fact that many experts and scientists believe in the long-term recovery of the tourism industry. In our opinion, the governments of the countries need to refocus on domestic tourism and deal with infrastructure development, search for new niches, formats, formation of new package deals in new - domestic - segment (new products' development (tourist routes, exhibitions, sightseeing programs, special rehabilitation programs after COVID) -19 in sanatoriums, etc.); creation of individual offers for different target audiences). Conclusions. Thus, the identified trends are associated with a decrease in the number of tourist flows, the negative impact of the pandemic on employment and income from tourism activities. International tourism needs two to four years before it returns to the level of 2019.

A Study on the Sanctuary of the Residence in East China Sea Skirts Area (동중국해권 민가의 성역(聖域)에 관한 연구)

  • Jeju Island, in Korea, shows many characteristics that are differentiated from the rest of Korea. Its culture is rooted in mythology which advocates a egalitarian, rather than hierarchical, social structure, the place of women in the home is relatively high, and the formation of buildings, the separation of cooking and heating facilities, and the living format of residential homes is dissimilar. These disparities in culture indicate that Jeju Island's heritage was not formed only from influences from the North, but also from other places as well. To fill in the blanks, residential homes in Jeju Island were compared with those scattered throughout the East China Sea, which connect the southern coastline of the Korean peninsula and Jeju Island. The regions encompassed by the East China Sea, sharing the Kuroshio current and a seasonal wind, can be considered as one cultural region integrating cultural aspects from the continental North and the oceanbound South. The unique characteristics of southern culture as seen in southern residences was examined through an investigation of the sacred places in which gods were considered to dwell. First, the myths of these areas usually concerned with the ocean, and a sterile environment made sustenance impossible without a dual livelihood, usually taking on the forms of half-farming and half-fishing, or half-farming, half-gardening. Although family compositions were strongly matricentric or collateral thanks to southern influence, a patriarchical system like those found in the North were present in the upper classes and in the cities. Therefore, residential spaces were not divided based on age or gender, as in hierarchical societies, but according to family and function. Second, these areas had local belief systems based on animism and ancestor worship, and household deities were closely related to women, agriculture and fire. The deities of the kitchen, the granary and the toilet were mostly female, and the role of priest was often filled by a woman. After Buddhism and Confucianism were introduced from mainland Korea, China and Japan, the sacred areas of the household took on a dual form, integrating the female-focused local rites with male-centered Buddhist and Confucian rites. Third, in accordance with worship of a kitchen deity, a granary deity, and a toilet deity led to these areas of the home being separated into disparate buildings. Eventually, these areas became absorbed into the home as architectural technology was further developed and lifestyles were changed. There was also integration of northern and southern cultures, with rites concerning granary and toilet deities coming from China, and the personality of the kitchen deity being related to the southern sea. In addition, the use of stone in separate kitchens, granaries, and toilets is a distinguishing characteristic of the East China Sea. This research is a part of the results gained from a project funded by the Korea Research Foundation in 2006.

An Analysis of Ten Year Trends of Cancer Incidence and Quality Control of Cancer Registration Data in Jeollabuk-do, Korea: 2001~2010 (전라북도의 10년간(2001~2010) 암 발생률 추이 및 암등록 자료의 질 관리 지표 분석)

  • Objectives: This study was aimed to analyze the trends of cancer incidence and evaluate the quality control of cancer registration data in Jeollabuk-do from 2001 to 2010. Methods: Incidence data of all cancers and indices of quality of cancer registration data in Jeollabuk-do for the 10-year period were obtained from the Population-based Regional Cancer Registry in the Jeonbuk Regional Cancer Center. Trends in crude incidence rate (CR) and age-standardized incidence rate (ASR) for all cancers and incidence rates for major cancer sites by gender were analyzed. Joinpoint regression tool was used to describe and quantify trends. And the completeness and validity of cancer registration data were analyzed. Results: The major cancer sites in males were the stomach (22.2%), lung (16.6%), colorectum (12.8%), liver (12.3%) and prostate (6.2%), and in females were the thyroid (17.8%), stomach (14.7%), breast (11.6%), colorectum (11.5%) and lung (7.7%). Between 2001 to 2010, ASR for all cancers increased 13.7% in men, 68% in women, and 36.5% overall. ASR for all cancers increased by 1.2% per year in males and by 6.7% per year in females from 2001 to 2010. In the quality control of the cancer registration data between 2001 and 2010, death certificate only (DCO%) for men was decreased from 5.6% to 1.3% and DCO% for women decreased from 6.1% to 1.8%. Microscopic verification (MV%) increased in both men and women. And mortality/incidence ratio (MI%) declined in both men and women. Conclusions: The cancer incidence during the 10 years (2001-2010) in Jeollabuk-do was increasing especially for the colorectum and prostate in men, and for the thyroid and breast in women. The overall quality control of the cancer registry was gradually improving.

The Measurement of Normal Talus in Korean Cadaver (한국인 사체에서의 정상 거골의 실측)

  • Purpose: To investigate the measured values of the talus in Koreans. Materials and Methods: We measured 88 tali from 44 cadavers that have been donated between December 2012 and December 2015. Of the cadavers, 27 were male and 17 were female. Their mean age was 73 years. The length and width of the talus were measured using a digital goniometer and vernier caliper. Results: The values of cadaveric measurement, mean maximal width and length, width and length of the dome anterior, width and length of the posterior facet, height and length of the trochlear medial facet, and height and length of the trochlear lateral facet were $43.6{\pm}2.6mm$, $56.5{\pm}3.3mm$, $32.5{\pm}2.0mm$, $42.2{\pm}2.7mm$, $22.2{\pm}2.2mm$, $34.7{\pm}2.0mm$, $15.3{\pm}1.3mm$, $33.3{\pm}2.9mm$, $25.3{\pm}3.3mm$, and $30.8{\pm}2.4mm$ for men and $38.9{\pm}1.6mm$, $53.6{\pm}2.4mm$, $27.9{\pm}2.1mm$, $37.4{\pm}3.2mm$, $20.6{\pm}0.8mm$, $31.9{\pm}1.2mm$, $13.6{\pm}2.6mm$, $28.4{\pm}2.5mm$, $24.9{\pm}2.1mm$, and $28.9{\pm}1.4mm$ for women, respectively. The size of the talus showed an accuracy of 86% when anteroposterior diameter was greater than 59 mm. A difference in the size of the right and left talus was not observed. The mean inclination and declination angles were $24.4^{\circ}{\pm}4.2^{\circ}$ and $28.2^{\circ}{\pm}5.4^{\circ}$ for men, and $24.6^{\circ}{\pm}3.6^{\circ}$ and $24.7^{\circ}{\pm}6.7^{\circ}$ for women (p=0.980, p=0.018), respectively, at least $15^{\circ}$, which showed a big difference for every object up to $37^{\circ}$. Conclusion: This paper, to the best of our knowledge, is the first study to measure the talus in Koreans. There were differences by gender and ethnicity in the in measured talus values. The measurements were smaller than European-Americans and greater than Japanese.
