표면활성제와 저용량 methylprednisolone으로 치료하였던 급성 호흡 곤란 증후군 1례 (A case of acute respiratory distress syndrome treated with surfactant and low dose methylprednisolone)

  • 급성 호흡곤란 증후군은 다양하게 유발되는 신체의 손상에 대한 방어기전으로 염증반응이 유발되어 생성되는 폐포 내의 부종으로 인해 호흡곤란과 저산소증이 초래되는 질환이다. 이 질환에서 표면활성제의 결핍과 폐포 표면 장력의 증가로 인해 폐포 동원에 장애가 오는 것이 결정적인 병인이고 표면활성제의 보충이 저산소증을 개선시킴으로써 중요한 치료 방안이 될 수 있다는 가설이 제시되었고 실제로 소아 환자들에서 저산소증을 개선시키고 증상의 호전을 얻었던 보고들이 여러 차례 있었다. 또한 이 질환의 주요 병인인 폐에서의 염증 반응에 주안점을 두고 스테로이드를 투여해 본 연구들에서 저용량 methylprednisolone으로 증상이 호전되고 생존율이 향상되었다는 결론을 얻은 바 있으나 두 치료 모두 ARDS에서 일반적인 적용은 확립되어 있지 않은 실정이다. 저자들은 급성 호흡곤란 증후군으로 진단받은 61일된 소아가 인공 호흡기 등의 보존적인 치료에 반응하지 않고 지속적으로 증상의 악화를 보이다가 표면활성제 120 mg/kg를 기도 내로 주입한 후 호흡곤란 증상과 저산소증 호전되고 $PaO_2/FiO_2$ 상승, 이후 저용량 methylprednisolone을 투여(2 mg/kg 14일간, 1 mg/kg 7일간, 0.5 mg/kg 7일간, 0.25 mg/kg 2일간, 0.125 mg/kg/d 2일간)한 후 2개월간의 추적 관찰에서 증상의 재발없고 방사선학적 검사상 섬유화 등의 합병증을 보이지 않았던 1례를 보고하는 바이다.

Analysis of ceramide metabolites in differentiating epidermal keratinocytes treated with calcium or vitamin C

  • Ceramides (Cer) comprise the major constituent of sphingolipids in the epidermis and are known to play diverse roles in the outermost layers of the skin including water retention and provision of a physical barrier. In addition, they can be hydrolyzed into free sphingoid bases such as $C_{18}$ sphingosine (SO) and $C_{18}$ sphinganine (SA) or can be further metabolized to $C_{18}$ So-1-phosphate (S1P) and $C_{18}$ Sa-1-phosphate (Sa1P) in keratinocytes. The significance of ceramide metabolites emerged from studies reporting altered levels of SO and SA in skin disorders and the role of S1P and Sa1P as signaling lipids. However, the overall metabolism of sphingoid bases and their phosphates during keratinocyte differentiation remains not fully understood. Therefore, in this study, we analyzed these Cer metabolites in the process of keratinocyte differentiation. Three distinct keratinocyte differentiation stages were prepared using 0.07 mM calcium (Ca$^{2+}$) (proliferation stage), 1.2 mM Ca$^{2+}$ (early differentiation stage) in serum-free medium, or serum-containing medium with vitamin C (50 ${\mu}L$/mL) (late differentiation stage). Serum-containing medium was also used to determine whether vitamin C increases the concentrations of sphingoid bases and their phosphates. The production of sphingoid bases and their phosphates after hydrolysis by alkaline phosphatase was determined using high-performance liquid chromatography. Compared to cells treated with 0.07 mM Ca$^{2+}$, levels of SO, SA, S1P, and SA1P were not altered after treatment with 1.2 mM Ca$^{2+}$. However, in keratinocytes cultured in serum-containing medium with vitamin C, levels of SO, SA, S1P, and SA1P were dramatically higher than those in 0.07- and l.2-mM Ca$^{2+}$-treated cells; however, compared to serum-containing medium alone, vitamin C did not significantly enhance their production. Taken together, we demonstrate that late differentiation induced by vitamin C and serum was accompanied by dramatic increases in the concentration of sphingoid bases and their phosphates, although vitamin C alone had no effect on their production.

칼슘, 베라파밀, 란타눔이 흰쥐 자궁근의 자발적 수축과 칼륨 경축에 미치는 효과 (Effects of $Ca^{++}$, Verapamil and $La^{+++}$ on the Spontaneous Contraction and K-contracture in the Isolated Rat Uterine Smooth Muscle)

  • The effects of $Ca^{++}$ and its antagonists (verapamil and $La^{+++}$) upon the spontaneous contraction and the contracture induced by 60 mM K-Tyrode solution were studied in the isolated uterine muscle. Longitudinal muscle strips were prepared from the rat uteri at estrous stage. All experiments were performed in tris-buffered Tyrode solution which was aerated with 100% $O_2$ and kept at 35^{\circ}$. The results obtained were as follows: 1) In the uterine strips contracting spontaneously, both the amplitude of peak tension and the area of contraction curve increased dose-dependently in the range of $0.5${\sim}8$ mM $Ca^{++}$. The frequency of contraction increased as the concentration of $Ca^{++}$ increased up to 2 mM, but above this concentration the frequency decreased. In $Ca^{++}-free$ media, however, contraction did not develop. In the contracture induced by 60 mM K-Tyrode solution, the developed tension increased dose-dependently as the concentration of external $Ca^{++}$ increased to 8 mM. In the absence of external $Ca^{++}$ K-contracture appeared, but it was not sustained. 2) The spontaneous contraction of rat uterus was suppressed by verapamil in proportion to an increase of its concentration and totally abolished at the concentration of $3{\times}10^{-4}\;g/l$, but the spontaneous contraction re-appeared by addition of $Ca^{++}$. The amplitude of peak tension recovered completely but the recovery of frequency was incomplete. K-contracture decreased in a dose-dependent manner after the treatment with verapamil and totally disappeared at its concentration of $3{\times}10^{-4}\;g/l$. Even in this case contracture developed again by extra $Ca^{++}$. 3) The spontaneous contractile activity was inhibited by $La^{+++}$. At the concentration of $10^{-4}$M $La^{+++}$, fibrillation appeared. In the strip inhibited by $10^{-5}M\;La^{+++}$, contractility recovered completely by extra $Ca^{++}$ while in the $10^{-4}M\;La^{+++}$ treated preparation, the rhythmic spontaneous contraction did not develop even at the concentration of 16 mM $Ca^{++}$. After the initial transient depression of contracture tension by $10^{-3}M$ of $La^{+++}$, the strip stowed considerably large size of contracture, hardly influenced by external $Ca^{++}$ or verapamil. The results obtained in this experiment suggest that in the rat uterine muscle there would be some competitive actions between $Ca^{++}$ and its antagonists. It is speculated that $Ca^{++}$ plays an important role in the conduction of excitation, and $La^{+++}$ influences upon cellular $Ca^{++}$ mobilization and re-uptake process as well as transmembrane $Ca^{++}$ transport in a K-depolarized state.

음식물 소멸기에서 발생하는 응축폐수의 Polyethylene 침지형 중공사막을 이용한 처리 특성 (Processing Characteristics of the Condensed Wastewater Resulting from Food Waste Disposal using a Submerged Polyethylene Hollow Fiber Membrane)

  • 연구는 음식물 쓰레기를 음식물 소멸기에서 미생물에 의해 발효하여 유기물을 감량화하고, 음식물 발효시 발생되는 가스를 냉각기에서 기체와 수분으로 분리하여 기체는 반응기로 다시 보내고 수분은 응축하여 침지식 MF 중공사 분리막이 장착된 MBR 시스템에 적용하여 처리하는 시스템에 관한 연구이다. (주)바이오하이테크에서 제작한 음식물 소멸기와 수처리 장치에 침지식 MF 중공사 모듈을 설치하여 90일간 H연구소 직원식당에서 발생되는 음식물 쓰레기를 연속 투입하여 처리하였다. 음식물 소멸기 초기 Seeding를 위하여 수분조절제로 미강, 왕겨, 톱밥을 305 kg 투입하였고 음식물은 운전기간동안 1,648 kg투입하였고 응축폐수는 1,600 L 발생되었다. 음식물 소멸기 운전 종료 후 배출된 발효 부산물은 386 kg으로 감량율은 약 80%로 조사되었다. 침지식 MF 중공사 분리막 모듈을 응축폐수의 MBR 시스템에 적용하여 유기물 처리한 결과 제거율은 각각 BOD 99.9%, COD 97.5%, SS 98.6%, T-N 54.6%, T-P 34.7%였으며, 총대장균은 100%가 제거되었다.

구강 악안면 영역의 암종 진단에 있어서 $[^{18}F]$-Fluorodeoxyglucose를 이용한 양전자방출 단층촬영의 임상적 연구 (CLINICAL STUDY OF POSITRON EMISSION TOMOGRAPHY WITH $[^{18}F]$-FLUORODEOXYGLUCOSE IN MAXILLOFACIAL TUMOR DIAGNOSIS)

  • Positron Emission Tomography(PET) is a new diagnostic method that can create functional images of the distribution of positron emitting radionuclides, which when administered intravenously in the body, makes possible anatomical and functional analysis by quantity of biochemical and physiological process. After genetic and biochemical changes in initial stage, malignant tumor undergoes functional changes before undergoing anatomical changes. So, early diagnosis of malignant tumors by functional analysis with PET can be achieved, replacing traditional anatomical analysis, such as computed tomography(CT) and magnetic resonance image(MRI), etc. Similarly, PET can identify malignant tumor without confusion with scar and fibrosis in follow up check. In the Korea Cancer Center Hospital(KCCH) from October 1997 to September 1999, clinical study was performed in 79 cases that underwent 89 times PET evaluation with [18F]-Fluorodeoxyglucose for diagnosis of oral and maxillofacial tumors, and the data was analysed by Bayesian $2{\times}2$ Classification Table. The results were as follows : Evaluation for initial diagnosis with FDG-PET (P<0.005) 1. Agreement rate or accuracy rate is 88.9%. 2. Sensitivity is 95.2%, and specificity 66.7%. 3. Positive predictive rate is 90.9%, and negative predictive rate 80.0%. 4. In consideration of tumor stage, diagnostic rate in less than stage II was 90% and in greater than stage III 100%. 5. In consideration of tumor size, diagnostic rate in less than T2 was 92.3% and in greater than T3 100%. After primary treatment, evaluation for follow up check with FDG-PET (P < 0.001) 1. Agreement rate or accuracy rate is 85.4%. 2. Sensitivity is 87.5%, and specificity 82.4%. 3. Positive predictive rate is 87.5%, and negative predictive rate 82.4%. 4. In 24 recurred cases, 6 had distant metastasis, and 5 of them were diagnosed with FDG-PET, resulting in diagnostic rate of FDG-PET of 83.3%. From the above results, Positron Emission Tomography with [18F]- Fluorodeoxyglucose appears to be more sensitive and accurate for detecting the presence of oral and maxillofacial tumors, and has various clinical applications such as early diagnosis of tumor in initial and follow up check and detection of distant metastasis.

Kinetic analysis of 64Cu-NODAGA-gluco-E[c(RGDfK)]2 for a tumor angiogenesis PET tracer

  • Molecular imaging with the radiolabeled RGD peptides for ${\alpha}_v{\beta}_3$ integrin has been an increasing interest for tumor diagnosis and the treatment monitoring. Recently, $^{64}Cu$-NODAGA-gluco-E[c(RGDfK)]$_2$ was developed for quantification of ${\alpha}_v{\beta}_3$ integrin and its biological properties was elucidated. To better understand the molecular process in vivo, we performed the kinetic analysis for the $^{64}Cu$-NODAGA-gluco-E[c(RGDfK)]$_2$. After preparation of a radiotracer, dynamic PET images were obtained in the U87MG xenograft mice for 60 min (n = 6). Binding potential values were estimated from the 3-tissue compartment model, reference Logan and simplified reference tissue model. In the early time frame (0-20 min), the liver, kidney, intestine, urinary bladder and tumor were visualized but these uptakes were diminished as time went by. The tumors showed a good contrast at 40 min after administration. $^{64}Cu$-NODAGA-gluco-E[c(RGDfK)]$_2$ showed the 2-fold uptake in the tumor compared with that in the muscle. The parametric maps for binding values also provide the higher tumor-to-background contrast than the static images. A binding value obtained from the 3-tissue compartment model was comparable to other modeling methods. From these results, we conclude that $^{64}Cu$-NODAGA-gluco-E[c(RGDfK)]$_2$ may be a promising PET radiotracer for the evaluation of angiogenesis.

The Effect of Remifentanil Preconditioning on Injured Keratinocyte

  • Background: Incisional site of surgical operation become transient ischemic state and then occur reoxygenation due to vasodilatation by inflammatory reaction, the productive reactive oxygen species (ROS) give rise to many physiologic results. Apoptosis have major role on elimination of inflammatory cell and formation of granulation tissue in normal wound healing process. Remifentanil can prevent the inflammatory response and can suppress inducible nitric oxide synthase expression in a septic mouse model. After cardiopulmonary bypass for coronary artery surgery, remifentanil can also inhibit the release of biomarkers of myocardial damage. Here we investigated whether remifentanil pretreatment has cellular protective effect against hypoxia-reoxygenation in HaCaT human keratinocytes, if so, the role of apoptosis and autophagy on this phenomenon. Methods: The HaCaT human keratinocytes were exposed to various concentrations of remifentanil (0.01, 0.05, 0.1, 0.5 and 1 ng/ml) for 2 h before hypoxia (RPC/HR group). These cells were cultured under 1% oxygen tension for 24h at $37^{\circ}C$. After hypoxia, to simulate reoxygenation and recovery, the cells were reoxygenated for 12 h at $37^{\circ}C$. 3-MA/RPC/HR group was treated 3-methyladenine (3-MA), autophagy inhibitor for 1h before remifentanil treatment. Cell viability was measured using a quantitative colorimetric assay with thiazolyl blue tetrazoliumbromide (MTT, amresco), showing the mitochondrial activity of living cells. To investigate whether the occurrence of autophagy and apoptosis, we used fluorescence microscopy and Western blot analysis. Results: The viability against hypoxia-reoxygenation injury in remifentanil preconditioning keratinocytes were increased, and these cells were showed stimulated expression of autophagy 3-MA suppressed the induction of autophagy effectively and the protective effects on apoptosis. Atg5, Beclin-1, LC3-II and p62 were elevated in RPC/HR group. But they were decreased when autophagy was suppressed by 3-MA. Conclusions: Remifentanil preconditioning showed the protective effect in human keratinocytes, and we concluded that autophagy may take the major role in the recovery of wound from hypoxia-reoxygenation injury. We suggest that further research is needed about the cell protective effects of autophagy.

공동주택의 분별해체 시험시공을 통한 건설폐기물 발생량 비교 분석 (A Comparative Analysis on Generated Construction Waste Quantities in a Case Study for Deconstruction of an Apartment)

  • 건설폐기물의 재활용과 재사용 비율을 높이기 위해서는 건축물의 분별해체 공법이 적용되어야 한다. 본 연구에서는 공동주택을 대상으로 시험시공을 실시하였다. 먼저 해체 과정에서 발생할 수 있는 모든 건설페기물을 건물의 각 부위별로 조사하였다. 해체대상 건물의 도면을 작성하고 이 도면을 분석하여 각 건설페기물의 발생량을 예측하였다. 공동주택의 한 빌딩씩 분별해체와 일반해체과정을 실시하면서 이 과정에 투입되는 인력 및 장비의 소요시간을 측정하고, 또한 각 해체과정에서 발생된 폐기물을 정밀 계측하여 이것의 체적, 단위중량 등을 제시하였다. 이러한 자료의 분석을 통해 도면분석 결과와 현장에서 발생된 건설페기물의 양을 비교분석하여 현장 회수율, 체적 및 중량 환산계수, 폐기물 원단위를 제안하였다. 이러한 계수는 건설페기물의 발생량 예측과 운반비 산정, 작업 공정계획 수립 등을 위한 기초적 자료로서 활용될 수 있을 것이다.

구강 내 디지털 인상채득을 통한 상악 전치부 임플란트 즉시 임시 보철 수복 증례 (Digital intraoral impression for immediate provisional restoration of maxillary single implant: A case report)

  • 디지털 기술의 발달로 무치악 부위의 임플란트 수복 시 그 진단과 치료 과정 전반에 걸쳐 구강 내 3차원 정보를 데이터화하여 보철물을 제작하는 것이 가능해졌다. 전통적인 인상채득 방식을 이용하여 모델을 만들고 이것을 스캔하여 보철물을 제작하던 CAD/CAM 이용 방식에서 벗어나 디지털 인상 코핑(scanbody)을 사용함으로써 모델 없이 디지털 인상을 채득하고 임플란트 임시 보철물 및 해부학적 지대주를 제작하는 술식으로 변화하고 있다. 그러나 아직 이러한 술식은 활발히 이루어지고 있지 않으며 특히 심미성이 요구되어 연조직 외형의 인기가 중요한 상악 전치부 임플란트 즉시 부하에서의 사용은 그 사례가 많지 않다. 이에 따라 본 증례 보고에서는 3명의 상악 전치부 단일 임플란트 수복 환자에서 디지털 인상 코핑(scanbody)을 체결한 후 구강 내 스캐너인 CEREC Omnicam (Sirona, Bensheim, Germany) 또는 Trios (3shape, Copenhagen, Denmark)로 디지털 인상채득을 통해 임플란트 지대주 및 임시 보철물을 제작하였다. 그 결과 디지털 인상채득을 통한 편리성, 정확성 그리고 최종 보철물의 양호한 심미성을 얻어 보고하고자 한다.

산소 챔버의 근육 피로회복효과에 대한 연구 (The Study of Effect on Oxygen Chamber for Recovery of Muscular Fatigue)

  • Objectives: This Study was designed to investigate the effect of Oxygen chamber to recover muscle fatigue. Methods: Twenty Subjects were divided into Oxygen Chamber Group (n=10) and Rest group (n=10). Subjects visited hospital two times, blood tests were performed 3 times for each visit. 1st blood test was performed in 4 hours hunger state. 2nd blood test was performed within 5 minutes after the 6 minutes bike exercise. 3rd blood test was performed after the 40 minutes Oxygen treatment or 40 minutes rest in bed. Blood test items were Lactate, Glucose, Lactate Dehydrogenase, Creatine Kinase, Free Fatty Acid. 2nd visit was applied 1st visit process, But Oxygen Chamber group was crossed over to Rest Group, Rest Group was crossed over to Oxygen Chamber group. The Blood test result was analyzed with paired T-test using SPSS for Windows version 21. Results: The reduction of Lactate in Oxygen Chamber Group (6.86±2.07 mmol/ℓ) was higher than Rest group (6.57±2.33 mmol/ℓ), but it was not statically significant (p=0.68). The reduction of Glucose in Oxygen Chamber Group (6.85±12.14 mg/dl) was lower than Rest group (7.60±9.83 mg/dl), but it was not statically significant (p=0.83). The reduction of Lactate Dehydrogenase in Oxygen Chamber Group (16.10±14.91 mmol/ℓ) was lower than Rest group (19.75±12.46 mg/dl), but it was not statically significant (p=0.41). The reduction of Creatine Kinase in Oxygen Chamber Group (13.40±5.69 U/ℓ) was lower than Rest group (15.25±8.01 U/ℓ), but it was not statically significant (p=0.41). The reduction of Free Fatty Acid in Oxygen Chamber Group (285.50±174.13 uEq/ℓ) was higher than Rest group (196.15±131.58 uEq/ℓ), but it was not statically significant (p=0.07). Conclusions: This study showed Oxygen chamber therapy could be effective to recover muscle fatigue.
