Dental Health Determinants of Elderly People (노인의 주관적 치아건강수준에 영향을 미치는 요인)

  • This study was Dental health determinants of elderly people. For this purpose, a questionnaire was done for 396 elderly people of 60 or more years old who have visited Seogwipo city public health center(branch) in Jejudo from May 27th to June 5th 2013 after they heard the intention of that investigation. Result of the most answers were 'normal' for the question asking the subjective status of teeth health of whom having more than 20 natural teeth as 73.3%; the most answers of whom with less than 19 natural teeth were 'bad' as 41.4%, which showed difference(p<0.05). The factors influencing the subjective teeth health were 4.3times higher in cases of women than men. The higher educational(high school or more)had they, the more monthly average income(2 million won or more) did they have, the higher was their subjective teeth health status(p<0.01). It is more important than anything that they reserve more than 20 natural teeth until they get old to keep and improve their oral and body health. In addition to that, the oral health project or oral health program to perform country-wide programs for free, consistent and systematic oral health examination and education should be planned.

The Effects of the Total Patterns of Proprioceptive Neuromuscular Facilitation on Balance Improvement in Patients with Low back pain (고유수용성 신경근 촉진법 통합패턴이 요통환자의 균형에 미치는 영향)

  • Purpose : The purpose of this study was to find out The effect of Low back pain on the total pattens of Proprioceptive neuromuscular facilitation(PNF). Methods : The subjects of the study were 62 low back pain patients. They were divided into two groups: 31 in the experimental group and 31 in the control group. the experimental group performed modalities and PNF total patterns three times for a week. the control group performed only modalities and ROM exercise three times for a week. Back muscle strength was measured by a back muscle strength measuring machine, the intensity of pain was measured by the Visual Analogue Scale (VAS), and the level of disability was measured by Oswestry low back pain disability index. Study measurements were compared before and after 6 weeks exercise program. The two groups of subjects were assessed by utilizing two different balance measurement: Static standing balance was measured by balance performance monitor (BPM) and Dynamic standing balance was measured by one leg standing(OLS). The scale for static standing balance was measured by using, sway area, sway path, max velocity. Results : The results of this study were as follow: 1) The score on visual analogue scale shows statistically significant increase on PNF group of post test(p<.05). 2) The score on Oswestry low back pain disability index. shows statistically significant decrease on PNF group of post test(p<.05). 3) The change sway area was statistically significant on pre-test and post-test(p<.05). 4) The change sway path was statistically significant on pre-test and post-test(p<.05). 5) The change max velocity was statistically significant on pre-test and post-test(p<.05). 6) The score on one leg standing shows statistically significant increase on PNF group of post test(p<.05). Conclusion : These results of this study indicated that Proprioceptive Neuromuscular Facilitation Total patterns which performed for six weeks had a statistically significant influence on low back pain. If the exercise for muscle strength is performed along with therapeutic stabilizing exercise, a better effect can be expected on low back pain. We hope that this study will provide a basic data for further research with a bigger group and on a long-term effect.

Cause Analysis and Development of Root Cause Analysis Map using Data of Chemical Laboratory Accidents (화학실험실 사고 Data를 이용한 근본원인분석 Map 개발 및 원인 분석)

  • To develop a Root Cause Analysis Map which determines the cause of the accident in chemical laboratory, The Root Cause Analysis(RCA) Map for the laboratory areas was sketched from Phase 1 of the accident element to Phase 3 of the accident element, based on the RCA Map which is applied in the petrochemical industry. On the basis of laboratory RCA Map which was classified by using such method. The root causes of the 211 accident cases in laboratories were classified from Phase 4 to Phase 5 by the Cause Factor Charting technique and The cause of the accident data were inputted to EXCEL program. After that, The causes of the accident data were sorted and classified by type and each step. So 'Approximate Primary RCA Map Draft' was written. In addition, it was reaffirmed whether the root causes of 211 accidents of laboratory were appropriate to 'Primary RCA Map Draft'. By complementing the cause which was expected to cause future accidents, the RCA Map for chemical laboratories was developed. Based on 'RCA Map' proposed in this study, the causes of accidents were analysed management systems 35%, monitoring 12.2%, Human Factor Eng. 15.1% and education training 12.1% by the size of the frequency from Phase 1 to Phase 5.

The effect of local application of thymoquinone, Nigella sativa's bioactive component, on bone healing in experimental bone defects infected with Porphyromonas gingivalis

  • Purpose: This study was performed to evaluate the influence of local application of thymoquinone (TQ) on bone healing in experimental bone defects infected with Porphyromonas gingivalis (PG). Methods: Forty-two female rats were randomly divided into 6 groups. A bone defect was created on the right tibia of all animals. The PG, PG/collagen membrane (COL) and PG/TQ/COL groups were infected with PG. In the COL and PG/COL groups, the defects were covered with a COL; in the TQ/COL and PG/TQ/COL groups, the defects were covered with a TQ-containing COL. After 28 days, all animals were sacrificed. Quantitative measurements of new bone formation and osteoblast lining, as well as semiquantitative measurements of capillary density and tissue response, were analyzed. Furthermore, the presence of bacterial infections in defect areas was evaluated. Results: The new bone formation, osteoblast number, and capillary density were significantly higher in the TQ groups than in the control groups (P<0.001, P<0.001, and P<0.01, respectively). In a comparison between the TQ/COL group, with a TQ-containing COL (TQ/COL), and the PG-infected TQ-containing COL (PG/TQ/COL) group, the newly formed bone and capillary density were higher in the TQ/COL group (P<0.01). When the control group was compared to the PG, PG/COL, and PG/TQ/COL groups in terms of tissue response, the differences were statistically significant (P<0.001, P=0.02, and P=0.041, respectively). The intensity of the inflammatory cell reaction was higher in the PG, PG/COL, and PG/TQ/COL groups (P<0.05). Conclusions: Within the limitations of this study, the local application of a TQ-containing COL positively affected bone healing even if the bone defects were infected. The results suggest that TQ increased angiogenesis and showed promise for accelerating bone defect healing. Further research is warranted to support these findings and reach more definitive conclusions.

Prognostic Value of Sarcopenia and Myosteatosis in Patients with Resectable Pancreatic Ductal Adenocarcinoma

  • Objective: The clinical relevance of myosteatosis has not been well evaluated in patients with pancreatic ductal adenocarcinoma (PDAC), although sarcopenia has been extensively researched. Therefore, we evaluated the prognostic value of muscle quality, including myosteatosis, in patients with resectable PDAC treated surgically. Materials and Methods: We retrospectively evaluated 347 patients with resectable PDAC who underwent curative surgery (mean age ± standard deviation, 63.6 ± 9.6 years; 202 male). Automatic muscle segmentation was performed on preoperative computed tomography (CT) images using an artificial intelligence program. A single axial image of the portal phase at the inferior endplate level of the L3 vertebra was used for analysis in each patient. Sarcopenia was evaluated using the skeletal muscle index, calculated as the skeletal muscle area (SMA) divided by the height squared. The mean SMA attenuation was used to evaluate myosteatosis. Diagnostic cutoff values for sarcopenia and myosteatosis were devised using the Contal and O'Quigley methods, and patients were classified according to normal (nMT), sarcopenic (sMT), myosteatotic (mMT), or combined (cMT) muscle quality types. Multivariable Cox regression analyses were conducted to assess the effects of muscle type on the overall survival (OS) and recurrence-free survival (RFS) after surgery. Results: Eighty-four (24.2%), 73 (21.0%), 75 (21.6%), and 115 (33.1%) patients were classified as having nMT, sMT, mMT, and cMT, respectively. Compared to nMT, mMT and cMT were significantly associated with poorer OS, with hazard ratios (HRs) of 1.49 (95% confidence interval, 1.00-2.22) and 1.68 (1.16-2.43), respectively, while sMT was not (HR of 1.40 [0.94-2.10]). Only mMT was significantly associated with poorer RFS, with an HR of 1.59 (1.07-2.35), while sMT and cMT were not. Conclusion: Myosteatosis was associated with poor OS and RFS in patients with resectable PDAC who underwent curative surgery.

A Study on the Adolescent Growth Spurt of Skinfold, Muscle and Bone Variables Aligned on Peak Height Velocity in Boys and Girls (PHV 척도를 기준한 피하지방후, 근과 골 변인의 사춘기 발육분출에 관한 연구)

  • The aim of this study was to examine the timing and magnitude of growth spurt in skinfold, body musle and bone related variables aligned on peak height velocity in boys and girls. In the study design, the subjects and the method were used by the cross-sectional investigation. The subjects participated in this study were 7 through 18 years of age belonged to typical primary, junior, senior high school students, and about 250 males and 250 females in each age group. The total subjects were 2,798 males and 2,762 females. All subjects of this study were lived in Pusan metropolitan city, Korea. The growth velocity magnitudes of sum of the four sites sknfold thickness, body musle and bone related variables. Velocity curve chart of physique was the smoothed according to an approximation of splines by the Sigma Plot-2001 graphic program. In this study, age at PHV of girls occurred eariler about 2 years than boys. In sum of four sites skinfold thickness, PV occurred -2 years from PHV in boys, and PV occurred +2 years from PHV in girls, respectively. In humerus breadth, two PV occurred before and after PHV in boys, whereas PV occurred -1 year from PHV in girls, respectively. In femur breadth, PFV and PHV appeared to occure the same time in boys, PV occurred -2 years from PHV in girls, respectively, In arm circumference, PV occurred after PHV in both sexes. In calf circumference, PV occurred +2 years from PHV in boys, PCCV and PHV appeared to occure the same time in girls, respectively. In magnitudes of peak velocity of body height, humerus breadth, femur breadth, arm circumference and calf circumfence, boys obtained higher than girls, on the other hand, girls obtained higher than boys in sum of four sites skinfold variable. we need to longitudinal and scientific investigation by Korean government level in adolescent growth spurt study, because childhood and adolescence achive higher positive physical education effect than the other ages.

Changes in Psoas Major and Quadriceps Cross Sectional Area in Elderly People after 12 Weeks of Exercise (고령자를 대상으로 12주간 운동이 대요근 및 대퇴부 근황단면적에 미치는 영향)

  • The purpose of this study was to investigate whether 12-weeks of movement training would increase the psoas major cross-sectional area (CSA) in senior men and women. Fifty eight men and women aged 65 to 80 years old ($69.6{\pm}3.7$, 30 male, 28 female) were divided into a control (n=19) and exercise group (n=39). Subjects were assessed before and after the training program for stature, body mass, and magnetic resonance imaging of the psoas major and the quadriceps muscle. The experimental group performed exercises using machines designed to improve the movement of the hip at a frequency of twice every week, with a total of 23 trainings in 12-weeks. Magnetic resonance images of both thighs and the abdomen and psoas major were obtained, aimed at 50% of the length of the greater trochanter and the lower edge of the femur and between the fourth (L4) and fifth (L5) lumbars. A 9.4% increase in the psoas major CSA in the training group was observed. In the male and female breakdown, a 11.5% and 8.4% change was observed in males and females, respectively. In the quadriceps, there was no significant statistical improvement in either males or females. Furthermore, in the control group, there was no significant change seen in either the psoas major or the quadriceps. As a result of conducting training that enables upkeep of posture and smooth linkage of the lumbar spine, the pelvis and thighbone, the psoas major CSA of older adults were improved in a short period of time. For this reason, the possibility of improving the psoas CSA, which decreases remarkably with increased age, by improving the linkage of the body trunk is also suggested.

Co-authorship patterns and networks of Korean radiation oncologists

  • Purpose: This research aimed to analyze the patterns of co-authorship network among the Korean radiation oncologists and to identify attributing factors for the formation of networks. Materials and Methods: A total of 1,447 articles including contents of ‘Radiation Oncology' and 'Therapeutic Radiology' were searched from the KoreaMed database. The co-authorship was assorted by the author's full name, affiliation and specialties. UCINET 6.0 was used to fi gure out the author's network centrality and the cluster analysis, and KeyPlayer 1.44 program was used to get a result of key player index. Sociogram was analyzed with the Netdraw 2.090. The statistical comparison was performed by a t-test and ANOVA using SPSS 16.0 with p-value < 0.05 as the significant value. Results: The number of articles written by a radiation oncologist as the first author was 1,025 out of 1,447. The pattern of coauthorship was classified into five groups. For articles of which the first author was a radiation oncologist, the number of singleauthor articles (type-A) was 81; single-institution articles (type-B) was 687; and multiple-author articles (type-C) was 257. For the articles which radiation oncologists participated in as a co-author, the number of single-institution articles (type-D) was 280 while multiple-institution articles (type-E) were 142. There were 8,895 authors from 1,366 co-authored articles, thus the average number of authors per article was 6.51. It was 5.73 for type-B, 6.44 for type-C, 7.90 for type-D, and 7.67 for type-E (p = 0.000) in the average number of authors per article. The number of authors for articles from the hospitals published more than 100 articles was 7.23 while form others was 5.94 (p = 0.005). Its number was 5.94 and 7.16 for the articles published before and after 2001 (p = 0.000). The articles written by a radiation oncologist as the first author had 5.92 authors while others for 7.82 (p = 0.025). Its number was 5.57 and 7.71 for the Journal of the Korean Society for Therapeutic Radiology and Oncology and others (p = 0.000), respectively. Among the analysis, a significant difference in the average number of author per article was indicated. The out-degree centrality of network among authors was 4.26% (2.03-7.09%) while in-degree centrality was 1.31% (0.53-2.84%). The three significant nodes were classified and listed as following: Choi, Eun Kyung for 1991-1995, Kim, Dae Young for 1998-2001, Park, Won and Lee, Sang Wook for 2003-2010. Choi, Eun Kyung and Kim, Dae Young appeared in two cases, and ranked as the highest degree in centrality. In the key player analysis, Choi, Eun Kyung and Lee, Sang Wook appeared in two cases, and ranked as the highest. From the cluster analysis, Sungkyunkwan University, Seoul National University and Yonsei University revealed as the three large clusters when Ulsan University, Chonnam National University, and Korea Institute of Radiological & Medical Science as the medium clusters. Conclusion: The Korean radiation oncologist's society shows a closed network with numerous relationships among the particular clusters, and the result indicates it is different from other institutions in the pattern of co-authorship formation of the major hospitals.

Factors Analysis Related to Health Administration Students' Satisfaction on Hospital Practice (보건행정 전공 대학생들의 병원실습 만족도에 영향을 미치는 요인분석)

  • Purpose .This study aims to assess the influence factors on the level of satisfaction with hospital training and to provide basic data for more efficient hospital practice of students' public health administration . Methods . We have conducted a survey on students satisfaction with their hands on practice against 200 students in Busan, Kyeongnam and Kyungbook area, who have completed their practice in public health administration. We have identified the general characteristics of the target respondents, the current training status of the department for the hospital practice and training venue; also analyzed the characteristics composed of the apprentice student's attitude before the practice, satisfaction with the department training program and hospital for practice, as well as their practice at hospital. The general characteristics of the target respondents and clinic practice status at the hospital were measured in percentage and frequency analysis; the average and standard deviations of hospital size were also measured; and the correlations between satisfaction with the department, attitude towards the practice and satisfaction with the hospital were analyzed. Results . Most public health administration students experienced their practice at the hospital after the first semester on the $2^{nd}$year and they have received the foundation training for the practice. The survey allowed multiple responses and the results are as follows: medical terminology 49.5%, medical coordinator 36.7%, Hospital administration 26.5%, Health insurance 17.3%, and medical recording and practice 13.8% respectively. In terms of each individual student's attitude towards the practice at the hospital, the respondents answered as follows: I have completed the obligatory subjects for the practice before the practice 89.5%; I have been fully informed on the hospital for my practice. 74.5%; I responsibly practiced and worked with professionalism and sense of belonging to the hospital as a member of staff 90%; I have actively learned and adopted 95%; My knowledge and skills from the practice was relevant to the theory and practice from my course 83.5%. The findings of satisfaction by size of hospital were in order of a small clinic($3.24{\pm}0.63$), medium sized clinic($3.27{\pm}0.65$), hospital ($3.20{\pm}0.61$), and large sized comprehensive hospital ($2.93{\pm}0.74$). Consequently the satisfaction rate shows no significant difference by size of hospital. In relations between practice attitude and satisfaction with subject, the matter of acquiring information and fully understanding of the hospital for practice before practice(p=0.04), the matter of a sense of belonging and responsibility at hospital during practice(p=0.33). the matter of active adaptation attitude during practice (p=0.42), and the matter of correlations with school curriculum during practice(p=0.00) showed significant results. In relations between practice attitude and satisfaction with the hospital for practice, the matter of acquiring information and fully understanding of the hospital for practice before practice(p=0.01), the matter of a sense of belonging and responsibility at hospital during practice(p=0.04), and the matter of correlations with school curriculum during practice(p=0.00) showed significant results. Conclusion . The most essential subject for the practice in public health administration is medical terminology and there is no significance in satisfaction with the practice by size of hospital. Students were content with the hospital where they can experience as much as possible. Students content with their major show positive attitude towards the practice and so do those content with the hospital. Those with the positive attitude towards the practice show the high correlation of satisfaction with both the major and hospital. As a result, the satisfaction with the major is the significant attribute to the practice in the hospital.

The Visitors Characteristics of Urban Ecological Park - The Cases of Gildong Ecological Park, Yeouido Tributary Ecological Park - (도시 생태공원 이용자 특성 연구 - 길동생태공원, 여의도샛강생태공원을 사례로 -)

  • Ecological parks enable the sustainable use of natural resources and can be used as a place for site based environmental education. This study presented an opportunity to expand the scope of user perceptions and leisure benefits into urban ecological parks. However, in-depth studies which are aimed at eco-park visitors seeking the leisure benefits of the eco experience programs are lacking. The primary purpose of this study was to examine the urban ecological park user' characteristics which were derived by analyzing the leisure benefits and attitudes for the environment. Also, the attitudes about the environment, and the leisure benefits of visiting urban ecological parks was found to influence visitors' overall satisfaction. To accomplish the purposes of this study, a questionnaire survey with 282 samples was carried out on the urban ecological park(Gildong ecological park, Yeouido tributary ecology park). The results of the analyses are as following: urban ecological park visitors had a positive attitude about the environment, and felt satisfied by the visitors' leisure benefits. The study also examined the relationships between visitors' expectations of the leisure benefits both before visiting and after visiting. The results showed that ecological park visitors had greater expectations regarding the leisure benefits of the ecological program. Leisure benefits have a positive influence on overall satisfaction, and a positive attitude for the environment does not affect satisfaction. This study reaches the conclusion that the ecology experience program utilizing ecological parks is very effective with regard to overall satisfaction in urban areas toward the general environment and ecological parks. One of the major implications of this study is to offer a way to improve user leisure benefits and promote ecological programs by developing ecological parks for the future.
