Excessive Bleeding after Implant Placement in the Anterior Mandible: Case Report (하악 전치부 임플란트 식립 후에 발생한 과다출혈: 증례보고)

  • Implant placement on the anterior mandible is considered a common and safe surgical procedure. However, severe hemorrhage can occur if branches of the sublingual artery, which run through the lingual cortical plate of the mandible, are damaged. Excessive hemorrhage caused by injury to the sublingual artery can result in life-threatening problems such as airway obstruction. A 54-year old male patient without any generalized systemic conditions was referred due to active bleeding after implant placement in the anterior mandible. Gauze compression with surgicel and bosimin were performed and hemostasis was achieved. The patient was discharged after 3 days without any supplementary bleeding.

Isolation and Characterization of Bacillus cereus A-139 Producing Auxin from East Coast Sand Dunes (동해안 사구로부터 Auxin을 생산하는 Bacillus cereus A-139의 분리 및 그 특성)

  • A bacterium, which was named to be Bacillus cereus A-139, secreting auxin was isolated from the east coast sand dunes in Korea. The secretion of auxin was confirmed by HPLC. When cultured in LB broth, Bacillus cereus A-139 produced $16.12\;{\mu}$g/mL auxin after 8 days in LB broth. Bacillus cereus A-139 produced $49\;{\mu}$g/mL auxin and $162.6\;{\mu}$g/mL by the addition of 2% tryptone and 0.1% tryptophan, respectively. The root growth of Arabidopsis thaliana was retarded by Bacillus cereus A-139 culture broth up to 57% but the formation of lateral roots was increased up to almost twice after 4 days incubation. Also the formation of lateral roots of mung bean was increased up to 57% after 10 days incubation.

Fatal Traumatic Subarachnoid Hemorrhage due to Acute Rebleeding of a Pseudoaneurysm Arising from the Distal Basilar Artery

  • Isolated traumatic pseudoaneurysms of the basilar artery are extremely rare but often fatal resulting in a mortality rate as high as 50%. A 51-year-old man presented with craniofacial injury after blunt trauma. A brain computed tomography (CT) scan showed thick basal subarachnoid hemorrhage associated with multiple craniofacial fractures, while CT angiography revealed contrast extravasation at the distal basilar artery with pseudoaneurysm formation. After this primary survey, the condition of the patient suddenly deteriorated. Conventional angiography confirmed the contrast extravasation resulted from pseudoaneurysm formation, which was successfully treated with endovascular coil embolization. Decompressive craniectomy and coma therapy with propofol were also performed. However, the patient died on the 7th hospital day because of the poor initial clinical condition. The current case is the first report of acute pseudoaneurysm rupture arising from the basilar artery within the first day after trauma. Our findings suggest the possibility that pseudoaneurysm rupture should be considered if brain CT shows thick traumatic subarachnoid hemorrhage on the basal cistern with a basal skull fracture.

The Effect of 12 Weeks Rope Skipping and Band Exercise on GH, IGF-1 and Metabolic Syndrome Risk Factors in Female Obese Middle School Students (12주간 줄넘기, 밴드운동이 비만 여중생의 GH, IGF-1 및 대사증후군 위험 인자에 미치는 영향)

  • The purpose of this study was to investigate the effect of rope skipping and band exercise on GH, IGF-1 and metabolic syndrome risk factors in female obese middle school students. Twenty female obese middle school students(%BF > 30%) were randomly assigned to a exercise group(EX, n=10) or control group(CON, n=10). The exercise group has performed rope skipping and band exercise for 12 weeks, 3time per week. Exercise intensity was increased gradually, from 40% to 70% of Heart Rate Reserve(HRR) and Rating of Perceived Exertion(RPE) 11-15. GH, IGF-1 and metabolic syndrome rist factors were GH, IGF-1 and metabolic syndrome risk factors were measured pre- and post-exercise program including rope skipping and band exercise. Levels of GH and IGF-1 were significantly increased (p<.01 and p<0.001, respectively) in exercise group after 12 weeks of exercise training. WC and TG were significantly decreased(p<.01) in exercise group after 12 weeks of training. HDL-C was significantly increased(p<.05) in exercise group after 12 weeks of training. In conclusion, rope skipping and band exercise were effective exercise program in improving GH, IGF-1 and metabolic syndrome risk factors in female obese middle school students.

Fiber optic smart monitoring of concrete beam retrofitted by composite patches

  • In order to extend the lifetime of buildings and civil infrastructure, patch type fibrous composite retrofitting materials are widely used. Retrofitted concrete columns and beams gain stiffness and strength, but lose toughness and show brittle failure. Usually, the cracks in concrete structures are visible to the naked eye and the status of the structure in the life cycle is estimated through visual inspections. After retrofitting of the structure, crack visibility is blocked by retrofitted composite materials. Therefore, structural monitoring after retrofitting is indispensable and self diagnosis method with optical fiber sensors is very useful. In this paper, we try to detect the peel out effect and find the strain difference between the main structure and retrofitting patch material when they separate from each other. In the experiment, two fiber optic Bragg grating sensors are applied to the main concrete structure and the patching material separately at the same position. The sensors show coincident behaviors at the initial loading, but different behaviors after a certain load. The test results show the possibility of optical fiber sensor monitoring of beam structures retrofitted by the composite patches.

  • Pretibial cyst formation is a rare complication after anterior cruciate ligament (ACL) reconstruction and there are only few cases reported. Pretibial cyst can be caused by variable reasons. Foreign body reaction due to breakdown of bio-absorbable screw for fixation, graft necrosis at tibial site, joint fluid leakage to tunnel and incomplete incorporation of graft to bony tunnel. The authors experienced one case of massive pretibial cyst after arthroscopic ACL reconstruction using bio-absorbable interference screw in 38-year-old male patient. Thus, authors report this rare case with literature view.

Remote Cerebellar Hemorrhage after Supratentorial Aneurysmal Surgery : Report of Six Cases

  • The case of postoperative hemorrhage occurring apart from the operative site as a complication of intracranial surgery is a rare malady, especially when it involves the cerebellum after supratentorial aneurysm surgery. In a review of the literature, the possible etiologies for cerebellar hemorrhage are: coagulopathy, intraoperative urokinase irrigation, excessive head rotation on positioning, brain shift due to excessive cerebrospinal fluid[CSF] and epidural hemovac drainage. We experienced six cases of cerebellar hemorrhage after supratentorial aneurysm surgery, and all of the patients were improved by instituting conservative medical treatment. The possible mechanism for the remote cerebellar hemorrhages seen in our series is probably a multifactorial effect, such as excessive epidural hemovac and CSF drainage, and jugular venous compression due to the operative position. The purpose of this report is to alert neurosurgeons to the existence of this syndrome and to suggest several ways of minimizing the possibility of their patients developing remote cerebellar hemorrhage.

Impact of Peer's Suicide on Mental Health of Adolescents (친구의 자살이 청소년의 정신 건강에 미치는 영향)

  • Objectives : This study examined the impact of peer's suicide on mental health of middle-school students. The aim of this study was to describe the course of posttraumatic stress and grief reaction, suicide ideation, and depression score among adolescents after exposure to peer's suicide and to examine the relationship between posttraumatic stress and other mental health scores in these subjects. Methods : Thirty seven middle school students who were exposed to the suicidal death of a peer completed self-report measures to assess levels of depression, grief reaction, post-traumatic stress, and suicide ideation at 1 month and 8 months after the peer's suicide. Results : There was no significant change in mental health scores between 1 month and 8 months. Level of posttraumatic stress after 8 months was related to acute grief response. Subjects who experienced a clinically significant level of suicidal ideation and posttraumatic stress showed more mental health problems. Conclusion : The results suggested that peer's suicide strongly impacted mental health issues of adolescents at a critical time of adolescent development. Clinicians should perform a thorough evaluation of mental health problems for youth who experienced peer's suicide and help them in management of their grief reaction.

Pneumonia after Dental Treatment under Ambulatory General Anesthesia in Mentally Retard Patient -A Case Report- (정신지체 환자에서 외래전신마취 하 치과치료 후 발생한 폐렴 -증례보고-)

  • A 15-years-old female patient with seizure disorder and pervasive developmental disorder was scheduled for dental treatment under ambulatory general anesthesia. She had past history of pneumonia and herpes encephalitis when she was 3 year old. Because of sever mental retardation and behavior disorder, routine laboratory test was substituted with physical exam and medical records of department of pediatrics. A few days before general anesthesia, she showed slight common cold, but pediatric consult had reported that there was minimal risk in general anesthesia. After 4-hour general anesthesia, she became critically sick with high fever, cough and malaise. After 10-day hospitalization with pneumonia and sepsis, she could go home.

Stability of Ta-Mo alloy on thin gate dielectric (박막 게이트 절연체 위에서 Ta-Mo 합금의 안정성)

  • This paper investigated the stability of Ta-Mo alloy on thin gate dielectric. Ta-Mo alloy was deposited by using co-sputtering process after thermal growing of 3.4nm and 4.2nm silicon dioxide. When the sputtering power of Ta and Mo were 100W and 70W, respectively, the suitable work function for NMOS gate electrode, 4.2eV, could obtain. To prove interface thermal stability of thin film gate dielectric and Ta-Mo alloy, rapid thermal annealing was performed at $600^{\circ}C$ and $700^{\circ}C$ for 10sec in Ar ambient. The results of interface reaction were surveyed by change of silicon dioxide thickness and work function after annealing process. Also, the reliability of alloy gate and gate dielectric could be confirmed by quantity of leakage current. Ta-Mo alloy was showed low sheet resistance and thermal stability, namely, little change of gate dielectric and work function, after $700^{\circ}C$ annealing process.
