Mesalizine-Induced Acute Pancreatitis and Interstitial Pneumonitis in a Patient with Ulcerative Colitis

  • Ulcerative colitis (UC) is a chronic idiopathic inflammatory bowel disease. Mesalizine for the first-line therapy of UC has adverse effects include pancreatitis, pneumonia and pericarditis. UC complicated by two coexisting conditions, however, is very rare. Moreover, drug-related pulmonary toxicity is particularly rare. An 11-year-old male patient was hospitalized for recurring upper abdominal pain after meals with vomiting, hematochezia and exertional dyspnea developing at 2 weeks of mesalizine therapy for UC. The serum level of lipase was elevated. Chest X-ray and thorax computed tomography showed interstitial pneumonitis. Mesalizine was discontinued and steroid therapy was initiated. Five days after admission, symptoms were resolved and mesalizine was resumed after a drop in amylase and lipase level. Symptoms returned the following day, however, accompanied by increased the serum levels of amylase and lipase. Mesalizine was discontinued again and recurring symptoms rapidly improved.

Complications of Donor Site in Latissimus Dorsi Muscle Flap (광배근 피판의 공여부에 대한 문제점의 분석)

  • Purpose : To investigate the complication of donor site in latissimus dorsi muscle flap. Materials and Methods : From April 1983 to March 1999, forty patients with latissimus dorsi muscle flap for reconstruction of extremity and with a follow-up period of more than 12 months were included. We analysed the complication, shoulder function, degree of shoulder muscle weakness, skin scar width after operation. Results After 1 year, skin scar widening in 12 cases(30%), limitation of shoulder motion in 9 cases(12.5%), muscle weakness in 14 cases(17.5%) were found. Conclusion. The rate of complication at donor site after latissimus dorsi flap operation is around 10%. To minimize the complication, avoiding axillary skin incision, minimal invasive harvesting by endoscopy, meticulous suturing of subcutaneous layer are needed.

The Survey on the Low Income Children's After-School Care (저소득층 취업모 아동의 방과후 생활조사)

  • This study attempted to investigate the state of the children's after-school care with low income employed mothers. The survey was conducted by means of the structured questionnaire. The questionnaire was built on the previous studies. The sample size was 418 mothers in Pusan. The results were summarized as follows : 1. Most of the children with low class employed mothers showed the self-care state. 2. Most of the children with low class employed mothers were found to spend more time alone in their houses. 3. Most of the children with low class employed mothers tended to have more difficulties in getting contact with their mothers and they were left to prepare their foods by themselves. 4. Most of the low class employed mother thought that safer environment and better education were immediate problems to be solved for their children's. 5. Most of the low class employed mothers needed after-school care facilities.

  • Tracheal injury is a rare complication of endo-tracheal intubation. However in neonates, the rates of morbidity and mortality are high. Recommendations for treatment are based on the several reports of this injury and are individualized. Conservative management can be effective in some cases. We describe the case of a neonate who presented with subcutaneous emphysema after intubation in a neonatal intensive care unit. This patient suffered full VACTERL syndrome and had 1.7 mm diameter subglottic stenosis. Conservative management resulted in no further increase in subcutaneous emphysema and after 10 days the patient was stable.

Aggravation and Relief after Surgical Resection of Post Infectious Pneumatocele in Very Low Birth Weight Infant

  • Pneumatocele (PC) is a thin-walled cyst of the lung that can occur at all ages and with various etiologies. However, there is no fully accepted consensus for the management of PC in a neonatal intensive care unit. Although the management of PC is generally expectant, it is difficult to decide how long conservative management should be maintained, especially under Korea's medical care environment and the parents' worry and anxiety. We report a male neonate, born at $27^{+5}weeks$ gestation, weighing 1,000 g, who had a post infectious PC caused by methicillin-resistant Staphylococcus aureus sepsis. We treated conservatively for about 100 days (roughly 14 weeks), but unfortunately after a few days of chest retraction, acute exacerbation occurred, video assisted thoracoscopic surgery (VATS) was deemed necessary and performed. The purpose of this publication is to describe the clinical course, aggravation and relief after VATS management with a review of the literature.

Facial diplegia as a delayed complication of scrub typhus

  • A 64-year-old man presented with facial diplegia occurring 2 weeks after scrub typhus diagnosis. The serum scrub typhus antibody titer was elevated to 1:5120. Brain magnetic resonance imaging revealed contrast-enhancement of the signal for both facial nerves. He was administered prednisolone. After two weeks, the symptoms improved, and after one month, he completely recovered from facial diplegia. This is the first case in the literature in which the patient exhibited facial diplegia, a delayed complication, in scrub typhus. Facial diplegia should be considered a type of cranial nerve palsy that may occur as a delayed complication of scrub typhus.

  • Recently, intra-Muscular Stimulation (IMS) therapy is being increasingly used for musculoskeletal pain. This procedure is generally regarded as a safe procedure for the general public. Some cases of iatrogenic pneumothorax caused by acupuncture have been reported in the medical literature. However, a case of an IMS therapy associated pneumothorax has not reported. We experienced two cases of iatrogenic pneumothorax after IMS therapy. A 62 year-old man received IMS therapy on the right shoulder due to posterior neck pain. After IMS therapy, acute dyspnea and chest discomfort developed. The other patient was a 74 year-old woman who also received IMS therapy. This patient experienced a nonproductive cough and acute dyspnea after the treatment. As the popularity of this form of alternative medicine increases, we might expect to see more cases of iatrogenic pneumothorax. Physicians should be aware of the adverse events associated with IMS therapy.

Symmetric dimethylarginine correlates with the urea, creatinine, potassium, and clinical scores in feline urethral obstructions

  • Background: A urethral obstruction (UO) is an emergency commonly observed in male cats, which can result in significant clinical and laboratory alterations, leading to complications and death. Objectives: This study aimed to correlate symmetric dimethylarginine (SDMA) with the urea, creatinine, potassium, and bicarbonate levels in cats with UO. In addition, the correlation between clinical score and time of obstruction was evaluated. Methods: Thirty male cats were selected and allocated into a control group (CG, n = 13) and an obstruction group (OG, n = 17). The laboratory analyses were conducted before treatment (M0) and at different times after treatment (12 h [M12], 24 h [M24], and 48 h [M48]). Correlations were established between SDMA and creatinine, urea, bicarbonate, potassium, time of obstruction, and the clinical score. Results: A strong correlation (r > 0.6) was observed between SDMA and creatinine, urea, and potassium in the OG. Furthermore, there was substantial agreement (kappa value) between SDMA and creatinine at M24. A higher clinical score was associated with a longer time of obstruction. In the OG, at M48, the SDMA and creatinine levels were 50% and 41.2% higher, respectively. Conclusions: A correlation was observed between SDMA and creatinine in obstructed cats, and significant agreement between these values was observed 24 h after the unblocking treatment. A correlation among SDMA, urea, and potassium was observed. Approximately 9% more cats continued to have elevated SDMA levels after 48 h of treatment compared to creatinine. This suggests a slightly lower sensitivity of the latter biomarker but does not exclude the possibility of congruent and normalized values after a longer evaluation period.

A Study on the Strategic Plan of Korea School Library Association (한국학교도서관협의회의 전략계획 수립에 관한 연구)

  • KSLA(Korea School Library Association) is a representative professional association related to school libraries and teacher librarians in Korea. This study tried to propose the strategic plan of KSLA. To do this, major works of KSLA are defined after analyzing current issues of school libraries and the internal environment. Among them, 12 works are selected as strategic works. The survey was carried out to decide which works are regarded as more important or less. After all, this study proposed the strategic plan for 4 works, which are the revision of school libraries promotion law, the revision of elementary and secondary education law, the investigation of teacher librarians' grievance cases and the collection of outstanding educational cases in school libraries.

Recognition of Food Labeling of High School Students in Yongin Region (용인지역 고등학생의 식품표시제에 대한 인식도)

  • This study investigated recognition of food labeling of high school students (males 94 and females 85) in Yongin region. Recognition of food labeling was very high at 81.6%. For checking food labeling at the time of purchasing, 53.6% of the respondents always checked and the main reason was healthier food choice (49.2%). Satisfaction of food labeling was very low at 18.4%. In spite of very low satisfaction, 70.9% of respondents recognized necessity for education of food labeling. In terms of education method, respondents preferred 'school lecture' the most with 48.6%, which are in the following order: TV/newspaper/journal/book (37.4%) > internet lecture (10.1%) > education from parents (3.9%). Moreover, 83.2% of respondents answered 'positive change (ex. checking food labeling)' after getting education. Consequently, high school students had low level of utilization of food labeling at the time of purchasing. However, they showed high level of perception and educational need of food labeling as well as possibility of positive change after receiving education. Therefore, appropriate school education for food labeling may be able to lead a healthy eating habit of high school students.
