Clinical Characteristics of Subacute Necrotizing Lymphadenitis in Pediatrics (소아에서의 아급성 괴사성 림프절염의 임상적 고찰)

  • Background:Subacute necrotizing lymphadenitis or Kikuchi's disease is unknown ethiology and self-limiting process. This disease predominantly affects young women age but rarely affects pediatrics, and usually manifests as lymphadenopathy and fever. Even though this disease is self-limited, benign process, many cases are misidentified as malignant lymphoma. The purpose of this study is to report the clinicopathologic finding, radiological finding and many labolatory test and to compare with characteristics of adult patients in this disease. Meterial and Methods:We reviewed 27 pediarics patients with subacute necrotizing lymphadenitis by excision biopsy or fineneedle aspiration cytology.Result:The most common symptomes were palpation of cervical lymh node(88.9%) and fever(66.7%). The common site of the involvement was cervical lymph node. The multiple involvement was 93% and bilateral involvement was 59%. Leukopenia(52%) and elevated erythrocyte sedimentation rates(93%) appeared in abnormal laboratory data. Microscopically, the characteristic finding was the wide area of florid nuclear dusts engulfed by histiocytes and well-circumscrbed area with eosinophilic fibrinoid material. There was a striking degree ofkaryorrhexis and an absence of granulocyte with paucity of plasma cell. All patients recovered with the conservative treatment and there was no specific complication and recurrence. Conclusion : We reviewed pediatric patients with this disease. Characteristics of this disease inpediatric patients were similar to adult patients.

A Study on the High School Students' Degree of Learning Desire in the Health Education of Military Drill Curriculum (교련교육과정에 포함된 건강관련내용에 대한 학생 학습요구도 조사)

  • The purpose of this study was to identify the degree of learning desire in the health education of military drill curriculum and to furnish basic data for the effective health education at high school. A total of 594 high school students were participated in the questionnaires and the 12 domains & the 55 questions were utilized for this study. The survey was conducted from March 15 to March 27, 1993 and the collected date were analized by T-test and F-test. The results of this study are as followings : 1. In the 12 domains, respondents indicated high degree of learning desire in 'human & sex', 'nutrition' and 'disease of adult' in that order. However 'accident & disaster', 'transport & management' and 'nursing' were not highly ranked. 2. In the 55 questions, respondents showed high degree of learning desire in 'artificial respiration' & 'cardiopulmonary resuscitation', 'precautionary of adult disease' and 'sex & sex moral' in that order, but low degree of learning desire in 'the management of many wounded persons' and the 'synopsis of nursing'. 3. Comparing the degree of learning desire by grade, the 1st, the 3rd and the End grade were ranked in that order. The 1st and 3rd graders showed higher degree of learning desire in 'human & sex', and the 2nd graders in 'nutrition'. 4. Also, female students showed higher degree of learning desire than male students in general. Female students indicated it in 'nutrition', 'human & sex' and 'adult disease' in that order while male students in 'human & sex', 'adult disease' and 'nutrition' in that order. 5. The academic high school students showed higher degree of learning desire than the vocational high school students. 'Human & sex' was highest ranked at both academic and vocational high school students.

The Usefulness of the Ivy Sign on Fluid-Attenuated Intensity Recovery Images in Improved Brain Hemodynamic Changes after Superficial Temporal Artery-Middle Cerebral Artery Anastomosis in Adult Patients with Moyamoya Disease

  • Objective : MR perfusion and single photon emission computerized tomography (SPECT) are well known imaging studies to evaluate hemodynamic change between prior to and following superficial temporal artery (STA)-middle cerebral artery (MCA) anastomosis in moyamoya disease. But their side effects and invasiveness make discomfort to patients. We evaluated the ivy sign on MR fluid attenuated inversion recovery (FLAIR) images in adult patients with moyamoya disease and compared it with result of SPECT and MR perfusion images. Methods : We enrolled twelve patients (thirteen cases) who were diagnosed with moyamoya disease and underwent STA-MCA anastomosis at our medical institution during a period ranging from September of 2010 to December of 2012. The presence of the ivy sign on MR FLAIR images was classified as Negative (0), Minimal (1), and Positive (2). Regions were classified into four territories: the anterior cerebral artery (ACA), the anterior MCA, the posterior MCA and the posterior cerebral artery. Results : Ivy signs on preoperative and postoperative MR FLAIR were improved (8 and 4 in the ACA regions, 13 and 4 in the anterior MCA regions and 19 and 9 in the posterior MCA regions). Like this result, the cerebrovascular reserve (CVR) on SPECT was significantly increased in the sum of CVR in same regions after STA-MCA anastomosis. Conclusion : After STA-MCA anastomosis, ivy signs were decreased in the cerebral hemisphere. As compared with conventional diagnostic modalities such as SPECT and MR perfusion images, the ivy sign on MR FLAIR is considered as a useful indicator in detecting brain hemodynamic changes between preoperatively and postoperatively in adult moyamoya patients.

Association Study between Vitamin D Receptor Gene Polymorphism and Adult Periodontitis in Korea

  • Adult periodontitis is a chronic inflammatory disease whose etiology is not well defined. Recent studies have shown that vitamin D receptor gene has been a candidate for the susceptibility of adult periodontitis. The purpose of this study is to investigate the frequency of Taq I restriction fragment length polymorphism (RFLP) in the vitamin D receptor gene in nan periodontically healthy controls and 28 adult periodontitis patients. Taq I RFLP in the vitamin D receptor gene was detected by PCR amplification, followed by restriction enzyme digestion and 2% agarose gel electrophoresis. There was no significant difference in the distribution of Taq I RFLP between healthy controls and adult periodontitis group (P > 0.05). Thus, Taq I RFLP in the vitamin D receptor gene may not confer the susceptibility to adult periodontitis in Korean population. However, t allele distributions of this RFLP showed various frequencies among ethnic groups studied. Further studies in other ethnic groups will be required.

THE RELATIONSHIP BETWEEN PERIODONTAL DISEASE SEVERITY AND Actinobacillus actinomycetemcomitans SEROTYPE & GENOTYPE DISTRIBUTION (Actinobacillus actinomycetemcomitans의 혈청형과 유전자형 분포가 치주질환 심도에 미치는 영향)

  • The present study was performed to evaluate the relationship between the serotype or the genotype of Actnobacillus actinomycetemcomitans (A. a.) and the severity of periodontal disease. Total 64 A. a. clinical isolates were sampled from 46 sites of 20 subjects classified into the group I (1 periodontally healthy subject, 2 gingivitis patients, 5 ealry adult periodontitis patients), group II (3 moderatelly adult periodontitis patients) and group III (1 advanced adult periodontitis patient, 8 RPP patients). Southern bolt hybridization (fingerprinting) patterns of the five reference strains, A. a. strain ATCC 29523 (serotype a), ATCC 29522 (Serotype b), ATCC 43719 (serotype c), IDH 781 (serotype d) and IDH 1705 (serotype e), were used as the five basic genotypic patterns (A, B, C, D, E). NT type was designated as one which did dnot represent any of those five basic types. The serotypes were determined by ELISA technique with the serum samples from pre-immunized rabbit. Based on subject-based analysis, it was noted that genotypes A and C, NT, and B, D, E were significantly related to the disease groups I, II, and III, respectively. It was also noted that both the serotypes a and c were significantly related to the disease group I and II, while serotypes were significantly related bm), and serotypes b and nd were frequently found in sites with severe attachment loss (LA>6mm). The results indicated that the significant relationship can be delineated beteen the genotypes and the serotypes of Acinobacillus actinomycetemcomitans and the periodontal disease severity. The results also indicated that genotyping can provide more detailed information on its relationship with the disease severity based on both the patient-based and the site-based analyses.

Inhaler Competency and Medication Adherence in Older Adults and Adults with Obstructive Lung Disease (폐쇄성 폐질환 노인 환자와 성인 환자의 흡입제 사용 숙련도와 투약이행도)

  • Purpose: The aims of current study were to assess the inhaler competency and medication adherence, and to identify association of inhaler competency with medication adherence in patients with obstructive lung disease. Methods: We did a secondary analysis of the Hanyang Obstructive Pulmonary Evaluation data in a single institution from June 2014 to April 2015 after an approval of Institutional Review Board. A total of 150 patients with asthma or chronic obstructive lung disease participated in the study. Inhaler competency was evaluated accuracy in each step for using metered dose inhaler. Medication adherence was calculated using actually dispensed doses based on the prescribed inhaler doses. Results: Older adults (${\geq}65$) had lower competency in using inhaler (66.7 vs 83.3, z=-4.52, p<.001) and poorer medication adherence (67.7 vs 91.8, $x^2=14.06$, p<.001) than adults (<65). Inhaler competency was associated with medication adherence (p=.26, p=.001). Surprisingly, more than 50% of patients were current smokers. Conclusion: Inhaler competency and medication adherence were lower in older adults with obstructive lung disease than those in adult-age patients. Therefore, an individual education program for older patients should be developed to improve the rates of proper use of inhalers. Nursing management for obstructive lung disease should focus on developing behavioral intervention strategies for smoking cessation.

  • Plastic bronchitis is a rare disease characterized by marked airway obstruction, via the formation of large gelatinous or rigid airway cast. In Korea, there were a few case reports with plastic bronchitis not in adults, but in children. So we report a case of an adult who was diagnosed as plastic bronchitis with eosinophilic casts, with no history of atopic and cardiac disease.

Interferon-γ-mediated secretion of tryptophanyl-tRNA synthetases has a role in protection of human umbilical cord blood-derived mesenchymal stem cells against experimental colitis

  • Mesenchymal stem cells (MSCs) are multipotent adult stem cells that present immunosuppressive effects in experimental and clinical trials targeting various rare diseases including inflammatory bowel disease (IBD). In addition, recent studies have reported tryptophanyl-tRNA synthetase (WRS) possesses uncanonical roles such as angiostatic and anti-inflammatory effects. However, little is known about the function of WRS in MSC-based therapy. In this study, we investigated if a novel factor, WRS, secreted from MSCs has a role in amelioration of IBD symptoms and determined a specific mechanism underlying MSC therapy. Experimental colitis was induced by administration of 3% DSS solution to 8-week-old mice and human umbilical cord blood-derived MSCs (hUCB-MSCs) were injected intraperitoneally. Secretion of WRS from hUCB-MSCs and direct effect of WRS on isolated $CD4^+$ T cells was determined via in vitro experiments and hUCB-MSCs showed significant therapeutic rescue against experimental colitis. Importantly, WRS level in serum of colitis induced mice decreased and recovered by administration of MSCs. Through in vitro examination, WRS expression of hUCB-MSCs increased when cells were treated with interferon-${\gamma}$ ($IFN-{\gamma}$). WRS was evaluated and revealed to have a role in inhibiting activated T cells by inducing apoptosis. In summary, $IFN-{\gamma}$-mediated secretion of WRS from MSCs has a role in suppressive effect on excessive inflammation and disease progression of IBD and brings new highlights in the immunomodulatory potency of hUCB-MSCs.

Body Image, Self Esteem, and Health related Quality of Life in Patients with Crohn's Disease (크론병 환자의 신체상, 자아존중감 및 건강 관련 삶의 질)

  • Purpose: The purpose of this study was to describe the degrees of body image, self esteem and Health Related Quality Of Life (HRQOL), and to identify factors related to these variables in patients with Crohn's disease. Methods: A descriptive cross sectional survey design was used. The 148 participants were the patients with Crohn's disease, recruited conveniently from one metropolitan hospital located in Seoul, Korea. The collected data were analyzed with SPSS/WIN program. Results: Patients with severe illness activity reported significantly lower body images than those of the others. Education levels and working states affected self-esteem positively, while the number of operations and illness activity affected self-esteem negatively. Hospitalization and illness activity affected HRQOL negatively. Body image, self esteem, and HRQOL were positively correlated to each other with statistical significance. Illness activity was a common factor to affect body image, self esteem, and HRQOL. Body image was an influential factor on HRQOL as well as illness activity in patients with Crohn's disease. Conclusion: As a result, this study suggested a necessity for developing an intervention study to lower the illness activity and heighten the body image among the patients with Crohn's disease.

Illness and Experiences of the Body Among Aged Women (만성질환을 지닌 여성 노인의 몸 체험)

  • Purpose: The purpose of present study was to discover the experience of the body of aged women, having had disease. Thus, the researcher tried to explore the perception of the informants and the context in which this perception emerged. Methods: 9 aged women who had disease or trauma were recruited by snow balling and theoretical sampling methods. The iterative data collection and analyzing process proceeded between September, 1999 and January, 2005. Questions posed to the informants included: "What major change in your body comes from the disease?" "How did you feel about yourself after having had disease?". Data from interviews and participant observation was taken as text. The text was analyzed using the ongoing process of qualitative content analysing method and taxonomy of Spradley. Results: Disease gives aged women a chance to reinforce the meaning of their body: the body as the most low valued component of a human, the body as a wholistic field of interacting each component of human and with natural environment and cosmos, and the body as a source of group identity. These meanings were constructed in their life world by the rules of hierarchy, reciprocity, and group cohesiveness. Conclusions: The human body is constructed as a cultural being by a social process. Nursing is concerned with the biological body and the social body. The results of this study can serve to help understand the socialization of the body and to construct a somology of nursing.
