선택적 함구증을 보인 6세 아동의 놀이치료 과정을 통해 나타난 정신역동 및 치료과정을 발달학적 관점에서 소개하였다. 비지시적 지지적 놀이치료를 실시하였고 치료자와의 관계속에서 환아는 놀이를 통하여 단계적으로 발달과정을 재경험하여 분리 불안 및 유기공포를 극복하고 자율성을 회복하므로써 증상의 소실과 함께 정상적 외디푸스시기의 발달을 이루게 되었다.

Objectives : The aim of this study was to examine the relationship between internet game addiction and psychiatric symptoms. Methods : A total of 447 adolescents who were recruited from a child and adolescent psychiatric clinic of university hospital completed a self-report questionnaire consisting of Korean Internet Addiction Self-scale (K-scale), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Korean-Attention-Deficit Hyperactivity Disorder Rating Scale (K-ARS), and Adolescent Happiness Index (AHI). They were classified into three internet user groups, non-addicted group (below or equal to 94), potential risk group (95 to 107) and high risk group (above or equal to 108) according to K-scale total score. Results : Significant mean differences in BDI, BAI, K-ARS, and AHI scores were observed among the three groups. The BDI, BAI and K-ARS scores showed positive correlation with K-score. The AHI score showed negative correlation with K-score. Conclusion : Findings of this study suggest that depression, anxiety, inattention, hyperactivity/impulsivity, and happiness are associated with internet game addiction.
Objective The aim of this study was to evaluate differences in psychopathology between offspring of parents with bipolar I disorder (BP-I) and those with bipolar II disorder (BP-II). Methods The sample included 201 offspring between 6 and 17 years of age who had at least one parent with BP-I or BP-II. The offspring were diagnostically evaluated using the Korean Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version. Psychopathology and Clinical characteristics were evaluated, including lifetime DSM-5 diagnoses, depression, and childhood trauma. Lifetime DSM-5 diagnoses were also compared between schoolchildren aged 6 to 11 years and adolescents aged 12 to 17 years. Results In lifetime DSM-5 diagnoses, offspring of parents with BP-I had significantly increased risk of developing MDD and BP-I than those with BP-II. Regarding clinical characteristics, ADHD rating scale and childhood trauma scale were significantly higher in offspring of parents with BP-I than that in those with BP-II. Conclusion The present study supports that BP-I may be etiologically distinct from BP-II by a possible genetic liability. Our findings indicate that additional research related to bipolar offspring is needed to enhance understanding of differences between BP-I and BP-II.

본 연구는 정서 행동적 문제를 자주 동반하는 것으로 알려진 아토피피부염 소아 청소년에 대해, 정신과적 문제의 정도와 병변 심각도에 영향을 미칠 수 있는 심리 사회적요인을 알아보고자 하였다. 서울대학교병원 아토피 크리닉을 방문한 아토피피부염 환자 70명(평균 나이 9.9세, 범위:$4{\sim}15$세)과 그 어머니를 대상으로 병력조사설문지, CBCL, MBRI, CDI, STAI을 주어 완성하도록 한 후, 정신과적 면담을 시행하였다, 환자의 피부 병변에 대해 SCORAD index를 이용하여 침범부위, 병변의 경중도, 주관적 증상으로 나누어 평가하였다. 16명(22.9%)이 임상적으로 유의한 정서 행동적 문제가 있을 것으로 예측되었으며 이들은 우울, 특성 불안, 어머니의 통제적 태도, 부정적평가 척도에서 다른 아토피 군보다 점수가 높았다. 병변의 경중도는 특성불안이나 우울정도와, 주관적 증상은 우울, 특성불안, 가족영향, 사회능력, 내재화 및 외현화 문제, 성문제, 아이에 대한 부정적 평가와 관련이 있었다. 아토피피부염을 가진 소아 청소년은 정신과적 문제의 고위험군이며 이러한 정신과적 문제는 양육태도와 함께 피부증상의 주관적 지각에 영향을 미칠 가능성이 높다고 할 수 있다. 향후 이에 대한 정신과적 개입이 필요할 것으로 생각된다.

Objectives : The purpose of this study was to assess anger of middle school adolescents in an urban community using State-Trait Anger Expression Inventory (STAXI)-Korean and Korean Child Behavior Checklist (K-CBCL), and to determine whether specific anger expression of adolescents is associated with certain psychopathology including depression. Methods : Data were collected from 395 middle school students and their parents in Incheon city. The students completed the STAXI, Children's Depression Inventory, Korean version of Mood disorder Questionnaire, and Conners-Wells' Adolescent Self-Report Scale (Short Form), and their parents completed the K-CBCL. Results : No significant correlation was observed between aggression subscale K-CBCL and each scale of the STAXI, except anger out (r=0.704). The scores for STAXI total and respective scales were significantly higher for female students than male students. A stepwise logistic regression model was used to explore the possible predictors for depression in adolescents and lower anger control and higher anger suppression were found to be predictors for depression in adolescence. Conclusion : There is a possibility of a large discrepancy between anger that the adolescents actually feel and parents' judgment of their children's anger. It must be considered in assessment of adolescent anger and use of further structured interviews is necessary. In addition, it may be useful to consider the anger expression style in adolescents who report depressive symptoms.

Objectives: This study investigated whether the Minnesota Multiphasic Personality Inventory-Adolescent Restructured Form (MMPI-ARF) can differentiate between two groups of adolescents, one diagnosed with internalizing disorders and another with externalizing disorders, and examined the clinical utility of the MMPI-A-RF by examining which subscales can significantly discriminate between these two groups. Methods: A total of 105 adolescents aged 13-18 years completed the MMPI-A-RF (53 internalizing disorder and 52 externalizing disorder groups). Independent t-test, chi-square test (χ2), and discriminant analysis were used to examine whether MMPI-A-RF can distinguish between the two groups. Results: Sixteen MMPI-A-RF scales best predicted differences between the groups with internalizing and externalizing disorders. Fourteen scales (Higher-Order Scale [Emotional/Internalizing Dysfunction], Restructured Clinical [RC] Scale [RC demoralization, Somatic Complaints (RC1), and Low Positive Emotions (RC2)], Personality Psychopathology Five Scale [Introversion/Low Positive Emotionality-Revised, Negative Emotionality/Neuroticism-Revised], Somatic/Cognitive Scale [Malaise, Head Pain Complaints, and Gastrointestinal Complaints], Internalizing Scale [Stress/Worry, Self-Doubt], Externalizing Scale [Negative School Attitudes], Interpersonal Scale [Social Avoidance, Shyness]) were associated with the internalizing disorder group, whereas two scales (Externalizing Scale [Conduct Problems, Negative Peer Influence]) were associated with the externalizing disorder group. Conclusion: The MMPI-A-RF can be an efficient assessment tool for a quick diagnosis as it can classify individuals with internalizing and externalizing disorders in clinical settings that lack a variety of assessment tools for children and adolescents.

Objectives : This study was conducted to investigate the prevalence of victims of bullying and the demographic characteristics of victims, and their related psychopathology, in a Korean nationwide sample of youths in middle and high school over a one month period. Methods : During the autumn of 2009, students in the 7th to 12th grades at 23 secondary schools participated in a nationwide, cross-sectional study. The study subjects completed the Adolescent Mental Health and Problem Behavior Screening Questionnaire-II (AMPQ-II) and Symptom Checklist-90-Revision (SCL-90-R). Based on the data acquired, descriptive statistics, correlation coefficients and multiple logistic regression analysis were performed. Results : Among the 3364 participants, 2272 (67.54%) completed the questionnaire. The prevalence of victimization was 28.9%. Male gender was positively associated with victimization, and grade level was negatively related to victimization. The AMPQ-II bullying score (Factor 4) was significantly (p<.001) and positively correlated to the AMPQ-II student total score (r= 0.50), Worry and thought (Factor 1 ; r=0.38), Mood and suicide (Factor 2 ; r=0.31), Academic and Internet-related problems (Factor 3 ; r=0.24), Rule violations (Factor 5 ; r=0.23), and AMPQ-II teacher total score (r=0.11). Somatization (r=0.23), Obsessive-compulsive behavior (r=0.24), Interpersonal sensitivity (r=0.30), Depression (r=0.33), Anxiety (r=0.26), Hostility (r=0.30), Phobic anxiety (r=0.22), Paranoid ideation (r=0.36), and Psychoticism (r=0.31) results from the SCL-90-R were also found to be positively related to the AMPQ-II bullying score, and remained significant after adjusting for age and gender. A total of 26% of the victims reported suicidal ideations as compared to 9% of non-victims over the month prior to the evaluation ($x^2$=119.595, df=1, p<.001). The multiple logistic regression analysis indicated that the AMPQ-II bullying score significantly increased the risk of suicidal ideation [Exp(b)=1.55, df=1, p<.001] after adjusting for age and gender. Conclusion : School bullying was highly prevalent among Korean middle and high school students. This study provided strong evidence that suicidal ideation and psychopathology were serious problems among the victims of bullying.

정신분열병이 있는 어머니 자녀들의 정신병리를 K-CBCL을 통하여 정상대조군과 비교하고, 이 두 군에서의 부모 양육 태도의 차이를 알아보며 이와 정신병리와의 상관관계를 알아보고자 하였다. 연구대상군은 국립서울정신병원에서 정신분열병 진단으로 외래 추적 치료를 받고 있는 여자 환자의 자녀로 하였고, 정상대조군으로 정신과적 문제가 없는 어머니의 자녀를 대상으로 하였다. 정신병리의 측정을 위하여 부모용과 청소년용(12-18세) 아동 ${\cdot}$ 청소년 행동평가 척도(K-CBCL)를 사용하였고, 부모자녀 결합형태 조사(PBI)를 통하여 부모양육태도를 알아보았다. 1) 정신분열병 어머니의 자녀군과 정상대조군 자녀의 인구학적 자료에서는 유의한 차이는 없었다. 2) 연구대상군이 정상대조군에 비해서 아동 ${\cdot}$ 청소년 행동평가척도상 문제 행동이 통계적으로 유의하게 많았다. 3) 어머니에 대한 부모양육태도상 연구대상군은 정상대조군에 비하여 과보호 척도가 유의하게 높고, 돌봄척도에서는 유의하게 낮았다. 4) 연구대상군에서 보이는 문제 행동들은 어머니의 과보호척도와 정적인 상관관계를 보였고, 어머니의 돌봄 척도와는 부적인 상관관계를 보였다. 정신분열병 어머니의 자녀 군이 정상대조군의 자녀에 비해 문제 행동들이 더 많았다. 또한 이들의 문제 행동들은 어머니의 과보호 척도와 정적인 상관관계를, 그리고 어머니의 돌봄 척도와는 부적인 상관관계를 보였다.

Objectives : The purpose of this study is to examine the effects of the characteristics of victim and sexual abuse on posttraumatic psychiatric symptoms in children and adolescents with a history of sexual abuse. Methods : A total of 137 children and adolescents were recruited from the Seoul Sunflower Children Center, a nation-funded sexual violence victim protection center, from January 2009 to December 2013. We collected the demographic data of the victims and the Trauma Symptom Checklist for Children (TSCC) from victims. We hypothesized victims' age, sex, and intelligence quotient, and the characteristics of sexual abuse as the affecting factors of posttraumatic psychiatric symptoms. Descriptive analysis and hierarchical regression analysis were performed for analysis of demographic data, TSCC scores, and psychiatric symptoms. Results : The victims' age and the characteristics of sexual abuse were significantly related to the traumatic distress of sexual abuse. R-square was 23% for anxiety, 39% for depression, 21% for posttraumatic stress, and 37% for dissociation on TSCC. Conclusion : This study suggests that victims' age, type, frequency and duration of exposure, and disclosure of sexual abuse are significant affecting factors on posttraumatic psychiatric symptoms in children and adolescents. Exploration of psychiatric symptoms other than posttraumatic symptoms, and relations between pretraumatic and posttraumatic psychiatric symptoms is needed through collection of larger samples.

Objectives : The research is designed in order to know how much Traumatic Symptom Checklist fur Children(TSCC) is effective in the case of child abuses, especially in the estimation of the psychopathology of child sexual abuse. Methods : A sample includes a group of thirty children of 4th, 5th and 6th grade whose sexual abuse have confirmed and also a group of thirty four normal kids who have no abuse. Along with TSCC, Children's Depression Inventory (CDI) and Revised Children's Manifest Anxiety Scale (RCMAS) were used to children and child behavior checklist (K-CBCL), sexual behaviors (CSBI), and Kiddie-Schedule for Affective Disorder and Schizophrenia-Present and Lifetime version (K-SADS-PL) record sheets were used to kids' parents. Results : In case of the sexual abuse group, the most meaningful difference from the analyzed result is shown at sexual concerns scale which is one of the clinical scale in TSCC (p<.01). Also, the sexual abuse group has the meaningful higher score than the general group's one in the stress scale after the other anxiety, depression, anger and dissociation (p<.01). Conclusion : The scale of TSCC has relatively shown the characteristic symptom and severity which were gained from the children who have experienced trauma, especially sexual abuse.
