Effects of Infection Control Strategies for Vancomycin Resistant Enterococci in Intensive Care Units (중환자실에서 적용한 반코마이신(Vancomycin) 내성 장구균의 감염관리 전략 효과)

  • Purpose: This study was to examine whether VRE infection control strategies have an effect on the decrease in incidence rates for VRE acquisition and VRE nosocomial infection in ICU. Methods: All the patients were examined for VRE carriers on ICU admission. Among them, patients hospitalized for over 48 hours were investigated for VRE acquisition rates and VRE nosocomial infection rate using VRE infection control strategies in ICU for the experimental group from September 2007 to April 2008. Before that, incidence of VRE acquisition and VRE nosocomial infection for the control group without Intervention were investigated from May to August 2007 retrospectively. Results: VRE acquisition rate in clinical specimens was 0.6% in the experimental group, that was significantly lower when compared to the control group. VRE carrier rate at admission to ICU was 15.4%. Out of 182 VRE carriers, 180 patients were identified by the active surveillance culture. Conclusion: These results suggested that active surveillance culture at admission was considered to be an essential measure for detection of VRE carrier. But without strict isolation and adherence rating after each intervention, hand washing and contact isolation alone did not significantly decrease VRE nosocomial infection, although it did significantly decrease incidence of VRE acquired from clinical specimen.

  • Next Generation Networks (NGN) is defined as IP-based networks with multi-services and with multi-access networks. A variety of services and access technologies are co-existed within NGN. Therefore there are numerous transport technologies such as Differentiated Services (DiffServ), Multi-protocol Label Switching (MPLS), and the combined transport technologies. In such an environment, flows are aggregated and de-aggregated multiple times in their end-to-end paths. In this research, a method for calculating end-to-end delay bound for such a flow, provided that the information exchanged among networks regarding flow aggregates, especially the maximum burst size of a flow aggregate entering a network. We suggest an admission control mechanism that can decide whether the requested performance for a flow can be met. We further verify the suggested calculation and admission algorithm with a few realistic scenarios.

  • Many of multimedia applications in distributed environments generate the packets which have the real-time characteristics for continuous audio/video data and transmit them according to the teal-time task scheduling theories. In this paper, we model the traffic for continuous media in the distributed multimedia applications based on the high-bandwidth networks and introduce the PDMA algorithm which is the hard real-time task scheduling theory for guaranteeing QoS requested by the clients. Furthermore, we propose the admission control to control the new request not to interfere the current services for maintaining the high quality of services of the applications. Since the proposed admission control is sufficient for the PDMA algorithm, the PDMA algorithm is always able to find the feasible schedule for the set of messages which satisfies it. Therefore, if the set of messages including the new request to generate the new traffic. Otherwise, it rejects the new request. In final, we present the simulation results for showing that the scheduling with the proposed admission control is of practical use.

Chernoff Bound and the Refined Large Deviation Approximation for Connection Admission Control in CDMA Systems

  • This paper proposes a transient (predictive) connection admission control (CAC) scheme using the transient quality of service (QoS) measure for CDMA cellular systems with bursts On-Off sources. We need an approximate and bounded approach for real-time CAC applications. We derive the transient outage probability as the QoS measure using the Chernoff bound and the refuted large deviation approximation. Numerical results show that the predictive CAC is a promising approach for the multicell CDMA systems.

  • 오늘날의 인터넷은 QoS를 보장하기 위해 다양한 방법을 사용하고 있는데, 본 논문에서는 Integrated-QoS 개념을 제안하고 이를 기반으로 Admission Control을 실행하는 Enhanced Differentiated Service(DiffServ)를 설계하였다. 본 논문은 IntServ(Integrated Service)와 DiffServ(Differentiated Service)의 장점을 결합하여, EF 클래스에 속하는 다수의 흐름들(flows)에 기존보다 QoS가 더욱 보장된 서비스를 제공할 수 있는 방안을 제시한다.

  • In this paper, a new real-time neural network connection admission controller is proposed. The proposed controller measures traffic flows, cell loss rate and cell delay periodically each classes. The Neural network learns the relation between those measured information and service quality by real-time. Also the proposed controller uses the DWRR multiplexer with buffer dedicated to every traffic source in order to measure the delay that cells experience in buffer. Experimental result shows that the proposed method can control effectively heterogeneous traffic sources with diverse QoS requirement.

  • 멀티미디어 서비스는 트래픽 특성이 비대칭이기 때문에 하향링크의 무선 자원 관리의 중요성이 부각되고 있다. 여기서 제안되는 알고리즘은 기존의 전력 기반의 알고리즘을 조합한 하이브리드 형태의 알고리즘으로 현재 셀의 상황에 따라서 admission control의 형태가 바뀌는 방식이다. 본 논문에서는 하이브리드 형태의 알고리즘과 기존의 전력 기반의 알고리즘을 simulation을 통해서 비교 분석한다.

The Effects of Admission Care on Bell's Palsy-Case Control Study (환자 대조군 연구를 통한 입원치료가 구안와사에 미치는 영향)

  • Objective : The aim of this study was to assess the effects of admission care on Bell's palsy by case control study. Method : 27 patients who diagnosed as Bell's palsy were shared 13 inpatients group treated by early admission care, and 14 outpatients group. They were equally treated with acupuncture, herb medicine and western medicine. The groups were evaluated by Yanagihara,s unweighted grading system at pre-treatment, after 5 days, after 10 days and after 15 days. Results : The Yanagihara,s scores of 2 groups showed stastically significant improvement in comparison with pre-treatment. In improvement index after treatment, the Yanagihara,s scores of 2 groups increased but were not stastically significant the difference. Conclusion : These results provided that the admission care may not be a valuable treatment for Bell's palsy. Further study is needed to evaluate the effect of early admission care on Bell's palsy.

QoS-Aware Call Admission Control for Multimedia over CDMA Network (CDMA 무선망상의 멀티미디어 서비스를 위한 QoS 제공 호 제어 기법)

  • Diverse multimedia services will be deployed at hand on 3G-and-beyond multi-service CDMA systems in order to satisfy different quality of service (QoS) according to traffic types. In order to use appropriate resources efficiently the call admission control (CAC) as a major resource control mechanism needs to be used to take care of efficient utilization of limited resources. In this paper, we propose a QoS-aware CAC (QCAC) that is enabled to provide service fairness and service differentiation in accordance with priority order and that applies the different thresholds in received power considering different QoS requirements such as different bit error rates (BER) when adopting total received power as the ceil load estimation. The proposed QCAC calculates the different thresholds of the different traffic types based on different required BER applies it for admission policy, and can get service fairness and differentiation in terms of call dropping probability as a main performance metric. The QCAC is aware of the QoS requirement per traffic type and allows admission discrimination according to traffic types in order to minimize the probability of QoS violation. Also the CAC needs to consider the resource allocation schemes such as complete sharing (CS), complete partitioning (CP), and priority sharing(PS) in order to provide fairness and service differentiation among traffic types. Among them, PS is closely related with the proposed QCAC having differently calculated threshold per each traffic type according to traffic priority orders.

  • In this paper, A Connection Admission Control(CAC) algorithm by Dynamic Bandwidth Allocation is proposed. The B-ISDN will be required to carry traffic generated by a various traffic sources. These source will have all traffic flow characteristics and quality of services requirements. When a connectionless message with burstiness arrives at a node during a renewal period, CAC will be dynamically determined connection request by using the predetermined connection admission bounds in estimating the number of input arriving cells. The proposed algorithm estimates the available bandwidth by observing cell streams of the aggregate traffic. A connection request is accepted if the available bandwidth is larger than declared peak rate, and otherwise rejected. The proposed CAC scheme is shown to be promising and practically feasible in obtaining efficient transmission of burst traffic with guaranteed quality of services.
