심장 CT 검사시 ASIR 적용에 따른 선량 평가 (Radiation dose Assesment according to the Adaptive Statistical Iterative Reconstruction Technique of Cardiac Computed Tomography(CT))

  • 심장 CT 검사시 적응식 통계적 반복 재구성법을 이용하여 체질량 지수에 따른 관전압 변화에 대한 방사선 피폭선량에 대하여 알아보고자 하였다. 심장 CT 검사를 시행 받은 환자를 BMI에 따른 관전압에 따라 네 개의 군으로 나누어[A군(n=20), Non-ASIR, BMI < 25, 100 kVp; B군(n=20), Non-ASIR, BMI > 25, 120 kVp; C군(n=20), 40% ASIR BMI < 25, 100 kVp; D군(n=20), 40% ASIR, BMI > 25, 120 kVp] 대동맥 중심부와 우관상동맥, 좌전하행동맥에 관심영역을 설정 한 후 CT값(number)측정하여 평균값과 표준편차를 분석하였다. 영상 잡음은 A군과 C군 사이에는 통계적으로 유의한 차이가 있었으며, A군이 C군보다 노이즈가 유의하게 높았다(group A, 494 ${\pm}$ 32 HU; group C, 482 ${\pm}$ 48 HU: P<0.05). 또한, B군과 D군 사이에는 통계적으로 유의한 차이가 있었으며, B군이 D군보다 노이즈가 유의하게 높았다(group B, 510 ${\pm}$ 45 HU; group D, 480 ${\pm}$ 82 HU: P<0.05). 영상의 정성적 분석에서 관상동맥 분절별로 임상평가 한 결과 평균값은 A군은 4.13${\pm}$0.2, B군은4.18${\pm}$0.1, C군은 4.1${\pm}$0.2, D군은 4.15${\pm}$0.1로 A군, B군, C군, D군 모두 통계적으로 유의한 차이가 없었으며(P>0.05), 모든 군에서 진단에 적절한 영상을 보였다. 피폭선량은 A군은 8.6${\pm}$0.9, B군은 14.9${\pm}$0.4, C군은 5.8${\pm}$0.5, D군은 10.1${\pm}$0.6 mSv 로 나타났다.

POCS와 정규화를 기반으로한 프레임간 압출 영사의 후처리 (Postprocessing of Inter-Frame Coded Images Based on Convex Projection and Regularization)

  • 본 논문에서는 프레임간 압축된 영상의 블록화 현상을 감소시키기 위해 재구성하기 전 차분 영상을 처리하는 새로운 복원 알고리듬을 제안한다. 프레임내 압축 영상의 블록화 현상은 오직 8×8 DCT에 의해서 일어나는 반면에, 프레임간 압축된 영상에서는 8×8 DCT뿐만 아니라 움직임 보상을 위해 사용한 16×16의 매크로 블록에 의해서 발생된다. 이러한 이유로 본 논문에서는 차분 영상에 대한 새로운 열화모델을 제시하고, 블록 경계와 내부의 불연속에 대한 POCS를 이용한 복원 알고리듬을 제시한다. 제안된 알고리듬은 DCT계수의 일부를 이용해 블록 경계의 방향을 고려하는 공간 적응적 저대역 통과 필터를 포함하는 표준 정규화의 변환된 형태이다. 일반적인 비디오 압축 표준은 블록을 기준으로 하는 움직임 보상과 블록 이산여현 변환(BDCT)을 이용한 혼성(hybrid) 구조를 채택하고 있다. 이러한 이유에서 블록화 현상은 블록 경계와 블록 내부에서 일어난다. 두 종류의 블록화 현상을 좀 더 완벽히 제거하기 위해서 복원된 차분 영상은 블록 경계와 블록 내부에서의 방향성 불연속과 같은 제약조건을 만족시켜야만 한다. 그러한 제약조건은 차분 영상을 복구하기 위한 convex set을 정의하는데 이용한다.

소형 도마뱀 운동 분석을 위한 마커리스 모션 캡쳐 알고리즘 (Markerless Motion Capture Algorithm for Lizard Biomimetics)

  • 본 논문에서는 마커를 부착하기 어려운 소형도마뱀의 관절을 측정하기 위한 마커리스 모션 캡쳐 알고리즘을 제안하였다. 제안한 알고리즘에서는 먼저 스테레오 비젼과 같은 다시점 영상에서 적응적 이진화를 통해 도마뱀의 실루엣 영상을 획득하고 세선화를 수행하여 도마뱀의 뼈대 영상을 획득한다. 이후, 직교-대각 성분 제거 알고리즘 및 A* Search를 통해 머리와 꼬리점, 및 머리와 꼬리를 잇는 척추라인을 구한다. 어깨관절과 고관절의 좌표는 $3{\times}3$ 마스크를 이용하여 척추라인과 다리가 만나는 지점을 구하여 획득하고 모폴로지 닫기 영상을 통해 발바닥 좌표들을 검출한다. 최종적으로 각각의 다리에서 어깨관절 및 고관절 좌표와 발바닥 좌표를 잇는 직선과 해당 다리의 뼈대 좌표간의 직교 거리 비교를 통해 무릎과 팔꿈치 좌표를 구한다. 최종적으로 제안한 알고리즘으로 검출된 각 관절의 다시점 영상의 2차원 좌표들로부터 각 관절의 3차원 좌표를 복원한다. 실제 도마뱀을 촬영한 스테레오 영상에 제안된 알고리즘을 적용하여 2차원 주요 관절 지점 검출 및 3차원 복원을 수행하여 제안된 알고리즘의 성능을 검증하였다.

골 밀도 평가를 위한 뼈의 전기 전도도 재구성: 시뮬레이션 (An Electrical Conductivity Reconstruction for Evaluating Bone Mineral Density : Simulation)

  • 골다공증은 골 조직의 량이 감소하여 골절의 가능성이 증가하는 상태이다. 뼈의 전기적 특성은 골 밀도와 관련이 있으며, 특히 뼈의 절기 저항은 골 소실이 증가할수록 감소하는 것으로 알려져 있다. 만일 신뢰성 있는 측정이 가능하다면, 뼈의 전기적인 특성은 골다공증 진단을 위한 유용한 변수가 될 수 있음을 의미한다 본 연구에서는 EIT (electrical impedance tomography) 기술을 이용하여 뼈의 전기 전도도 평가를 시도하였다. 뼈와 주위의 연조직 간의 전기적인 특성의 차이가 크기 때문에 (100 정도), 일반적으로, 뼈의 EIT를 얻기는 쉽지 않다. 본 연구에서는 이상 경계 검출을 위해 개발된 adaptive mesh regeneration 기법을 이용하여, 주어진 경계면의 기하학적 조건하에서, 경계 내부의 전기전도도를 복원할 수 있도 록 하였다. 수치 해석은 균일한 타원형 (장경 17 mm, 단경 15 mm) 뼈와 주위의 연 조직으로 구성된 반경 40 mm인 원통형 팬텀으로 모의한 경골에 대해 수행하였다. 타원형 뼈는 팬텀의 원형 단면 중심에서 15 mm 위쪽에 위치하도록 하였다. 연 조직의 전기 전도도는 4 mS/cm로 고정하였고, 뼈에 대해서는 0.01 - 1 mS/cm에서 변하도록 하였다. 측정 오차에 의한 효과를 분석하기 위해 시뮬레이션은 측정 오차를 포함하도록 하였다. 시뮬레이션 결과, 만일 측정 오차가 5 % 미만으로 유지될 경우, 재구성된 뼈의 전기전도도는 10 % 이내의 오차를 갖는 것으로 나타났다. 예측할수 있듯이 복원의 정확도는 뼈의 전기 전도도가 증가함에 따라 증가하였다. 이것은 사용된 EIT가 골 소실이 진행된 뼈에 대한 좀더 정확한 정보를 제공할 수 있음을 시사한다. 본 연구는, 뼈의 해부학적인 정보를 알고 있을 때 뼈와 주위의 연조직에 대한 단순한 이상 영상에 한정된 결과를 제공하지만, EIT 기술이 골다공증 골절을 예측하는 새로운 방법으로 활용될 가능성을 시사한다.

ASIR를 이용한 두부 CT의 영상 잡음 평가 및 피폭선량 분석 (Evaluation of Image Noise and Radiation Dose Analysis In Brain CT Using ASIR(Adaptive Statistical Iterative Reconstruction))

  • 본 연구는 두부 컴퓨터 단층 촬영 검사 시 적응식 통계적 반복 재구성법인 알고리즘을 적용하여 노이즈 및 화질평가, 피폭선량의 감소에 대하여 알아보고자 하였다. 두부 CT 검사 시 ASIR를 적용하지 않은 군[A군], ASIR 50 % 적용한군 [B군]으로 나누어 검사하였다. 팬텀연구에서 측정된 CT 노이즈 평균값의 측정결과는 B군이 A군보다 중심부(A)와 주변부(B, C, D)에서 각 각 46.9 %, 48.2 %, 43.2 %, 47.9 %가 감소되었다. 영상화질 평가에서 정량적 분석방법으로 CT 값(number)을 측정하여 잡음(noise) 정도를 분석하였다. 영상 잡음은 A군과 B군 사이에는 통계적으로 유의한 차이가 있었으며, A군이 B군보다 영상 잡음이 유의하게 높았다(group A ;우엽에서 31.87 HU, 좌엽에서 31.78 HU, group B ; 우엽에서 26.6 HU, 좌엽에서 30.42 HU : P<0.05). 영상의 정성적 평가방법으로 두부 임상 영상 평가표에 의해 평가한 결과 80점 만점에 A군의 관찰자 1의 점수는 73.17 점, 관찰자 2의 점수는 74.2 점으로 평가하였으며, B군의 관찰자 1의 점수는 71.77 점, 관찰자 2의 점수는 72.47 점으로 평가하였다. 통계적으로 유의한 차이가 없었으며(P>0.05), 진단에 적절한 영상을 보였다. 피폭선량은 ASIR 50 % 적용하여 검사함으로써 영상의 질적 저하 없이 방사선 피폭선량을 47.6 % 감소 시킬수 있었다. 결론적으로 임상 부위에 ASIR가 적용이 된다면 훨씬 더 적은 선량으로도 검사가 가능할 것으로 사료되며, 검사 시에 검사자가 판단하는데 있어 긍정적인 요인이 될 것으로 사료된다.

콘빔 CT 및 MLC 로그데이터를 이용한 전달 선량 재구성 시 오차 분석 (Error Analysis of Delivered Dose Reconstruction Using Cone-beam CT and MLC Log Data)

  • 본 연구에서는 콘빔 단층촬영영상(cone beam CT; CBCT) 및 다엽 콜리메이터(multileaf collimator;MLC) 로그데이터를 이용한 적응형 방사선치료기법의 체계를 구축하고, 그 과정에서의 선량 계산 오차의 양상을 팬텀을 이용하여 분석하고자 하였다. Catphan-600 (The Phantom Laboratory, USA) 팬텀을 CT와 CBCT 촬영 후 CT 영상을 이용하여 간단한 단계별조사(step-and-shoot) 방식의 세기조절방사선치료(intensity-modulated radiation therapy; IMRT) 계획을 수립하였다. 이후 빔전달 시 생성된 MLC 로그데이터(Dynalog)를 이용하여 실제 전달된 세그먼트 별 모니터단위(MU) 가중치와 MLC 엽(leaf)의 위치를 구한 후 이를 다시 Pinnacle3에 넣고 선량을 재계산하였다. 초기 치료 계획은 치료 계획용 CT 영상($CT_{plan}$) 및 CBCT 영상($CBCT_{plan}$)에 대하여 계산되었으며, 재구성된 선량 역시 치료 계획용 CT 영상($CT_{recon}$) 및 CBCT 영상($CBCT_{recon}$)에 대하여 계산되었다. 각 선량 계산을 $CT_{plan}$을 기준으로 하여 2차원 선량분포, 감마 인덱스, 선량-부피 히스토그램(dose-volume histogram; DVH)을 이용하여 분석하였다. 2차원 선량분포 및 DVH 분석 모두에서 원래의 치료 계획보다 실제 전달된 선량이 다소 많은 것으로 나타났으나 임상적인 의미는 미미했다. 감마 인덱스의 경우 CBCT에 선량을 계산했을 때 치료 계획 정보나 재구성된 선량 정보를 이용한 경우 모두 오차가 크게 발생했다. 재구성된 선량은 빔의 경계 부분에서 오차가 크게 발생하였으나 그 영향은 CT 및 CBCT 영상 간 차이에 의한 것보다 작았다. CBCT 영상에 전달된 선량을 재구성하게 되면 두 영향이 복합적으로 작용하여 오차는 더 줄어들게 되지만 $CT_{plan}$$CBCT_{plan}$의 차이에 비하여 $CBCT_{plan}$$CBCT_{recon}$ 차이는 상대적으로 작아 전달된 선량의 오차를 평가할 때 불확실성이 커졌다. 그러므로 선량 계산 오차의 양상은 셋업 오차, CBCT 영상을 이용한 선량 계산 오차 및 재구성된 선량 계산의 오차로 나누어 분석될 필요가 있을 것이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord. The obtained collagen was treated by pepsin to remove telopeptides, and finally, the immune-free atelocollagen was produced: The cross linked atelocollagen was highly resistant to the collagenase induced collagenolysis. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter. At the first stage, background noise is removed and at the next stage, only spectrum of QRS complex components is passed.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
