봉약침 시술 후에 발생한 Pain Shock 환자에 대한 임상보고 (A Clinical Study on the cases of The Pain Shock Patients after Korean Bee-Venom Therapy)

  • Objective : There has been no known report on the pain shock after administering Korean bee-venom therapy. Three accounts of pain shock were observed at the Sangji university affiliated Oriental medicine clinic from July 2001 through September 2001. This thesis will inform clinical progression and cautions on administering Korean bee-venom therapy. Methods: We were able to witness different patterns of pain shock during the treatment of degenerative knee joint, progressive oral paralysis, and A.L.S. In order to reduce heat toxicity of the bee venom, needling points were first massaged with the ice for 10 minutes before injecting $0.1{\sim}0.2cc$ of the bee venom. Points of injection were ST36, LI11, LI4 and others. Pain shock occurred after injecting on inner xi-an, outer xi-an and LI4. The phenomena associated with pain shock was recorded in chronological order and local changes were examined. Results: Through examining 3 patients with the pain shock, we managed to observe clinical progression, duration, and time linked changes on specific regions. We also managed to determine sensitive needling points for the pain shock. Conclution: Following results were obtained from 3 patients with the pain shock caused by Korean bee-venom therapy from July 2001 to September 2001. 1. Either positive or negative responses were shown after the pain shock. For case 1, extreme pain was accompanied with muscular convulsion and tremble, ocular hyperemia, delirium, stiffening of extremities, and hyper ventilation which all suggest positive responses. For case 2 and 3, extreme pain was accompanied with facial sweating, asthenia of extremities, pallor face, dizziness, weak voice, and sleepiness which are the signs of negative responses. 2. The time required to recover to stable state took nearly an hour (including sleeping time) and there was no side effect. 3. Precautions required to prevent the pain shock includes full concentration from the practitioner, accurate point location, precise amount of injection, physiological condition and psychological stability of the patient 4. Coping with the pain shock should be similar with a needle shock, and since extreme pain is accompanied, sufficient psychological rest must be provided. 5. Pain shock occurs because the patient cannot tolerate stimulation on the needling point. Thus, symptoms were similar to the needle shock in addition to excruciating pain. Further investigation and research must be done to have better understanding of an immune response and the pain shock associated with Korean bee-venom therapy.

특발성 외안근마비의 한의학적 치료 경향 분석 (The Analysis of Korean Medicine Treatment for Idiopathic External Ophthalmoplegia)

  • Objectives : This study is designed for analysis of trend and curative effect of Korean medicine for idiopathic external ophthalmoplegia. Methods : Research studies related to objectives were gathered through OASIS and KTKP and analyzed. Results : 21 case reports were selected. 5 korean medicine therapies, Herb, acupuncture, moxibustion, electroacupuncture, and pharmacopuncture, were executed. Restorative herb including Bojungikki-tang was frequently used in herb therapy. Facial and cephalic acupoints and non-facial, non-cephalic acupoints which individually differ were mostly simultaneously executed as acupuncture therapy. Also, moxibustion was partly performed. Electroacupuncture was carried out to affected area with electric stimulation at the strength of 1Hz and 4Hz. Hominis placental pharmacopuncture, 0.1cc or 0.5cc for each acupoint, was injected on disease area. After analyzing number of Korean medicine treatment and period of Korean medicine treatment from 21 case reports, the recovery period of idiopathic external ophthalmoplegia, 36.6 treatment session/36.69 days, was assessed to be more than one month shorter than previously known recovery period. Number of treatment sessions and treatment period for 11 cases when pharmacopuncture or electroacupunture was applied were 10/29, 35/29.96, which was 15 days shorter in terms of treatment period and 10 times less in terms of number of treatment sessions compared to cases when pharmacopuncture or electroacupunture was not applied. Conclusions : Korean medicine treatment can be effective for recovery of idiopathic external ophthalmoplegia.

원락(原絡) 배혈(配穴) 전침 자극이 백서의 족과 염좌(捻挫) 통증에 미치는 영향(影響) (Effect of Electroacupuncture Applied to the Won Point and Rak Point in the Rat Model of Ankle Sprain Pain)

  • Objectives : In the present study, the effect of electroacupuncture (EA) applied to SI6 and won-rak point on the ankle sprain model was examined. A common source of persistent pain in humans is the lateral ankle sprain. Methods : To model this condition, the rat's right ankle was bent repeatedly, overextending lateral ligaments, for 4 min under enflurane anesthesia. The rat subsequently showed swelling of the ankle and a reduced stepping force of the affected limb for the next several days. The reduced stepping force of the limb was presumably due to a painful ankle. EA was applied to the several acupuncture point on the contralateral forelimb for 30 min under gaseous anesthesia. After the termination of EA, behavioral tests measuring stepping force and Paw volume were Periodically conducted during the next 4 h and 18 h respectively. Results : EA applied to $SI_6$ with won-rak point produced more powerful improvement of stepping force of the sprained foot than to $SI_6$ alone lasting for at least 4 h. However, neigher $KI_4$ point nor $BL_{64}$ point produced any significant increase of weight bearing force. The improvement of stepping pressure was interpreted as an analgesic effect. The analgesic effect was specific to the acupuncture point since the analgesic effort on the ankle sprain pain model could not be mimicked by EA applied to $KI_4$ or $BL_{64}$. In addition, EA applied to $SI_6$ with won-rak combination point showed inhibitory effect on the paw edema induced by ankle sprain. Also, COX-2 protein expression increased by ankle sprain were suppressed by the EA stimulation. Conclusion : These data suggest that EA with won-rak combination point produces a more potent analgesic effect on the ankle sprain pain model in the rat and that EA with won-rak combination point induced anti-inflammatory effect through the suppression of COX-2 protein expression.

족삼리(足三里) 배혈(配穴)에 따른 전침(電鍼)이 흰쥐 대뇌피질(大腦皮質)의 NADPH-diaphorase와 nNOS, NPY, VIP 신경세포(神經細胞)에 미치는 영향(影響) (Effect of Joksamni combination on NADPH-diaphorase, neuronal Nitric Oxide Synthase, Neuropeptide Y and Vasoactive Intestinal Peptide in the cerebral cortex of Spontaneously Hypertensive Rat)

  • Objective: The aim of this study was to investigate the effects of Joksamni(ST36) combination on NAD PH-diaphorase, neuronal nitric oxide synthase(nNOS), neuropeptide Y(NPY) and vasoactive intestinal peptide (VIP) in the cerebral cortex of spontaneously hypertensive rat. Methods: The experimental groups were divided into four groups: Normal, Joksamni(ST36), Joksamni(ST36)+Eumneungcheon(SP9), and Joksamni(ST36)+Gokji(LI11). Needles were inserted into acupoints at the depth of 0.5cm with basic insertion method. Electroacupuncture was done under the condition of 2Hz electrical biphasic pulses with continuous rectangular wave lasting for 0.2ms until the muscles produced visible contractions. Such stimulation was applied continuously for 10 minutes, 1 time every 2 days for 10 sessions of treatments. Thereafter we evaluated changes in NADPH-d positive neurons histochemically and changes in nNOS, NPY and VIP positive neurons immunohistochemically. Results: The optical densities of NADPH-d positive neurons of the Joksamni(ST36)+Eumneungcheon(SP9) group in all areas of cerebral cortex and Joksamni(ST36)+Gokji(LI11) group in primary somatosensory cortex, visual cortex, auditory cortex, perirhinal cortex were significantly increased as compared to the Joksamni(ST36) group. The optical densities of NADPH-d positive neurons of the Joksamni(ST36)+Gokji(LI11) group were significantly decreased as compared to the Joksamni(ST36)+Eumneungcheon(SP9) group with the exception of primary somatosensory cortex. The optical densities of nNOS positive neurons of the Joksamni(ST36)+Eumneungcheon(SP9) group in all areas of cerebral cortex and Joksamni(ST36)+Gokji(LI11) group in auditory cortex, perirhinal cortex, insular cortex were significantly increased as compared to the Joksamni(ST36) group. The optical densities of nNOS positive neurons of the Joksamni(ST36)+Gokji(LI11) group were significantly decreased in all areas of cerebral cortex as compared to the Joksamni(ST36)+Eumneungcheon(SP9) group. The optical densities of NPY positive neurons of the Joksamni(ST36)+Gokji(LI11) group were significantly decreased in primary motor cortex, primary somatosensory cortex, cingulate cortex as compared to the Joksamni (ST36) and Joksamni(ST36)+Eumneungcheon(SP9) groups. The optical densities of VIP positive neurons of the Joksamni(ST36)+Eumneungcheon(SP9) group were significantly increased in all areas of cerebral cortex except for cingulate cortex as compared to the Joksamni(ST36) group. The optical densities of VIP positive neurons of the Joksamni(ST36)+Gokji(LI11) group were significantly decreased in auditory cortex, cingulate cortex, perirhinal cortex as compared to the Joksamni(ST36) group. The optical densities of VIP positive neurons of the Joksamni(ST36)+Gokji(LI11) group were significantly decreased in all areas of cerebral cortex as compared to the Joksamni(ST36)+Eumneungcheon(SP9) group. Conclusions: The result demonstrated that electroacupuncture on Joksamni(ST36) and its combination change the activities of the NO system and peptidergic system in the cerebral cortex of SHR and that acupoint combination is one of the important parameters for the effects.

족삼리 전침자극에 의한 흰쥐 hypothalamus의 유전자 발현 profile 분석 (Gene Expression Profile of Rat Hypothalamus Treated with Electroacupuncture at ST36 Acupoint)

  • Electroacupuncture (EA) has been reported to increase pain threshold, and to enhance the NK cell activity by up-regulation of IFN-γ and endogenous β-endolphin. For the purpose of understanding the molecular mechanism of EA stimulation, we analyzed the gene expression profile of rat hypothalamus, treated on Zusanli (ST36) with EA, in comparison with control group by oligonucleotide chip microarray (Affymetrix GeneChip Rat Neurobiology U34 Array) and real-time RT-PCR. Sprague-Dawley (S-D) male rats were stimulated at the Zusanli (ST36) acupoint in restriction holder. Simultaneously the control group was given only holder stress without EA stimulation. In order to prove the appropriateness of EA treatment, we measured spleen NK cell activity with standard 51Cr release assay. NK cell activity of EA group was significantly increased comparing to control group. The microarray and PCR results show that EA treatment up-regulates expression of genes associated with 1) nerve growth such as NGF induced factor A and VGF, 2) signal transduction such as 5HT3 receptor subunit, AMPA receptor binding protein and Na-dependent neurotransmitter transporter, and 3) anti-oxidation such as superoxide dismutase and glutathione S-transferase. In addition, the activity of the anti-oxidative enzyme, SOD of hypothalamus, liver and RBC was enhanced compared to that of control. The list of differentially expressed genes may implicate further insight on the mechanism of acupuncture effects.

금연이침저주파지극기(HBN-001)의 개발 및 금연효과 Pilot Test (A Pilot Test on Stop-Smoking and Development of HBN-001)

  • 최근 수명 연장 등으로 건강에 대한 관심이 높아지면서 흡연자들의 금연에 대한 욕구가 높아지고 있다. 한의학에서는 금연치료법으로 이침요법(Auricular Acupuncturing)을 사용하는데, 저주파 자극을 통하여 금연에 효과 있는 폐점(Lung point)과 내분비점(Endocrine point)을 자극할 수 있는 기기를 만들어 실제 효과가 있는지 알아보았다. 먼저 이혈에 저주파 자극과 헤드셋이 결합된 금연이침저주파자극기(HBN-001)시제품을 개발하였고 이 기기를 이용하여 20명의 피험자에게 Pilot Test를 해보았다. 흡연량은 시술 전 12.19 개비였으며, 5회 이상 시술하여 2주후에는 10.34개비로 줄었으나 통계적으로 유의한 차이는 없었다. 수술 후 흡연에 대한 욕구는 변화 없다 4명(20%), 약간감소 4명(20%), 25-49% 감소 4명(20%), 50-75% 감소 6명(30%), 75-99% 감소 1명(5%), 욕구가 완전히 사라졌다 1명(5%) 이었다. 담배 맛의 변화에 대해서는 피험자 중 약간 좋아졌다 2명(10%), 변화없다 7명(35%), 약간 나빠졌다 6명(30%), 많이 나빠졌다 5명(25%) 이었다. Pilot Test 상으로는 긍정적으로 금연을 도울 수 있을 것으로 보이며, 향후 더 깊은 연구가 필요할 것으로 보인다.

냉온 자극의 다양한 온도경계조건들에 대한 피부 내 온도 분포의 수치해석 (Numerical Analysis for the Conjugate Heat Transfer of Skin Under Various Temperature Conditions of Contrast Therapy)

  • 본 연구에서는 냉온 자극요법의 열전달 특성을 정량적으로 파악하고자 한다. 축대칭 비압축성 Navier-Stokes 방정식 및 에너지 방정식의 해법을 위해 SIMPLE 알고리즘에 기반을 둔 유한체적법을 사용하였다. 혈액의 관류를 고려하기 위해 Pennes bio-heat 방정식을 추가로 적용하였다. 다양한 온도조건에 따른 피부 아래 각 지점에서의 열 침투양상을 분석하였다. 수치해석을 통하여 다양한 입력온도들에 대하여 역치 온도에 도달하는 영역들을 발견하였다. 또한, 페니스 효과로 인하여 피부 심부로 갈수록 외부 자극에 대한 온도변화가 완만함을 확인하였으며, 진피부분의 온도가 역치에 도달하기 위해서는 온 자극 및 냉 자극은 47도 및 7도 내외로 주어져야 함을 발견하였다. 본 논문의 수치해석을 통해 구축된 데이터베이스는 경혈 냉온 자극 치료기의 최적설계 시에 활용된다.

12경맥(經脈)의 오수혈중(五輸穴中) 합혈(合穴) 침자(鍼刺)가 백서(白鼠)의 혈위(穴位) 조직내 NO, nNOS와 조직 및 혈장 Norepinephrine의 변화에 미치는 영향(影響) (Effects of Acupuncture at the Sea Point on the Changes of Plasma and Tissue Levels of NO, nNOS, Norepinephrine in Rats)

  • Objectives : This study is to observe the changes in the expression of neurotransmitters, such as NO, nNOS, and NE, upon the needle insertion to the sea points, which is one of the five transport points. Methods : Needles were inserted into rats, on both left and right sides of all sea points, including the LU5, PC3, HT3, LI11, TE10, SI8, SP9, LR8, KI10, ST36, GB34, and BL40, which are the sea points of five transport points for 12 meridian vessels. After insertion, needles were retained for five minutes. After the retention, blood was drawn via cardiac puncture, and tissues of each point near meridian vessels were extracted to examine the changes in the expression of NO, nNOS and NE. Results : In terms of the effect in NO production, there was a significant decrease only in the LU5 point, whereas there was a significant increase in the TE10 point alone. In terms of the expression of nNOS within tissues, none of the experimental groups showed significant changes based on the results of immunohistochemistry and western blotting. Regarding the formation of norepinephrine within tissues, the HT3, SP9, and KI10 point showed a significant decrease, while the PC3 and LR8 point showed a significant increase. Production of plasma norepinephrine was significantly increased at the TE10, SP9, LR8, GB34, and BL40 point. Conclusions : The effect of needles applied at the sea points of five transport points of 12 meridian vessels on the functions of NO, nNOS, and NE could be observed, and it is considered that the effect of needle stimulation on nervous system disorders could be studied through additional researches based on this one.

흰쥐에 대한 양계(LI5), 양곡(SI5), 지구(TE6) 침자가 Neuronal, Inducible 및 Endothelial Nitric Oxide Synthase와 Nitric Oxide의 변화에 미치는 영향 (Effects of Acupuncture at LI5, SI5, TE6 on Changes of NO and NOSs in Rats)

Methods : Based on rats, needle was inserted on both sides of LI5, SI5 and TE6, which are river points of three yang meridians on the forefoot, and were retained for five minutes. After the retention, blood was drawn via cardiac puncture and tissues from each point around meridian vessels were extracted to be examined on the changes of the expression of NO, as well as of nNOS, iNOS and eNOS. Results : In terms of the effect on NO creation in tissues, none of the experimental groups showed any significant change compared to the Normal group. In terms of the effect on expression of nNOS within tissues, LI5 and SI5 showed significant increase based on the results of immunohistochemistry. In iNOS within tissues, LI5 and SI5 showed significant increase based on the results of immunohistochemistry. In eNOS within tissues, SI5 showed significant increase based on the results of immunohistochemistry. Conclusions : The effect on the function of NO, nNOS, iNOS and eNOS of needle insertion on the river points of three yang meridians on the forefoot could be observed, and based on this study, it is considered that the effect of needle stimulation on the changes of nervous system could be found out through additional research.

백서의 경혈(해계, 곤륜, 양보) 침자가 Nitric Oxide Synthase 및 Nitric Oxide의 변화에 미치는 영향 (Effects of Acupuncture at ST41, BL60, GB38 on Changes of Nitric Oxide and Nitric Oxide Synthase in Rats)

  • Objectives : To observe the changes in the expression of nNOS, iNOS, eNOS and NO by the needle insertion on river points, one of the five transport points of three yang meridians of the foot. Methods : Based on rats, needle was inserted on both left and right sides of ST41, BL60 and GB38 and retained for five minutes. After the retention, blood was drawn via cardiac puncture and tissues from each point around meridian vessels were extracted to observe the changes in the expression of nNOS, iNOS, eNOS and NO. Results : In terms of the effect on expression of nNOS within tissues, ST41 showed significant decrease based on the results of immunohistochemistry. In terms of the effect on expression of iNOS within tissues, none of the experimental groups showed any significant change compared to the Normal group.
