Objectives : Siguan points (LI4, LR3) have been most frequently applied for various diseases, especially different digestive disorders such as constipation, abdominal pain or various intestinal inflammatory diseases. The fact that gastrointestina movement is closely connected with physiologic functions or pathologic process of alimentary canal led us to ask the question if Siguan points affects on intestinal motility. Design: To investigate the effect of Siguan acupuncture on the intestinal movement in both physiologic and pathologic conditions, we divided the experimental animals into 12 groups. Six groups were pre-treated with loperamide (0.5 mg/kg, sc) or scoploamine (0.5 mg/kg, sc) to suppress the intestinal movement and another three groups were pretreated with carbachol (0.5 mg/kg, po) to activate it, whereas the rest three groups didn't receive any pretreatment to be kept in the physiological condition. After the administration with charcoal meal, mice were acupunctured bilaterally on sham point or Siguan points as the manner of tap-stimulation, with the exception of no acupuncture groups. Methods : Mice were scarified in twenty minutes after the administration of charcoal to measure the distance of charcoal passage from stomach-duodenal junction. The effect on intestinal movement was presented by calculating the relative distance where charcoal arrived to total length of small intestine. Results : In physiological state, charcoal meal passed around 53%, and there was no significant difference between Siguan points and sham points groups. On the other hand. Siguan points-sitimulation significantly ameliorated loperamide or scoplolamine-induced suppressed travel rate of 17.3 % and 18.6% in sham point into 26% and 26.3% respectively (p<0.05). In carbachol-induced accelerated condition, Siguan points-stimulation didn't affect intestinal motility comparing to sham point group passed about 97.6%. Conclusions : These results postulate that acupuncture at Siguan points have a therapeutic effect by restoring cholinergic activity on pathogenically suppressed intestinal peristalsis, but does not affect the gastrointestinal motility in the normal or accelerated condition.
Objectives : The purpose of this study is to review RCTs on pharmacopuncture treatment for musculoskeletal diseases and to establish standards of pharmacopuncture treatment model. Methods : We searched articles up to date of March 2009 via computerized databases of Pubmed, The Journal of Korean Acupuncture & Moxibustion, Journal of Korean institute of Herbal Acupuncture, Journal of Oriental Rehabilitation and Journal of Korean Oriental Medicine. Only Randomized Controlled Trials (RCT) concerning the effects of pharmacopuncture on musculoskeletal diseases. The pharmacopuncture treatment methods were assessed based on STRICTA and items considering the feature of pharmacopuncture. And the methodological quality of the trials was assessed by FEAS and modified Jadad score. Results : Eighteen trials of pharmacopuncture on musculoskeletal diseases were analyzed. Except for 4 trials comparing the effect of SBV and BV, positive outcome was reported in ten trials. Among eighteen trials; most of the trials were about Bee Venom acupuncture, and most of the trials used about five acupuncture points, mainly local acupuncture points. But, the amount of injection to each point and total injection were various. And most of trials were lack in the information about method of stimulation. The adjusted FEAS score ranged from 0 to 12, and modified Jadad scoreranged from 1 to 5. Conclusions: To standardize pharmacopuncture treatment, we need more well-designed, high quality clinical trials. And methodological assessment tools designed for pharmacopuncture treatment are also needed.
Objective : The central opioid mechanism of acupuncture analgesia has been fairly well documented in acute behavioral experiments, but little electrophysiological study has been performed on the peripheral mechanism and subtypes of opioid receptors responsible for acupuncture-induced antinociception in chronic animal models. In the present electrophysiological experiment, we studied the peripheral mechanism and opioid receptor subtypes which Were implicated in electroacupuncture-induced antinociception in the rat with chronic inflammatory and neurogenic pain. Methods : In the rat with complete Freund's adjuvant-induced inflammation and spinal nerve injury, dorsal horn cell responses to afferent C fiber stimulation were recorded before and after electroacupuncture (EA) stimulation applied to the contralateral Zusanli point for 30 minutes. Also studied Were the effects of specific opioid receptor antagonists and naloxone methiodide, which can not cross the blood-brain barrier, on EA-induced inhibitory action. Results : EA-induced inhibitory action was significantly attenuated by naloxone methiodide, suggesting that EA-induced inhibition was mediated through peripheral mechanism. Pretreatment, but not posttreatment of naltrexone and spinal application significantly blocked EA-induced inhibitory actions. In inflammatory and neurogenic pain models, ${\mu}-$ and ${\delta}-opioid$ receptor antagonists (${\beta}-funaltrexamine$ & naltrindole) significantly reduced EA-induced inhibitory action, but ${\kappa}-opioid$ receptor antagonist had weak inhibitory effect on EA-induced antinociception. Conclusion : These results suggest that 2Hz EA-stimulation induced antinoeiceptive action is mediated through peripheral as well as central mechanism, and mainly through ${\mu}-$ and ${\delta}-opioid$ receptors.
Objectives : The purpose of this research was to develop the magnetic acupuncture system which used solenoid coil for magnetizing acupuncture needle. The system could generate the meridian electric potential (MEP) similar to the potential by manual acupuncture. Thus, we tried to confirm the therapeutic effect that is caused by the MEP generation. Methods : To confirm the MEP, we stimulated the magnetic acupuncture with at 2Hz, $92.7{\pm}2mT$, PEMFs (Pulsed Electro-Magnetic Fields) at ST37 and measured the evoked potential between ST36 and ST41. Also, we conducted a fatigue recovery test using isokinetic exercise in order to identify the therapeutic effect on musculoskeletal disorders. We chose LR9 as a stimulation point. To observe the state of fatigue, we measured the EMG and analyzed median frequency and peak torque for 20minutes. Results : We observed that MEP which incurred from magnetic acupuncture was higher than he reported MEP induced by manual acupuncture. Moreover, its modes were divided into two types by the direction of magnetic flux. When generating magnetic flux in the direction of acupoint, the positive peak voltage of the MEP was generated. In contrast, negative peak voltage of the MEP was generated whenever meganetic flux generated in the outward direction. As a result of fatigue recovery, the median frequency (MF) of the magnetic acupuncture group were recovered faster than that of the non-stimulation group. However, the peak torques of both groups were not restored until after 20 minutes. Conclusions : We confirmed that the magnetic acupuncture system can lead to the MEP similar to manual acupuncture. Moreover, the MEP had a therapeutic effect on the musculoskeletal disorders.
Stimulation of the auricle is known to be effective in reducing obesity. The aim of the present study is to investigate whether stimulating a specific auricular acupuncture point is effective on suppression of appetite. Cholesystokinin (CCK) is known to induce a powerful feeding response after central administration and particularly abundant in the cerebral cortex. In food-deprived rats exhibiting a strong drive for feeding, decreased CCK release was markedly detected in the cerebral cortex of the brain. Needling the fasted rats on the specific auricular region increased the CCK level of the cerebral cortex. In conclusion, stimulating the specific auricular points increases CCK-expression in the cerebral cortex of the fasted rats and may be useful for controlling obesity.
Objectives : This study was designed to evaluated the effect of electro acupuncture on Heart Rate Variability of Patients with functional voiding disease. Methods: Subjects were voluntarily recruited by newspapers and internet. Electro acupuncture was performed three times a week for 3 weeks. Acupuncture point for EA group was B32, Electrical stimulation frequency was 2Hz, duration 20min, and intensity was up to pain threshold according to patients. The patients' Heart Rate Variability was measured before, after 3 weeks of treatment for 5 minutes. Results : In time domain analysis, There were significant increase of SDNN and RMSSD(p<0.05) after 3 weeks treatment. In frequency domain analysis, There were significant increase of LF and LF /HF ratio(p<0.05) after 3 weeks treatment. There were no significant changes in other components of heart rate variability. Conclusion : This study suggests that electroacupuncture treatments can be applicable to improve autonomic nervous system in patients with functional voiding disease.
Objectives : The acupuncture about acupoint affects the production of NO, NOS, and NE.Local action of acupuncture is important for acupuncture treatment. To prove this, the revelation degree of NO, NOS, and NE was observed by stimulating the acupuncture at the connecting point of SP4, KI4, and LR5 in the depths of Superficial layer, Middle layer and Deep layer. Methods : Needles were inserted into rats, on each right and left sides of the connecting point, SP4, KI4 and LR5 acupoints which are the stream points of the foot meridian. After insertion, needles were retained for three minutes. After the retention, rat was sacrificed via cardiac puncture, and tissues of each SP4, KI4 and LR5 point near meridian vessel was extracted to examine the changes in the expression of NO, NOS and NE. Results : In terms of the effect in NO production, there was significant increase in the Superficial layer, Middle layer and Deep layer at KI4. In terms of the effect in NE production, there was significant decrease in the Superficial layer at SP4 and increase in the Superficial layer, Middle layer and Deep layer at LR5. In terms of the effect in nNOS production, there was significant increase in the Superficial layer, Middle layer and Deep layer at SP4 also in the Superficial layer at KI4. In terms of the effect in eNOS production, there was a significant increase in the Superficial layer, Middle layer and Deep layer at SP4, KI4 and LR5. In terms of the effect in iNOS production, there was significant increase in the Superficial layer, Middle layer and Deep layer at SP4, KI4 and LR5. Conclusions : The effect of acupuncture applied at the connecting point of six meridians of the foot on the activities of NO, NOS and NE could be observed, and it can be induced from the effect of needle stimulation on disrupted local and systemic nervous responses.
Objectives : The usage of acupuncture has gained popularity as an alternative method of treatment for certain chronic pain conditions. However, the efficacy of acupuncture in various diseases has not been fully established and the underlying mechanism is not clearly understood. In the present study, the effect of electroacupuncture (EA) applied to foot samli$(ST_{36})$ on the carrageenan-induced knee arthritic pain was examined. Methods : A common source of persistent pain in humans is the knee arthritis. Knee arthritis was induced by injection of 2 % carrageenan $50\;{\mu}l$ into the knee joint cavity. When rats developed pain behaviors, EA was applied for 30 min. under enflurane anesthesia with repeated train stimuli at the intensity of 10X of muscle twitch threshold. The weight bearing force of the hind limb was measured for an indicator of pain level after each manipulation. Results : The average weight borne by the hind limb during normal gait was 55% of total body weight, which was reduced to less than 10% after knee arthritis. EA improved the weight bearing of the arthritic hind limb significantly for the duration of 4 hr. EA applied to $ST_{36}$ point produced a significant improvement of stepping force of the arthritic foot lasting for at least 4 h. However, $GB_{31}$ point did not produce any significant increase of weight bearing force. The analgesic effect was specific to the acupuncture point since the analgesic effect on the knee arthritis model could not be mimicked by EA applied to a nearby point, $GB_{31}$. The relations between EA-induced analgesia and endogenous nitric oxide(NO) and inducible NO synthase(iNOS)/neuronal NOS was also examined. Results were turned out that both NO production and nNOS/iNOS protein expression which is increased by arthritis were suppressed by EA stimulation applied to $ST_{36}$ point. Conclusions : The data suggest 1) that EA produces a potent analgesic effect on the rat model of chronic knee arthritis pain in a point specific manner and 2) that EA-induced analgesia modulate endogenous NO through the suppression of nNOS/iNOS protein expression.
Objectives: Acupuncture has been shown to relieve pain and modulate biological function by stimulating the organ-specific meridians and enhances parasympathetic activity and suppresses sympathetic activity. The aim of this review is to summarize and understand the effects of acupuncture on autonomic nervous system. Methods: We reviewed a total of 29 studies published from 2000 to 2010 searched by PueMed and various domestic oriental medicine journals to obtain acupuncture studies related with ANS. Each article was classified by ANS measurement index and reviewed for study objectives, outcomes, acupuncture points, experimental and control interventions. Results: In the study of acupuncture about EEG, HRV, SCR, the experiment results are not the same by acupoints. Although same acupoint the results differ by subject's condition. But the study showed some clear tendency. In brief, in normal states acupuncture enhanced either vagal or sympathetic tone depending on the stimulated acupuncture point sites. On the other hand, most of studies demonstrated that acupuncture restored the autonomic dysfunctions in various kinds of tired or stressful states. Conclusions: We reviewed studies that contributed to an understanding of the effects and mechanisms of acupuncture on autonomic nervous system. Although the relationship between acupuncture and ANS response is still uncertain, acupuncture could be a excellent treatment method for modulating autonomic dysfunction.
Objectives : In the present study, the effect of electroacupuncture (EA) applied to hand yang meridian on the ankle sprain model was examined. Methods & Results : A common source of persistent pain in humans is the lateral ankle sprain. To model this condition, the rat's right ankle was bent repeatedly, overextending lateral ligaments, for 4 min under halothane anesthesia. The rat subsequently showed swelling of the ankle and a reduced stepping force of the affected limb for the next several days. The reduced stepping force of the limb was presumably due to a painful ankle. EA was applied to the several acupuncture point on the contralateral forelimb for 30 min under gaseous anesthesia. After the termination of EA, behavioral tests measuring stepping force were periodically conducted during the next 4 h. EA applied to SI-6 point produced a significant improvement of stepping force of the sprained foot lasting for at least 2 h. However, neigher LI-4 point nor TE-3 point produced any significant increase of weight bearing force. The improvement of stepping pressure was interpreted as an analgesic effect. The analgesic effect was specific to the acupuncture point since the analgesic effect on the ankle sprain pain model could not be mimicked by EA applied to a nearby point, LI-4 or TE-3. The analgesic effect of EA applied to SI-6 was more powerful when EA was applied by low-frequency and high-intensity stimulation. In addition, this effect need to be stimulated more than 15 min. Conclusions : These data suggest that EA produces a potent analgesic effect on the ankle sprain pain model in the rat. This analgesic effect is produced by applying EA to a Tae-Yang meridian at opposite side from the painful area in a stimulus point-specific way.
