Three Case Reports of Patients Treated with Korean Medicine after Rotator Cuff Repair

  • Objectives : This study examined the effects of Korean medicine treatment in three patients following rotator cuff repair. Methods : Patients were treated with acupuncture, bee venom, moxibustion, and herbal medicine. Treatments were performed for an average of 3 weeks. A numeric rating scale (NRS) and range of motion (ROM) were used for evaluation of treatment effects. Results : The NRS score decreased and the ROM increased after treatment. In Case 1, the NRS score decreased from 10 to 6; flexion increased from $25^{\circ}$ to $180^{\circ}$ and abduction increased from $35^{\circ}$ to $180^{\circ}$. In Case 2, the NRS score decreased from 10 to 7; flexion increased from $30^{\circ}$ to $125^{\circ}$ and abduction increased from $15^{\circ}$ to $100^{\circ}$. In Case 3, the NRS score decreased from 10 to 3; flexion increased from $40^{\circ}$ to $120^{\circ}$ and abduction increased from $60^{\circ}$ to $95^{\circ}$. Conclusion : Korean medicine treatment following rotator cuff repair was effective in decreasing the NRS score and increasing ROM. Although the study only involved 3 cases, Korean medicine treatment may reduce the duration of rehabilitation.

침도침 시술을 가미한 복합한방치료를 시행한 결절성 다발 동맥염의 치험례 (Case Report of Polyarteritis Nodosa Treated with Oriental Treatments Including Acupotomy)

  • Objectives : Polyarteritis nodosa is a progressive disease of connective tissue that is characterized by nodules along arteries; nodules may block the artery and result in inadequate circulation to the particular area. This report is intended to estimate the efficacy using oriental complex treatment on a patient with Polyarteritis nodosa. Materials and Methods : From 19th March, 2012 to 12th May, 2012, One male inpatient diagnosed with polyarteritis nodosa was treated with general oriental medicine therapy : needle-embedding therapy ; acupuncture ; pharmacopuncture ; acupotomy therapy and herbal medication. VAS(visual analogue scale) was used for evaluation of both leg pain. Other subjective symptoms including night sweat, tinnitus, upper heat were evaluated by percentage comparing the symtoms before and after treatment. Results : The patient showed a certain degree of improvement in both leg pain and other subjective symtoms. Conclusions : Oriental treatments such as needle-Embedding therapy, acupuncture and moxibustion therapy, pharmacopuncture therapy, acupotomy therapy and herbal medication can be effective for controlling pain and other accompanied symtoms due to polyarteritis nodosa.

Clinical Research Using Salviae Miltiorrhizae Radix-Pharmacopuncture for Lumbar Herniated Intervertebral Disc: Analysis of Trends

  • This review of national and international randomized controlled trials of Salviae miltiorrhizae radix pharmacopuncture for lumbar herniated intervertebral discs was performed to assess its clinical efficacy. There were 5 online databases (PubMed, EMBASE, CNKI, NDSL, OASIS, and RISS) searched on June 1st, 2020. Studies were selected according to the inclusion and exclusion criteria and were reviewed by risk of bias assessment. This review included 14 Chinese studies. The sample sizes ranged from 50 to 100. The numbers of treatments ranged from 20 to 30, with most patients receiving 20 treatments. The longest treatment periods were 10-15 and 15-20 days, of which each accounted for 29% of the studies. The most frequently used evaluation indices were the Japanese Orthopedic Association and the Visual Analog Scale scoring method. The most frequently used acupoints were EX18 and BL25, which accounted for 31% of the total number of acupoints. In 50% of the studies, the pharmacopuncture injection volume was 2 mL. Acupuncture treatment was the most common control group. Eleven studies reported that the intervention group had significantly improved symptoms. However, most of the included studies were of low quality.

Effects of Complex Korean Medicine Treatment on a Patient with Knee Pain and Ankylosis Following a Distal Femur Osteotomy: A Case Report

  • Distal femur osteotomy (DFO) is a controlled surgical break of the femur performed to allow realignment of the limb. Redistribution of the load aims to correct the abnormal mechanical weight-bearing axes in patients with abnormal alignment of the lower extremities, and degenerative changes in the knee joint. This report describes a complex Korean medicine treatment for a patient complaining of knee pain and stiffness following a DFO. Post-operative care for the patient lasted 78 days with treatment including pharmacopuncture, acupuncture, herbal medicine, cupping therapy, and physiotherapy. The effectiveness of the treatments was evaluated using the numerical rating scale, range of motion of the knee, and by physical examination. After treatment, these evaluation indicators improved, suggesting that the complex Korean medicine treatment received by the patient was an appropriate treatment for knee pain and stiffness following a DFO.

비만환자의 비만감소를 위한 침술요법에 대한 국내외 논문 분석 (Analyses of stidies on acupuncture therapies for reducing obesity in obeses patients)

  • 본 연구의 목적은 비만환자의 비만을 감소시키기 위한 침술요법의 현황을 파악하고 침술요법의 내용과 효과를 파악하는 것이다. 문헌검색은 '비만', '침', '이침', '전기침' 등을 검색어로 하여 한국학술정보, 한국교육학술정보원, 국회도서관, Pubmed, CINAHL 등을 통해 2003년 1월부터 2017년 12월까지 발표된 연구논문을 검색하였다. 연구자가 원본을 확인한 후, 선정한 19편의 논문을 최종 분석 하였다. Scottish Intercollegiate Guideline Network의 체크리스트로 논문의 질적 평가를 실시한 결과 10점 만점에 9점인 연구가 1편, 10점 만점에 7-8점인 연구가 15편, 10점 만점에 6점인 연구가 2편, 10점 만점에 3점인 연구가 1편이었다. 침술요법의 내용을 파악한 결과 비만을 감소시키기 위한 경혈점은 shemen, hunger, LI-11(Quchi), LI-4(Hegu), St-25(Tianshu), St-44(Neiting) 등이었다. 침술요법 총 시행 횟수는 평균 16.9회이었고, 침술요법 중재 기간이 20일 혹은 72일인 연구에서의 침술요법 중재 빈도는 1회/1-2일이었고, 침술요법 중재 기간이 4-18주인 연구에서의 침술요법 중재빈도는 1-3회/주이었다. 분석한 연구결과에 의하면 침술요법은 신체계측수치의 감소, 혈당 및 혈중 지질 농도의 감소, 혈중호르몬 변화 등을 일으켜 비만환자의 비만을 감소시키는 효과가 있었다. 향후 비만환자의 비만감소를 위한 침술요법의 효과성을 평가하기 위해 잘 설계된 연구가 시급히 필요하다.

거위발 점액낭염의 중의학적 치료에 대한 2014-2024년도의 연구 동향 분석: China National Knowledge Infrastructure를 중심으로 (Analysis of Research Trends in Traditional Chinese Medicine Treatments for Pes Anserine Bursitis from 2014 to 2024: Using the Search Results of China National Knowledge Infrastructure)

  • Objectives The aim of this study is to investigate the research trends in traditional Chinese medicine (TCM) treatment for pes anserine bursitis using the search results from the China National Knowledge Infrastructure (CNKI). Methods We searched for clinical studies on pes anserine bursitis in the CNKI electronic database. We analyzed the publication year, study type, participant demographics, treatment methods and duration, evaluation criteria, adverse events, and the results of each article. Results Seventeen studies published from January 1, 2014 to January 1, 2024 were selected. Among the 17 selected studies, 14 were randomized controlled trials and 2 were case reports. The most frequently used treatment intervention was acupuncture. Nine types of acupuncture were identified, including electro-acupuncture, pharmacopuncture, acupotomy, floating acupuncture, and other new types of acupuncture. The most commonly used evaluation outcome was effective rate. Conclusions Various TCM treatments for pes anserine bursitis were introduced, and we could identify the effectiveness of TCM in treating pes anserine bursitis. However, it is anticipated that more well-designed and high-quality evaluation studies will be required in the future.

도플러 초음파검사를 이용한 안면마비 환자의 안면부 경혈의 혈류 관찰 (Clinical Observation on Blood Flow of Facial Acupuncture Points in Bell's Palsy Patients by Doppler Ultrasound)

  • Objectives : The aim of this study was to investigate whether Doppler ultrasound device is applicable for the evaluation of facial palsy patients. Methods : A total of 25 patients participated in the first assessment and 22 of them finished a follow-up assessment one week later. An assessment comprised of a Minimax-Doppler-K device examination on bilateral acupoints $ST_5$, $SI_{19}$, $ST_2$, and $BL_1$, House-Brackmann grading, and subjective symptom questionnaire. Measurement reliability was assessed and clinically meaningful variations of Doppler measurement values were explored. Results : Doppler ultrasound examination revealed a significant difference of some measurement values between values of the symptom side and the contralateral side, between groups of severe paralysis and moderate paralysis, and between groups of short duration(0-3 weeks) and long duration (3 weeks-5 months) by non-parametric analyses (p<0.05). Several re-examination values showed a significant correlation with the first examination values by Spearman's correlation tests (p<0.05). Conclusions : This study revealed several possibilities for the clinical application of this device. Further validity tests and device improvements for a user's convenience would be helpful for its practical application.

자동 미세침이 말초성 안면마비 환자의 증상 정도 및 안전성에 미치는 영향에 대한 임상 관찰 (Clinical Observation of Effect on Severity of Symptoms and Safety of Auto-microneedle Therapy in Patients with Peripheral Facial Paralysis)

  • Objectives : This study was to observe clinical application of auto-microneedle therapy system (AMTS) in patients with peripheral facial paralysis. Methods : 27 peripheral facial paralysis patients were observed after taking AMTS at Facial Palsy Center, Kyung Hee University Oriental Medicine Hospital from March 1, 2011 to January 9, 2012. We assessed the symptoms of facial paralysis with Yanagihara unweighted grading system, Sunnybrook facial grading system(SBGS) and facial disability index(FDI), and observed adverse events and total safety of the treatment. Results : The scores of facial palsy scales increased after AMTS in Yanagihara grading system and Sunnybrook facial grading system. AMTS-related adverse events were mild pain(5.9%) and fatigue(3.5%), which needed no extra treatment. The total safety evaluation was between 'safe' and 'nearly safe' level. There were no other serious adverse events. In addition, patients were satisfied with subjective improvement including facial tingling and numbness. Conclusions : AMTS can be applied as an adjunctive treatment for patients with peripheral facial paralysis due to its safety and clinical usefulness. It is easier to stimulate wide skin area in a short time. Further clinical research is required to investigate the effectiveness of ATMS in a more rigorous RCTs.

월경곤란증의 자하거 약침효과에 관한 연구 (Effect of Hominis placenta Pharmacopuncture on the Dysmenorrhea (A Pilot study, Single blind, Randomized, Controlled Clinical Trial))

  • Objective: This study was designed to evaluate the effect of Hominis placenta Pharmacopuncture treatment on Dysmenorrhea of Women. Methods: 49 subjects who were suffering from dysmenorrhea volunteered to answer the MMP(Measure of Menstrual Pain) and MSSL(Menstrual Symptom Severity List) questionnaire. They were divided into two groups, a Hominis placenta Pharmacopuncture treatment group(Experiment al group, n=25) and a Normal Saline(N/S) treatment group(Control group, n=24). The two groups were injected on the $CV_4,\;S_{36},\;Sp_9$ and $Sp_6$ acupuncture point. They were treated totally five times depending on the individual menstruation cycles. The scores of MMP and MSSL were measured overall three times before and after the menstruation cycle. The collected data were analyzed as paired t-test, independent t-test using SPSS 12.0 WIN Program. Results: As a result of the evaluation by MMP and MSSL, a significant improvement on dysmenorrhea was made in the two groups(p<0.05), and both scores of Experiment group were decreased more than Control group. But there was no significant difference between the two groups. Conclusions: The Hominis placenta Pharmacopuncture treatment and the Normal Saline treatment were effective in decreasing the symptom of Dysmenorrhea.

안면신경평가기준에 따른 구안와사(口眼喎斜)의 임상관찰 (The clinical observation on Bell's palsy according to facial nerve grading system)

  • Background and Objective : The evaluation of facial nerve function is a complex procedure. The House-Brackmann facial nerve grading system(H-B grade)and the Yanagihara grading system(Y-system) have been recommended as universal standards for assessing the degree of facial nerve palsy. The purpose of this study is to compare the effectiveness of Y-system and H-B system and to know the advantages of each system for each stage of incomplete recovery or sequelae of facial paralysis. Materials and Methods : A comparison between H-B grade and Y-system was studied with 137 evaluations of 61 cases of incomplete recovery or sequelae of peripheral facial paralysis. Each case was graded by using H-B system as a gross system and Y-system as a regional system before treatment, after 12 weeks and 6 months Results and Conclusions : The range of score in the Y-system 0-6, 8-16, 14-22, 24-34, 32-38, 38-40 were matched with grade VI, V, IV, III, II and I in the H-B system. The percentage of H-B grade III was the greatest among 137 evaluations and y-system showed the greatest score range. H-B system is easy to use but Y-system is more objective, quantitative and convenient to use in the incomplete recovery or sequelae state.
