Evaluation of Real-time Measurement Liver Tumor's Movement and $Synchrony^{TM}$ System's Accuracy of Radiosurgery using a Robot CyberKnife (로봇사이버나이프를 이용한 간 종양의 실시간 움직임 측정과 방사선수술 시 호흡추적장치의 정확성 평가)

  • Purpose: This study aimed to quantitatively measure the movement of tumors in real-time and evaluate the treatment accuracy, during the treatment of a liver tumor patient, who underwent radiosurgery with a Synchrony Respiratory motion tracking system of a robot CyberKnife. Materials and Methods: The study subjects included 24 liver tumor patients who underwent CyberKnife treatment, which included 64 times of treatment with the Synchrony Respiratory motion tracking system ($Synchrony^{TM}$). The treatment involved inserting 4 to 6 acupuncture needles into the vicinity of the liver tumor in all the patients using ultrasonography as a guide. A treatment plan was set up using the CT images for treatment planning uses. The position of the acupuncture needle was identified for every treatment time by Digitally Reconstructed Radiography (DRR) prepared at the time of treatment planning and X-ray images photographed in real-time. Subsequent results were stored through a Motion Tracking System (MTS) using the Mtsmain.log treatment file. In this way, movement of the tumor was measured. Besides, the accuracy of radiosurgery using CyberKnife was evaluated by the correlation errors between the real-time positions of the acupuncture needles and the predicted coordinates. Results: The maximum and the average translational movement of the liver tumor were measured 23.5 mm and $13.9{\pm}5.5\;mm$, respectively from the superior to the inferior direction, 3.9 mm and $1.9{\pm}0.9mm$, respectively from left to right, and 8.3 mm and $4.9{\pm}1.9\;mm$, respectively from the anterior to the posterior direction. The maximum and the average rotational movement of the liver tumor were measured to be $3.3^{\circ}$ and $2.6{\pm}1.3^{\circ}$, respectively for X (Left-Right) axis rotation, $4.8^{\circ}$ and $2.3{\pm}1.0^{\circ}$, respectively for Y (Crania-Caudal) axis rotation, $3.9^{\circ}$ and $2.8{\pm}1.1^{\circ}$, respectively for Z (Anterior-Posterior) axis rotation. In addition, the average correlation error, which represents the treatment's accuracy was $1.1{\pm}0.7\;mm$. Conclusion: In this study real-time movement of a liver tumor during the radiosurgery could be verified quantitatively and the accuracy of the radiosurgery with the Synchrony Respiratory motion tracking system of robot could be evaluated. On this basis, the decision of treatment volume in radiosurgery or conventional radiotherapy and useful information on the movement of liver tumor are supposed to be provided.

Effects on the Thermal Change of the Face Follow Electroacupunctyre on Hapkok($LI_4$), Sangan($LI_3$) (合谷($LI_4$), 三間($LI_3$)의 電針刺戟이 顔面部 領域 溫度變化에 미치는 影響)

  • The back ground and purpose : The acupuncture of oriental medicine is very important in treatments. Until now it has been researched according to the meridian and qi xue(氣血) phenomenon of oriental medicine's theory. As electroacupuncture is one of acupuncture treatments, it will show more objective index to observe the meridian and qi xue(氣血) phenomenon. And then, I studied the effects on the thermal change of the face following electroacupuncture treatment. Objective and Methods : This study was performed from January 1999 to March 1999 on 10 healthy students. The objective was divided into three groups, those were the control group A(n=10), the group B(n=10) of electroacupuncture on Hapkok($LI_4$), Samgan($LI_3$) and the group C(n=10) of electroacupuncture on Shinmun($H_7$), T' ongni($H_5$). First, in the control group A, we took a picture for 10 men without any stimulation with the Digital Infrared Thermograph Imaging(D.I.T.I.) and did 3min after, 10min after, 15min after, 25min after, 45min after respectively. Second, in the electroacupuncture treatment group B, we took a picture for 10 men without any stimulation, and then treat electroacupuncture on Hapkok($LI_4$), Samgan($LI_3$) and took a picture immediately(3min after), 10min after, 15min after and remove needle and took a picture in the same way respectively. Third, in the electroacupuncture treatment group C, we took a picture for 10 men without any stimulation, and then treat electroacupuncture on Shinmun($H_7$), T'ongni($H_5$) and took a picture in the second way respectively. Results: 1. In healthy men, average skin temperture about Yonghyang($LI_{20}$) area was higher than Soryo($G_{25}$) or Chich'ang($S_4$) area. They were Soryo($G_{25}$) area $31.495{\pm}0.766^{\circ}C$, Rt. Yonghyang($LI_{20}$) area $31.664{\pm}0.936^{\circ}C$, Lt. Yonghyang ($LI_{20}$)area $31.686{\pm}0.767^{\circ}C$, Rt. Chich'ang($S_4$) area $31.226{\pm}0.875^{\circ}$, Lt. Chich'ang ($S_4$) area $31.453{\pm}0.855^{\circ}C$. 2. In the control group A, the skin temperature of Soryo($G_{25}$) showed the increase or decrease in below ${\Delta}0.1^{\circ}C\;except\;0.265{\pm}0.594^{\circ}C$ in 25min, but not significantly. 3. About Soryo($G_{25}$) area, the skin temperature decreased significantly after electroacupuncture immediately. ${\Delta}T $of the group B was $-0.970{\pm}0.87\;1^{\circ}C$, which was larger than one of the group C which was $-0.707{\pm}0.624^{\circ}C$ at 3min. And then ${\Delta}T$ of the group C was increase valuable at 25min, 45min. 4. About Yonghyang($L1_{25}$) area, the left ${\Delta}T$ of the group B showed below $0.2^{\circ}C$ or so in contrast to the right it. In the group C, on the both side showed continous increase of temperature as following times. 5. About Chich'ang($S_4$) area, the skin temperature increased valuable $0.3^{\circ}C$ or so on the both side and later inclined to decrease in the group B but not significantly. In the group C, it increased valuable on the both side. 6. The skin temperature of electroacupuncture treatment group B, C were more increase than the control group A except Lt. Yonghyang($LI_{20}$) area in the group B. The temperature of group C were more increase than the group B wholly. Conclusion : The above results indicate that D.I.T.I. is a useful method to observe and fallow-up the effects and the changes by electroacupuncture stimulation on objective evaluation of phenomenon for the meridian system and character. Thus, continuous thermographic study will be needed for more clinical application such as acupuncture and medicine or laser therapy according to oriental medicine.

Revision and Evaluation of Korean Outpatient Groups-Korean Medicine (한의 외래환자분류체계 개선 및 평가)

  • Objectives: This study aimed at revising the Korean Out-patient Groups for Korean Medicine (KOPG-OM, version 1.0) based on clinical similarity and resource use, by using the accumulated claims data, and evaluating the validity of the revised classification system. Methods: A clinical specialist panel involving 19 specialists from 8 Korean medicine (KM) specialty areas reviewed the classification tree, diagnosis groups and procedure groups in terms of clinical similarity. Several models of outpatient grouping were formulated, with the validity of each tested based on the $R^2$ coefficient of determination for the treatment costs of all visits. To add age splits, the variances of treatment costs by age groups were also analyzed. These statistical analyses were performed using KM claims data of National Health Insurance from 2010 to 2012. Results: The classification tree designed via panel discussions was used to allocate outpatient cases to 26 diagnosis groups, with cases involving procedures such as acupuncture, moxibustion and cupping, then allocated to 9 procedure groups in each diagnosis group. The cases without procedures were categorized into the visit index - medication group. This process resulted in 298 outpatient groups. The $R^2$ values for treatment costs of all visits ranged from 0.38 to 0.69 depending on the providers' types. Conclusions: The revised model of KOPG-KM has a higher validity for outpatient classification than the current system and can provide better management of the costs of outpatient care in KM.

A Review on Korean Traditional Medicine Research against Cervical Dysplasia - Focused on Studies of Cervical Cancer and Cervical Dysplasia- (자궁경부 이형증과 관련한 한의약 연구 고찰 - 자궁경부암 및 자궁경부 이형증 논문을 중심으로 -)

  • Purpose: This review is to figure out evidence that suggest effectiveness of Korean Medicine treatments against cervical dysplasia. Methods: Studies on cervical cancer and cervical dysplasia were searched through 6 databases: Korean Studies Information Service System(KISS), National Discovery for Science Leaders (NDSL), Korea Institute of Science and Technology Information (Korean TK), Oriental Medicine Advanced Searching Integrated System (OASIS), the Journal of Korean Medicine, and the Journal of Korean Obstetrics & Gynecology. After that, the articles were extracted with reference point of Korean Traditional Medicine. Results: 37 articles were included lastly according to selection criteria. 3 of them were case reports on cervical dysplasia, and 34 were in-vitro studies on Human Papilloma Virus (HPV) positive cancer cell. In case reports, acupuncture, moxibustion, medical herbs and pharmacoacupuncture were used for treatments of cervical dysplasia with about 3 months. Experimental studies on cervical cancer cell showed that several herbs function with clear heat, eliminate stasis (淸熱解毒, 化瘀消腫) have anti-cancer effects inducing apoptosis. Conclusions: The results of articles are not enough to use in practice. Therefore, we indicates more advanced research methodology as follows: development of Korean Medicine treatment protocol with oral and external, in-vivo experimental study, and evaluation immunity index.

A Case Report of Patient with Alcoholic Derogation Acute Abdominal Pain Using Oriental Medicine Combined with Conventional Treatment (한양방 협진 치료를 통해 호전을 보인 주상(酒傷)으로 인한 급성 복통 환자 1례)

  • Objective : This study was aimed to report a patient accompanied by alcoholic derogation acute abdominal pain treated with oriental medicine combined with conventional treatment. Methods : A 36 year old female patient complained of acute abdominal pain for one year from Apr. 2009. She had treated at local general hospital. But the symptom was not reduced. Instead, it was aggravated on Jul. 2010. The main symptoms were left flank pain, chest discomfort, nausea and anorexia. The patient was treated by oriental medicine (herbal medicine (Chunggan-plus), acupuncture, moxibustion, etc) combined with conventional medicine from Jul. 10th, 2010 to Jul. 17th, 2010. We observed the patient with evaluation of laboratory test and imaging scan. Results : Most of the symptoms of the patient were improved. Especially, the abdominal pain were disappeared from grade 3 to grade 0. No side effects were noted during treatment. Conclusion : This results suggest that the oriental medicine combined with western medicine may effective to treat alcoholic derogation acute abdominal pain.

Systematic review on the Randomized Controlled Clinical Trials of Qigong in the Korean Journals (국내 기공관련 무작위 임상연구에 대한 현황 검토)

  • Background : Qigong is an energy-healing intervention used to prevent and cure ailments and to improve health through regular practice. Although qigong-neither itself nor its postulated mechanism of action-are within the paradigm of modern Western medical science, its effects on the human body could be possible. Objectives : This study aims to review the bibliography, biological responses and therapeutic effects of Qigong. In the process, this review will grasp trends in this field of studies and will direct further researches into the right direction. Method : The computerized Korean databases were searched from their respective inceptions up to January 2008. The search terms used were 'qi', 'qigong', 'doin', 'training', 'bioenergy', 'life nurturing' and random or Korean language terms related to qigong. Several specialized journals were also manually searched for relevant articles. Result : Since the 1990s, Qigong papers in the Korean Literature are increased. The articles on Korean traditional medicine had been published more than those on physical education or nursing etc. However, since the majority of the trials determine questionnaire, it was difficult to determine the efficacy of a specific intervention. The methodological quality of the trials was generally low (Jadad score: mean, 1.3; range, 1 to 4), questioning their reliability. Conclusions : More profound studies for Qigong are needed. Further rigorous clinical trials with more objective outcome measures that include sham procedures should be performed. Specifically, we think it should be clinical studies and qualitative research methods for evaluation are needed.

  • Functional dyspepsia refers to upper abdominal pain or discomfort with or without symptoms of early satiety, nausea, or vomiting with no definable organic cause. In this study, we recognized that dyspepsia was decreased with herbal medication (Banhabakchulchunma-tang) and acupuncture therapy. And we aimed to evaluate the Gastrointestinal Symptoms Rating Scale (GSRS) in these cases. The GSRS indicated that Oriental medicine treatment could be effective in the Functional dyspepsia. And it is helpful in decreasing symptoms of patients and in improving quality of life.

A Case Report of Olivopontocerebellar Atrophy (Multiple Systemic Atrophy-Cerebellum) Patient Complicating Sleep Disorders and Gait Disturbance (수면장애와 보행장애를 호소하는 올리브교소뇌위축증 환자 증례보고 1례)

  • This case report describes a patient with olivopontocerebellar atrophy accompanied by sleep disorder and gait disturbance whose condition was improved by treatment with Korean medicine. The 61-year-old woman, who was diagnosed with olivopontocerebellar atrophy (Multiple Systemic Atrophy-Cerebellum), was admitted to hospital twice and treated with Korean medicine (acupuncture and herbal medicine) and rehabilitation. The Korean medicine was Gwibiondam-tang-gami and Jaeumgeonbi-tang-gami. Clinical symptoms were assessed by the Modified Bathel index, functional independent measurement, Berg balance scale, and Unified Multiple System Atrophy rating scale. A brain MRI at the one-year follow up after onset showed similar progress but clinical symptoms were improved after treatment, and the evaluation index score increased. Multiple system atrophy, a type of degenerative neurological disease, has no targeted treatment. In this situation, although this report describes a single case, Korean medicine treatment could provide a meaningful improvement in the sleep disturbance and gait disorder symptoms of patients with olivopontocerebellar atrophy.

Clinical Study of Diarrhea-Type Irritable Bowel Syndrome Patient (설사형 과민성 장 증후군 치험례)

  • This study was designed to report the clinical effects of oriental medical therapy on a patient whose irritable bowel syndrome did not improved under western medical therapy, Herbal medication, acupuncture and moxibustion therapy were applied to the patient for 12 days. The status and frequency of feces were checked and self-recognized symptoms (abdominal pain and bowel sounds) were checked to evaluate improvement. According to the above evaluation items, the treatment was beneficial. The status and frequency of feces became normal. Abdominal pain and bowel sounds ceased. The above results suggest that oriental medical therapy is beneficial for the patients with irritable bowel syndrome.

  • Objectives : The essential tremor (ET) is characterized by a tremor of the whole body, including both arms. Recent studies suggest that its psychiatric and cognitive factors are more important for treatment and management than motor symptomsymptoms. This study was performed to report the effects of Korean medicine for essential tremor aggravated by Hwa-byung. Methods : We treated a 42 year- old man with Korean medicine (acupuncture, herb-medication and moxibustion) and mental therapy. The effects of the treatment for tremor and skin symptoms were measured by VAS and Severity score (3 level). We used SCL-90-R, BDI, BAI, PSQI and instrument of oriental medical evaluation for Hwa-Byung score (IOMEHB) for a psychological condition. Results : After treatments, tremor, Hwa-byung and accompanying symptoms have improved and subsided. VAS, SCL-90-R, BDI, BAI, PSQI, and IOMEHB have decreased. Conclusions : This result suggests that Korean medical treatment and mental therapy might be effective for the essential tremor aggravated with Hwa-byung.
