Clinical Study on 2 Cases of Cervical Spondylotic Myelopathy Treated by Korean Traditional Medicine (한의학적 치료를 통하여 호전을 보인 경추증성 척수증 환자 치험 2예)

  • Cervical spondylotic myelopathy (CSM) is a severe disease that usually needs surgical treatment. We have treated two CSM patients with Korean traditonal medicine, which includes acupuncture, pharmacopuncture, herbal medicine and manual treatment (Chuna). For objective evaluation, we used Nurick's classification, JOA (Japanses Orthopedic Association) score, grip and release test and hand grasping power measurement. Post-treatment analysis has shown that grip and release test score and grasping power of two patients were both remarkably improved, along with the improvements on both Nurick's classification and JOA score. This study showed that Korean traditional medicine could be effective conservative treatment for CSM.

Genotoxicity Evaluation of Capsaicin-Containing (CP) Pharmacopuncture, in an In Vivo Micronucleus Test

  • Objectives: Capsaicin-containing (CP) pharmacopuncture was developed to treat neuropathic pain. This study was conducted to assess the toxicity of CP extract for pharmacopuncture, using a micronucleus test. Methods: First, a dose range finding study was conducted. Then an in vivo micronucleus test was performed to determine the induction of micronuclei in mouse bone marrow cells after intramuscular administration of CP twice with a 24-hour interval to 8-week-old ICR mice. A high dose of 0.2 mL/animal was selected, and this was sequentially diluted by applying a geometric ratio of 2 to produce two lower dose levels (0.1 and 0.05 mL/animal). In addition, negative and positive control groups were set up, and an HPLC analysis was conducted to confirm the capsaicin content of CP. Results: The incidence of micro-nucleated polychromatic erythrocytes in polychromatic erythrocytes in the CP-treated group was similar to that in the negative-control group, while that in the positive-control group was significantly greater. In addition, the ratio of polychromatic erythrocytes to total erythrocytes in the CP treatment group and the positive control group was not significantly different from the negative control group. In the HPLC analysis, capsaicin in the CP was identified through a comparison with the retention time of the capsaicin standard of 27 min. Conclusion: CP did not show any indication of any potential to induce micronuclei formation in bone marrow cells of ICR mice under the conditions of this study. Further toxicity studies are necessary to ensure the safety of the use of CP in clinical practice.

Treatments of Korean Medicine for Pediatric Growth: A Literature Review of Clinical Studies (소아 성장에 대한 한의학 치료: 국내 임상연구에 대한 문헌적 고찰)

  • Objectives The purpose of this study is to provide evidence of treatment of growth disorder Methods We have reviewed clinical studies of growth disorder in children and adolescent through 6 databases until February, 2020. The searching keywords were "short stature OR dwarfism OR growth disorder" AND "acupuncture OR electric acupuncture OR electroacupuncture OR moxibustion OR herb medicine OR cupping OR Korean medicine OR oriental medicine OR chuna OR pharmacopuncture OR qigong OR traditional medicine OR traditional Korean medicine OR Korean medicine". There was no limit to time and language. Results As a result of the initial search, a total of 270 papers from six domestic databases were found. Among these papers, 156 papers were selected after excluding duplicated papers, and 109 of them were further excluded after checking the title and the abstract. Additionally, 28 papers were excluded by reviewing the full text. The author, year, number of patients, treatment, evaluation tools, and results of a total of 19 papers were included in this study and were summarized. 90.5% of the studies have shown that herbal remedies have improved childhood growth. Conclusions Random control studies and large-scale observational studies are needed in future to show high-quality evidence for the treatment of Korean medicine in pediatric growth.

A Review of Clinical Studies for Chinese Medicine Treatment of Idiopathic Edema Using the CNKI Database (특발성 부종의 중의치료에 대한 임상연구 동향 - CNKI검색을 중심으로)

  • Objectives: This study investigated the latest clinical studies on Korean medicine treatment of idiopathic edema in the Chinese National Knowledge Infrastructure Database (CNKI). Methods: We searched the last 9 years of clinical studies discussing Oriental medicine-based treatments for idiopathic edema in the CNKI database. The search focused on the authors, publication year, type of study, purposes of study, method and duration of treatment, evaluation criteria, and results of the selected articles. Results: The search identified 7 randomized controlled trials (RCTs) and 9 others. All studies used oral herbal medicine for treatment. The most commonly used herbs were Poria Sclerotium (茯苓), Atractylodis Rhizoma Alba (白朮), and Alismatis Rhizoma (澤瀉), which are the constituent herbs of Oryeong-san (五苓散). All 16 studies confirmed the efficacy of Oriental medicine treatments. Conclusion: More varied and scientifically designed clinical studies are required to develop treatments for idiopathic edema. The results of this study could be used as basic data for further studies on idiopathic edema.

A Case Report of Facial Dermatitis Herpetiformis with Allergy Diseases Improved by Administration of Mahaengeuigam-tang (마행의감탕(麻杏薏甘湯)으로 호전된 알레르기 질환을 동반한 안면부 포진상 피부염 치험 1례)

  • This study aims to report a case of facial dermatitis herpetiformis with allergy diseases improved by administration of Mahaengeuigam-tang (Maxingyigan-tang). Multiple blisters appeared on the patient's face, Erythema, papules, and scales were also observed on the neck, back, and knee areas of the patient. The patient has had cough, sputum, and dyspnea symptoms since she was 5-6 months old. After diagnosing the patient's condition as dermatitis herpetiformis, atopic dermatitis, and bronchial asthma through history taking and symptom observation, Mahaengeuigam-tang (Maxingyigan-tang) was prescribed for about 30 days. As a result of evaluation through our own criteria, dermatitis herpetiformis symptoms improved from 18 to 2 points. Atopic dermatitis and asthma symptoms also decreased from 12 to 0 and 20 to 3 points, respectively. Since this study has proven the efficacy of Mahaengeuigam-tang (Maxingyigan-tang) for dermatitis herpetiformis patient, this prescription may be fully utilized as the treatment for other inflammatory skin diseases.

A Case of Patient with Brady-arrhythmia Complaining Dizziness Treated with complex Korean Medicine treatments (어지럼증을 호소하는 서맥성 부정맥 환자의 한의복합치료 1례)

  • Brady-arrhythmia is defined as an arrhythmia in which the heart rate slows to less than 60 beats per minute. Brady-arrhythmia reduces cardiac output and causes changes in blood flow. As a result, dizziness occurs because not only ischemia occurs in the central nervous system, but also blood flow in major organs decreases. The insertion of an artificial pacemaker is known as almost the only treatment alternative for patients with brady-arrhythmia with symptoms. This study reports a case of 85 year old male diagnosed with brady-arrhythmia complaining presyncope type of dizziness. The patient underwent complex Korean medicine treatments by herbal medicine, acupuncture and moxibustion. The daily average value of pulse rate, K-DHI score, NRS score and the patient's subjective expression of symptoms were used as evaluation tools. This study suggested a significant improvement in symptoms through complex Korean medicine treatments without the use of anti-arrhythmic drugs or insertion of an artificial pacemaker in the patient with brady-arrhythmia complaining dizziness.

A Case Report of Korean Medical Treatment on Parkinsonism Patient Complaining of Motor Disorder and Aphonia (한방치료로 운동 기능장애와 실성증이 호전된 파킨슨증후군 환자 치험 1례)

  • ■Objectives This case study is to report the effectiveness of Korean medicine in Parkinsonism patient's treatment. ■Methods We used the acupuncture, electro-acupuncture, moxibustion, cupping therapy, herbal medicine, especially Palmulgunja-tang to the Parkinsonism patient with motor disorder such as Postural Instability and Gait Difficulty(PIGD) and aphonia. Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait pattern, voice dB and self-evaluation of speed and volume were used to assess the change of symptoms. ■Results ‌After treatment, the UPDRS score decreased in overall category and the walking pattern has improved. In addition, the improvement was observed in voice volume and in self assessment of the patient. ■Conclusion This case suggests the effect of Korean medical treatment on motor disorder and aphonia in Parkinsonism.

A Case on Combined Korean Medicine of Adult Chronic Urticaria with Papular Form (구진성 만성 두드러기 환자의 한방 단독 치료 보고)

  • Objectives : The purpose of this study is to report the use of Korean medicine on a chronic urticaria patient. Methods : We treated an adult patient had chronic urticaria with herbal medicine, pharmacopuncture, acupuncture, photodynamic therapy. The effects of the treatment were evaluated through urticaria activity score and photo evaluation. Results : The urticaria activity score started at 6 and ended with 0. After 4 months of follow-up, no recurrence was confirmed. Conclusions : This study has demonstrated that Korean medicine could be effective to treat chronic urticaria with papular form.

Effect of Auriculotherapy on Musculoskeletal Pain: A Systematic Review and Meta-Analysis (이혈요법이 근골격계 통증에 미치는 효과: 체계적 문헌 고찰 및 메타분석)

  • Purpose: The aim of this study was to evaluate the effect of auriculotherapy on musculoskeletal pain in adults. Methods: A total of 885 studies were retrieved from nine databases (PubMed, Scopus, CINAHL, Web of Science, Ovid Medline, Cochrane Library, RISS, KMbase, and KISS). Sixteen studies were selected for meta-analysis, which satisfied the inclusion criteria and the evaluation of risk of bias. Demographic data, auriculotherapy types, intervention characteristics, auricular points, and outcomes related to pain (subjective pain scale, and amount of analgesic) were extracted from all included studies. The effect size of auriculotherapy was analyzed through comprehensive meta analysis 3.0, and the presence of publication bias was analyzed through a funnel plot and Egger's regression. Results: The results of the meta-analysis (n = 16) revealed that the auriculotherapy was significantly superior to the control group on present pain in adults (Hedges' g = - 0.35, 95% Confidence Interval [CI] = - 0.55~- 0.15). According to the results of subgroup analysis, the effect size of auricular acupuncture therapy (Hedges' g = 0.45, 95% CI = - 0.75~- 0.15) was higher than the auricular acupuncture (Hedges' g = 0.27, 95% CI = - 0.53~0.00): the longer the intervention period, the greater the effect size. Conclusion: In this study, auriculotherapy demonstrates a significant reduction in musculoskeletal pain in adults. Therefore, it is necessary to refine the curriculum to include auriculotherapy as a nursing intervention to relieve musculoskeletal pain in adults and encourage its use in clinical settings.

A Case Report of Korean Medical Treatment on Atypical Parkinsonism Patient Complaining of Gait Disturbance Using 3-Demensional Gait Analysis System (3차원 보행분석기를 통해 보행장애의 호전이 확인된 비정형 파킨슨증후군 환자 한방치험 1례)

  • ■Objectives This case study is to report the effects of Korean medicine on parkinsonism patient's Gait Disturbance. ■Methods During 12 days of hospitalization, the patient was treated by acupuncture, pharmaco-acupuncture, moxibustion, herbal medicine, especially Cheongsimyeonjatang-gamibang. In order to assess the change of symptoms, we used a 3-Dimensional(3D) gait analysis system, Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait video and self-evaluation of discomfort. ■Results After treatment, The improvements of walking pattern were observed in both objective analysis results of gait analysis system and subjective video analysis. And the UPDRS score decreased, especially Part III score decreased more than minimal clinically important difference(MCID). In addition, There was improvement in self assessment of the patient. ■Conclusion This study suggests that Korean medical treatment might be effective in motor disorder of parkinsonism patient.
