퇴행성관절염(고관절, 수지관절염) 한의표준임상경로 적용연구 프로토콜 (Hip and Finger Degenerative Arthritis Case Series Protocol for Applying Korean Medicine Clinical Pathway)

  • Objectives : The aim of this study is to set out case series study protocol to complete clinical pathway (CP) of hip and finger degenerative arthritis by applying CP, based on Korean medicine clinical practice guideline developed by clinical experts, to clinical field. Methods : The treatments included Manual acupuncture, acupoint injection, electroacupuncture, laser acupuncture, cupping, moxibustion, chuna, and physiotherapy. They were conducted in the 2nd week of admission and 4th week of out patient department (OPD) days. We carried out 10-point Likert scale questionnaires on the clinical usefulness and the satisfaction of patients and staff after applying CP. Appropriateness and improvement on patients were conducted using a 10-point Likert scale. An open-ended questionnaire was also conducted to ask if there was any requirement to be added. Results : In past research studies, there were no related studies about Korean medicine CP on hip and finger degenerative arthritis. Final version of CP is going to be completed based on the questionnaire. Conclusions : This evidence-based case series study protocol is expected to contribute development of hip and finger degenerative arthritis.

  • Objective ; This Study was carried out to review the history of acupuncture & moxibustion in Korea under the control of imperialized Japan and to compare the history among Korea, China and Japan. Results ; Under the control of imperialized Japan the science of acupuncture & moxibustion in Korea has experienced the dark ages because of the momentary disordered medical laws made by Japan. As we compare the history of acupuncture & moxibustion among Korea, China and Japan, Korea was simple but systemic, China was complicate but complex and Japan was nearly surgical. Conclusion ; The science of acupuncture & moxibustion of Korea was mostly advanced in the late Chosun and did not just imitate that of China and has advanced uniquely.

침구(鍼灸) 치료와 활락탕(活絡湯) 투여가 급성요통 및 간기능에 미치는 영향에 관한 임상적 고찰 (A Clinical Study of the Effects of Acupuncture, Moxibustion and Whuallak-tang (Huoluo-tang, 活絡湯) on Acute Back Pain and Liver Function)

  • Objective : The purpose of this study was to investigate the effect of acupuncture, moxibustion and whuallak-tang on acute back pain and liver function. Methods : We investigated 34 acute back pain patients. We treated them with acupuncture, moxibustion and bed rest, herbal medicine (Whuallak-tang, Huoluo-tang, 活絡湯) and physical therapy and we observed clinical effects and checked the changes of AST,ALT,ALP on admission and 1week later. Results : The result obtained is as follows. The patients were 58.8% male and 41.2% female ranging in age from 29 to 76 years. after treatment, of the 34 cases, 10(29.4%) were obtained excellent results and 23(67.7%) were good, and 1(2.9%) case was obtained fair result, with the total effective rate being 97.1%. AST,ALT,ALP was within normal range on admission and 1week later. and AST and ALP were decreased but had no statically significance.(P>0.05) Conclusion : This study suggests that acupuncture, moxibustion and Whuallak-tang are effective on acute back pain and should not induce hepatotoxicity. even though there are few toxic herbs in Oriental medicine, acupuncture and general herbal medication by a oriental doctor should not injure liver function of human.

  • Objectives : This research was projected to verify the validities of needling or moxibustion- acupoints in Illustrated Manual for the Practice of Acupuncture and Moxibustion with the Help of a Bronze Figure bearing Acupuncture Points (IMPAM) and The Classic of Supplementing Life with Acupuncture and moxibustion(CSLAM) Methods; We investigated acupoints which have prohibitions, notices or adverse events when performing needling or moxibustion in IMPAM and CSLAM, then verified it in various literatures and today's medical knowledge. Results; In IMPAM, 2 acupoints were prohibited both needling and moxibustion, 22 acupoints were prohibited from needling and 29 were prohibited from moxibustion. In CSLAM, 3 acupoints were prohibited from both needling and moxibustion, 24 acupoints were prohibited from needling and 31 were prohibited from moxibustion. Most of the prohibition, the notices and adverse events of the acupoints written in IMPAM and CSLAM had medical or philological bases, and the number of them increased compared to that in A-B Classic of Acupunture and Moxibustion(ABCAM). Conslusion : The prohibotions, the notices and the adverse events when performing acupunture or moxibustion in IMPAM and CSLAM reflected the medical experience and knowledge of the time. And the knowledge was increased compared to that in ABCAM.

COVID-19 전후에 따른 지역별 대전대학교 한방병원의 외래 환자에 대한 통계적 분석 (Statistical Analysis of Outpatients Trends at Korean Medicine Hospitals of Daejeon University by Region before and after COVID-19)

  • Objectives: This study is designed to statistically analyze data of outpatients visiting for recent 3 years. The purpose is to identify tendencies of patients who visit the hospitals before and after COVID-19. Methods: This study retrospectively analyzed the medical records of 452,487 patients who visited to the Daejeon·Cheonan·Cheongju Korean Medicine Hospital of Daejeon University from January 1, 2018 to August 31, 2020. The data is classified according to year, month, gender, age, and visit type. The statistical analysis was performed using IBM SPSS 25.0. Results: The total number of patients decreased in 2020 compared to before 2020, and the number of patients in each hospital also decreased from the previous year. According to the year of each hospital, the difference by year was not statistically significant at Daejeon Hospital, but Cheonan and Cheongju Hospital showed statistical difference. The change in monthly according to the year by hospital has continued to decrease from January to March in 2020, unlike the previous year, which recovered from a decline in March. In the analysis of patients by age, there was no statistically significant difference in the number of patients in the above 60s according to the year, while those under 60s have a tendency to significantly decrease. Conclusion: We expect that the results of this study will be used as reference materials in analyzing effects of COVID-19 at health care utilization.

일개 한방병원에서의 흉요추 압박골절 입원 환자에 대한 통계적 분석: 후향적 차트리뷰 (Statistical Analysis on Hospitalized Patients with Thoracolumbar Compression Fracture at Single Korean Medicine Hospital: Retrospective Review)

  • Objectives: This study was designed to statistically analyze patients hospitalized for thoracolumbar compression fractures at Daejeon University Daejeon Korean Medicine Hospital from January 1, 2017 to December 31, 2021. Methods: A total of 62 patients were analyzed through electronic medical records in this study. The extracted data was analyzed using IBM SPSS ver.27.0. Results: 1. Traffic accident patients were more likely to reduce pain by more than half compared to falls and other patients. Patients without spinal disease were more likely to reduce pain by more than half compared to those without. The shorter the absolute bed rest and the longer the hospital stay, the higher the probability of pain reduction by more than half. 2. The duration of pain half-reduction was longer in patients with diabetes than in patients without diabetes. 3. The longer the hospitalization period and the shorter the absolute bed rest period, the less pain was reported upon discharge. Males complained of less pain at discharge than females, and patients without spinal disease complained less than those without. Patients who did not receive absolute bed rest complained of less pain upon discharge than those who did not. Conclusions: This study included patients hospitalized for thoracolumbar compression fractures and showed that etiology, absolute bed rest period, hospitalization period, gender, spinal disease, diabetes statistically affected the degree of pain reduction.

  • Objective : By precess of treatment for a case which diagnosed as Unknown Neurologic bladder and admitted from the 15th, April, 2002 to the 27th, June, 2002, the result is as follows. Methods : The change in clinical symtom of Urinary Retension was described when this patient was treated with Korean Medicine, especially Acupuncture & Moxibustion. Results : Acute Urinary Retension at admission is improved remarkably. Conclusion : We could get the effective result that case of Neurologic bladder treated with Korean Medicine, especially Acupuncture & Moxibustion.

태평성혜방과 동인침구경의 침구금기혈에 대한 고찰 (A Study of Acupuncture or Moxibustion Contraindications in Taipingshenghuifang and Tongrenzhenjiujing)

  • Objectives : This research aimed to verify the validities of contraindicated acupoints in acupuncture or moxibustion treatments in Taipingshenghuifang and Tongrenzhenjiujing. Methods : We investigated contraindicated acupoints when performing needling or moxibustion in Taipingshenghuifang and Tongrenzhenjiujing, then verified them in various literatures and today's medical knowledge. Results : In Taipingshenghuifang and Tongrenzhenjiujing, 15 acupoints were described to need a careful approach in acupuncture treatment, and 24 acupoints were described so in moxibustion treatment in common. And additionally GB1 was described to need a careful moxibustion treatment in Taipingshenghuifang. Most of the descriptions for the contradindicated acupoints can be explained based on medical knowledge. Conclusions : The contraindicated acupoints in acupuncture or moxibustion treatments in Taipingshenghuifang and Tongrenzhenjiujing seem to have been classified based on the experience of the doctors at that time. Therefore they may be referred to for safe acupuncture or moxibustion treatments at these points.

퇴행성 슬관절염의 매선 치료에 대한 체계적 문헌 고찰 (A Systematic Review on Thread Embedding Therapy of Knee Osteoarthritis)

  • Objectives : This study was performed to review the efficacy of national and international randomized controlled trials (RCTs) investigating evidence on thread embedding therapy for knee osteoarthritis. Methods : Online databases (PubMed, EMBASE, Cochrane, NDSL, OASIS, CNKI) were searched for studies where thread embedding therapy was performed for Knee Osteoarthritis from their inception to July 2018. Two researchers independently performed the search. Only RCTs were selected. Eligible studies were selected first by the abstract and the title and then included after full-texts were read. Risk of bias of the included studies were evaluated using the Cochrane risk of bias assessment tool. Data were narratively summarized. Results : There were 334 studies retrieved from the databases, resulting in analysis of 3 RCTs. There was an average of 1.5 treatment visits over a 7 day period and evaluation tool used was efficacy rate, with traditional acupuncture being the most common control used in the trials. Statistically significant improvement by thread embedding therapy was reported. None of the included RCTs reported on adverse reactions. The risk of bias of the included studies was generally unclear. Conclusion : The review suggests that thread embedding therapy can be effective in knee osteoarthritis. But there was a lack of detailed information about the treatment procedures, and the risk of bias was unclear. Therefore, there is insufficient evidence for thread embedding therapy for knee osteoarthritis.

초음파를 통해 경과관찰한 비복근 부분파열 환자의 한의학적 치료: 증례보고 (Korean Medical Treatment for Partial Rupture of Gastrocnemius Muscle Observed by Ultrasonography: A Case report)

  • Gastrocnemius muscle partial rupture is a common muscle injury. This case is report on a patient with a gastrocnemius muscle partial rupture who was continuously observed with ultrasonography while receiving Korean medicine treatment. Acupuncture, pharmacoacupuncture, herbal medicine, physical therapy and rehabilitation treatment were performed on a patient diagnosed with gastrocnemius partial rupture. The improvement of symptoms was evaluated using Numeric Rating Scale (NRS), Range of Motion (ROM) of ankle, circumference of calf and size of hematoma by using ultrasonography. NRS decreased more than 90%, ROM of ankle joint gradually improved, calf circumference improved, and hematoma reduced by more than 90% when measured by ultrasonography. In this study, gastrocnemius muscle partial rupture was observed with ultrasonography, and it is considered that the Korean medical treatment is useful for gastrocnemius muscle partial rupture.
