본태성 진전 환자의 진전특성에 대한 수행과제 및 부위의 영향 (Effects of Task and Part on Tremor Characteristics in Patients with Essential Tremor)

  • Essential tremor is a neurological disorder with a tremor of the arms and hands. It is well known that essential tremor is characterized by the postural tremor and the action tremor. There has been no report on the quantitative difference in the characteristics of two tremor types. The purpose of this study was to investigate the possible difference in tremor characteristics of postural and action tremors. Seventeen patients with essential tremor ($68.9{\pm}7.9years$, 7 men, 10 women) participated in this study. Patients performed the tasks of postural maintenance (arms outstretched) and daily actions (spiral drawing). Three-axes (pitch, roll and yaw) gyro sensors were attached on index finger, back of hand and forearm, from which the segment and the joint angular velocities were calculated. Outcome measure was the tremor amplitude defined as the root-mean-square mean of the vector-sum angular velocity at segments and joints. Two-way ANOVA showed that task and joint had main factor on the tremor amplitude (p < 0.05). Post-hoc analysis revealed that tremor amplitude at the metacarpo-phalangeal joint was not affected by task (p > 0.05). However, tremor amplitude at the wrist joint differed among the tasks (p < 0.05), and it was greater in the action tasks than in postural task. Tremor was greater at finger segments than at hand and forearm and it increased in action tasks. The results of this study would be helpful for the understanding and task-specific treatments of the essential tremor.

  • Tremor is a rhythmic and involuntary muscular contraction characterized by oscillations of a part of the body. The most common of all involuntary movements, tremor can affect various body parts such as hands, head, facial structures, vocal cords, trunk, and legs; most tremors, however, occur in the hands. Clinically, tremor is classified into postural tremor, resting tremor, action tremor, and other kinds of tremor, and treated according to the causes. The author reports the improvement of the patient, hospitalized at Dong Seo Medical Center, whose tremor was not classified specifically in western medicine but was diagnosed as Ganhyeolbujok (肝血不足) by Korean medicine.

머리 떨림 환자에 대한 한방 치험 1례 (A Case Report of a Tremor Patient with Stenosis of the Left Carotid Artery)

  • The carotid artery is one of the main vessels supplying blood to the brain. Carotid artery stenosis is mostly caused by atherosclerosis, a disease where cholesterol is deposited in the arterial blood vessels. Tremor refers to rhythmic shaking of a body part. Tremor is a symptom of many diseases, including Parkinson's disease, essential tremor, orthostatic tremor, cerebellar disease, peripheral neuropathy, and alcohol withdrawal. Tremors may be classified as postural, rest, and action tremors. Tremor of a patient with stenosis of the left carotid artery decreased with acupuncture treatment. The acupoints were GB20, TE17, GV8 and GV11. This case shows that the acupuncture treatment is effective against tremor.

원발성 서자진전 B형, 서자자세-특수성인가? (Primary Writing Tremor Type B, Writing Posture-specific?)

  • Purpose : Primary writing tremor(PWT) can be classified as either type A or type B depending on whether tremor appeared during writing or whilst writing and also on adopting the hand postures normally used for writing. Through the clinical experience author has had an impression that PWT type B may not be purely dependant on specific writing postures. The objective of this study was to clarify whether PWT type B have writing posture-specificity or not. Results : The data indicated that type B PWT is not writing posture-specific. Various pronation and supination postures could evoke tremor as well as writing postures. Furthermore most of other pronation- and supination-related tasks could evoke tremors as well as action of writing. Conclusions : The present data suggest that PWT should be limited only on the pure form of task-spesific PWT type A.

  • Tremor is a rhythmic and involuntary muscular contraction characterized by oscillations of body parts. Clinically, tremor is classified into postural tremor, resting tremor, action tremor, and other kinds of tremor, and treated according to the causes. In oriental medicine, Bangpungtongsung-San has been used to treat tremor when diagnosed as wind-heat. Bangpungtongsung-San-gamibang was administered to patients, with tremor from chromic alcoholic derogation and diagnosed as wind-heat and damp-heat. Both patients, treated with Bangpungtongsung-San-gamibang improved in symptoms, so it is here reported for the better treatment.

  • Tremor is a rhythmic, involuntary muscular contraction characterized by oscillations of a part of the body and is the most common movement disorder. Clinically, tremor isclassified into postural tremor, resting tremor, action tremor, or other kinds of tremor, and treated according to the causes. The appropriate treatment of tremor depends on accurate diagnosis of its cause. We have recently examined a number of tremor-patients diagnosed as cerebellar infarction. Based on traditional Korean medicine curative way, we diagnosed this patient as deficiency of Qi and blood(氣血兩虛). On this account, suitable treatment had been provided. Our results show the clinical symptoms had improved through treatment with herbal medicine (Palmultang-gamibang) and acupuncture, so we report it for better treatment.

  • Tremor refers to rhythmic shaking of a body part. Tremor is a symptom of many disorders, including Parkinson's disease, essential tremor, orthostatic tremor, cerebellar disease, peripheral neuropathy and alcohol withdrawal. Tremors may be classified as postural, rest or action tremors. Symptomatic treatment is tailored to the tremor type. Because Dansambohyul-tang has been used to treat patients differentiated with an insufficiency of the heart and the spleen(心脾兩虛) in oriental medicine, we treated a 78 year-old female patient who suffered from tremor in mouth, chin and hand and insomnia, with improvement of general condition, who was differentiated with an insufficiency of the heart and the spleen(心脾兩虛) with Dansambohyul-tang, herb complex. After 78 days of treatment with Dansambohyul-tang and some other herb complex, we observed improvement of tremor, insomnia and general condition So Dansambohyul-tang shows therapeutic effects on tremor.

중뇌종양에 의한 적핵진전에 대한 Vim 시상핵절제술 - 증례보고 - (Vim Thalamotomy for Intractable Rubral Tremor Associated with Midbrain Tumor - Case Report -)

  • This 26-year-old man gradually developed a disabling midbrain tremor involving both distal and proximal part of left upper arm. On neurologic examination, oculomotor palsy, and ataxia of the left arm were noted. Radiologic examination revealed a mass lesion on midbrain tegmentum. He was tentatively diagnosed as brain tumor (presumably germinoma) without stereotactic biopsy. Conventional radiation therapy was given for this lesion. Although there was improvement in the radiologic imaging, his midbrain tremor worsened and became untolerable. The authors performed MR-guided stereotactic Vim-thalamotomy. With macroelectrode stimulation and radiofequency lesioning, his resting, postural and action tremors were almost completely abolished in both distal and proximal part of left upper extremity. Authors consider that Vim thalamotomy is still an effective means of controlling midbrain tremor involving proximal upper limb.

조등산(Diaoteng-san, Choto-san, 釣藤散)과 GB34(陽陵泉) 전침치료를 활용한 약인성파킨슨증후군 환자의 증례 1례 (A Case Report of Drug-Induced Parkinsonism Treated with Jodeung-san and Electro-acupuncture at GB34)

  • Drug-induced Parkinsonism is the most frequently observed type among the cases of secondary Parkinsonism. Besides typical parkinsonian symptoms, such as tremor, rigidity and bradykinesia, drug-induced Parkinsonism manifests with additional simultaneous symptoms like orobuccolingual dyskinesia, mixed type of tremor (resting, action), and symmetry of expressions. We present a case of drug-induced Parkinsonism, affected by taking the antiulcer drug cimetidine. Jodeung-san extract (Choto-san, Tsumura Co. 10) was administered for 7 days and acupuncture (electronic, auricular, pharmacopuncture) was conducted 3 times. The clinical outcomes were then evaluated through the patient's global impression of change, visual analogue scale, and Hoehn and Yahr stage. After the treatment, the clinical features, such as tremor and orobuccolingual dyskinesia, disappeared. The combination of Jodeung-san and electro-acupuncture at GB34 could therefore be a remedy for the patients with drug-induced Parkinsonism.
