Development of Nursing Clinical Judgment Scale (간호사의 임상판단 측정도구 개발)

  • Purpose: This study aimed to develop a nursing clinical judgment scale (NCJS) and verify its validity and reliability in assessing the clinical judgment of nurses. Methods: A preliminary instrument of the NCJS comprising 38 items was first developed from attributes and indicators derived from a literature review and an in-depth/focus interview with 12 clinical nurses. The preliminary tool was finalized after 7 experts conducted a content validity test based on a data from a preliminary survey of 30 hospital nurses in Korea. Data were collected from 443 ward, intensive care unit, emergency room nurses who voluntarily participated in the survey through offline and online for the verification of the construct validity and reliability of the scale. Results: The final scale comprised 23 items scored on a 5-point Likert scale. Six factors - integrated data analysis, evaluation and reflection on interventions, evidence on interventions, collaboration among health professionals, patient-centered nursing, and collaboration among nurse colleagues - accounted for 64.9% of the total variance. Confirmatory factor analysis supported the fit of the measurement model, comprising six factors (root mean square error of approximation = .07, standardized root mean square residual = .04, comparative fit index = .90). Cronbach's α for all the items was .92. Conclusion: The NCJS is a valid and reliable tool that fully reflects the characteristics of clinical practice, and it can be used effectively to evaluate the clinical judgment of Korean nurses. Future research should reflect the variables influencing clinical judgment and develop an action plan to improve it.

Survey on the Needs of Korean Medicine Doctor for Community Care Education Programs (한의약 건강돌봄 교육 프로그램에 대한 한의사 요구도 조사)

  • Objectives : This study conducted a survey on the needs of Korean medicine doctors for health care education programs specializing in traditional Korean medicine. Methods : The study selected Korean medicine doctors who had experience participating in outpatient consultations. Data collected through surveys underwent frequency analysis on performance, importance, difficulty, and educational needs using SPSS 24.0. Additionally, an Importance-Performance Analysis (IPA) was conducted using importance and performance data. Results : According to the results of the IPA analysis, in the area of "keep up the good work" there were activities such as fee Claims (A3), comprehensive assessment (B4), care plan development (B5), client and caregiver interviews (C8), chronic disease monitoring (C9), musculoskeletal and other pain management, musculoskeletal rehabilitation (C10), mental health management (C11), and fall prevention (C15). In the "concentrate here" priority action area, skin care including pressure sore management (C13) was identified. Conclusions : The traditional Korean medicine community care service is expected to expand further, so it is anticipated that the developed educational programs will contribute to the activation of traditional Korean medicine health care business.

Hysteroscopic evaluation of endometrial changes and fallopian tubal functions in women using progestin-only contraceptives

  • The purpose of this prospective study was to investigate whether office hysteroscopy (OH) can be used to assess the mechanisms of action of progestogen-only contraceptives (POCs), diagnose possible local causes of abnormal uterine bleeding (AUB), and support the treatment plan of symptomatic patients using POCs compared with those who do not use hormones. The study included 140 women who were divided into two groups. Group A consisted of 70 women who used POCs, whereas group B consisted of 70 women who did not use hormones. They were successively examined using transvaginal ultrasonography (TVS), OH, and endometrial sampling. The TVS results were consistent with those of OH and histopathology. The changes in endometrial thickness and vasculature, as well as fallopian tube (FT) functions, were significantly more pronounced in POC users than in non-POC users. There was a significant reduction in the peristalsis of the proximal part of the FT, as well as a reduction in the bubble flow test in group A compared with group B. In addition, the combination of peristalsis and the bubble flow test (Darwishscope test) was significantly lower in group A. It was concluded that using OH as a simple diagnostic tool in women with POCs would contribute to a better understanding of the mechanisms of endometrial and FT effects and explain some local endometrial causes of AUB. This ensures that the combination of TVS and OH would limit routine endometrial sampling in POCs users.

The Effect of Taking Gamitaeeumjowee-tang for 2 Weeks on Blood Pressure, Pulse Rate and Body Weight in Overweight or Obese Patients: A Retrospective Chart Review (가미태음조위탕 2주 복용이 과체중 또는 비만 환자의 혈압, 맥박수 및 체중에 미치는 영향: 후향적 차트리뷰)

  • Objectives: This study was aimed to evaluate changes in blood pressure, pulse rate and body weight of overweight or obese patients after two weeks of taking Gamitaeeumjowee-tang. Methods: A retrospective chart review was conducted for patients with auscultatory blood pressure records who visited the clinic after two weeks from the first visit between July 8, 2019 and August 30, 2019. Results: A total of seventy-two patients were included. There were significant differences in systolic blood pressure (121.69±14.3 mmHg to 117.37±12.3 mmHg, P=0.002) and pulse rate (78.56±9.3 bpm to 82.99±9.0 bpm, P<0.001). There was no significant difference in diastolic blood pressure. There were significant differences in body weight (76.28±13.5 kg to 73.41±13.1 kg, P<0.001) and body mass index (28.82±4.4 kg/m2 to 27.75±4.3 kg/m2, P<0.001). Conclusions: In this study, blood pressure tended to decrease and pulse rate slightly increased after two weeks of taking Gamitaeeumjowee-tang. It is necessary to notice the possibility of pulse rate increase and the plan of action when using Gamitaeeumjowee-tang. Additional well-designed investigations are needed.

  • Interest in the future of the battery market is growing as Tesla announces plans to increase production of electric vehicles and to produce batteries. Tesla announced an action plan to reduce battery prices by 56% through 'Battery Day', which included expansion of factories to internalize batteries and improvement of materials and production technology. In the trend of automobile electrification, the expansion of the battery market, which accounts for 40% of the cost of electric vehicles, is inevitable, and the size of the electric vehicle battery market in 2026 is expected to increase more than five times compared to 2016. With the development of materials and process technology, the energy density of electric vehicle batteries is increasing while the price is decreasing. Soon, electric vehicles and internal combustion locomotives are expected to compete on the same line. Recently, the mileage of electric vehicles is approaching that of an internal combustion locomotive due to the installation of high-capacity batteries. In the EV battery market, Korean, Chinese and Japanese companies are fiercely competing. Based on market share in the first half of 2020, LG Chem, CATL, and Panasonic are leading the EV battery supply, and the top 10 companies included 3 Korean companies, 5 Chinese companies, and 2 Japanese companies. All-solid, lithium-sulfur, sodium-ion, and lithium air batteries are being discussed as the next-generation batteries after lithium-ion, among which all-solid-state batteries are the most active. All-solid-state batteries can dramatically improve stability and charging speed by using a solid electrolyte, and are excellent in terms of technology readiness level (TRL) among various technology alternatives. In order to increase the competitiveness of the battery industry in the future, efforts to increase the productivity and economy of electric vehicle batteries are also required along with the development of next-generation battery technology.

The Effects of Perception of Fun Factors for Participation in Park Golf on Psychological Happiness and Behavioral Intentions : Focusing on the New Silver Generation (파크골프 참여에 대한 재미요인 인지가 심리적 행복감 및 행동 의도에 미치는 영향 : 뉴실버세대를 중심으로)

  • In this study, a total of 255 New Silver Generations residing in Chungcheong Province were analyzed from September 16 to December 10, 2023 to find out how the fun factors of park golf, which has established itself as a representative silver sport, affect psychological well-being and behavioral intentions. First, it was found that the fun factor had an effect on psychological well-being. Second, it was found that the fun factor influenced the behavioral intention. Finally, it was found that the psychological well-being of park golf participants had a significant effect on the intention of action. In fact, it was found through the results of the study that park golf participants felt psychological well-being according to the fun factor and were also related to future actions. Recognizing that it is the time when it is required to understand the range of New Silver people's behavior and provide customized programs to encourage participation, it is also necessary to devise a plan to activate it as a daily sport.

Regional cooperation of NOWPAP MERRAC against marine litter from sea-based activities in the Northwest Pacific region (북서태평양 지역의 해상기인 해양쓰레기 저감을 위한 NOWPAP 방제지역활동센터의 지역협력 활동현황 분석 및 향후 발전방향)

  • The marine litter generated as by-products of human activities and economic development enters to the sea through rivers or stream Indirectly, and through sea-based activities directly. It is commonly comprised of materials that degrade very slowly, such as various plastic products, polystyrene, glass, rubber, metal, wood, derelict fishing nets, wire, rope and so on. Such litter is found in the water column and on the seafloor as well as coastal areas In the Northwest Pacific region. It causes injury or death of human and other living organisms and also accident or damage of the vessel. It is not only a problem of country but also regional and/or global problem because it is transported by currents and winds from one country to another. In this regard, Northwest Pacific Action Plan (NOWPAP) Marine Litter Activity (MALITA) project had been carried out during 2006-2007 biennium and NOWPAP Regional Action Plan on Marine Litter (RAP MALI) has been also continuously implemented in the 2008-2009 biennium as next phase step of MALITA. MERRAC, one of four Regional Activity Centres (RACs) of NOWPAP, has developed monitoring guidelines, sectoral guidelines, and brochures related to sea-based marine litter and port reception facilities and services through MALITA project. Based upon these output, MERRAC will continuously implement relevant activities of RAP MALI in order to help to establish and improve a regional mechanism to deal with the sea-based marine litter problem. This paper aims to introduce MERRAC activities under MALITA and RAP MALI, and to suggest several recommendations to reduce marine litter in the NOWPAP region.

Comparison of Three Different Helmet Bolus Device for Total Scalp Irradiation (Total Scalp의 방사선 치료 시 Helmet Bolus 제작방법에 관한 연구)

  • Purpose: This study evaluated the usefulness of Helmet bolus device using Bolx-II, paraffin wax, solid thermoplastic material in total scalp irradiation. Materials and Methods: Using Rando phantom, we applied Bolx-II (Action Products, USA), paraffin wax (Densply, USA), solid thermoplastic material (Med-Tec, USA) on the whole scalp to make helmet bolus device. Computed tomography (GE, Ultra Light Speed16) images were acquired at 5 mm thickness. Then, we set up the optimum treatment plan and analyzed the variation in density of each bolus (Philips, Pinnacle). To evaluate the dose distribution, Dose-homogeneity index (DHI, $D_{90}/D_{10}$) and Conformity index (CI, $V_{95}/TV$) of Clinical Target Volume (CTV) using Dose-Volume Histogram (DVH) and $V_{20}$, $V_{30}$ of normal brain tissues. we assessed the efficiency of production process by measuring total time taken to produce. Thermoluminescent dosimeters (TLD) were used to verify the accuracy. Results: Density variation value of Bolx-II, paraffin wax, solid thermoplastic material turned out to be $0.952{\pm}0.13g/cm^3$, $0.842{\pm}0.17g/cm^3$, $0.908{\pm}0.24g/cm^3$, respectively. The DHI and CI of each helmet bolus device which used Bolx-II, paraffin wax, solid thermoplastic material were 0.89, 0.85, 0.77 and 0.86, 0.78, 0.74, respectively. The result of Bolx-II was the best. $V_{20}$ and $V_{30}$ of brain tissues were 11.50%, 10.80%, 10.07% and 7.62%, 7.40%, 7.31%, respectively. It took 30, 120, 90 minutes to produce. The measured TLD results were within ${\pm}7%$ of the planned values. Conclusion: The application of helmet bolus which used Bolx-II during total scalp irradiation not only improves homogeneity and conformity of Clinical Target Volume but also takes short time and the production method is simple. Thus, the helmet bolus which used Bolx-II is considered to be useful for the clinical trials.

A Study on the Current State of the Integrated Human Rights of the Elderly in Rural Areas of South Korea (농촌지역 거주 노인의 통합적 인권보장 실태에 관한 연구)

  • This study purported to investigate the current state of human rights of older adults residing in rural areas of Korea. The study utilized, as an analytic framework, 4 priority directions (1. "older persons and development", 2. "rural area development", 3. "advancing health and well-being into old age", and 4. "ensuring enabling and supportive environments") with 13 task actions recommended by Madrid International Plan of Action on Ageing (MIPAA). Furthermore, the study examined gender differences in all items included in the analytic framework. Data was collected by the face-to-face survey on 800 subjects aged 65 and over. Statistical analyses were conducted using STATA 13.0 program. The main results were summarized in order of 4 priority directions as follows. First, average working hours per day were 6.2, and men reportedly participated in economic activities and needed job training more than women, while women participated in lifelong education programs more than men. Awareness of fire and disaster prevention facilities was low in both genders. Second, accessibility to the support center for the elderly living alone as well as protective services for the vulnerable elderly was found to be low. IT-based services and networking were used more by men than women, and specifically, IT-based financial transactions and welfare services were least used. Third, medical check-ups and vaccinations were well received, while consistent treatments for chronic illnesses and long-term care services were relatively less given. In addition, accessibility to mental health service centers was considerably low. Fourth, although old house structures and the lack of convenience facilities were found to be circumstantial risk factors for these elders, experiences of receiving housing support services were scarce. The elderly were found to rely more on informal care, and concerns for their care were higher in women than men. Plus, accessibility to elderly abuse services was markedly low. Based on these results, discussed were implications for implementing policies and practical interventions to raise the levels of the human rights for this population.

A Study Seeking the Practical Implementation of the Yellow Sea Large Marine Ecosystem Project (황해광역해양생태계 프로젝트의 실효성 확보에 관한 연구)

  • The Yellow sea, as described in article 123 of UNCLOS, is semi-enclosed sea surrounded by the Republic of Korea, the People's Republic of China and North Korea. In addition, the Yellow Sea is one of the 66 large marine ecosystems as it contains large amounts of marine resources. According to article 194 of UNCLOS, states should be aware of rights and duties with respect to the protection and preservation of the marine environment to be engaged with countries directly as regional entity or indirectly. Therefore, the legal blank is urgent in terms of trans-boundary environmental pollutant issues. The UNDP has conducted a project called Yellow Sea Large Marine Ecosystem (YSLME) which has reached the 2nd phase. The project has some notable achievements, namely performing joint activities on analysis of diagnostic trans-boundary issues in collaboration with China and South Korea, developing a strategic action plan based on TDA, and establishing regional strategic action plan. However, on the other hand, the project could not reflect the full participation of North Korea as a state party. As a result, the project has a limitation on effective implementation of RSAP. Therefore, this study focuses on the suggestion of a legally-binding trilateral treaty as a blue print for the next, 3rd phase of the project. By analyzing the best practice of the Wadden Sea Trilateral Treaty case, the study verifies the validity of legislative measures on establishing and managing a legally-binding trilateral YSLME Commission. By suggesting a three phase treaty, incorporating a joint declaration by establishing the commission, the signing of the treaty, and formulating an umbrella convention and implementation arrangement, the study expects to guarantee the consistency and sustainability of the trilateral treaty regardless of political issues pertaining to North Korea.
