Application of Prefabricated Retaining Walls with Steel Lagging (강재 요소를 적용한 조립식 흙막이 벽체에 관한 연구)

  • It has been known that the conventional retaining wall system with timber lagging and H pile has several problems such as the irregular gap between H-piles, cutting or adding to standard timber, back fill over first step excavation, and especially break-down accident at the disjoint of wall system. In the practical excavation, these problems may lead to worker's accident and the inefficiency of construction economy. To solve the above problems, a new method using prefabricated retaining wall was proposed and verified. The characteristics of the new method is to replace timber wall as free-sliding steel-lagging and connector. To check its verification and application, laboratory tests such as bending strength, tensile strength, and fatigue strength were carried out. Also, a pilot test in the field and numerical simulations under various ground conditions were performed. From the researches, it is found that the prefabricated retaining wall plate can be superior to the conventional timber lagging plate in the strength. It is also found that the proposed methods can be effective in the reuse of retaining wall plate and safe in the disjoint of wall system. Finally, it is desired that the proposed method will be effective in the reduction of the imported timbers and helpful in the safety of retaining wall construction.

Perception of the Nurse on the Nursing Malpractice and Its Case (간호사고와 관련판례에 대한 임상간호사의 지각)

  • The purpose of this study is to provide some basic data for the direction of nursing education and practice to prevent nursing malpractice as investgating perception of the nurse on the nursing malpractice and its case. Data were collected from 450 nurse working in four general hospitals which were located in Chonbuk province from November 9 to 21, 1998 through questionnare. The structured instruments developed by the reseacher were used for collecting data. The Results were as follows: 1) As for the cognition of the nursing malpractice case, the transfusion accident was 37.3%, the side-effects of KCL injection(19.3%), hymen rupture by uterus cancer test, the side-effects of aminophyllin injections on dyspnea patient and the others were 10% below. 2) With regard to the channel of the perception of the nursing malpractice case, mass-communication was taken first and followed through by neighbors, In-service education, school education, continuing education 3) As for the direct${\cdot}$indirect experience of the nursing malpractice case, transfusion accident was 51.3%, the fall of NPO patient after an operation 40.1%, the side-effect of KCL injection 32.5%, suicide of the psychiatric patient 32.5%, and the others were 30% below. 4) The possibility of nursing malpractice case was an average 2.57(${\pm}$0.91) and the highest was the fall of the NPO patient after an operation. 5) The perception on the responsibility of the nursing malpractice in its case was indicated as a joint-fault in ten nursing malpractice instances. As compared to the real decision, nurse's perception of the responsibility in the fall of the NPO patient after an operation, the side-effect of KCL injection, the tracheal edema of the patient who had a thyroid operation, the suicide of psychiatric patient, the hymen rupture by uterus cancer test accorded real decisions. But the other cases were different from the real decisions. These cases were perceived as ones of joint fault even in cases determined as Dr's single fault cases or those in which both doctor and nurse were declared free of fault. 6) Knowledge levels of the nursing malpractice, school education and In-service education were perceived as low but anxiety levels of the malpractice were high. 7) With regard to the countmeasure of the hospital after nursing malpractice, the rate answered as "the practice settled the accident temporarily and forced the person in charge to be punished" was highest. In conclusion, the level of the cognition of the clinical nurse on nursing malpractice cases was low. As nurses' perception on the responsibility of the nursing malpractice case was compared to the real decision, there was a difference in five cases out of the ten cases.

Shoulder Arthrodesis in Brachial Plexus Injury Patient (상완신경총 손상 환자에서 시행한 견관절고정술)

  • Purpose: To analyze the clinical and radiologic results of the shoulder arthrodesis in brachial plexus injury patients with flail upper extremity. Material and Method: From Aug 1978 to April 2008, 29 shoulders in 29 patients with brachial plexus injury with shoulder fusion, we evaluated 20 shoulders in 20 patients, more than 1 year follow up. The average follow-up period was 6.45years (range: 1year~24years). There were 13 men and 7 women, and the mean age at the time of trauma was 32.0years(2~65 years). The type of injury was a motorcycle accident in 11 patients, in car accident in 5, pedestrian accident in 3, and fall from a height in 1. The lesion of injury was root and trunk in 1 patient, trunk and cord in 1, trunk in 18. Surgery was performed on the whole arm type paralysis in 12 patients, lower arm type paralysis in 8 patients. The preoperative visual analog scale score was 8.7(7~10). When the trapezius and serratus anterior muscle were in function, operation was performed. 18 patients were processed to the additional operation. Gracilis free flap in 6 patients, neurotization in 3, Steindler flexor plasty in 6, and tendon transfer in 3 were performed. Fixation was conducted with cancellous screws in 13 patients, Knoles pins in 5, and cancellous screws and Knoles pins in 2. The position of the arthrodesis at operation was $28.5^{\circ}$($20~45^{\circ}$) in abduction, $30.3^{\circ}$($20~45^{\circ}$) in flexion, and $30.8^{\circ}$($20~40^{\circ}$) in internal rotation. Result: The follow up visual analog scale score was 3.4(0~7). Postoperatively, shoulder spica cast was applied for 15.3weeks(8-20weeks). The median time to bony union was 17.7weeks(9~28weeks). Average range of motion was $32.0^{\circ}$($15~40^{\circ}$) of abduction, $24.0^{\circ}$($10~40^{\circ}$) of flexion, and $18.5^{\circ}$($10~30^{\circ}$)of internal rotation. Conclusion: The shoulder fusion in brachial plexus injury patients is one of the good methods to relieve pain, improve the function and stabilize the flail shoulder joint.

Study for the Homogeneity of Freeway Free-Flow Speed under the State of LOS-A Low Density (LOS A 저밀도 상태에서 고속도로 자유교통류 속도의 동질성에 관한 연구)

  • Freeway FFS (Free-Flow Speed) under the state of LOS (Level of Service) A has been regarded as a homogeneous state to all levels of traffic volume. The features of low-density FFS is served as the basic data for a decision on limited maximal speed, accident analysis, simulation modeling etc. A few researches for the macroscopic characteristics of design and operation speed have been reported, and any study for the microscopic features of freeway free-flow speed under the state of low density has not been done. Therefore, the characteristics of low-density FFS according to the level of traffic volume (1-3 veh/30 sec) and daily hour periods (0-5, 6-8, 9-11, 12-19, 20-23) is microscopically analyzed in this study with huge volume and speed data. It was found that speed distributions are changed and show different behaviors under the conditions of traffic volume levels and daily time periods. V85s (85th percentile speed) at early morning and night periods decrease when levels of traffic volume increase, whereas V85s at day time do not show considerable differences. Especially, FFSs of levels of traffic volume at early morning and night periods, despite low density, is analyzed as heterogeneity, but homogeneity at day time.

Clinical Review of Hypertensive Patients Admitted in the Hospital (병원에 입원한 고혈압 환자에 대한 조사연구(I))

  • As the seriousness of hypertension in adults is increasingly recognized, attention is being focussed on its control through continuous health supervisions. Complications arising from high blood pressure exceed those of many other adult health problems and affect the nursing needs of these patients. In order to contribute to effective nursing care planning and improved health supervision, 248 medical records of hypertensive patients admitted to Internal Medicine at Ewha Medical Center from January 1971 to August 1976 were examined. Results were as fellows: 1. The sample group averaged 5.0% of the total number of patients admitted to internal medicine during the period studied. The proportion increased slightly with each successive year. 2. Patients with hypertension averaged 31.4% of all patients with cardiovascular discease. This proportion was stable over the period. 3. Hypertensive patients were primarily in their 40s. The average age was 55.1 years. 4. Women were most frequently affected in their 60s and men in their 40s. 5. The bloo pressure of these in their 30s was not well controlled on discharge. Rates for those in their 30s averaged 151.5/107mmhg, for those in their 40s 161.5/110mphg, and for those in their 60s 169/100mmhg. 6. Complications increased with each age range from the 40s to the 60s for both men and women. 7. Complications related to systolic hypertension occurred most frequently when the rate exceeded 200mmhg. Cerebral vascular accident was the primary complication, presenting implications for nursing intervention. 8. Complications related to diastolic hypertension occurred most frequently when the rate was in the $110{\sim}129mmhg$ range. C.V.A. again was the primary complication needing nursing care planning. 9. Diets ordered for patients were low salt 79.%, salt free 4.19%, diabetic 6.1%, and protein free ('no protein') 2.0%. Caloric, salt end protein levels were not defined. Recommendations based on the findings were directed to diet and other therapies.

This Paper investigated the impacts on the hands-free cellular Phone use while driving to the drivers Performance among normal and two different level of secondary task driving conditions in the urban of Japan. As the results, quantitative differences of drivers eye movement, subjective mental workload, steering wheel angle entropy, and cognitive Performance of secondary tasks were compared between straight and curve (right turn) section for each driving condition.

  • The purpose of this article is to examine maritime accident and corporate criminal liability in comparison with cases and laws in UK and US. In Anglo-American law, a corporation can be convicted of and sentenced for a criminal offence. However, some theoretical difficulties lie in fixing a corporation with the appropriate mens rea. The Corporate Manslaughter and Corporate Homicide Act 2007 in England is to solve those difficulties and punish a corporation like a natural person. Comparing to Anglo-American law, a corporation is difficult to be punished in Korean law because it is a well recognized theory that only natural person is capable of committing a crime. However, safety in society and workplace is earning great concern in Korea, and emphasis is put on responsibilities of corporations. This article discusses the need for legislation on corporate manslaughter act in Korea with regard to the sinking of the MV Sewol.

Quasi-Static Analysis of Block Impact Against the Ground Due to Sling Failure During Block Lifting (권상 작업 중 슬링 파손으로 인한 블록 지상 낙하 충격에 대한 준정적 해석)

  • Recently, shipyards are making many efforts to reduce the number of the mounted blocks by increasing the block size. This is to improve productivity and reduce related costs by minimizing block movement and shortening the building period. However, as the blocks become larger, the weight increases considerably. If the target block has a damage due to an unexpected accident during block lifting, it may seriously cause a problem of the reusability of the block. In this study, a large-sized block of the offshore structure weighing 480 tons was lifting with a total of seven sling belts, and one sling belt was broken while it was moving, resulting in a situation in which a part of the edge of the block collided with the ground. The aim of this paper is to verify the structural integrity of the block that directly collides with the ground in the form of free fall due to the sling breakage. Considering that the hook loads acting on several sling belts holding the block are redistributed when a sling belt is broken, the hook loads were recalculated at the angle just before the sling breakage. These loads were used to check the safety of the sling belts. In addition, FE analysis was performed by calculating the amount of impact from the free fall condition, obtaining the impact area by using Hertz's contact theory, and then applying the impact load to the area.

Formant Frequency as a Measure of Physical Fatigue

  • Objective: The current study investigated a non-obtrusive measure for detecting physical fatigue based on the analysis of formant frequencies of human voice. Background: Fatigue has been considered as a main cause in industrial and traffic accidents. Therefore, it is critical to detect worker's fatigue for accident prevention. Method: After running exercises on a treadmill, participants were instructed to read a sentence and their voices were recorded under four different physical fatigue levels. Korean vowels of "아", "어", "오", "우", and "이" from the voice recorded were then used to collect formant 1 frequencies. Results: Results of separate ANOVAs showed a significant main effect of physical fatigue on formant 1 frequency of "아", "어", and "이". Furthermore, post-hoc comparisons revealed that formant 1 frequency of "아" was most sensitive to physical fatigue level employed in this experiment. Conclusion: Formant 1 frequencies of some vowels significantly decrease as the physical fatigue level increases. Application: Potential application of this study includes the development of a measure of physical fatigue state that is free from sensor attachment and requires little preparation.

Pedicled Perforator Flaps for Reconstruction of Bilateral Knee Defects: A Case Report

  • Reconstruction of soft tissue defects of the knee has always been a challenging task for plastic surgeons. Various reconstructive choices are available depending on the location, size, and depth of the defect relative to the knee joint. Defects on the knee joint have several characteristic features. The use of a free flap is preferred for reconstructions involving obliteration of large-cavity defects, but recipient pedicle isolation can be difficult because of the extent of the injury zone. Furthermore, the true defect during knee joint flexion is larger than during knee joint extension, and a durable flap is necessary for joint movement. We report for the first time on the use of pedicled perforator flaps for reconstruction of bilateral knee defects in a 76-year-old woman. The operative procedure required skeletonizing the perforators of an antero-lateral thigh flap and antero-medial thigh flap and rotating the flap in the defect. The patient returned to normal daily activity and had a full range of motion two months after the accident. The shorter operating time with decreased donor site morbidity and its durability make this flap a valuable alternative for soft tissue reconstruction of the knee.
