Study on the Improvement of Event Data Recorders through Accident Analysis (사고분석 사례를 통한 사고기록장치 개선방안에 대한 고찰)

  • This study was initiated to improve of the defect investigation method using event data recorders (EDR) and suggested a solution through the regulation and system analysis of EDR. The EDR data has been used for various purposes such as the vehicle defect investigation and the traffic accident investigation. However the EDR regulation has not been updated since the implementation in 2012. "Trigger Threshold" can be used to analyze a single accident such as the frontal crash, the side crash, and the rollover. In the case of a complex accident in which a rollover accident and a crash accident occur simultaneously, it is difficult to analyze a complex accident due to current "Trigger Threshold". This study proposed the method of separating the "Trigger Threshold" into a crash accident and a rollover accident so that accidents can be analyzed using the EDR data even when a complex accident occurs. In addition, it proposed the improvement method to quickly use the data of EDR in accident reconstruction software.

  • This study presented quantitative risk analysis in case of transporting explosive materials by railway. Accident types were classified into accidents of in station and in transit. And the study presented an initial value of accident frequency through derailment accident and crushing one according to each type, and drew the results of accident frequency through event tree analysis. Damage impact evaluation used TNT equivalent method and probit analysis method. As the result of risk evaluation, railway transportation of explosive materials passing through areas which are high in population density is appeared to be able to cause a large number of personnel injury when occurring accidents. Specially, the accident of explosive transportation combined with petroleum was forecasted as easily resulting in large explosive accident. Consequently, there is a necessity to reduce consequences by decreasing passage through areas where are high in population density, and take measures for lessening the risks in case of transporting dangerous explosive materials.

The Gender Difference in the Occupational Hazards and Injuries of Cleaning Workers and Janitors

  • Objective: The purpose of this study is to analyze the accident characteristics according to the gender of the injured workers in building cleaning and to reflect them in the Industrial Accident Prevention Policy. Background: An analysis of industrial accidents is an essential process for establishing systematic industrial accident prevention measures. In order to establish industrial accident prevention measures for workers effectively, it is necessary to analyze accident characteristics by job type for workers who do the same work. Method: In this study, we analyzed the accident characteristics of 1,645 janitors who were approved of work-related injuries in 2015. We also analyzed the characteristics according to gender by dividing them into worker-related factors and accident-related factors. Results: The accidents caused to the janitors showed different characteristics according to gender, age, work experience, agency of accident, and distribution of original cause materials. In other words, 70.2% occurred to workers over 60 years old and 56.2% occurred to unskilled workers with less than a year of work experience. In the case of accident pattern, 79.1% occurred in tripping (slip) hazards, and 68.2% of accidents occurred on the floor (including the ground) and the stairs, indicating that the accident occurred most frequently during cleaning work on the floor or stairs. Conclusion and Application: The results of the study on the accident characteristics of the janitors can be used as basic data for systematic preventive measures against accidents occurring to the elderly female workers in the service industry.

Development of Emergency Restoration Scenarios for Railway Accident using Analytic Network Process (네트워크분석적 의사결정기법을 이용한 철도사고 임시복구시나리오 개발)

  • The emergency restoration scenarios for efficient railway accident management and restoration were developed. The emergency restoration procedures defined by the worst case of emergency restoration and the important events was proposed based on questionnaires from specialists and the result of survey. Via these studies, the railway accident in the tunnel could be the worst case among all railway accident types. Therefore, educations for a restoration team in confined area condition should be planned and performed to recover the worst case accident. In order for the emergency restoration, when a railway accident is occurred, the restoration should be performed in orders of handing collapse of facilities, burying track, and derailment of vehicle in tunnel based on the statistical analysis. The result of priorities were established by the period of restoration. The standard operation system for efficient railway accident management was developed by synthesizing the worst case for rapid emergency restoration, and important events for the standard operation procedures according to each emergency restoration type. Through this study, the restoration operation system of railway accident are recommended. This paper suggests to develop emergency restoration scenarios for the efficient railway accident management and recovery system. The study results will contribute not only for insuring punctuality, but also for minimizing delays from accidents. Therefore, emergency restoration scenarios will play a major role in the SOP for the damage limitation and the prevention of accident spread.

  • The safety accident is one of the formidable issues in construction field, and in order to prevent safety accident, contractors perform safety education, safety control document, meeting with labors, and safety instruction in construction phase. However, safety accident is occurred continuously, it is needed to analyze which factors are affected to safety accident. day of the week, daily construction cost, and daily work task were selected as those factors. The result in this research represented that those factors were not considered as an critical one in safety accident. Because of approach limitation to safety accident, only two case studies were collected. If more safety accident cases are gathered in future study, it will clarify the cause of safety accident and will prevent safety accident.

The Generic Analysis Method for Core Flow Instability

  • The generic analysis method for core flow instability is suggested to confirm that the core flow instability would not occur on PWR conditions. For the confirmation, the stability criteria of each fuel type are provided. Instability investigations in various accident conditions prove that the locked rotor accident is the most limiting case to instability. Parametric Effects are surveyed and in good agreement with available studies. The effects of heat flux distribution become negligible as the subcooling number is decreased. The power margin to instability is calculated quantitatively in various accident conditions.

A Study on the Systematic Cause Analysis of Shipboard Fire Accident Case using STAMP Methodology

  • The ship system is complex and advanced, and the operation relationship between each element is very high. So it is necessary to approach it in terms of an overall and integrated system in addition to the traditional sequential approach of finding and removing the direct cause of the accident when analyzing the accident. In this study, it is analyzed the recent fire accidents on ships occurred the Korean terrestrial water using a STAMP methodology that is different from conventional accident analysis techniques. This analysis reviews a range of factors, including safety requirements to prevent fires in ships, inappropriate decisions and actions, situations, equipment defects, and recommendations derived from accident analysis results. Through a comprehensive approach to accident prevention using STAMP, alternative evaluations are presented at the component level within the entire system of ships, and they are systematically used for accident prevention and risk evaluation as well as simple accident analysis.

  • Integrated severe accident codes should be capable of simulating not only specific physical phenomena but also entire plant behaviors, and in a sufficiently fast time. However, significant uncertainty may exist owing to the numerous parametric models and interactions among the various phenomena. The primary objectives of this study are to present best-practice uncertainty and sensitivity analysis results regarding the evolutions of severe accidents (SAs) and fission product source terms and to determine the effects of mitigation measures on them, as expected during a short-term station blackout (STSBO) of a reference pressurized water reactor (optimized power reactor (OPR)1000). Three reference scenarios related to the STSBO accident are considered: one base and two mitigation scenarios, and the impacts of dedicated severe accident mitigation (SAM) actions on the results of interest are analyzed (such as flammable gas generation). The uncertainties are quantified based on a random set of Monte Carlo samples per case scenario. The relative importance values of the uncertain input parameters to the results of interest are quantitatively evaluated through a relevant sensitivity/importance analysis.

  • Steel and iron manufacture works exist that many latency risk as melting liquid of high temperature, work of high place, and so on. Once in a while, the accident case make use of basic data for latency risk analysis in a place of business. In this paper, we investigated the cause of the accident in steel an iron works. The result, we came across that many latency risk in steel and iron manufacture works. The main type of risk are fall, narrow, come flying, etc. Most of the latency risk type are repetition and conventional accident. Accordingly, steel and manufacture works must prevent to repetition and conventional accident.

A Study of Emergency Response for the Leakage Accident of Hazardous and Noxious Substances in a Port (항만에서의 위험·유해물질(HNS) 누출사고 대응에 관한 연구)

  • In general, lots of containers including various dangerous materials are transported to the port located in big cities such as Busan where massive residents live. Thus, it's really important how to make the emergency response for the leak accidents of dangerous materials and evaluate the direct or indirect damages to adjacent areas. In this study, in order to make reasonable emergency plans, CA (Consequence Analysis) is employed after selecting a key hazardous and noxious material, hydrogen fluroide. This material accounts for the third largest portion of cargo volume among all dangerous materials and can cause a huge damage in case of leakages. As a case study, Busan North port is selected as a test port since the portion of dangerous materials is higher than that of other ports in Busan. It is assumed that 1 ton of hydrogen fluoride is spilled at Busan North port. CA is performed to assess the impact of this accident. Throughout CA, the ERPG-2 range of a leak accident can be evaluated and this result can be used for decision making tools for mitigating the impact of a leak accident. To mitigate the damage of this accident, suitable a protective equipment and resident evacuation procedures should be prepared. Finally, this study can provide a systematic approach to make the emergency plan for reducing economical and personal losses.
