Strategy to Overcome Drug Resistance That Develops during Treatment of Chronic Hepatitis B in Children

  • Development of antiviral resistance to lamivudine is the most important factor for the treatment failure. It is necessary to establish proper guidelines to overcome drug resistance for children with chronic hepatitis B. Primary treatment with lamivudine should be considered if patients are in immune-clearance phase and have persistently elevated ALT levels more than twice the upper limit of normal value. Before initiating the therapy, careful consideration of the patient's status is required to exclude abnormal liver function tests due to other causes. The treatment option should be carefully decided to suppress the viral replication effectively. To obtain good compliance, clinicians should educate patients and their parents. Appropriate monitoring for virologic breakthrough and genotypic resistance is important in deciding to change the treatment plan. Sequential monotherapy should be avoided and a combination of drugs in other categories is recommended. New antiviral agents, such as entecavir and tenofovir, which have high potency and high genetic barrier, are soon expected to be available for use with children.

A Clinical Observation of Seroconversion on Chronic Hepatitis B Treated with a Herb Prescription

  • Objective : Report on a hepatitis B patient with abnormal AST/ALT, HBeAg(+), and habitual diarrhea stemming from irritable colon. Methods : Lab-evaluation of intervals of herb medicine treatment for a patient with hepatitis B with HBeAg and irritable colon. Result & Conclusion : AST/ALT level was normalized and did not rise again. Seroconversion to HBeAb repeatedly appeared just druing the time of the treatment, but typically repeated seroconversion between treatment period and no treatment period confirmed that the treatment was effective for the immune system against hepatitis B. The exact mechanism is not clear but the result provides an indication that oriental herbal medicine has potential capacity to treat liver diseases.

  • Objective & Method : By investigating physiological function of tri-energizer, symptoms, and organ coordination, we obtained following conclusion. Tri-energizer is the membrane structure that surrounds the five visceral organs and six hollow organs, and filled with fluid. Tri-energizer acts as the passage for the flow of qi energy. Result & Conclusion : Therefore, dysfunction of the tri-energizer is caused by abnormal evaporation and metabolism. Upper-energizer regulates cardiopulmonary function, middle-energizer regulates spleen and stomach functions, and lower-energizer regulates liver, kidneys, small and large intestines, and bladder functions. Such a functional specialization is possible by receiving the source of qi through the wall wrapping around the internal organs. Tri-energizer represents the exterior and interior relationship by acting as the membrane structure supporting the five visceral organs and six hollow organs and at the same time, as the pericardium surrounding the heart.

  • An intact female, 5-year old, Shihtzu, weighing 3.2 kg with a history of depression, hypersalivation, dullness, and ataxia for 3 days was referred. Radiographic findings included microhepatica, bilateral renal calculi, and a cystic calculus. Ultrasonography showed microhepatica, decreased visibility of portal veins in the liver, bilateral renal calculi, a cystic calculus, and an abnormal tortuous dilated vessel in the craniodorsal abdomen. A single congenital extrahepatic portoazygous shunt was confirmed through operative mesenteric portography. The patient received surgery using an ameroid constrictor ring and recovered well.

Congenital Diaphragmatic Eventration: Report of 4 Cases (선천성 횡경막 내번증)

  • Congenital diaphragmatic eventration is a rare disease and generally accepted as an abnormally high position of part or all of the diaphragm, usually associated with a marked decrease in muscle fibers and a membranous appearance of the abnormal area. There were 4 cases of the congenital diaphragmatic eventration at the Dept. of Thoracic Surgery, Seoul National University Hospital, from 1957 to 1977. They were two boys and two girls and ranging from 1 day to 3 years of age. They were all repaired by surgical operation and one was expired postoperatively, another one was dead one year later due to complication. The ratio between right and left was 1:3 and their symptoms were cyanosis, dyspnea and frequent respiratory disease. In physical examination there was noted decreased breathing sound on the affected lung field and bowel sound was audible in some cases. Diagnosis was done by Chest X-ray and plication of the affected diaphragm was usually done in operation. There were noted atelectasis and cystic change of the affected side lung. And the liver, colon, spleen and small intestine were found in the dome of the eventrated diaphragm.

A Case Study of Poisoning Symptoms Caused by Misusing Xanthii Fructus, Treated with Korean Medicine (도꼬마리 열매 복용으로 발생한 중독증상(中毒症狀)의 한방치료 1례)

  • This is a clinical report of one patient with mental disorientation, abnormal behavior, spasm, and other symptoms caused by poisoning with Xanthii Fructus, who recovered completely with treatment. The patient was treated with Korean medical treatments, such as herbal medicine and acupuncture. After treatment, various symptoms caused by the Xanthii Fructus poisoning disappeared completely and laboratory findings, such as liver function tests (aspartate transaminase and alanine transaminase), were also close to the normal ranges. This case shows that Korean medicine therapy can be applicable in treating various symptoms caused by Xanthii Fructus poisoning. It is considered that further research is needed to confirm the safe use of Xanthii Fructus.

  • Hitiocytosis X, recently termed Langergans cell histiocytosis, is a rare disease histologically characterized by an abnormal proliferation of histiocytes, and of which the pathophysiology is still ambiguous. The clinical presentation is highly varied. Typically, there is a solitary or multiple bony involvements, and less frequently, lesions are found in other organs such as the lungs, liver, lymph nodes, skin, and mucosae. Interestingly, so far, there has been no report which shows multiple developments in both soft and hard tissues. In this report, we present a case of hitiocytosis X in both mandible and lung. This may provide some information for understanding the pathophysilogy of the histiocytosis X.

microRNA biomarkers in cystic diseases

  • microRNAs (miRNAs) are small non-coding RNAs that regulate gene expression by targeting the 3'-untranslated region of multiple target genes. Pathogenesis results from defects in several gene sets; therefore, disease progression could be prevented using miRNAs targeting multiple genes. Moreover, recent studies suggest that miRNAs reflect the stage of the specific disease, such as carcinogenesis. Cystic diseases, including polycystic kidney disease, polycystic liver disease, pancreatic cystic disease, and ovarian cystic disease, have common processes of cyst formation in the specific organ. Specifically, epithelial cells initiate abnormal cell proliferation and apoptosis as a result of alterations to key genes. Cysts are caused by fluid accumulation in the lumen. However, the molecular mechanisms underlying cyst formation and progression remain unclear. This review aims to introduce the key miRNAs related to cyst formation, and we suggest that miRNAs could be useful biomarkers and potential therapeutic targets in several cystic diseases.

A segmentation method of abnormal liver using abdominal CT images (복부 CT 영상을 이용한 비정상 간의 세그멘테이션 기법)

  • 일반적으로 복부 CT 영상에서 간암이나 다른 병변들을 갖고 있는 않은 정상 간은 고른 그레이값 분포 범위를 가지고 있다. 그 그레이값 범위는 대개 90 에서 92 사이의 값이다. 그러나. 복부 CT 영상에서 간암이나 여러 병변들을 가지고 있는 비정상간의 경우는 정상간의 경우와 같이 90 에서 92 사이의 일정 간격의 그레이값들만으로 구성되어 있지 않다. 비정상간의 경우는 병변들로 인하여 건강한 간의 실질 부분의 그레이값만을 나타내지는 못하기 때문이다. 이는 복부 CT 영상에서 간 부분을 세그멘테이션할 때 정상간 부분과 비정상간 부분의 세그멘테이션 방식이 다를 수 있음을 말해준다. 보통 기존에 있는 정상간의 세그멘테이션 기법은 위치 정보와 함께 일정 간격의 그레이값 분포 정보를 이용하여 수월하게 간을 세그멘테이션 했다. 그러나, 이 방식은 비정상간을 세그멘테이션하지 못하는 경우가 대부분이다. 본 연구는 간의 위치 정보, 거리 정보를 이용하고 각도선 조절 기법 등을 사용하여 비정상간을 세그멘테이션하였다. 그리하여, 본 연구는 세그멘테이션이 어려운 간암 보유 복부 CT 영상에 적용되어 효과적인 간의 세그멘테이션을 가능하게 하였다.

The Clinical Implications of Hepatic Enzymes in Metabolically Healthy Obese Men (대사적으로 건강한 비만남성에서 간 효소의 임상적 의의)

  • Increased hepatic enzymes are associated with insulin resistance, metabolic complications, and type 2 diabetes mellitus. Metabolically healthy obese (MHO) phenotype is not accompanied by metabolic complications and maintains insulin sensitivity, despite excessive body fat. The purpose of this study was to evaluate the clinical implications of hepatic enzymes in MHO men. The diagnostic criteria for MHO were based on NCEP-ATP III and obesity in adults was defined using WHO Asian-Pacific criteria. We used the data from 9,683 obese men aged between 20 and 70 years. The subjects were divided into three groups according to the diagnostic criteria: The metabolically healthy non-obese (MHNO, N=2,878), metabolically healthy obese (MHO, N=5,427), and metabolically abnormal obese (MAO, N=1,378). Obesity criteria were classified according to the standards set forth by WHO Asia-Pacific Criteria. AST, ALT, and GGT were significantly lower in the MHO group than in the MAO group (p<0.001, respectively). However, the hepatic enzyme levels were higher in the MHO group than in the MHNO group (p<0.001). Liver enzymes were associated with metabolic syndrome risk factors. Waist circumference, fasting glucose, total cholesterol, triglyceride, and HDL-C were risk factors for metabolic syndrome affecting liver enzymes. In conclusion, hepatic enzymes were found to predict metabolic abnormalities in metabolically healthy obese men.
