Difference of the Obesity Index, Blood Pressure and Serum Lipids in Abdominal and Non Abdominal in Men and Women (복부형비만군과 정상군간의 비만지수 혈압 및 혈청지질의 성별 차이)

  • This study was done to compare the difference of obesity index(waist-hip ratio, body fat, body mass index, relative body weight), blood pressure and serum lipids in abdominal obesity and non abdominal in both men and women. Abdominal and non abdominal obesity was divided into waist-hip ratio above 0.85 in women and 0.95 in men. The subjects were 412 adults (age range 40-59), who had regular health examinations between 1996 to 1997 at the S-Hospital in Seoul. The data were analyzed using ANCOVA (for adjusted for age) and Pearson correlation coefficient. The results were as follows: 1. 39.9% of men and 42.5% of women had abdominal obesity. The average age group of abdominal obesity was 50.8 which is older than the non abdominal obesity group(48.0). 2. After they were adjusted for age, the group of men who have abdominal obesity had higher levels in body fat, body mass index, relative body weight, blood pressure, total cholesterol, LDL- cholesterol and triglyceride than the group of non abdominal obesity group. The group of women with abdominal obesity had higer levels in body fat, body mass index, relative body weight , blood pressure and triglyceride than the group of non abdominal obesity. 3. In the group of non abdominal obesity, the waist-hip ratio was significantly correlated to body fat, body mass index, relative body weight, blood pressure and serum lipids the group of abdominal obesity in men and women.

Diagnostic laparoscopy in a leopard cat (Prionailurus bengalensis) with intercostal abdominal hernia and hepatic lipidosis

  • Intercostal abdominal hernia in the 11th intercostal space was identified in a leopard cat. Although mild leukopenia was found in laboratory examinations, no remarkable abnormality was revealed in medical imaging. To investigate abdominal organs, diagnostic laparoscopy was performed after hernia repair. In laparoscopic view, closure of the herniation site and a lesion with whitish discoloration in the liver (left medial lobe) were observed. Subsequently, laparoscopic liver biopsy was performed against the affected hepatic tissue. Histologically, the sample was diagnosed as mild hepatic lipidosis. Laparoscopy is considered useful for abdominal visceral examination and liver biopsy in a leopard cat patient.

  • One woman, Sixty three years old, suffered from chest pain, abdominal pain, nausea and vomiting for about three months. Although many examinations, brain magnetic resonance imaging(MRI), chest computed tomography(CT), abdominal CT, gastric endoscope, abdominal ultrasonography, and so forth, was performed from every point of view, those were non-significant. At first, I wrongly diagnosed her case as disease of Soumin and gave Soumin Gwakhyangjeongkisan. But she complained of diarrhea and chest discomfort. I re-diagnosed her case as Soyangin Gyeolhyung and treated with Hyeongbangdojeoksan, herbal medicine, consisted of Rhizoma Rehmanniae(生地黃), Lignum Akebiae(木通), Radix Scrophulariae(玄蔘), Semen Trichosanthis(瓜蔞仁), etc. Three weeks have passed from first visit and she did not suffered from pain. I experienced the excellent effect of Hyeongbangdojeoksan to Soyangin Gyeonlhyung.

Study on the Possibility of Quantitative Measurement of Abdominal Examinations in Korean Medicine - A Focus on Diagnosis of Abdominal Coldness in Functional Dyspepsia Patients - (한의 복진 정량화 연구 - 기능성 소화불량 환자의 복냉 진단을 중심으로 -)

  • Objective: This study was designed to investigate the possibility of quantification of the diagnosis of abdominal coldness (AC) in patients with functional dyspepsia (FD). Methods: Forty-four patients with FD were enrolled in this study. Three Korean medicine doctors each randomly examined all abdomens. Diagnosis of AC was made by consensus of at least two of the doctors. Body temperature (oral by digital oral thermometer) and skin temperature (by digital infrared thermal imaging [DITI]) were measured, followed by administration of the Cold and Heat questionnaire (CHQ) and the Instrument of Pattern Identification for Functional Dyspepsia (IPIFD). Results: Of the 44 patients with FD, 22 were assigned to the AC group and 22 to the non-AC group. The concordance rate of diagnosis among the three doctors was 63.6% (28/44), with a ${\kappa}$ of 0.504, indicating means moderate agreement). Neither the oral nor the skin temperatures showed statistically significant differences between the AC and non-AC groups. However, the CHQ scores and 'Simultaneous Occurrence of Cold and Heat Syndromes pattern' scores of the IPIFD were higher in AC group and showed statistically significant differences (p=0.010 and 0.009). Conclusions: This is the first study conducting quantitative measurements of abdominal coldness in patients with FD. Although oral and skin temperature showed no statistical significance between AC and non-AC groups, the concordance rate of diagnosis of AC among the three Korean Medicine doctors was moderate. The CHQ scores and 'Simultaneous Occurrence of Cold and Heat Syndromes pattern' scores of the IPIFD also suggest that diagnosis of AC is relevant to cold and heat patterns, and these questionnaires could be utilized as supportive data for the diagnosis of AC. Further studies should be conducted for the purpose of quantifying and standardizing abdominal examinations in Korean Medicine.

Eating control and eating behavior modification to reduce abdominal obesity: a 12-month randomized controlled trial

  • BACKGROUND/OBJECTIVES: Abdominal obesity is associated with metabolic disorders, and, in recent years, its prevalence in Korea has continuously increased. The change of lifestyle, particularly diet, is critical for the reduction of abdominal obesity. This study evaluated the effectiveness of an intervention focused on dietary self-efficacy and behaviors on the improvement of abdominal obesity. SUBJECTS/METHODS: Abdominally obese adults with additional cardiovascular risk factors were recruited through 16 medical facilities in South Korea from the year 2013 to 2014. The participants were randomly divided into 2 groups: an intensive intervention group (IG) that received a multi-component intervention to reduce abdominal obesity, by mainly focusing on dietary attitude and dietary behavior change, and a minimal information intervention group (MG) that received a brief explanation of health status and a simple recommendation for a lifestyle change. The interventions were provided for 6 mon, and health examinations were conducted at baseline, 3-, 6-, and 12-mon follow-ups. A path analysis was conducted to identify the process governing the changes in abdominal obesity. RESULTS: The IG showed an improvement in self-efficacy for eating control and diet quality at 6-mon follow-up. Abdominal obesity improved in both groups. Waist circumference was observed to be decreased through the path of "improved self-efficacy for eating control in food availability-eating restriction-improved dietary quality" in IG. Most changes in follow-ups were not significantly different between two groups. CONCLUSIONS: The intensive program targeting the modification of dietary behavior influenced management of abdominal obesity, and the effect occurred through a step-by-step process of change in attitude and behavior. Generally, improvements were also seen in the MG, which supports the necessity of regular health check-ups and brief consultation. The results can be used for further development and implementation of more successful interventions.

  • In diagnostic X-ray examinations, dose assessments for pregnant female and fetus are realistically difficult, and related research is also lacking. Therefore, in this study, the purpose of the simulation was to analyze the dose and fetal dose for pregnant female during abdominal X-ray examination. Based on the data presented in ICRP 89, this study produced phantom reconstructed of the existing prenatal phantom, which was used to analyze the evaluation of the organ dose and fetal dose of pregnant female according to pregnancy week and the difference between the dose of the existing phantom and the reconstructed phantom. As a result, the abdominal X-ray test showed a tendency to show higher doses for organs close to the direction of the source joining. In addition, it was confirmed that fetal doses in posteroanterior position were reduced by more than 65% compared with anteroposterior position.

  • Objectives : If patients notice a symptom indicating inveterate dyspepsia but they don't have any problem around gastroscope, they get diagnosed as a functional dyspepsia or an imaginary stomach disease, but to overcome the limitations of these diagnoses, we are analyzing them for the common feature and are looking for a new diagnostics for them. Methods : Based on our survey with 122 patients with inveterate dyspepsia, we analyzed the period of onset, eating habits, the main symptoms, and observations on the gastroscope. We also analyzed the function of the stomach and intestines by EAV examinations, and the outer walls of the stomach and intestines by subdividing the level of coagulation into six with abdominal palpation. Results : We figured out that people who appeals about inveterate dyspepsia have had long period of onset, and that they had bad eating habits, shoulder stiffness, neck stiffness, headache, dizziness, etc. These are all the similar symptoms beside dyspepsia, which indicates that it is a syndrome. From about 70%, they didn't had particular problem in gastroscope, and as to be seen from the result of EAV examination, their stomachs and intestines had become functionless. Also, we were able to feel a stiffened tissue through abdominal palpation. Conclusion : Through this investigation, we found out that what the gastroscope can not find so that gets diagnosed as a functional dyspepsia or an imaginary stomach disease can be diagnosed as a syndrome called damjeok by overcoming the limitation utilizing the survey, EAV examinations, and abdominal palpation. We can find a mighty significance from the fact that it can be diagnosed as a syndrome.

Analysis of Health Insurance Standards and Utilization of MRI in Korea: Based on Health Insurance Claim Data (한국의 MRI 건강보험 급여기준 및 진료이용에 관한 연구)

  • This study analyzed the current status of MRI (frequency, amount of treatment) based on the history of application of the MRI health insurance benefit standard and health insurance claim data. MRI examinations began as a health insurance benefit in 2005. In 2005, the indications were restricted for some diseases, but coverage for benefits in 2010, 2013, 2016, and 2018 was expanded. In 2021, the Ministry of Health and Welfare decided to apply health insurance for all MRI examinations. From 2010 to 2017, the number of MRI examinations increased by 86.7% in 2017 compared to 2010, and the amount of treatment increased by 53.5%. According to general characteristics, the number of MRI examinations was higher in women than in men. By age, the number of examinations was the highest among ages 70-79. Outpatient examinations were more frequent than inpatient examinations, and the number of examinations in the tertiary hospitals was the highest among the types of hospitals. The number of brain MRI examinations was the highest in each exam site. In December 2013, the standard of MRI was expanded for heart disease and Crohn's disease, the number of cardiac MRI and abdominal MRI examinations increased in 2014 compared to 2013. However, the number of examinations is small and not associate with the disease, it would be difficult to say that it affected the increase in the total number of MRI examinations. To assess health insurance sustainability and policy effectiveness, monitoring will be necessary.

  • The occurrence of incidentally detected pancreatic cystic lesions (PCLs) is continuously increasing. Radiologic examinations including computed tomography and magnetic resonance imaging with magnetic resonance cholangiopancreatography have been widely used as the main diagnostic and surveillance methods for patients with incidental PCLs. Although most incidentally detected PCLs are considered benign, they have the potential to become malignant. Currently, we have several guidelines for the management of incidental PCLs. However, there is still debate over proper management, in terms of accurate diagnosis, optimal follow-up interval, and imaging tools. Because imaging studies play a crucial role in the management of incidental PCLs, the 2017 consensus recommendations of the Korean Society of Abdominal Radiology for the diagnosis and surveillance of incidental PCLs approved 11 out of 16 recommendations. Although several challenges remain in terms of optimization and standardization, these consensus recommendations might serve as useful tools to provide a more standardized approach and to optimize care of patients with incidental PCLs.

Surgical Management and Long-Term Follow-Up of a Giant Hepatic Cyst with an Internal Septum in a Cat

  • A 1-year-old spayed female Scottish Fold cat presented with a 1.5-month history of vomiting, intermittent dyspnea, and abdominal distention. Radiographic, ultrasonographic, and computed tomographic examinations suggested a tentative diagnosis of a fluid-containing cystic mass with an internal septum. The mass was surgically removed. Histological examinations revealed that the mass was a non-neoplastic hepatic cyst. The patient had no recurrence of the cystic structure but died of acute renal failure 5 years after the surgery. This report describes the unusual case of a giant hepatic cyst with clinical signs that resolved after mass removal.
