  • Pericardial rupture due to blunt trauma is very rare, but can lead to serious complications. It occurs mainly on the left, is found incidentally during surgery, and is seldom discovered radiologically unless accompanied by cardiac herniation. The following case describes a 53-year-old traffic-accident victim who received emergency pericardial repair and bleeding control via an exploratory thoracotomy and an exploratory laparatomy. The patient was discharged without any complication and remained healthy at six month after injury.

  • In the case of a high-valuable asset, the Operation and Maintenance (O&M) phase requires heavy charges and more efforts than the installation (construction) phase, because it has long usage life and any accident of an asset during this period causes catastrophic damage to an industry. Recently, with the advent of emerging Information Communication Technologies (ICTs), we can get the visibility of asset status information during its usage period. It gives us new challenging issues for improving the efficiency of asset operations. One issue is to implement the Condition-Based Maintenance (CBM) approach that makes a diagnosis of the asset status based on wire or wireless monitored data, predicts the assets abnormality, and executes suitable maintenance actions such as repair and replacement before serious problems happen. In this study, we have addressed several aspects of CBM approach: definition, related international standards, procedure, and techniques with the introduction of some relevant case studies that we have carried out.

Accidental High Epidural Block -A case report- (우발성 고위경막외차단 경험 1예 -증례보고-)

  • Accidental high epidural block is a rare but serious complication. It can result from many factors, which include the volume and concentration of drug, posture, puncture site, age, pregnancy or intra-abdominal mass, and patients' height and weight. We had a case of accidental high epidural block recently. This is a case report which was confirmed by an epiduragram. A healthy 50-year-old woman with a huge uterine myoma was scheduled for a total abdominal hysterectomy under continous epidural analgesia. Epidural catheterization was carried out smoothly. However, an unexpected hypotension was noticed after an epidural injection of 2% lidocaine 25 ml. Thereafter, the patient was intubated and her respiration was controlled during the operation. Using the 5mg of ephedrine, her blood pressure and pulse were well maintained. The scheduled operation was carried out for one hour uneventfully, but after the operation, she felt paresthesia on her hands in the recovery room. To differentiate between the high epidural and the subdural blocks. We injected 5 ml of a water soluble Niopam 300 through the catheter postoperatively. It was observed on the epiduragram that the catheter was placed in the epidural space. It was suggested that the high epidural block was induced from the widespread diffusion through the narrowed epidural space due to the engorgement of the epidural venous plexus by the patient's huge uterine myoma.

Interesting Cases of Foreign Bodies in Air way -Report of Seven Cases- (최근 경험한 흥미있는 기도이물 7증례)

  • Foreign bodies in the airway are very import-tant and not uncommon conditions in the field of E.N.T., and practically there are many reports of them. Occasionally, it can be followed by some pulmonary complications and technical difficulty to remove. Therefore, foreign body in the trachea-bronchial tree is serious and, on occasion, fatal. Recently, the authors experienced 7 interesting cases of foreign bodies in the airway and a brief review of literature was made. Case 1. (1 year old male): A bit of walnut was coughed out spontaneously. Case 2. (2 years old male): A bit of raw chestnut was removed from the right main bronchus. Case 3. (21 Years old male): A pushpin was removed from the left main bronchus. Case 4. (1 year old male): A kind of bean was removed from the right main vronchus. Case 5. (4 years old male): A coiled steel spring (1.1cm$\times$2cm) was removed from the subglottic region. Case 6. (5 years old female): A shell of pine nut which had been impacted in the wall of right main bronchus was removed during right middle and lower lobectomy. Case 7. (2 years old male): A metal nail was removed from the rightposterior basal segmental bronchus during right lower lobectomy.

Anaphylaxis Induced by Surgical Prophylactic Cefotetan and The Application of World Allergy Organization Guide: A Case Report (사례보고: 수술예방적 항생제 Cefotetan에 의한 아나필락시스 보고 및 World Allergy Organization 가이드라인활용)

  • The definition of anaphylaxis is 'a serious, life-threatening generalized or systemic hypersensitivity reaction' and is considered as the life threatening adverse drug reaction. We experienced a case of cefotetan induced anaphylaxis with negative pre-skin test, used for surgical prophylaxis. A 82-year-old female was scheduled for total knee replacement therapy. She had no previous history of allergy and her skin test results were also negative. On her right knee surgery, she underwent cefotetan therapy as a surgical prophylaxis for a week with no problems identified. Next left knee surgery, she also received the prophylaxis of intravenous cefotetan. However, a few minutes later, anaphylactic reaction developed with vomiting, severe hypotension, bronchospasm, and dyspnea. After immediate intensive care treatment, she recovered without significant complications. Though commonly used laboratory data in case reports, such as the specific IgE, tryptase, histamine, or allergic skin prick test were limited, we successfully confirmed anaphylaxis based on clinical criteria for diagnosing anaphylaxis based on WAO 2011 guideline with through concurrent patient°Øs medical history review and the process of identifying the causes.

Case Report of Korean Medicine Treatment for a Patient with an Aneurysmal Subarachnoid Hemorrhage (뇌동맥류 파열로 인한 지주막하 출혈 환자에서 중재적 치료 후 조절되지 않은 두통 치험 1례)

  • Subarachnoid hemorrhage (SAH) is a disease that causes bleeding in the subarachnoid space; 70%-80% of nontraumatic subarachnoid hemorrhages are caused by saccular aneurysms. If the patient has already experienced a ruptured aneurysm that causes subarachnoid bleeding, rebleeding can result in a high mortality rate and serious sequelae. Therefore, if the patient can undergo surgical or interventional treatment, it should always be performed. This patient was diagnosed with acute aneurysmal subarachnoid hemorrhage and hydrocephalus. The patient was hospitalized for uncontrolled headache and vertigo after aneurysm coil embolization and ventriculoperitoneal shunting. The patient was treated with Yangkyuksanwha-tang and acupuncture and was observed with a symptom checklist for 25 days. Headache improved, from a visual analog scale (VAS) score of 7 to 0. Vertigo also improved, from a numeric rating scale (NRS) of 6 to 2, and the vertigo pattern changed from rotational to nonrotational. This case suggests that Korean medicine treatment is helpful in managing subarachnoid hemorrhage sequelae.

A Case Study of a Soyangin Patient with Palpitation and Epistaxis that Occurred after Taking Panax Ginseng (인삼 복용 후 발생한 비출혈 및 심계항진을 주소로 한 소양인 환자 1례)

  • Objectives The purpose of this case study was to report improvement of palpitation, epistaxis, nausea, and anorexia after treatment with Dojeokgangki-tang gamibang in a middle-aged woman who has been taking Panax Ginseng continually. Methods Upon beginning her Dojeokgangki-tang gamibang regimen the patient was asked to self-evaluate the degree of discomfort in her symptoms using a using a number scale from 0 (no symptoms) to 10 (most severe). Evaluation of palpitation and chest discomfort was made a high priority. Results The palpitation the patient experienced, and self-evaluated as an "8" on the number scale at the time of her admission to the hospital, had disappeared (become "0") by the fourth day of hospitalization. The numerous symptoms which accompanied her palpitations (chest discomfort, nausea, anorexia, general weakness, weight loss, and depression) also improved accordingly. Conclusions Health functional food can pose serious health risks such as sudden bleeding and palpitations in individuals with pre-existing health problems. One must exercise extreme caution in deciding to take such supplements. As seen in the case of the middle-aged woman, the quality of life of patients who had experienced adverse reactions to such supplements can be treated through the administration of Dojeokgangki-tang gamibang.

Two Cases of Colonoscopic Retrieval of a Foreign Body in Children: A Button Battery and an Open Safety Pin

  • Ingestion of foreign body in children is a relatively common problem among paediatric population. The foreign bodies mostly pass spontaneously through the gastrointestinal tract. However, complications can occur according to its anatomical location, the characteristics of the foreign body, and delays in management. Although the cases of ingested button batteries or sharp objects impacted at the gastrointestinal tract can be very serious, there have been very only a few cases have reported colonoscopic removal of these dangerous foreign bodies in adults, and there have been no case reports in children. We report one case of a button battery and one case of an open safety pin, both impacted in the terminal ileum that had moved from the stomach within a few hours of ingestion and were eventually managed by colonoscopy without any complications.

  • Social media marketing has gained attention from marketers due to the growing number of social media users. Marketers around the globe have a serious concern over how to utilize social media as a successful marketing tool. Many of them show a lack of understanding of the essential feature of social media and effective social media marketing strategies, which brings about a higher chance of social media marketing failures. Analyzing the failure cases of social media marketing is vital because it provides rich insights for marketing experts who have difficulty in developing effective social media marketing strategies. Therefore, this study conducted multiple case studies by selecting five failure cases of social media marketing which we defined as paradigmatic social media marketing failures which happened in the last five years in South Korea. From the case studies, we derived five successful social media marketing strategies. This study has a theoretical implication because it is the first to suggest effective social media marketing strategies based on the analysis on social media marketing failure cases. It also has a practical implication since it proposes specific social media marketing strategies which can help facilitate successful social media marketing.

A Study of Mycoplasmal Pneumonia developed around a Chunnam Coastal Area in 1993 (1993년 전남의 한 해안지역에서 발생한 Mycoplasmal 폐렴에 대한 고찰)

  • Mycoplasma pneumoniae(M. pneumoniae) is a primary pathogene of the respiratory tract in children. Most studies of such pneumonia involve a group of admitted patients in hospital, usually with major medical illness. We retrospectively studied the epidemiologic and clinical feature of 105 patients with serologically proven M. pneumoigic pneumonia treated at Gwang-Yang Hospital during a epidemic period of Jun. 1993 to Dec. 1993. All cases of pneumonia developed in this period were also reviewed and compared with serologically proven group. The results were as follows. 1. There were 63 males and 42 females, and the male /female ratio was 1.5 : 1. 2. More than half(57%) of cases belonged to 5-9 years of age group, and mean age was 6.5 years old. 3. Mean age was steadily decreased as prevalence of mycoplasmal pneumonia had been subsided. 4. The date of index case was June 26th, and that of median case was 3 months after the index case. 5. A major determinant of the outbreak seemed to be the population density rather than the population size. 6. Recurrence and serious complication were not observed in this period. 7. All cases of pneumonia developed in this period might be attributed to M. pnemoniae. These epidemiologic and clinical characteristics would contribute to the prevention and diagnosis of mycoplasmal pneumonia.
