국내 간호학 분야 메타분석 논문의 질 평가 (A Quality Assessment of Meta-Analyses of Nursing in South Korea)

  • Purpose: The purpose of the study was to assess the quality of meta-analyses on nursing published in South Korea. Methods: Relevant meta-analyses were identified through searches of the National Assembly Library, KISS (Korean Studies Information Service System), and the DBpia and RISS4U databases from 1990 to May 2013. Quality assessments were conducted using AMSTAR, a validated tool for assessing the quality of systematic reviews. Results: Forty-two meta-analyses were included in this study. Twenty-nine published between 1990 and 2010, and 13, between 2011 and May 2013. Two high quality studies and 11 moderate quality studies were published in the latter period. The mean score for the reviews was 5.61 (range 3-10); 11 studies were rated as low quality, 29 as moderate quality, and two as high quality. Conclusion: Although an improvement in the quality of meta-analyses conducted by nursing researchers in South Korea was observed across the study period, the study results indicate a need to use of more rigorous research methods when conducting systematic reviews or meta-analyses.

딕카이트의 열적 특성 연구 (Thermal Behavior of Dickite)

  • Thermal behavior of dickite was studied by thermal analysis, X-ray diffraction analysis, electron microprobe analysis, and scanning electron microscopy, Dickite has an endothermic peak at about$ 650^{\circ}C$ and an exothermic one at $960^{\circ}C$ in the differential thermal analysis. The endothermic reaction is assigned to the decomposition of dickite to meta-dickite. Hydroxyl radicals are removed from dickite structure by the reaction, resulting in the weight loss about 10.5~14.5% and appearance of a 14$\AA$ phase different from other kaolin minerals. The reaction slowly proceed in the range of $200^{\circ}C$. As the completion of decomposition, aciclular mullite forms at the expense of meta-dickite plates with random crystallographic relationship. Mullites have diverse silica versus alumina ratio. The exothermic reaction without weight loss seems to be due to the formation of spinel and amorphous silica. The spinel phase shows cryptocrystalline globular morphology accompanying a little amount of silica. From this work, it is suggested that mullite is formed from meta-dickite much lower temperature than the reported one in the previous works.

Non-Identical Parallel Machine Scheduling with Sequence and Machine Dependent Setup Times Using Meta-Heuristic Algorithms

  • This paper considers a non-identical parallel machine scheduling problem with sequence and machine dependent setup times. The objective of this problem is to determine the allocation of jobs and the scheduling of each machine to minimize makespan. A mathematical model for optimal solution is derived. An in-depth analysis of the model shows that it is very complicated and difficult to obtain optimal solutions as the problem size becomes large. Therefore, two meta-heuristics, genetic algorithm (GA) and a new population-based evolutionary meta-heuristic called self-evolution algorithm (SEA), are proposed. The performances of the meta-heuristic algorithms are evaluated through compare with optimal solutions using randomly generated several examples.

데이터통합과 메타분석을 위한 그래프 통계량 추출과 검증 (Statistical Data Extraction and Validation from Graph for Data Integration and Meta-analysis)

  • 본 연구의 목적은 개별연구들이 정확한 통계량을 제시하지 않고 그래프로만 나타내었을 경우 그래프로부터 통계량을 추출해내는 구체적인 방법을 기술한 것으로서 데이터통합과 정량적합성을 위한 메타분석을 가능하게 한다. 특히 메타분석(meta-analysis)은 체계적·객관적으로 대상문헌을 선택한 후 개별 연구들의 결과를 계량화하여 이를 통합된 효과크기(effect size)로 제시함으로써 근거중심의학(evidence based medicine)를 위한 올바른 의사결정을 할 수 있게 하는 중요한 분석도구이다. 데이터통합과 메타분석을 위해서 그래프로부터 정확한 통계수치를 추출하는 전산도구인 Adobe Acrobat Reader 와 Python기반의 JupyterLab 소프트웨어의 소개와 적용에 대한 주요사항을 알아보았다. 사용된 예제자료는 선행연구를 통해서 통계결과가 검증되어졌고 ClinicalTrials.gov에서 원자료 확보가 가능한 것을 사용하였다. 원자료와 각 전산도구들의 측정값을 각각 메타분석한 결과 통계적으로 유의한 차이는 없었다. 또한 연구자들간의 측정 신뢰도를 확인하였으며 높은 일치도를 나타내었다. 만약 그래프로부터 통게수치를 추출해야만 할 경우 연구의 완결성 유지를 위해서 전통적 사용 방법들보다는 전산 도구를 이용한 측정이 권고된다.

간호사간 직장 내 괴롭힘 관련변인에 대한 체계적 문헌고찰과 메타분석 (The Related Factors to Workplace Bullying in Nursing: A Systematic Review and Meta-analysis)

  • Purpose: The purpose of this study was to review and identify factors relevant to workplace bullying in nursing. Methods: Twenty-three studies that met the criteria were selected from a sample of twenty-six studies. These articles were retrieved from a central literature databases (N=13,241). The total correlational effect size (ESr) for each related factor was calculated from Fisher's Zr. A funnel plot inspection (similar to scatter plot) with a trim-and-fill method was used to assess the publication bias of the meta-analyzed studies. Results: From the systematic review, fifty-one factors were identified as having an influencing effect. Fourteen factors (five individual and nine organizational factors) were eligible for meta-analysis. The individual factors included, self-esteem (ESr=-.31), psychological capital (ESr=-.26), and marital status (ESr=-.06) which were significantly correlated with workplace bullying. Organizational factors included, organizational tolerance (ESr=.48), supervisor incivility (ESr=.47), job stress (ESr=.46), group morale (ESr=-.36), group support (ESr=-.35), supervisor leadership (ESr=-.35), group identity (ESr=-.33), and structural empowerment (ESr=-.27). These factors were significantly correlated with workplace bullying. There were no publication biases except for both individual and organizational factors. Conclusion: Organizational factors have more of an greater impact than individual factors on workplace bullying. The results of this study support the need for intervention at the organizational level.

입원 환자에서 STRATIFY의 예측 타당도 메타분석 (Predictive Validity of the STRATIFY for Fall Screening Assessment in Acute Hospital Setting: A meta-analysis)

  • Purpose: This study is to determine the predictive validity of the St. Thomas Risk Assessment Tool in Falling Elderly Inpatients (STRATIFY) for inpatients' fall risk. Methods: A literature search was performed to identify all studies published between 1946 and 2014 from periodicals indexed in Ovid Medline, Embase, CINAHL, KoreaMed, NDSL and other databases, using the following key words; 'fall', 'fall risk assessment', 'fall screening', 'mobility scale', and 'risk assessment tool'. The QUADAS-II was applied to assess the internal validity of the diagnostic studies. Fourteen studies were analyzed using meta-analysis with MetaDisc 1.4. Results: The predictive validity of STRATIFY was as follows; pooled sensitivity .75 (95% CI: 0.72~0.78), pooled specificity .69 (95% CI: 0.69~0.70) respectively. In addition, the pooled sensitivity in the study that targets only the over 65 years of age was .89 (95% CI: 0.85~0.93). Conclusion: The STRATIFY's predictive validity for fall risk is at a moderate level. Although there is a limit to interpret the results for heterogeneity between the literature, STRATIFY is an appropriate tool to apply to hospitalized patients of the elderly at a potential risk of accidental fall in a hospital.

Complications and Carcinogenic Effects of Mustard Gas - a Systematic Review and Meta-Analysis in Iran

  • Background: Catastrophic effects of mustard gas as a chemical warfare agent have always been a major problem for those exposed to this agent. In this meta-analysis it was tried to evaluate carcinogenesis, ocular, cutaneous and respiratory complications of mustard gas exposure among Iranians who had been exposed to this agent during the Iran-Iraq war. Materials and Methods: In this meta-analysis, the required data were collected using keywords "mustard gas", "sulfur mustard", "cancer", "neoplasm", "respiratory complications", "ocular complications", "lung disease", "chronic complication", "eye", "skin", "cutaneous complication", "carcinogenesis" and their combination with keywords "Iran", "Iranian", "prevalence", "mortality" and their Farsi equivalent terms from the databases of SID, Iranmedex, Magiran, Pubmed, Science Direct, Google Search engine, Gray Literature and Reference of References. To determine the prevalence of each complication and perform meta-analysis, CMA: 2 (Comprehensive Meta-Analysis) software with a randomized model was used. Results: Of the 542 articles found, 7 national articles, consistent with the aims of this study were selected. Meta-analysis of seven papers revealed that cancer risk, especially cancer of the respiratory system was elevated, so that the relative risk (RR) of cancer role of mustard gas was inconsistent from 2/1 to 4 in this survey. Also prevalence of delayed skin disorders due to sulfur mustard was 94.6%, pulmonary complications 94.5% and ocular complications 89.9%. The incidence of various cancers in victims exposed to mustard gas was 1.7% worldwide where the rate was 2.2% in Iranian victims of the Iraq-Iran war. Conclusions: Based on present study the prevalence of delayed mustard gas related cutaneous, pulmonary and ocular complications is above 90% and risk of carcinogenesis is higher in comparison to worldwide statistics. This may suggest need for long-term and persistent follow-up and rehabilitation procedures for populations exposed to this agent.

Association between the XRCC3 Thr241Met Polymorphism and Gastrointestinal Cancer Risk: A Meta-Analysis

  • Background: The x-ray repair cross-complementing group 3 (XRCC3) encodes a protein involved in the homologous recombination repair (HRR) pathway for double-strand DNA repair. Associations of the XRCC3 Thr241Met polymorphism with various cancers have been widely reported. However, published data on links between XRCC3 Thr241Met and gastrointestinal (GI) cancer risk are inconsistent. Objective and Methods: A meta-analysis was conducted to characterize the relationship between XRCC3 Thr241Met polymorphisms and GI cancer risk. Pooled odds ratios (ORs) and 95.0% confidence intervals were assessed using random- or fixed- effect models for 28.0 relevant articles with 30.0 studies containing 7,649.0 cases and 11,123.0 controls. Results: The results of the overall meta-analysis suggested a borderline association between the XRCC3 Thr241Met polymorphism and GI cancer susceptibility (T vs. C: OR=1.18, 9 % CI=1.0-1.4, POR=0.04; TT vs. CT+CC: OR=1.3, 95 % CI=1.0-1.6, POR=0.04). After removing studies not conforming to Hardy-Weinberg equilibrium (HWE), however, this association disappeared (T vs. C: OR=1.00, 95 % CI=0.9-1.1, POR=0.96; TT vs. CT+CC: OR=0.9, 95 % CI=0.8-1.1, POR=0.72). When stratified by ethnicity, source of controls or cancer type, although some associations between XRCC3 Thr241Met polymorphism and GI cancer susceptibility were detected, these associations no longer existed after removing studies not conforming to HWE. Conclusion: Our meta-analysis suggests that the XRCC3 Thr241Met polymorphism is not associated with risk of GI cancer based on current evidence.

3R Variant of Thymidylate Synthase 5'-untranslated Enhanced Region Contributes to Colorectal Cancer Risk: A Meta-analysis

  • Background: Studies investigating the association of 2R/3R polymorphism in the thymidylate synthase 5'-untranslated enhanced region (TSER) and colorectal cancer (CRC) risk have reported conflicting results. Thus, a meta-analysis was performed to summarize the data on the potential association. Methods: Pubmed, Embase and CBM databases were searched for all available studies. Links between the TSER 2R/3R polymorphism and CRC risk were estimated by odds ratios (ORs) with 95% confidence intervals (CIs). Results: Seven case-control studies with a total of 2723 cases and 4030 controls were included in this meta-analysis. The results showed that the 3R variant of TSER 2R/3R polymorphism contributes to CRC risk in two comparison models (OR 3R vs. 2R =1.10, 95%CI 1.02-1.18, P = 0.015; OR Homozygote comparison model = 1.22 1.04-1.43, 95%CI 1.04-1.43, P = 0.012). Subgroup analyses by ethnicity further demonstrated a contribution in Caucasians with three comparison models (OR 3R vs. 2R = 1.10, 95%CI 1.02-1.19, P = 0.015; OR Homozygote comparison model = 1.21, 95%CI 1.03-1.41, P = 0.019; OR Recessive comparison model = 1.18, 95%CI 1.05-1.33, P = 0.008). However, the association in the Asian population was still uncertain due to the limited data (all P values were more than 0.05). Conclusions: Our meta-analysis suggests that the 3R variant of Thymidylate synthase 5'-untranslated enhanced region 2R/3R polymorphism contributes to gastric cancer risk in the Caucasian population, while any association in Asian populations needs further study.

The Correlation between E-Selectin S128R Gene Polymorphism and Ischemic Stroke in Chinese Population : A Meta-Analysis

  • To perform a systematic review of the data collected from case-control studies conducted earlier to investigate the correlation between E-selectin S128R polymorphism and ischemic stroke (IS) risk among the Chinese population. The PubMed, Web of Science, Chinese biomedical literature database (CBM), Chinese databases China National Knowledge Infrastructure (CNKI), WanfangData knowledge service platform (Wanfang Data), and information resource integration service platform (VIP) Databases were searched to retrieve case-control studies on the correlation between E-selectin gene S128R polymorphism and IS from the inception of the database till June 2019. The literature was screened, data were extracted, the risk of bias was reviewed, and the studies included were assessed independently by two reviewers. Stata ver. 12.0 software (Stata Corp LLC, College Station, TX, USA) was used to perform the meta-analysis. A total of 2907 cases from eight case-control studies involving 1478 IS patients and 1429 controls were included in this study. The R allele and RS genotype in E-selectin were found to be associated with the risk of IS as per the results of the meta-analysis (R vs. S : odds ratio [OR], 2.75; 95% confidence interval [CI], 2.15-3.51; p<0.00001; RS vs. SS : OR, 2.50; 95% CI, 1.95-3.19; p<0.00001; RR+RS vs. SS : OR, 2.85, 95% CI, 2.21-3.67; p<0.00001). The E-selectin gene S128R polymorphism is likely related to IS based on the results of a meta-analysis in the Chinese population, and the R allele and RS genotype of E-selectin may be IS risk factors.
