Comparative Analysis of Salivary Cortisol in Young Adult Patients with Temporomandibular Disorders

  • Purpose: The goal of this study was to investigate the causative factors related to the stress of temporomandibular disorders (TMDs) by evaluating salivary cortisol concentration in young adult TMD patients and control groups. Methods: Saliva was obtained from 32 young adult TMDs patients and 34 control patients without a history of TMDs who visited Chosun University Dental Hospital between June 1 and August 31, 2021. Enzyme-linked immunosorbent assay was conducted to measure the salivary cortisol concentration. Results: The salivary cortisol concentration in the TMD patient group and the control group differed significant significantly (p<0.05). The salivary cortisol concentration according to the duration of the clinical symptom of TMD differed significantly difference between the two groups in the male. The salivary cortisol concentration according to perceived stress level differed significantly in the mild and moderate groups (p<0.05). There was no significant difference in salivary cortisol concentration between the two groups related to bruxism or clenching (p>0.05). Conclusions: The salivary cortisol concentration in the TMD patient group and the control group showed statistical relevance, indicating that stress was a causative factor.

Aerodynamic Characteristics of Young and Elderly Adult Patients with Voice Disorders during Continuous Speech (젊은 성인 및 노인 음성장애 환자의 연속발화시 공기역학적 특성 비교)

  • This study was performed to compare the aerodynamic characteristics of young and elderly adult male patients with voice disorders during continuous speech. Aerodynamic measurements were obtained after 12 young male patients and 9 elderly male patients read a paragraph. The elderly group showed longer duration, lower airflow rate and air volume than the younger group, but the differences were not significant except phonation time. So, when interpreting the meaning of aerodynamic measures of elderly voice disorder patients in the aspects of airflow and air volume, it should take into account various conditions(e. g. reading materials, pulmonary functions) as well as age.

  • Objectives: The goal of this study was to estimate the overall satisfaction level of dental implant patients and further evaluate factors influencing satisfaction. Materials and Methods: Self-administered questionnaires were mailed to patients who received dental implant therapy at Seoul National University Bundang Hospital (Seongnam, Korea) from October 2003 to April 2005. The main portion of the questionnaire was shared to evaluate the level of satisfaction with implant therapy. The questionnaires contained evaluations of influencing factors, which were classified as pain-related, service-related, and complication-related. Results: The responses from 93 patients (41 males, 52 females) with a total of 325 implants were included in the analysis, and the mean score for overall satisfaction level with implant therapy was 8.26. Female patients showed higher visual analogue scale (VAS) scores for both pain during (P=0.000) and after implant surgery (P=0.016). Male patients showed more 'negative' values for the reasonability of treatment cost (P=0.008) and the adequacy of the treatment period (P=0.022). Conclusion: The subjective satisfaction of patients was influenced by various factors, especially complication-related factors.

  • Patients with lumbar disc herniation are faced with lowered quality of life. For this reason, we investigated the effects of Oriental Intensive Therapy (OIT) including acupuncture, pharmacopuncture, manual therapy and herbal medicine on patients with lumbar disc herniation. Ten patients, diagnosed with lumbar disc herniation by MRI in hospital, were treated with OIT for 12 weeks. Symptoms of the patients were assessed using Verbal Numerical Rating Scale (VNRS) and Oswestry Disability Index (ODI). All patients were investigated every a week. In our results, intensity of pain was reduced after 4 weeks treatment significantly by VNRS, Quality of life has also improved after 4 weeks treatment by ODI. In conclusion, this study suggests that OIT may be effective to treat patients with lumbar disc herniation.

Purpose : 30 patients with uterine cervical cancer who had been treated at Department of radiation Oncology of Korea university Hospital from June, 1, 1994. to December. 31, 1994. were analyzed to the radiation field accuracy using verification films. 2. Methods and Materials : 30 patients were divided by 6 groups, 5 patients respectively : Group (1) : Standard physique, supine not-fixed Group (2) : Standard physique, supine fixed Group (3) : Obesity physique, supine, not-fixed Group (4) : Obesity physique, supine, not-fixed Group (5) : Obesity physique, prone, not-fixed Group (6) : Obesity physique, prone, fixed All groups were taken verification fimls during the whole treatment period, and it compared with simulation films and portal fimls, 3. Results : This results suggest that all patients should be immovilized and especially obesity physique patients should be treated by prone position in order to reduce radiation field discrepancy. In addition, this results should help to prescribe appropriate safty margins for patients simulation.

Caregiver Burden and Health-related Quality of Life Among Stroke Caregivers (뇌졸중 환자가족의 가족부담감과 건강관련 삶의 질)

  • Purpose: This study was performed to examine the relationship between caregiver burden and health-related quality of life (HRQOL) in family caregivers of stroke patients in the community. Methods: Subjects were 33 stroke patients who had a experience of hospitalization during last 1 year and their family. Data were obtained from personal interview by nursing students in October, 2003. We assessed the functional status of stroke patients, caregiver burden and the HRQOL of caregivers using SF-8. Results: A higher subjective burden was related to dependent functional status of patients than independent(P<.01). The independent cognitive function of stroke patients and good health of caregivers were related to high HRQOL of caregivers. PCS and MCS of HRQOL were negatively correlated to subjective burden, -.34(p<.01) and -.37(p<.05). Conclusion: We founded functional status of stroke patients. caregiver burden and HRQOL of caregivers were related to each other. To improve the HRQOL of caregiver, decreasing burden of caregivers and the functional status improvement of stroke patients must be considered.

  • Purpose: This study aimed to understand the essence of experiences of patients and family members during flexible visiting in an intensive care unit (ICU). Methods: This is a qualitative study using interviews with open ended questions. We used Colaizzi's method of phenomenological interpretation. Results: Flexible visiting in the ICU impacted the patients and their families in various ways. The following categories were extracted from the patients' experiences with flexible visiting: 1) the opportunity to feel the presence of the family and 2) the burden of unrestricted visiting. The following categories were extracted from the families' experiences with flexible visiting: 1) psychological comfort by convenience 2) being aware of health care professionals and critical care nursing in the intensive care unit, and 3) double trouble. Conclusions: These results showed that flexible visiting in the ICU affected the patients and their families positively and negatively. Therefore, nursing staff need to design psychological and social interventions that address the needs of patients and their families.

  • Objective : Microvascular decompression[MVD] at root exit zone[REZ] of the facial nerve has been largely popularized and it has become the standard treatment for patients with hemifacial spasm[HFS]. This sturdy is performed to evaluate the efficacy of MVD over the course of time. Methods : From 1994 to 2003, 50 patients with HFS who underwent MVD were followed up for more than 6 months. We retrospectively analyzed results with medical records and telephone researches. Results : The mean age of patients at the time operation was 57.6 years and 84% of the patients were female. The mean duration of follow-up after operation was 3.4 years [range $0.5{\sim}7.8\;years$]. One day after MVD, 54% of patients had complete relief of spasm immediately. Continuous improvements of HFS were observed during the follow-up period and these improvements were statistically significant with time (P< 0.05). Until 6 months after operation, complete relief of spasm was observed in 84% of patients. The delayed relief of spasm was observed in 35.7% of our patients who experienced complete relief. Conclusion : The efficacy of MVD in HFS is improves with time. Continuous follow-up evaluations for the duration of more than at least 6 months after MVD are important for the decision of its clinical results because delayed relief of spasms occurs.

Clinical Characteristics of Acute Appendicitis in Children and Elderly Patients (소아와 고령자에서 급성 충수염의 임상적 특성)

  • Acute appendicitis is one of the most common diseases of young people that requires emergency operation. This is especially true for those age 10 years old and older. However, the numbers of cases of appendicitis are increasing in both the young and the elderly. The main purpose of this study is to understand the clinical features of acute appendicitis in children and elderly patients. We retrospectively compared clinical characteristics in 4 groups of patients who underwent appendectomy in our hospital. There were 16 patients in the pre-school age group, 9 were males and 7 females. The total number of patients in the juvenile group were 287, 156 were males and 131 females. The total numbers of patients in the adult group were 794, 436 were males and 358 females. The numbers of elderly patients were 189, 91 were males and 98 females. Complicated appendicitis was found in 15 patients (93.7 %) in the pre-school age group, 79 patients (27.5 %) in the juvenile group, 332 patients (41.8 %) in the adult group, and 96 patients (50.7 %) in the elderly group. Four patients (40 %) had generalized panperitonitis in the pre-school group. The occurrence of perforated appendicitis was the highest in the pre-school age group and the lowest in the juvenile group. Since generalized panperitonitis has a higher incidence in the pre-school age group, prudent and careful diagnosis and treatment are required for the pre-school age group.

Microsatellite Instability Is Associated with the Clinicopathologic Features of Gastric Cancer in Sporadic Gastric Cancer Patients

  • Purpose: Replication error is an important mechanism in carcinogenesis. The microsatellite instability (MSI-H) of colorectal cancers is associated with the development of multiple cancers. The influence of MSI-H on the development of multiple gastric cancers in sporadic gastric cancer patients has not been defined. This study was performed to reveal the association between the clinicopathologic features and MSI in sporadic gastric cancers. Materials and Methods: Between July 2004 and March 2009, the clinicopathologic characteristics, including MSI status, were evaluated in 128 consecutive patients with sporadic gastric cancers. None of the patients had hereditary non-polyposis colorectal cancer of familial gastric cancer. The markers that were recommended by the NCI to determine the MSI status for colorectal cancers were used Results: MSI-H cancers were found in 10.9% of the patients (14/128). Synchronous gastric cancers were shown in 4 patients (3.1%). Synchronous cancers were found in 2 of 14 patients with MSI-H gastric cancer (14.3%) and 2 of 114 patients with MSS gastric cancer (1.8%; P=0.059, Fisher's exact test). Among the patients with synchronous cancer 50% (2/4) had MSI-H cancer, but 9.7% of the patients (12/124) without synchronous cancer had MSI-H cancer. MSI-H (RR, 24.7; 95% CI, 1.5~398.9; P=0.024) was related with to synchronous gastric cancer, but age, gender, family history, histologic type, location, gross morphology, size, and stage were not related to synchronous gastric cancer. Conclusions: MSI is associated with the intestinal-type gastric cancer and the presence of multiple gastric cancers in patients with sporadic gastric cancer. Special attention to the presence of synchronous and the development of metachronous multiple cancer in patients with MSI-H gastric cancer is needed.
