Purpose: This study was conducted to develop & to determine the effect of an tailored falls prevention exercise for older adults. Method: Subjects consisted of 59 older adults (experimental group : 29, control group : 30) living at nursing homes. Experimental group participated in tailored falls prevention exercise for 16 weeks(3 times a week, 50 min every session). Data were collected before the exercise, 16 weeks & 24 weeks after the beginning of exercise. Results: 1) the experimental group significantly improved the muscle strength of hip extensor & flexor, knee extensor & flexor, ankle dorsiflexor, & plantar flexor compared to the control group. 2) the experimental group significantly decreased the mean time for 10 times chair stand. 3) the experimental group significantly increased the standing time on one leg and the number of heel raise for 30 seconds compared to the control group. In addition more older adults in the experimental group completed the tandem stance & semi-tandem stance for 10 seconds than the control group. 4) The experimental group significantly decreased the mean time of 6m walk and the fall frequency compared to the control group. Conclusions: This results suggest that tailored falls prevention exercise for older adults can improve muscle strength, static & dynamic balance and decrease the fall frequency of older adults.
Purpose: The purpose of this study was to investigate the effects of group art therapy on old adults' powerlessness, loneliness. depression and self-image (for 55-year old or older people). Method: An experimental group of 10 persons were given group art therapy for 1 hour per week for 8 weeks. Results: The result of this study showed that group art therapy is effective in decreasing older adults' powerlessness (p<0.05) and their loneliness and depression (p<0.05). However, there was no significant difference in the score of loneliness (p>0.05). A tree test was executed to analyze self-image change. and the result of the tree test showed a significant change in the tree shape. which mean a significant positive change in self-image. Conclusions: The scores of older adults' powerlessness, loneliness and depression were lowered by the group art therapy. Therefore it is thought that group art therapy program was an effective intervention for improving older adults' quality of life. Thus, this study suggests to apply group art therapy as a supportive nursing intervention in order to manage powerlessness, loneliness, depression and self-image in old low-income adults.
The authors, to collect some necessary data to promote correcting the toothbrushing method, the most basic home dental care, and to arrest the attention to the dentifrices in Korea, observed and evaluated the cervical abrasion experience rate in 8,000 Korean male and female adults from 19 to 52 years in the age, which had been extracted by the stratified random sampling method. The obtained results were as follows;
1. Cervical abrasion experience rate was 18.6% in Korean adults.
2. Cervical abrasion experience rate in male adults was higher than that in female adults.
3. Cervical abrasion experience rate was gradually increased by ageing.
4. It seemed the cervical abrasion experience rates in the adults in Honam and Youngnam district to be higher than those in the other districts.
5. In Korean adults, the further detail inveatigations into the cervical abrasion were required.
Purpose: This descriptive survey-based study aimed to identify the factors associated with obese adults' quality of life (QOL) using a secondary analysis of data collected through the 8th Korea National Health and Nutrition Examination Survey (2019). Methods: In this study, factors affecting the health-related quality of life of obese adults were analyzed by categorizing them into three independent variables: general characteristics, health behaviors, and mental health characteristics. Using t-test and analysis of variance (ANOVA), an analysis was performed to determine the differences in obese adults' QOL depending on their general characteristics, health behaviors, and mental health conditions. A hierarchical regression analysis was performed to identify the factors that affect obese adults' QOL. Results: In obese adults' general characteristics, the risk factors for QOL were older age, male, lower-income level, presence of spouse, and occupation. With regard to health behaviors, the risk factors were lack of exercise. In terms of mental health conditions, the risk factors were stress and suicidal ideation. Conclusion: Obese adults' QOL was affected by age, sex, income level, absence/presence of a spouse, occupation, health behaviors (e.g., exercise), and mental health conditions (e.g., stress and suicidal ideation).
Purpose: This study aims to understand the current use of and demand for assistive eating devices. These device improve the self-feeding abilities in older adults. Methods: This study used a cross-sectional study design. Descriptive statistics were performed using SPSS 22.0 for Windows. The survey participant of 282 older people(47.7%), 42 family caregivers(7.4%), and 266 staff members(44.9%). Results: Whereas most of the older adults living in their home(87.3%) did not experience difficulty while eating, older adults residing in facilities(26.4%) expressed difficulty with the process of putting food into a spoon. The difference between the two groups was statistically significant(p<.001). Wile the older adults living in home considered assistive eating devices(34.5%) as the most important requirement for self-feeding, older adults in facilities considered receiving help(39.3%) as their biggest requirement(p<.009). Older adults, family caregivers, and nursing home staff were not familiar with assistive eating devices and were not aware of their necessity. Conclusion: The study could help increase awareness about assistive eating devices and the importance of self-feeding in research and nursing educational settings. Improvement in self-feeding ability may future enhance the daily quality of life of older adults.
Objective : The adolescent presentation of tethered cord syndrome (TCS) is well-recognized, but continues to pose significant diagnostic and management controversies. The authors conducted a retrospective study of clinical outcomes after surgical intervention in 24 school-aged children, adolescents, and young adults with TCS. Methods : All 83 patients with a lipomyelomeningocele (LMMC) underwent untethering surgery for caudal cord tethering between 1987 and 2007. The clinical charts and follow-up data were reviewed. Of these patients, 24 school-aged children, adolescents, and young adults with TCS were studied with respect to the clinical, radiologic, pathologic features, and surgical outcomes. Results : Untethering procedures were performed in 24 patients (age range, 7-25 years) for TCS of various origins (lipoma, lipomyelomeningocele, and tight filum terminale). Specific circumstances involving additional tugging of the already tight conus, and direct trauma to the back precipitated the onset of symptom in 50% of the patients. Diffuse and non-dermatomal leg pain, often referred to the anorectal region, was the most common presenting symptom. Progressive sensorimotor deficits in the lower extremities, as well as bladder and bowel dysfunction, were also common findings, but progressive foot and spinal deformities were noted less frequently. The most common tethered lesions were intradural lipomas, thickened filum and fibrous band adhesions into the placode sac. The surgical outcome was gratifying in relation to pain and motor weakness, but disappointing with respect to resolution of bowel and bladder dysfunction. Of the 24 patients with TCS, pre-operative deficits improved after surgery in 14 (58.3%). remained stable in 8 (33.4%). and worsened in 2 (8.3%). Conclusion : The pathologic lesions of tethered cord syndrome in school-aged children, adolescents, and young adults, are mostly intradural lipomas and tight filum. It is suggested that the degree of cord traction results in neurologic dysfunction in late life due to abnormal tension, aggravated by trauma or repeated tugging of the conus during exercise. Early diagnosis and adequate surgical release might be the keys to the successful outcome in school-aged children, adolescents, and young adults with TCS.
본 연구는 성인초기여성의 섭식장애를 설명하기 위해 가설적 모형을 구성하고, 영향요인의 효과를 확인하고자 수행한 경로분석 연구이다. 2017년 8월, 약 1개월 동안 19세 ~ 29세 성인초기여성 193명을 대상으로 일반적 특성, 자존감, 분노, 우울, 신체불만족 및 섭식장애를 자가보고 설문지법을 이용하여 조사하였다. 자료는 SPSS 21.0 프로그램과 AMOS 24.0 프로그램으로 통계 처리하였다. 수집한 자료를 분석한 결과는 다음과 같다. 대상자의 평균 연령은 24.02세이었으며, 변수들의 평균은 섭식장애 8.99점, 신체불만족 92.05점, 우울 14.96점, 분노 18.99점, 자존감 32.17점이었다. 또한 섭식장애 점수 20점을 기준으로 구분하였을 때, 고위험 섭식장애 대상자는 9.8%이었다. 섭식장애는 신체불만족, 우울, 분노와 양(+)의 상관관계가 있었고, 자존감과는 음(-)의 상관관계가 있었다. 신체불만족은 우울, 분노와 양(+)의 상관관계가 있었고, 자존감과는 음(-)의 상관관계가 있었다. 우울은 분노와 양(+)의 상관관계가 있었고, 자존감과 음(-)의 상관관계가 있었다. 수정모형의 적합도는 ${\chi}^2$(p) 1.89 (p = 0.756), ${\chi}^2$/df 0.47, GFI 0.99, AGFI 0.99, NFI 0.99, SRMR 0.022, RMSEA 0.001으로 나타났다. 각 변수들이 섭식장애에 미치는 효과를 살펴보면, 자아존중감은 우울과 신체불만족을 매개한 간접효과와 총 효과가 유의하였다. 분노는 우울과 신체불만족을 매개한 간접효과와 총효과가 유의하였다. 우울은 신체불만족을 매개한 간접효과와 총효과가 유의하였다. 신체불만족은 직접효과 및 총효과가 유의하였다. 이들 변수들에 의한 설명력은 46%이다. 따라서 성인초기여성의 섭식장애 개선을 위한 전략 수립에 신체불만족, 우울, 분노, 자존감을 주요요인으로 고려해야 함을 확인하였다.
This study was undertaken to compare the gait characteristics between the Korean elderly and young adults, we measured the plantar pressure and contact time of gait with barefoot along a walkway at their preferred walking speed. The results indicate that older people exhibited significantly less plantar pressure than young adult in all 3 regions (FF, MF and RF) and significantly less time % on the initial contact phase (ICP), forefoot push-off phase (FFPOP) and significantly more % forefoot contact phase (FFCP) and foot flat phase (FFP). The converted plantar pressure value to percentage, it showed more pressure in forefoot (FF) in the elderly person than the young adults. It could be explained that the forward shifting in plantar pressure are associated with a more flexed posture of elderly such as actual stabilizing fearrelated adaptations. Longer total foot contact time in the elderly means that the old people show the decreased gait velocity. In other words, lower velocity was found to be associated with pre-existing fear of falling. With longer contact time and slower stepping movement, the elderly become more unstable. With these findings, it could be confirmed that there were significant changes in foot characteristics which contribute to alter the plantar pressure and contact time during gait with advancing age. Further research is required to establish possible links to risk of falling and development of footwear in the elderly adults.

Sarcopenia, a common clinical syndrome in older adults, is defined as decreased muscle mass, strength, and physical performance. Since sarcopenia is associated with the incidence of functional decline, falls, and even mortality in older adults, researchers and health care providers have been keen to accumulate clinical evidence to advocate the screening and prevention of sarcopenia progression in older adults. The factors that may accelerate the loss of muscle mass and function include chronic diseases, inactivity, and deficiency in appropriate nutritional support. Among these, nutritional support is considered an initial step to delay the progression of muscle wasting and improve physical performance in community-dwelling older adults. However, a nationwide study suggested that most Korean older adults do not consume sufficient dietary protein to maintain their muscle mass. Furthermore, considering age-associated anabolic resistance to dietary protein, higher protein intake should be emphasized in older adults than in younger people. To develop a dietary protein recommendation for older adults in Korea, we reviewed the relevant literature, including interventional studies from Korea. From these, we recommend that older adults consume at least 1.2 g of protein per kg of body weight per day (g/kg/day) to delay the progression of muscle wasting. The amount we recommend (1.2 g/kg/day) is 31.4% higher than the previously suggested recommended daily allowance (i.e., 0.91 g/kg/day) for the general population of Korea. Also, evidence to date suggests that the combination of exercise and nutritional support may enhance the beneficial effects of protein intake in older adults in Korea. We found that the current studies are insufficient to build population-based guidelines for older adults, and we call for further researches in Korea.
본 연구는 우리사회에서 가장 취약한 집단이라고 할 수 있는 여성 독거노인을 대상으로 이들의 우울이 시간의 흐름에 따라 어떠한 변화패턴을 갖는지 그리고 이러한 우울에 영향을 미치는 요인이 무엇인지를 규명하기 위한 종단연구이다. 동시에 본 연구에서는 동일한 노인일지라도 연령에 따른 차이가 크다는 점을 고려하여 여성 독거노인을 전기(the young-old)와 후기노인(the old-old)으로 구분하여 각 집단의 우울에 영향을 미치는 예측요인이 서로 어떠한 차이가 발생하는지를 확인하였다. 주요 분석결과를 살펴보면 다음과 같다. 첫째, 여성 독거노인의 우울은 시간이 지나면서 감소하는 패턴을 갖는 것으로 나타났다. 조건적 모형에서 여성 독거노인의 우울궤적의 초기수준에 영향을 미치는 요인을 살펴보면 그 외 도시보다는 대도시 지역에 사는 노인이, 주관적 건강상태가 좋을수록, 운동을 하지 않는 노인이 초기 우울값이 낮은 것으로 나타났다. 다음으로 여성 독거노인의 우울의 변화율(기울기)에 영향을 미치는 요인을 살펴본 결과, 연령이 높을수록, 대도시에 사는 노인이, 주관적 건강상태가 좋을수록, 친교활동이 적을수록, 사회활동을 많이 할수록 우울수준이 큰 폭으로 감소하는 것으로 나타났다. 마지막으로 여성 독거노인의 우울의 초기값과 기울기에 영향을 미치는 경로에 전기노인과 후기노인 사이에 일부 차이가 있는 것으로 나타났다. 구체적인 내용을 살펴보면, 후기노인의 경우 초기에 사회활동 참여정도가 높을수록 우울의 변화폭은 커지는 반면 전기노인은 유의미한 영향관계가 없었다. 전기노인이 후기노인 보다 초기 주관적 건강상태가 양호할수록 우울의 변화폭이 컸다. 후기노인에게서만 초기에 주기적으로 운동을 하는 노인이 그렇지 않은 노인에 비해 초기 우울값이 높았다. 이상의 분석결과를 바탕으로 여성 독거노인의 우울을 감소시키기 위한 제언을 제시하였다.
