  • Objectives : The aim of this study was to suggest the optimal cut off for best, very good, good, slightly bad, and bad grades. Methods : The subjects were recruited from 4 areas of South Korea and 487 questionnaires were analyzed. The nominal and continuous self-rated health questions were used to reveal the optimal cut off and the Short Form-12 Health Survey questionnaire (SF-12) was additionally used. Frequency, Pearson's correlation coefficient, and ROC-curve analysis were used; the significance level was <.05. Results : Subjects assigned 15(3.1%), 90(18.5%), 237(48.7%), 130(26.7%), and 15(3.1%) to best, very good, good, slightly bad and bad groups respectively. The self-rated health score was associated with total Component (r=.563, p<.001), Physical Component (r=.520, p<.001) and Mental Component of SF-12 (r=.303, p<.001). The optimal cut off was 80.5, 70.5, 53.5, and 40.5 for best, very good or more, good or more, and under slightly bad respectively and area under curve was 0.898, 0.908, 0.945, and 0.908 accordingly. Conclusions : This study suggests that the self-rated health score and grade could be integrated with the optimal cut off.

  • Purpose: As a result of dramatic advances in the medical and surgical management of congenital heart disease (CHD), many babies born with cardiac anomalies today can expect to reach adulthood. The main purpose of this study was to examine the relationship between parenting stress and personal resources of mothers of children with CHD. Method: Fifty-one mothers of children with CHD were recruited at the pediatric cardiac outpatient clinic from July 14th to September 25th 2006. Abidin's parenting stress index/short form (PSI/SF) and Brandt and Weinert's personal resource questionnaire (PRQ) were used. PSI has 3 sub-concepts; parental role distress, dysfunctional parent-child interaction, and difficult child. PRQ has 4 sub-concepts; intimacy, social integration, worth, and assistance. Data were analyzed using SPSS 13.0 version. Results: Correlation analysis showed that parenting stress was significantly related to 'intimacy', 'social integration', and 'worth' of mothers. Multiple regression analysis showed that parenting stress was significantly related to personal resource of mother and information by internet. Conclusion: Mothers who felt they had supportive friends and family, high self esteem, and social integration reported lower parenting stress. Also, internet may be an effective method to provide information and share experience for mothers of children with CHD.

  • Purpose: This study investigated intentions and attitudes towards future parenthood and awareness of fertility among university students in South Korea. Methods: The participants comprised 166 female and male undergraduate students enrolled at five universities. A cross-sectional survey was conducted from May to July 2019 using the Korean version of the Fertility Awareness Questionnaire and Attitudes of Parenthood. The data were analyzed using descriptive statistics based on participants' general characteristics, the χ2 test to identify differences in intentions, and the t-test to evaluate attitudes towards parenthood and awareness of fertility in female and male students. Results: Both female and male students desired to have two children, but they lacked awareness about fertility. The possibility of combining work and having children, along with the availability of childcare resources, impacted the desire for parenthood. Male students tended to consider parenthood as less impactful on their lives and careers than female students. Social structures might also impact the decision to have children. Conclusion: It is important to provide health education emphasizing fertility awareness and parenthood in young adulthood so participants can consider these facts in advance. In addition, the government should provide resources for couples making parenthood decisions.

  • Significant advances in the diagnosis and medical care of children with chronic kidney disease (CKD) are major reasons for the better survival rates of children and adolescents with CKD than the survival rates reported in previous decades. These patients are reaching adulthood, and therefore require a transition to adult medical care. This transition phase is well-recognized to be associated with considerably increased morbidities and medical problems, such as non-adherence, graft loss after transplantation, and loss to follow-up. Low adherence increases morbidity and medical complications and contributes to poorer qualities of life and an overuse of the health care system. However, these tragic outcomes may be avoidable through a structured and well-defined transition program. In the last decade, there has been increasing interest to resolve these medical and psychological problems that occur during the transfer of young adult patients from pediatric to adult renal units. The aims of a successful transition from pediatric to adult medical care include enhancing the individual development of better health-competence and stabilizing, or even improving, the state of health. This review will focus on various aspects of the transition phase of adolescents who have CKD or who underwent kidney transplantation from pediatric to adult nephrology care.

  • Prevalence of obesity in Korean children and adolescents has dramatically increased since the last 10-20 years. It is important to initiate prevention efforts early in childhood because prevalence of obesity in adolescence is the strongest predictor of its prevalence in adulthood. Intrauterine life, infancy, and preschool years may comprise the critical periods that are essential for the long-term regulation of energy balance therefore, obesity-prevention strategies should be initiated in utero and continued throughout childhood and adolescence. Families with high-risk children should be provided early education about maintaining normal weight. Encouraging physical activity and, especially, avoiding inactivity, are key challenges in the prevention of future obesity. Schools should be primarily involved in educating parents to discourage their children from excessively watching TV or playing computer games and eating unhealthy snacks and food. The involvement of medical practitioners is also important, especially, in the case of obese parents, obesity prevention strategies should be promoted from the first visit of pregnant women to the physicians. Health professionals can also be involved in obesity prevention because they are ideally equipped to identify young children at risk of obesity. Community and nation-wide efforts to increase awareness and promote environments that encourage physical activity and healthy nutrition are required.

  • Kawasaki disease (KD), an acute vasculitis that primarily affects young children, is the most common acquired paediatric cardiovascular disease in developed countries. While sequelae of arterial inflammation in the acute phase of KD are well documented, its late effects on vascular health are increasingly unveiled. Late vascular dysfunction is characterized by structural alterations and functional impairment in term of arterial stiffening and endothelial dysfunction and shown to involve both coronary and systemic arteries. Further evidence suggests that continuous low grade inflammation and ongoing active remodeling of coronary arterial lesions occur late after acute illness and may play a role in structural and functional alterations of the arteries. Potential importance of genetic modulation on vascular health late after KD is implicated by associations between mannose binding lectin and inflammatory gene polymorphisms with severity of peripheral arterial stiffening and carotid intima-media thickening. The changes in cholesterol and lipoproteins levels late after KD further appear similar to those proposed to be atherogenic. While data on adverse vascular health are less controversial in patients with persistent or regressed coronary arterial aneurysms, data appear conflicting in individuals with no coronary arterial involvements or only transient coronary ectasia. Notwithstanding, concerns have been raised with regard to predisposition of KD in childhood to accelerated atherosclerosis in adulthood. Until further evidence-based data are available, however, it remains important to assess and monitor cardiovascular risk factors and to promote cardiovascular health in children with a history of KD in the long term.

  • Objectives : The early onset of mood symptoms in bipolar disorder has been associated with poor outcomes in many studies. However, aspects of the clinical course of bipolar disorder in children and adolescents are controversial. The goal of this article is to review the clinical characteristics and longitudinal course of children and adolescents with bipolar disorders. Methods : Searches were conducted in MedLine, PsycINFO, KISS, and RISS using the terms phenomenology, clinical course, outcome, BPD, pediatric, children and adolescents. Twenty-one reports were selected : either original articles reporting symptoms and clinical characteristics of subjects (ages 5-18 years), or published articles in reviewed journals about bipolar disorder in children and adolescents. Results : Approximately 70% of subjects with bipolar disorder recovered from their index episode, and 50% had at least 1 syndromal recurrence, particularly depressive episodes. For 60% of the follow-up time, subjects had syndromal or subsyndromal symptoms with numerous changes in symptoms and shifts of polarity. Approximately 20% of BP-II subjects converted BP-I. Conclusion : Bipolar disorders in children and adolescents are characterized by episodic illness with subsyndromal and syndromal episodes with mainly depressive and mixed symptoms and rapid mood changes. Extensive follow-up time is needed to evaluate the continuity of bipolar disorder symptoms from childhood to adulthood.

  • To provide the physician with adequate information to diagnose and treat sleepwalking and sleep terrors, the author reviewed clinical features, epidemiology, causative and precipitating factors, polysomnography, diagnosis, differential diagnosis, and treatment for these disorders. Sleepwalking and sleep terrors have been defined as disorders of arousal that occur early in the night and have their onset during stage 3 or 4 sleep. In both disorders, patients are difficult to arouse, and complete amnesia or minimal recall of the episode is frequent. Genetic, developmental, and psychological factors have been identified as causes of both sleepwalking and sleep terrors. Sleepwalking and sleep terrors typically begin in childhood or early adolescence and are usually outgrown by the end of adolescence. When sleepwalking or sleep terrors have a post-pubertal onset or continue to adulthood, psychopathology is a more significant causative factors. The behavior that occur from deep slow-wave sleep can be painful or dangerous to the individual and/or disturbing to those close to that individual. The assessment of patients suspected of having these conditions requires a thorough medical and sleep history. The most important consideration in managing patients with sleepwalking or sleep terrors episodes is protection from injury.
