『교주부인양방(校注婦人良方)』에 수재된 설기(薛己)의 부인과 의안(醫案) 연구 (Xueji's Gynecological Medical Records In Xiaozhufurenliangfang)

  • Objectives : Medical records of Xueji in the "Xiaozhufurenliangfang" were examined in this study which aimed to look at the medical situation in gynecology of China's Ming Dynasty period, in hopes for it to yield implications and treatment directions to gynecology in $21^{st}$ century Korea. Methods : The medical records were systematically organized with a medical anthropological approach along with overall analysis of the entire records, which lent meaningful statistical information in numeric form. A bibliographical review of the text as historical artifact was undertaken as well. Results : In managing gynecological conditions, Xueji frequently attributed them to depletion of Qi and Blood of the Spleen and Liver. In terms of pathogenic factors, he frequently mentioned Fire and Heat, and as etiological factors, emotional distress. For treatment, he frequently used 'Bu Zhong Yi Qi Tang(補中益氣湯)', 'Xiao Yao San(逍遙散)' and 'Gui Pi Tang(歸脾湯)'. Conclusions : Through studying the medical records of Xueji in "Xiaozhufurenliangfang" a close look into a master's insight on gynecological disorders in terms of diagnosis and treatment was achieved. The formulas he used are widely applied even today, and this study shows that the formulas's clinical application could be expanded even wider.

임산(臨産) 이경맥(離經脈)에 대한 연구 - 맥리(脈理)와 임상활용을 중심으로 - (Research on the Imminent Labor Pulse - A Focus on Pulse Theory and Clinical Practicality -)

  • Subject : The Imminent Labor Pulse(臨産 離經脈, extra-meridian pulse approaching labor) is one of many practical clinical knowledges, unique to Traditional Korean(Eastern) Medicine. Objective : This research explores the origin and change of perception on the subject throughout history, with objective to sort out practical knowledge applicable in the clinic of today. Also, it draws clinical meaning of pulse diagnosis according to the physiological theories, and suggests directions for future clinical researches. Method : First, the Imminent Labor Pulse was examined based on major publications such as the "Nanjing", "Maijing" and theories of major doctors. Second, this previous study was examined through clinical observation research. Third, further research was carried out on pulse theory according to the Traditional Medical theories. Fourth, their clinical practicality and points of further research were sorted out. These points need to be examined through additional clinical research. Conclusion : The following conclusions could be drawn from study of the Imminent Labor Pulse. From the Imminent Labor Pulse which indicated the beginning of the first period of labor in the "Maijing", it developed into trying to grasp the beginning of the second period of labor which is the actual time of labor, through "Zhubingyuanhoulun" and "Shizaizhifang". By Xueji, another pulse that could be felt on the middle finger of the mother had been discovered. The middle finger is where the Pericardium Meridian flows. The child's connection to the uterus becomes disconnected on the first period, but that with the Pericardium meridian which provides blood meridian to the Taichongmai of the child continues throughout labor until the umbilicus chord is cut. Therefore, this middle finger pulse could serve as showing the heart condition of the mother and child during labor. The time of its appearance and disappearance, and their correlation with the child's heartbeat need to be clinically examined.

의학연구 방법론으로서 '격물치지' 이해 (Comprehension of 'gewuzhizhi' as the medical research methodology)

  • 본 연구는 의학연구 방법론으로서 격물치지(格物致知)를 설명해 줄 수 있는 구조와 이 구조에서 지식을 획득하는 방법을 고찰하였다. 먼저, "예기(禮記)" "대학(大學)"에 보이는 격물치지(格物致知)를 "학기(學記)" "소의(少儀)"와 관련시켜 물(物)의 관찰과 물(物)의 이치를 터득하는 구조로 학(學)-­격물치지(格物致知)­-'유(游)'를 도출하였다. 그 다음, 산수화가이자 의학연구자, 의사였던 왕리(王履)의 화산(華山) 유람 목적과 송대 화론인 "도화견문지(圖?見聞誌)"와 "임천고치(林泉高致)"에서 화의(?意)를 산출하는 '유(游)'의 함의를 고찰하였다. 고찰의 과정에서 다음을 밝혔다. 첫째, 왕리(王履)의 화산(華山) 유람과 송대의 "도화견문지(圖?見聞誌)", "임천고치(林泉高致)"에서 보이는 '유(游)'는, 학(學)­-격물치지(格物致知)-­'유(游)'의 구조에 속한다. 둘째, 학(學)­-격물치지(格物致知)-­'유(游)' 구조에서 격물치지(格物致知) 의미는 송대에 심화되었다. "학기(學記)"의 '유(游)'는 유행(游行)으로 견문을 넓히는 과정에서 물(物)에 대한 지(知)를 획득하였다. 그러나 송대 화론인 "도화견문지(圖?見聞誌)", "임천고치(林泉高致)"에서 '유(游)'는 물(物)을 적극적으로 관찰하여 知를 얻는 활동으로, 이러한 '유(游)'로 인하여 격물치지(格物致知)는 그 의미가 심화되었다. 셋째, 송대 화론의 '유(游)'가 물(物)의 이치를 터득하는 방법은 물(物)에 대한 관찰(찰(察), 탐적구심(探蹟鉤深), 간(看), 면면간(面面看), 역역나열흉중(歷歷羅列胸中) 등)이다. 넷째, 학(?)­-격물치지(格物致知)­-'유(游)'의 구조는 송대에 회화영역에서 실천되었고, 송대의 회화관을 추종한 왕리(王履)에서 의학연구로 실천영역이 확장되었다. 다섯째, 왕리(王履)의 "화산(華山)을 배웠다[사(師)]"는 주장을 통하여, 왕리(王履)는 인식의 근원을 자연(화산(華山))에 두고 있음을 알 수 있다. 이상을 통하여 "예기(禮記)" "대학(大學)"에 보이는 격물치지(格物致知)는 수양론과 인식론에 관여되며, 인식론에 관여하는 구조는 학(學)­-격물치지(格物致知)-­'유(游)'로 구별할 수 있다. 그리고 여기서 인식의 방법은 관찰(찰(察), 탐적구심(探蹟鉤深), 간(看), 면면간(面面看), 역역나열흉중(歷歷羅列胸中) 등)임을 규명하였다. 이러한 규명을 통하여 의학연구 방법론으로서 격물치지(格物致知)를 이해할 수 있다.
