RNSXI(real-name shooting X-ray of inspector) Settlement Realization applying PACS Database, In Digital Medical environment (PACS Database를 활용한 촬영실명제 정착화 실현)

  • As developing the medical treatment image portion with the change of these times, PACS, which is able to digitalize image portion data, has a lot of data-based image data. Applying this PACS, we would like to settle down RNSXI(real-name shooting X -ray of inspector) system. We interviewed with P ACS's operators of university hospitals which is using PACS in Seoul about the present conditions whether using of RNSXI or not. And we inquired the RNSXI equipments, applying PACS database, and Interface conditions undertook to do in our hospital. All university hospitals in Seoul are set up the P ACS system. But no hospital use the RNSXI. In our hospital, we can check inspector' name or initials who exposure x-ray with the PACS Viewer by looking over equipments(CR, DR, US, MG, MR, CT) and Interface of the DICOM Header data. However, some equipments like RF and Angio can not check inspector' name or initials. Under the Film/System environment, RNSXI system has been used frequently like that inspector's signature or initial added to a patient data. Though the digital medical treatment was developed, RNSXI system was declined. It is necessary to using RNSXI system in order to improving radiologists' rights, even if it is not under the application of the medical treatment image laws. If RNSXI system use, radiologists should specialize in their major and the Repeat rate should be reduced. In environment of PACS, RNSXI system can be used by linking both the equipments and the Interface with a production enterprise of P ACS. Therefore RNSXI system applying the P ACS datebase should settle down in our medical system for being provided lots of data.

Development of Image Reconstruction Algorithm for Chest Digital Tomosynthesis System (CDT) and Evaluation of Dose and Image Quality (흉부 디지털 단층영상합성 시스템의 영상 재구성 알고리즘 개발 및 선량과 화질 평가)

  • Recently, digital tomosynthesis system (DTS) has been developed to reduce overlap using conventional X-ray and to overcome high patient dose problem using computed tomography (CT). The purpose of this study was to develop image reconstruction algorithm and to evaluate image characteristics and dose with chest digital tomosynthesis (CDT) system. Image reconstruction was used for filtered back-projection (FBP) methods and system geometry was constructed ${\pm}10^{\circ}$, ${\pm}15^{\circ}$, ${\pm}20^{\circ}$, and ${\pm}30^{\circ}$ angular range for acquiring phantom images. Image characteristics carried out root mean square error (RMSE) and signal difference-to-noise ratio (SDNR), and dose is evaluated effective dose with ${\pm}20^{\circ}$ angular range. According to the results, the phantom image with slice thickness filter has superb RMSE and SDNR, and effective dose was 0.166 mSv. In conclusion, we demonstrated usefulness of developed CDT image reconstruction algorithm and we constructed CDT basic output data with measuring effective dose.

Analysis of the Manufacturing Techniques for the KwangDahoe Tying on the Sword in Joseon Dynasty (조선시대 도검 패용 광다회의 제작기법 분석)

  • Dahoe is a traditional braid in Joseon Dynasty. There are many Dahoe artifacts, which can give us a glimpse of the costume, culture, and life of that era. The study of Dahoe is necessary in various fields, but it is difficult because this manufacturing technique has been passed down from hand to hand. Few studies have been done so far. This research examines the manufacturing technique of KwangDahoe which is passed down by Maedeupjang (Decorative Knotting) and is generally used for knotting and/or tying objects. The main characteristic of TieKwangDahoe, made through the same method as WonDahoe, is the square hole in the middle. It was impossibile to remake the original braid because there is no confirmed number of the strand. Especially it is very difficult to do conservation and restoration on serious degradation state of the fiber. Therefore, it is necessary to analyze the non-destructive manufacturing techniques method for Dahoe and assess their applicability. First, we analyzed the artifacts' manufacturing technique based on the database of the Dahoe's manufacturing technique. In order to do that, we undertook schematization, restoration, morphological analyzation of the Dahoe. And then, X-ray CT scans were performed to improve the reliability of the DB. These results of scanning were interpreted based on the manufacturing technique. The selected Tie-KwangDahoe on the sword for the study are artifacts including artistic value and symbolism in Joseon Dynasty. Based on the analysis of the manufacturing technique, we found that both artifacts were made of 20-strand braid of single cross according to the length-scale measure. It was manufactured using 8-strand on left-right side, 12-strand on front-back side by the braiding manufacturing technique method Finally, this research suggests non-destructive analysis method of Dahoe's manufacturing technique is based on the database and the analysis results. I hope this research can be useful in various professional fields of Dahoe in the future. Moreover, I hope this can be of any help in preserving Korean cultural heritage.

The Effect of Fe and Fe2O3 Powder Mixing Ratios on the Pore Properties of Fe Foam Fabricated by a Slurry Coating Process (슬러리 코팅 공정으로 제조된 Fe 폼의 기공 특성에 미치는 Fe 및 Fe2O3 분말의 혼합 비율의 영향)

  • Metal foams have a cellular structure consisting of a solid metal containing a large volume fraction of pores. In particular, open, penetrating pores are necessary for industrial applications such as in high temperature filters and as a support for catalysts. In this study, Fe foam with above 90% porosity and 2 millimeter pore size was successfully fabricated by a slurry coating process and the pore properties were characterized. The Fe and $Fe_2O_3$ powder mixing ratios were controlled to produce Fe foams with different pore size and porosity. First, the slurry was prepared by uniform mixing with powders, distilled water and polyvinyl alcohol(PVA). After slurry coating on the polyurethane(PU) foam, the sample was dried at $80^{\circ}C$. The PVA and PU foams were then removed by heating at $700^{\circ}C$ for 3 hours. The debinded samples were subsequently sintered at $1250^{\circ}C$ with a holding time of 3 hours under hydrogen atmosphere. The three dimensional geometries of the obtained Fe foams with an open cell structure were investigated using X-ray micro CT(computed tomography) as well as the pore morphology, size and phase. The coated amount of slurry on the PU foam were increased with $Fe_2O_3$ mixing powder ratio but the shrinkage and porosity of Fe foams were decreased with $Fe_2O_3$ mixing powder ratio.

Development of an MCNP-Based Cone-Beam CT Simulator (MCNP 기반의 CBCT 전산모사 시스템 개발)

  • We have developed a computer simulator fur cone-beam computed tomography (CBCT) based on the commercial Monte Carlo code, MCNP. All the functions to generate input files, run MCNP, convert output files to image data, reconstruct tomographs were realized in graphical user-interface form. The performance of the simulator was demonstrated by comparing with the experimental data. Although some discrepancies were observed due to the ignorance of the detailed physics in the simulation, such as scattered X-rays and noise in image sensors, the overall tendency was well agreed between the measured and simulated data. The developed simulator will be very useful for understanding the operation and the better design of CT systems.

The width of the incisive canal and labial alveolar bone of the incisive canal: an assessment on CT images (전산화단층영상에서 절치관 및 순측 치조골 폭경에 관한 연구)

  • Purpose : To assess the width of the labial alveolar bone of the incisive canal and the width of the incisive canal on spiral computed tomographic images of the anterior portion of the maxilla. Materials and Methods : Study materials included 38 CT scans taken for preoperative planning of implant placement. Axial cross-sectioned image entirely showing the incisive canal was selected and scanned with 600 DPI resolution. The width of the labial alveolar bone of the incisive canal at an orifice to the oral cavity, middle portion, and an orifice to the nasal cavity and the diameter of the incisive canal at the middle portion were determined by two specialist using Digora for Windows 2.1 The statistical analyses were carried out using SPSS 12.0.1. Results : When the maxillary central incisors remained, the mean labial alveolar bone width were $6.81{\pm}1.41mm,\;6.46{\pm}1.33mm$, and $7.91{\pm}1.33mm$. When the maxillary central incisors were missed the mean width were $5.42{\pm}2.20mm,\;6.23{\pm}2.29mm$, and $7.89{\pm}2.13mm$. Conclusions : The labial alveolar bone width at middle portion and an orifice to the nasal cavity were of no statistical significant difference according to presence of the maxillary central incisors (P>0.05). The width between oral cavity and nasal cavity, middle portion and to nasal cavity revealed statistically significant difference (P<0.05).

A Case Report of Echocardiography of Non-rheumaic Giant Left Atrium (비-류마티스성 거대 좌심방의 심장 초음파 검사 증례 보고)

  • In this case, non-rheumatic GLA was observed and it was intended to report a case of ultrasound examination. The case patient, a 60-year-old male, visited the emergency room for dyspnea and palpitations. For radiology examination, chest X-ray examination and CT examination were performed, and a giant left atrium was confirmed. Echocardiography was performed to find the cause of the giant left atrium. Echocardiography The size and volume of the left atrium were evaluated by Simpson's method, and the giant left atrium with LVEDVI 6 times larger than that of the general giant left atrium could be evaluated. Also, a giant left atrium in a patient without rheumatic heart disease is evaluated as a very rare case. Since non-rheumatic giant left atrium could be caused by functional mitral regurgitation, diastolic dysfunction. It was confirmed that ultrasonography, which allowed both morphological and hemodynamic examinations, could be a useful case.

CdZnTe Detector for Computed Tomography based on Weighting Potential (가중 퍼텐셜에 기초한 CT용 CdZnTe 소자 설계)

  • Room-temperature operating CdZnTe(CZT) material is an innovative radiation detector which could reduce the patient dose to one-tenth level of conventional CT (Computed Tomography) and mammography system. The pixel and pixel pitch in the imaging device determine the conversion efficiency of incident X-or gamma-ray and the cross-talk of signal, that is, image quality of detector system. The weighting potential is the virtual potential determined by the position and geometry of electrode. The weighting potential obtained by computer-based simulation in solving Poisson equation with proper boundaries condition. The pixel was optimized by considering the CIE (charge induced efficiency) and the signal cross-talk in CT detector system. The pixel pitch was 1-mm and the detector thickness was 2-mm in the simulation. The optimized pixel size and inter-pixel distance for maximizing the CIE and minimizing the signal cross-talk is about $750{\mu}m$ and $125{\mu}m$, respectively.

Study of Absorbed Dose and Effective Dose for Prostate Cancer Image Guided Radiation Therapy using kV Cone Beam Computed Tomography (kV Cone Beam Computed Tomography (CBCT)를 이용한 전립선암 영상유도방사선치료 시 흡수선량 및 유효선량에 관한 고찰)

  • Purpose: To evaluate the results of absorbed and effective doses using two different modes, standard mode (A-mode) and low-dose mode (B-mode) settings for prostate cancer IGRT from CBCT. Materials and Methods: This experimental study was obtained using Clinac iX integrated with On Board Imager (OBI) System and CBCT. CT images were obtained using a GE Light Speed scanner. Absorbed dose to organs from ICRP recommendations and effective doses to body was performed using A-mode and B-mode CBCT. Measurements were performed using a Anderson rando phantom with TLD-100 (Thermoluminescent dosimeters). TLD-100 were widely used to estimate absorbed dose and effective dose from CBCT with TLD System 4000 HAWSHAW. TLD-100 were calibrated to know sensitivity values using photon beam. The measurements were repeated three times for prostate center. Then, Evaluations of effective dose and absorbed dose were performed among the A-mode and B-mode CBCT. Results: The prostate absorbed dose from A-mode and B mode CBCT were 5.5 cGy 1.1 cGy per scan. Respectively Effective doses to body from A mode and B-mode CBCT were 19.1 mSv, 4.4 mSv per scan. Effective dose from A-mode CBCT were approximately 4 times lower than B-mode CBCT. Conclusion: We have shown that it is possible to reduce the effective dose considerably by low dose mode(B-mode) or lower mAs CBCT settings for prostate cancer IGRT. Therefore, we should try to select B-mode or low condition setting to decrease extra patient dose during the IGRT for prostate cancer as possible.

A Study on measurement of scattery ray of Computed Tomography (전산화 단층촬영실의 산란선 측정에 대한 연구)

  • Purpose : Computed tomographic equipment is essential for diagnosis by means of radiation. With passage of time and development of science computed tomographic was developed time and again and in future examination by means of this equipment is expected to increase. In this connection these authors measured rate of scatter ray generation at front of lead glass for patients within control room of computed tomographic equipment room and outside of entrance door for exit and entrance of patients and attempted to ind out method for minimizing exposure to scatter ray. Material and Method : From November 2001 twenty five units of computed tomographic equipments which were already installed and operation by 13 general hospitals and university hospitals in Seoul were subjected to this study. As condition of photographing those recommended by manufacturer for measuring exposure to sauter ray was use. At the time objects used DALI CT Radiation Dose Test Phantom fot Head (${\oint}16\;cm$ Plexglas) and Phantom for Stomache(${\oint}32\;cm$ Plexglas) were used. For measurement of scatter ray Reader (Radiation Monitor Controller Model 2026) and G-M Survey were used to Survey Meter of Radical Corporation, model $20{\times}5-1800$, Electrometer/Ion Chamber, S/N 21740. Spots for measurement of scatter ray included front of lead glass for patients within control room of computed tomographic equipment room which is place where most of work by gradiographic personnel are carried out and is outside of entrance door for exit and entrance of patients and their guardians and at spot 100 cm off from isocenter at the time of scanning the object. The results : Work environment within computed tomography room which was installed and under operation by each hospital showed considerable difference depending on circumstances of pertinent hospitals and status of scatter ray was as follows. 1) From isocenter of computed tomographic equipment to lead glass for patients within control room average distance was 377 cm. At that time scatter ray showed diverse distribution from spot where no presence was detected to spot where about 100 mR/week was detected. But it met requirement of weekly tolerance $2.58{\times}10^{-5}\;C/kg$(100 mR/week). 2) From isocenter of computed tomographic equipment to outside of entrance door where patients and their guardians exit and enter was 439 cm in average, At that time scatter ray showed diverse distribution from spot where almost no presence was detected to spot with different level but in most of cases it satisfied requirement of weekly tolerance of $2.58{\times}10^{-6}\;C/kg$(100 mR/week). 3) At the time of scanning object amount of scatter ray at spot with 100 cm distance from isocenter showed considerable difference depending on equipments. Conclusion : Use of computed tomographic equipment as one for generation of radiation for diagnosis is increasing daily. Compared to other general X-ray photographing field of diagnosis is very high but there is a high possibility of exposure to radiation and scatter ray. To be free from scatter ray at computed tomographic equipment room even by slight degree it is essential to secure sufficient space and more effort should be exerted for development of variety of skills to enable maximum photographic image at minimum cost.
