The Influence of Home Position(H.P.) and Random Position(R.P.) on the Shape of Prepared Tooth Surfaces -Upper Left 1st Molar for Full Cast Crown- (시술자세가 치관 삭제면 형태에 미치는 영향 -상악 좌측 제 1 대구치의 전부 주조관-)

  • To properly prepare teeth, dentists require a direct view of the working area and are often placed in a difficult position, moreover, a high proportion of dentists suffer from headache and back pain. Dentists who make use of the dental mirror and position their patients carefully to gain a proper view report less headache, pain and discomfort in the shoulders. It is recommended that dentists learn the "Home Position(H.P.)" which among the various "Random Position(R.P.)" methods, enables dentists to approach their patients in a stable posture. The purpose of this study was to compare tooth preparation in the H.P. and the R.P., and evaluate the clinical efficacy of the H.P.. Tooth preparation for a full cast crown was performed on the maxillary left 1st molar using the H.P. and the R.P., and the shapes of the prepared tooth surfaces at the two different operating positions were compared. The amount of occlusal reduction, marginal width, height, and axial taper were measured and analyzed. A T-test was performed separately to compare the results of the H.P. and the R.P. with respect to the accuracy of the corresponding tooth reduction. The results were as follows. ; 1. The amount of occlusal reduction was excessive on the mesiobuccal cusp(P<0.05), and deficient on the lingual cusp in the H.P.(P<0.01). In the R.P., this was excessive on the buccal cusp and deficient on the fossa and distolingual cusp(P<0.01). 2. The amounts of marginal width were excessive in all areas except on the lingual and mesial surfaces in the H.P. and lingual surface in the R.P.(P<0.01). 3. The marginal heights were achieved more accurately in the R.P. than the H.P.(P<0.01). 4. Axial surface taper was excessive in all areas in the H.P.(P<0.01). But the axial taper of measured areas was even, and tooth is more like to retain its original axis after reduction. In the R.P., axial surface taper was excessive on the mesial and buccal area, and deficient on the distolingual area(P<0.01), and therefore, the axis of the prepared teeth was tipped in the distolingual direction. 5. The times needed for preparation were 12 minutes and 49 seconds in the H.P., and 11 minutes and 35 seconds in the R.P., and the R.P. was statistically faster(P<0.01). The tooth preparation in the H.P. achieved its goal, in that it enabled the operator to make even tooth reduction. In conclusion, the H.P. system offers an improved method that can be used in clinic after specific training.

  • Traditionally, keyboards have been one of the most frequently used input devices for information processing using computers. As most computers adopt the Microsoft Windows for their operating system however, the usage of mouse has recently increased to a great extent. Moreover, the mouse has been used as the leading input device in such areas as industrial design and computer aided design. Excessive uses of mouse may cause a severe pain and fatigue on neck and upper limb due to the intensive and repetitive use of corresponding muscles, which renders a decline in efficiency and leads to musculoskeletal disorders. The main purpose of this study is to find the best working conditions to prevent musculoskeletal disorders when using mouse in a neutral posture. Utilizing electromyogram amplitude and Borg's scale, the role change and strength imposed on the muscles were measured and analyzed with and without the forearm support concerned. Also investigated were the effects of changes in elbow forearm supporter.

Musculoskeletal Workload Evaluation in Physical Therapist (물리치료사의 근골격계 부담작업 유해요인 조사)

  • This study investigated the symptoms of work-related musculoskeletal disorder(WMSDs) for physical therapists who have different work duties. We analyzed the symptoms in the musculoskeletal system and the degree of work-harmfulness by the survey of the symptom in the musculoskeletal system and rapid upper limb assessment(RULA) for pain control group(n=56), adult exercise group(n=53), and pediatric exercise group(n=22). As a result, 69.6% of the pain control group, 84.9% of the adult exercise group, 81.8% of the pediatric exercise group show the observable symptom. The adult exercise group has the biggest work load per hour in the final wrist & arm score of the ergonomic risk assessment using RULA. The action level of the pain control group is $3.0{\pm}0.9$; the Action Level of adult exercise group is $3.3{\pm}0.6$; the Action Level of the pediatric exercise group is $3.2{\pm}0.8$, and so it is shown that the adult exercise group has a problem of working posture. It is considered that devices and education system for preventing from WMSDs should come into wide use.

  • Since 1970, MRI has greatly been developing in terms of strength of magnetic field, the number of receipt channels, and short time of examination. With the development of digital systems and wireless network, hospitals have also acquired, saved, and managed digital images taken by various kinds of medical imaging equipment. However, domestic universities fail to provide practice training course independently thanks to expensive practice equipment and high maintenance cost, and rely on clinical training. Therefore, this study developed a MR patient diagnosis program based on Windows PC to help out students before their working in clinical filed. The designed Relational Database of MRI Simulator is made up of seven tables according to functions and data characteristics. Regarding the designed patient information, each stepwise function was classified by the patient registration method in clinical field. In addition, on the assumption of the basic information for diagnosis, each setting and content were classified. The menu by execution step was arrayed on the left side for easy view. For patient registration, a patient's name, gender, unique ID, birth date, weight, and other types of basic information were entered, and the patient's posture and diagnosis direction were set up. In addition, the body regions for diagnosis and Pulse Sequence were listed for selection. Also, Protocol name and other additional factors were allowed to be entered. The final window was designed to check diagnosis images, patient information, and diagnosis conditions. By learning how to enter patient information and change diagnosis conditions in this program, users will be able to understand more theories and terms learned in practice and thereby to shorten their learning time in actual clinical work.

The sensitivity analysis for ergonomic checklists associated with musculoskeletal disorders (근골격계질환 위험도관련 주요 평가도구들에 대한 민감도 분석)

  • It has been very important to have an exact evaluation for risk factors in order to prevent MSDs(Musculoskeletal Disorders). However, most MSDs evaluation checklists have always been some problems of possibilities associated with subjective evaluation. Therefore, this study tried to conduct a sort of sensitivity analysis on three major evaluation checklists(OWAS, RULA, REBA). Specifically, major subjects in the study consisted of three parts as follows; comparison of the results between experienced and inexperienced subjects, analysis for the consistency of the results in terms of different evaluation times. The results of the study were summarized as follows; 1) There was statistically significant difference of the results by RULA and by REBA between experienced and inexperienced subjects. This might due to the fact that experienced subjects have had better ability to detect the bad working posture during evaluation. However, the results by OWAS did not give any significantly different results between experienced and inexperienced subjects., 2) All three checklists showed significantly different results in terms of different evaluation times. Further study on this subject would eventually provide a sophisticated evaluating guidelines for MSDs regarding determination of subject-specific evaluation, identification of repetitive number of evaluations for stable results for each checklist, determination of job-specific evaluation methods, and so on.

Ergonomic Design of Medic Work Table (MWT) for Medical Technologist

  • Objective: The purpose of this study was to develop and validate the guidelines for Medic Work Table (MWT) based on the anthropometric data of medical technologists. Background: Users' anthropometric data such as sitting height, sitting elbow height, knee height, and so on are significant factors for designing comfortable and useful furniture. Thus, many guidelines for different types of desks and chairs based on the users' anthropometric data have been suggested to many researchers. However, few researches have been conducted to provide design guidelines for MWT for blood collecting task. Medical technologists often use their upper extremities to perform blood collecting task with high repetitions. These repeated motions could be a critical factor in the prevalence rate of Work-related Musculoskeletal Disorders (WMSDs). Therefore, a study on ergonomic design of MWT would be essential in preventing the WMSDs and improving the quality of the working environment of medical technologists. Method: This study suggested design guidelines for ergonomic MWT by focusing on the heights of the upper side and underside, depths of the inside and outside, and width of MWT through anthropometric studies and literature reviews. Afterwards, a new MWT was made using the suggested design guidelines for this study. Five healthy medical technologists participated to evaluate the original MWT and new MWT. All participants took part in the range of motion (ROM) test, electromyography (EMG) muscle activity test, and usability test to validate the suggested guidelines in this study. EMG signals of related muscles (Flexor Carpi Ulnaris, Extensor Carpi Ulnaris, Deltoid Anterior, and Biceps Branchii) were recorded through the surface electromyography system from both the original MWT and the new MWT. The ROM test of the shoulder and elbow flexion was also assessed using motion sensors. Results: The newly designed MWT showed decreased ROMs of the shoulder and elbow up to 22% and 18% compared to the original MWT. The muscle activities in the new MWT also showed a decrease of 13% in Anterior Deltoid, 6% in Biceps Brachii, 5% in Flexor Carpi Ulnaris, and 8% in Extensor Carpi Ulnaris muscle groups, compared to the original MWT. In the usability test, the satisfaction score of the new MWT was also 56.1% higher than that of the original MWT. Conclusion: This study suggested guidelines for designing MWT and validating the guidelines through qualitative and quantitative analyses. The results of motion analysis, muscle activity, and usability tests demonstrated that the newly designed MWT may lead to less physical stress, less awkward posture, and better physical user interface. Application: The recommended guidelines of the MWT would be helpful information for designing an ergonomic MWT that reduces physical loads and improves the performance of many medical technologists.

The Eastern and Western Medical Investigation on the Relation with I.I.C.P and Kwul (두개내압상승(頭蓋內壓上昇)과 궐의(厥) 상관성(相關性)에 대(對)한 동서의학적(東西醫學的) 고찰(考察))

  • The purpose of this study is the approach to I.I.C.P. centered on the meaning of consciousness disorder and the pathological aspect of Kwul (Jose consciousness ; faint, fall into a coma). The meaning of consciousness disorder and apoplexy is evidently involved the definition of Kwul. 1. It is found that the etymological interpretation on Kwul which the energy rises back to go through blocked space and the meaning interpretation of regarding Kwul as apoplexy with medical viewpoint, are related with consciousness disorder and motor disturbance in IICP in the aspect of the rise of Kwul and the abnormal rising of vital energy and blood, In addtion, the overall of meaning of Kwul is showed in table <1-1> by reference to doctors of many generations, 2. The pathology of Kwul includes abnormal rising, sthenia-syndrome in the upper part and asthenia in the lower, the origin of Kwul, the lower, looking like Yin by too sthenic Yang and looking like Yang by too sthenic Yin. The headache, vomiting, papilledema, paralysis of nervi craniales, coma, blood pressure rising, tachycardia by I.I.C.P can be regarded as a conception of trouble of vital energy, sthenia-syndrome of Kwul. The pulse pressure, brachycardia, bradypnea can be regarded as the conception of looking like Yin by too sthenic Yang. 3. In the emergency of Kwul, the abnormal ternimal reversion of the Kwulyin channel, Kuyang channel, and three Yins are related with the phenomenon in I.I.C.P. It is considered that the reverse movement of materials, I.I.C.P. can be closely observed by giving meaning on the meridian of Kwul in Somunkwulron. And the content of phrases of Naelyung which includes consciousness disorder refered in the chapter of Kwul, is compared with I.I.C.P. 4. The followings should be considered; examination of optic symptom and abnormal posture in cerebral herniation ; understanding and working out counterplans of factors and symptoms of consciousness disorder by the observation of vital sign, check of general stages, neurologic inverstigation, clinical diagnosis, and subsidiary diagnosis; application of morphological change of opinion; addtion of the conception of demonstration centered on Yunkyung, Samyinkwulruk, asthenia and thenia of healthy energy in oriental medicine. 5. The similarity of Kwul and I.I.C.P. can be found from etiology and pathotenic factor. The similarity is clearly found by investigation of etiology, pathotenic factor, symptoms and thrapy of Kwul, disease symptom ar.d other symptoms.

Study on the Environmental Factors and Symptoms of VDT Syndrome (VDT 증후군의 환경적 요인과 증상에 대한 연구)

  • Purpose: Recently incidence of VDT syndrome has gradually increased as extensive use of computers. VDT syndrome reported by VDT workers include musculoskeletal disorder, neuropsychiatric disoders and eye symptoms such as eye strain, tired eyes, irritation and blurred vision. The environmental factors of VDT syndrome include electromagnetic waves, size, brightness and lighting of computer screen, height of a monitor and a worktable, working hours, kind of task, distance between screen and workers, indoor humidity and temperature, indoor air contamination and ventilation. In this study, we investigated the environmental factors related to body symptoms and health effects included in VDT syndrome. Methods: Study subjects were total 120 persons (54 male, 66 female) with age from 19 to 28. We surveyed the body symptoms and physical discomfort when doing an activity in a short distance such as reading book or paper, computer work. The questionnaire included main body symptoms, self-consciousness symptoms of eye, satisfaction of working environment, pain of the wrist when using keyboard and mouse. Results: Most of people (70%) felt physical pain from long time work of computer, paper, electrical apparatus. They mainly complained pain of neck and low back (57.1%), eye (45.2%) and head (31%). With the environmental factors, 78.3% of the subjects complaint pain of eye from inappropriate illumination. Most of the symptoms included 'eye fatigue'(38.3%), 'dryness of eye'(31.9%) and 'blurred vision'(23.7%). Subjects in this study complained discomfort of their chairs and most of them experienced pain in the wrist when using keyboard or mouse. Conclusions: When people use electrical apparatus or work with paper, people would get their eye fatigue and feeling of physical fatigue because of not harmonizing various environmental factors such as light, space, posture, worktable with theirselves. Therefore, workers should develop preventive method such as self-control of adequate break time to avoid fatigue while VDT work. Work environment should be changed to ergonomic design for optimal visual environment to prevent musculoskeletal disorder through constant research.

A Study on the Optimum Field Condition for the Performance of Rice Transplanter (수도이앙기(水稻移秧機)의 이앙작업(移秧作業)을 위한 적정포장조건(適正圃場條件)에 관한 연구(硏究))

  • For purpose of investigation the proper paddy field condition in performance of rice transplanter according to the various elapsed times (0.5, 1, 1.5, 2 days) after puddling and plowing (12, 15, 18 cm depth), this experiment was carried out on the paddy field located in Chil Am Dong, Jin Ju City, from April to May in 1980. The results are summarized as follows; 1. The practical working power for the elapsed time 0.5 days and plowing depth 18cm was about 0.8 ps, which was the highest among the plots, so the power out-put(2.5~3.5 p.s) of these engines are considered to be enough for the transplanting under these field conditions. 2. The percentage of slip increased proportional1y to the plowing depth and decreased proportionally to the elapsed time after puddling, and the highest and lowest percentages of slip were 42.5% in elapsed time 0.5 days, plowing depth 18 cm, and 26.5% in elapsed time 2 days, plowing depth 12 cm, respectively. 3. In the plot of elapsed time 2 days and plowing depth 12 cm, the planting distance was 13.9 cm, which was closed to the proper planting distance 14 cm. 4. The percentage of missing hill was lowest(1.5%) in the plot of elapsed time 2 days and plowing depth 12 cm. 5. The planted depth in the plot of the elapsed time 2 days and plowing depth 15 cm was 2. 95 cm, which was closed to the proper planting depth 3 cm. 6. The angle of planting postures in the plot of elapsed time 2 days and plowing depth 12 cm was $89^{\circ}$, which was closed to the desirable posture angle $90^{\circ}$. 7. The deviation from the straight transplanting line was lowest in the plot of the elapsed time 2 days and plowing depth 12 cm. 8. From the results above mentioned, it is recommended that the field condition under the elapsed time 2 days and plowing depth 12 cm is the most favorable one for the working performance of rice thansplanter.

Dental Hygienists Work on the Impact of Factors Associated with Musculoskeletal Pain (치과위생사 작업과 관련된 근골격계 통증의 영향요인)

  • The purpose of this study was to work related musculoskeletal disorders are a major. Occupational disease of the dental care profession is no exception. The survey was self-reported questionars of 300 dental hygienists that 268 dental hygienists reply to self-reported survey. This study results are as follows: Subjects of research analyzing the degree of physical musculoskeletal disorders pain, shoulder 90.3%, neck 89.2%, leg 83.6%, 81.7% back, hand/wrist/fingers 75.7%, arm/elbow, according to 52.8%. Therefore the work province of the research object people the musculoskeletal disorders appeared different. Generally characteristic was taller dental hygienists lower back pain and were out of less weight, study subjects had neck and arm pain. 29~33 year-old age the shoulder, over the age of 34 the arm/elbow to be high (p<0.05). Working environment to become a career, the more hand/wrist/fingers and the pain increased (p<0.05). The neck, shoulders (p<0.05), arm (p<0.01), waist high in the 3~4 years experience. And leg/foot was in the 1~2 years experience. This increase in working hours had increased pain in the neck but the hand/wrist/finger pain in the small hours of experience in the high pain(p<0.01). Conclusion of the musculoskeletal disorders of the dental hygienists often than the average for this risk is recognized. When it occurs early in treatment can be simple, but time is left to revert to normal when you do not already. Therefore, maintaining proper posture and dental hygienists, pain or fatigue appeared to accumulate immediately treated continued efforts are needed.
