Korea paid attention to the work injury insurance in the early economic development stage as other developed countries did. Thus "The industrial Accident Compensation Insurance Law" was legislated in 1963 and it is now expanded to apply to the workplace with five or more employees. Beside the low coverage of the number of employees, the scope of benefits are the problem of work injury insurance. The work injury insurance's main benefits are for the loss of income and the medical cost caused by occupational accidents. On the other hand, the investment on the prevention accident and the medical, social and employment rehabilitation is very low, even though the occupational accident pension is higher than that in other OECD countries. Practically lay-off benefits and survivor' benefit for some special workers are paid more than they need, therefore they want to remain under the benefit condition and the medical care days are longer. This paper indicates the problems of work injury insurance and suggest how to improve the functions of it.
The aim of this study is to explore the factors that influence the health care utilization of disabled workers who returned to their original workplace after an occupational accident. The study sample consisted of 457 workers who suffered from occupational accidents and were rated as disabled based on the data from the PSWCI in 2021. Data analysis was performed using a hierarchical multiple regression analysis with SPSS WIN 26.0. The study findings revealed that the number of outpatient visits was affected by factors such as daily wage payment, economic status, physical activity limitations, subjective health status, and chronic diseases. Moreover, health care expenditures were determined by factors such as having a caregiver for daily living, burn injuries, and physical activity limitations. Therefore, to increase the accessibility of health care utilization for disabled workers, the role of nurses in the workplace should be expanded, employers should include a health care utilization plan when developing a return-to-work plan, and research on the introduction of a paid sick leave system that allows workers to rest when they are sick is needed. It is also necessary to alleviate the economic burden by increasing the coverage of benefits for burn and rehabilitation treatments.
Background: According to the previous studies, the work-related accident rate decreased in Korea after the introduction of occupational health and safety management system (OHSMS), but there were several disasters in Korea such as subway worker's death at Guui station in 2016 and the Taean thermal power plant accident in 2018, which escalated the social demand for safety. In 2018, OHSMS became an international standard, as ISO45001 was announced. Methods: A survey was conducted to research the implementation status of OHSMS and changes in people's perception, and the results were compared with those of a past survey. Results: Enhanced social demand and various stakeholders' (not only buyer) needs, and social responsibility are perceived as the motivation for the introduction of OHSMS rather than legal compliance or customer demand. In the questionnaire about problems with the implementation of OHSMS, the factors with higher response rate in 2018 than 2004 were "excessive cost" and "complicated documentation management." In the questionnaire about how to promote OHSMS in organizations, most people answered "reduction of workers' compensation insurance rate" in 2004, but most people answered "exemption from health and safety supervision" in 2018. Conclusion: For the effective implementation of ISO45001, emphasis is placed on social demand, training to recognize health and safety as a part of management, and the reduction of certification and consulting costs to promote the introduction of OHSMS. Incentives such as insurance premium cuts and exemptions from health and safety supervision are needed.
Purpose: The purpose of this study was to examine the effects of a day care rehabilitation program managed by nurses on physical and emotional function of patients with job-related injuries. Method: A one group pre-test and post-test quasi-experimental design was used. Thirty patients participated in a day care rehabilitation program and 9% of those were unable to complete the 16 weeks program due to absence. The physiotherapist, occupational therapist, and clinical psychologist offered the day care rehabilitation program, 5 times a week for 16 weeks. Outcome measures included physical and emotional function. Results: The program participants had significantly greater motor function (t=-2.85, p=.008) and activity of daily living (t=-5.34, p<.001), and lower depression (t=5.20, p<.001), state anxiety (t=4.71, p<.001), and trait anxiety (t=4.40, p<.001). Conclusion: The nurse managed day care rehabilitation program significantly improved physical and emotional function in patients with job-related injuries. The program should be further tested in a larger sample to validate the findings.
Purpose: This study was to examine the cardiopulmonary symptoms, quality of sleep, and depression, and to identify the influencing factors in the hospitalized coal worker's pneumoconiosis patient. Methods: 137 hospitalized patients who received pneumoconiosis treatment were conveniently selected. A descriptive correlational study was conducted. Data were collected using structured questionnaires and were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Results: The mean score of cardiopulmonary symptoms was relatively high ($3.97{\pm}0.56$, out of 5). The 5 and over score of quality of sleep, sleep disturbance was 94.2%, and the reported depression was 95.6%. The levels of depression depended on the oxygen therapy and family/acquaintance visiting. The depression was significantly correlated with the cardiopulmonary symptoms and quality of sleep. The influential factors affecting depression were cardiopulmonary symptoms and quality of sleep, which explained about 34.1% of the variance. Conclusion: The results of this study indicate that nursing interventions are needed to reduce depression, and to improve cardiopulmonary symptoms and quality of sleep in the hospitalized pneumoconiosis patient. These results can provide for nursing intervention to facilitate reduction of depression.
Objectives: The purpose of this study was to identify the health promotion activities of the elderly Korean aged 65 or older and to examine the related factors associated with the health promotion activities. Methods: Data were obtained from 2008 Social Statistics Survey of Korea National Statistical Office of 6,207 people aged 65 or older. We measured the socio-demographic characteristics, physical health status, social health status, and health promotion activities. Statistical analyses were employed through the $X^2$-test and Odds ratio using Logit Model. Results: In our study, health promotion activity practice rates were varied among the socio-demographic characteristics, physical and social health status. Our findings also support that better socio-demographic and physical health status explain the higher practice rates of health promotion activities. In addition, the higher social health status was associated with better practice rates of health promotion activities. Conclusion: We found that the health promotion activities of the elderly could be encouraged by better socio-demographic status and physical and social health status. To better accomplish the health promotion for the elderly in our community, policy-makers should need careful political deliberation for executing health promotion services considering the distinctions of programme and target groups.

Objective: The objective of this study was to estimate the numbers of employees occupationally exposed to noise according to their industry and size. Methods: A Delphi panel consisting of 15 occupational health experts estimated the exposure prevalence of noise in workplace. Data on Industrial Accident Compensation Insurance provided from Occupational Safety and Health Research Institute were combined to produce the exposure population. Results: In Korea, 16.0% of employees, 2,539,890 out of 15,838,926, was estimated to exposed to noise occupationally. The rate was 32.7% and 10.3% in manufacturing sector and non-manufacturing sector, respectively. The highest rate, 52.5%, was found in manufacturing industries of wood and of products of wood and cork(except furniture) and of other transport equipment. Sorted by their size of business, the rate was higher as the number of employee was larger in manufacturing sector. Conclusions: Compared to the same rate estimated in the US, 17.2%, the result of this study seems to be in a resonable range.
