Model Development of Nursing Care System for Women's Health : Based on Nurse-Midwifery Clinic (여성의 건강을 위한 간호전달체계 모형개발 - 조산원 중심으로 -)

  • The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.

Multidimensional Factors Influencing Health of Low-Income Women: The Moderating Effect of Social Support Resource (저소득층 여성의 건강에 영향을 미치는 다차원적 요인 : 사회적 지지 자원의 조절효과)

  • This study aims to figure out factors influencing health and moderating effect of social support resource between those factors and health of low income women in diverse spheres. The data of low income women were 239 cases and were utilized SPSS ver. 21 program. The main results of this study are as follows: firstly, health of low income women is identified medium level. Secondly, education level, self-efficacy and social support resource are related to the self-rated health of low income women. Especially, social support resource functions as moderators on between education and self-rated level of the health of low income women. Consequently, the findings suggest the need to expand the social intervention range to promote health of low income women from cost benefit to psychological support such as capacity building for self-management and establishment of safety-net to extend social support. And subsequently, the time attribute of the factors related to low-income women's health and the diverse characteristics of the subjects should be considered.

  • Japanese imperialism initiated the Sino-Japanese War in 1937 and promulgated the Total National Mobilization Act in 1938, establishing a wartime total mobilization system. In the beginning of the wartime general mobilization system, Japanese colonialism focused on women's labor force and mobilized women both domestically and abroad. Women were forcibly mobilized to the Labor Patriotism Unit and Jeongshindae(Korean Women Labor Corps). Women had to take the place of home work as well as the work of men who had already been mobilized, and at the same time faced a poor situation of being forced to mobilize for war. The mobilization of Jeongshindae took place in various forms, such as recruitment, voluntary support by government offices, propaganda through schools or groups, job fraud, coercion or threats. Jeongshindae which was a representative victim of the forced mobilization of women during the Japanese colonial period, was individually litigated and remains an unresolved problem. In order to uncover the reality of the forced mobilization of women during the wartime general mobilization system, continuous research and social education through related organizations are required.

Knowledge and Attitude Regard to Cord Blood of Early Postpartum Women after Donating Cord Blood or Storing Cord Blood (제대혈 기증.보관한 산부의 제대혈 관련 지식 및 태도)

  • Purpose: There is increasing necessity of using cord blood cell worldwide. In order to improve the effectiveness of cord blood's usage in Korea, the study of knowledge and attitude regarding cord blood is needed. Method: As descriptive correlation study, data was collected from 212 early postpartum women who stored or donated cord blood from 2007 April to July. Data was analyzed using $X^2$-test, t-test, ANOVA, and logistic regression. Result: Total score of knowledge and attitude of early postpartum women who donated cord blood was higher than who stored. According to the source of education and the source of influence toward decision of using cord blood, early postpartum women who educated or influenced by mass media showed difference in attitude. Job status and income, and attitude of cord blood were the predictor of cord blood donation. Conclusion: Attitude regard to cord blood affect to usage of cord blood, and the attitude and knowledge of cord blood in early postpartum women who donated it higher than who stored.

Infertile Women's Perception on the National Support Program for Infertile Couples (난임 여성들의 난임부부 시술비 지원사업에 대한 인식)

  • Purpose: The purpose of this study was to explore the perception of infertile women on the use of the national support program that provides medical expense aid to infertile couples. Methods: Thirty Korean infertile women participated in five focus groups. Data were collected from January to August 2014. After obtaining permission from the participants, each session of the focus group was audio-taped and transcribed. The responses were analyzed using qualitative content analysis. Results: The main themes identified from the sessions with the participants were "feeling thankful for the reliable support program," "feeling happy or unhappy," "enduring inconveniences," and "hoping for a more comprehensive support service." Although most of the participants perceived the benefits of the national support service positively, they stated that the service was not comprehensive because it did not cover all the medical expenses for tests and other medical treatments. Conclusion: The benefits given to infertile couples by the program should be increased by covering all the medical expenses, expanding its criteria to include more eligible candidates, and by including special leave benefits for working women. Furthermore, it is essential to take measures for infertility prevention at the individual and national policy levels.

A Study on the Stress and Coping with Farm Work of Rural Women (여성농업인의 농작업 스트레스와 대처방법에 관한 연구)

  • The objectives of this study were to understand the working stress of rural women and their coping strategies, and to provide basic data related with rural welfare policy. The data were obtained through 318 samples of rural women under sixty years old who work full-time at their farms with their husbands. The questionnaire employed Likert-type scale with four-point, and then the data were analyzed by statistical methods of frequency, percentage, mean score, analysis of variance, t-test and Duncan test through SAS program. The significance level was p < .05. The results of this study are as follows ; 1) The average stress level of rural women was over 2.9 points out of 4. 2) The stress level showed meaningful correlation with such variables as age, education, perception on economic status, and recognition by their husbands and other family members. The rural women who are over forty years old, below high school, disregarded from their family members were marked high stress level. 3) The level of rural women's stress coping strategies was quite as low as 1.95 out of 4. 4) The level of coping strategies showed meaningful correlation with such socio-demographic variables as age, perception on economic status and job satisfaction level. Those who aged over 40, perceives low economic staus, lives together with husbands'parents, and shows “average” job satisfaction scored significantly lower coping strategy levels. 5) The level of stress didn't indicate significant relation with the type of coping strategy.

Effects of a Coaching-based Childbirth Program on Anxiety and Childbirth Self-efficacy among Primigravida Women (코칭 출산교육 프로그램이 초임부의 불안과 출산 자기효능감에 미치는 효과)

  • Purpose: Childbirth self-efficacy plays an important role in women's ability to cope with labor and delivery. Coaching has been gaining popularity as a way to promote cognitive, emotional and behavioral change. This study aimed to test the effects of a Coaching-based childbirth program on anxiety and childbirth self-efficacy among primigravida women. Methods: The study design was a quasi-experimental pre-post design. A coaching-based childbirth program was developed synthesizing concepts and techniques from the literature on coaching and was verified by an expert panel. It consisted of four weekly 2-hour small group sessions. Pregnant women were recruited from H hospital in Seoul. Childbirth self-efficacy and state anxiety were measured before and after the program. Results: Although there was a no significant reduction in anxiety, there were statistically significant increase for childbirth self-efficacy in the experimental group when compared to the control group. Conclusion: The program appears to increase childbirth self-efficacy for pregnant women. Future studies may benefit from using mixed coaching modalities and consider measuring health behaviors and obstetric outcomes to gain insights on its long-term impact.

Inverse association of improved adherence to dietary guidelines with metabolic syndrome: the Seoul Metabolic Syndrome Management program

  • BACKGROUND/OBJECTIVES: The Seoul Metabolic Syndrome Management (SMESY) program is a 1-yr lifestyle modification program targeting metabolic syndrome (MetS) in Seoul residents. This study investigated the associations between adherence to dietary guidelines and MetS among the SMESY program participants. SUBJECTS/METHODS: Data of 54,385 participants aged 20-64 yrs who completed the SMESY program in 2015, had information on adherence to dietary guidelines, and were not medicated for diabetes, hypertension, or dyslipidemia were analyzed. Participants underwent MetS screening and completed a lifestyle questionnaire including adherence to 10 dietary guidelines before and after participation. Participants were classified according to the number of MetS risk factors at baseline (MetS group, ≥ 3; risk group, 1-2; healthy group, none). Adherence to dietary guidelines was determined from the number of "yes" responses regarding the fulfillment of each guideline on ≥ 5 days/week. Multiple logistic regression was used to evaluate associations between newly diagnosed MetS and changes in adherence to dietary guidelines. RESULTS: In the MetS group, MetS prevalence decreased after the SMESY program (men, -41.9%p; women, -48.7%p), and all risk factors were significantly improved (P < 0.0001). All groups exhibited improved adherence to all dietary guidelines after participation (P < 0.0001). In the MetS group with positively changed adherence scores, the MetS prevalence decreased by -44.1%p for men and -49.5%p for women, whereas the prevalence in those with negative changes decreased by -38.1%p for men and -48.6%p for women. In the risk group, those with positively changed adherence scores had significantly decreased odds ratios (ORs) for newly diagnosed MetS compared with those with negative changes (OR, 0.70; 95% confidence interval [CI], 0.61-0.80 for men; OR, 0.88; 95% CI, 0.79-0.99 for women). CONCLUSIONS: The SMESY program may effectively reduce the risk of MetS among adults with risk factors by improving adherence to dietary guidelines.

A Study on Improvement of Import Insurance for Importers

  • South Korea, despite the financial turbulences has marked the trade volume of USD 1 trillion and marked $7^{th}$ largest exporter in 2013. Approximately 40% of the entire import of Korea was for export. In the South Korean trade structure, import and export are closely related, requiring proactive import financial assistance policies. Recognizing this, K-sure has made import insurance policy available on the market since July 2010. The K-sure insurance policy targets both financial institutions and importers. This is the reason why this research seeks to review the import insurance terms for importers and compare with foreign import insurance products to find out ways to improve. K-sure's import insurance for importers is to cover a loss of a policyholder when the policyholder or importer domestically addressed made a prepayment but cannot receive goods. The import insurance is applied to import transactions of goods or resources. K-sure's import insurance coverage needs to be expanded to intermediary trade and consignment processing trade, etc. In this sense, a more systematic educational program should be introduced about K-sure's import insurance.

A study of the relationship between oral health behaviors, use of dental clinics and periodontitis (구강건강행태 및 치과의료서비스 이용과 치주질환과의 관련성)

  • Objectives: This study was designed to examine the effects of oral health behaviors and use of dental clinics on periodontal diseases in women, the purpose of this study is to provide basic information to develop program for dental health promotion in young women. Methods: We conducted a survey targeting 486 women that agreed with the survey and were going to dental clinic from October 1, 2013 to October 18, 2013. We surveyed them by self-administered questionnaire and analyzed it by SPSS Win 19.0 program. Results: 1. For differences in periodontitis according to general characteristics, if th age is high, ratio of periodontitis is high. Married woman is higher than single one in ratio of periodontitis. Also the more number of birth, the higher ratio of periodontitis is.(p<.05) 2. For differences of periodontitis by dental care behavior, ratio of periodontitis of non-smokers is significantly low. For one that brush one's teeth broadside, the ratio of periodontitis is high.(p<.05) 3. For differences of periodontitis by use of dental clinic service, ratio of periodontitis of one that have more times visit to a dental clinic, more annual average cost for dental care, and regular dental check-ups is relatively low.(p<.05). 4. For determinant factors influencing on periodontitis, ratio of periodontitis of one that is old, have more stress, have a low level of education, and have preventive dental care is high. Ratio of periodontitis of one that have less toothbrushing and brush one's teeth after having lunch or a snack and before sleeping is significantly low. Conclusions: From this study, dental health behavior as the factor influencing on periodontitis of young women is statistically meaningful. Because this dental health behavior has a lot of potential to be improved by government and local community efforts such as education and social support, we think that systematic and various educational program development is needed to strengthen self dental care ability effectively with policy support.
