The Concept of Wind in Traditional Chinese Medicine

  • The use of folk medicine has been widely embraced in many developed countries under the name of traditional, complementary and alternative medicine (TCAM) and is now becoming the mainstream in the UK and the rest of Europe, as well as in North America and Australia. Diversity, easy accessibility, broad continuity, relatively low cost, base levels of technological inputs, fewer side effects, and growing economic importance are some of the positive features of folk medicine. In this framework, a critical need exists to introduce the practice of folk medicine into public healthcare if the goal of reformed access to healthcare facilities is to be achieved. The amount of information available to public health practitioners about traditional medicine concepts and the utilization of that information are inadequate and pose many problems for the delivery of primary healthcare globally. Different societies have evolved various forms of indigenous perceptions that are captured under the broad concept of folk medicine, e.g., Persian, Chinese, Grecian, and African folk medicines, which explain the lack of universally accepted definitions of terms. Thus, the exchange of information on the diverse forms of folk medicine needs to be facilitated. Various concepts of Wind are found in books on traditional medicine, and many of those go beyond the boundaries established in old manuscripts and are not easily understood. This study intends to provide information, context, and guidance for the collection of all important information on the different concepts of Wind and for their simplification. This new vision for understanding earlier Chinese medicine will benefit public health specialists, traditional and complementary medicine practitioners, and those who are interested in historical medicine by providing a theoretical basis for the traditional medicines and the acupuncture that is used to eliminate Wind in order to treat various diseases.

동의보감(東醫寶鑑) 중(中) 세신(細辛)이 주약(主藥)으로 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察) (A Study on Application of Radix Asari Main Blended Prescription From Dongeubogam)

  • This report describes the remedial fields, symptoms, pathology, dosage, prescriptional constitution of 69 studies related to the use of Radix Asari main blended prescriptions from Dongeubogam. The following conclusions were reached through investigations on the prescriptions that use Radix Asari as a key ingredient. Radix Asari blended prescriptions are utilized for 14 therapeutic purposes, for example, in symptoms of tooth, eye, paralysis, ear-nose, skin infection and carbuncle. In particular, 14.49% of the prescriptions appear in the chapter of tooth, and 10.14% of those appear in the chapter of eye. Prescriptions that utilize Radix Asari as the main ingredient are used in the treatment of dental disease, othalmology, nose and ear disease, throat disease, common cold, dermothology, paralysis, musculoskeletal disease, disease of the head, asthma, genital disease, the injury of the cuts and they are also used for treating 50 different types of diseases. Radix Asari is used in Six pathogenic factors such as wind, wind-cold, wind-heat, wind-dampness heat, wind-dampness, dampness, paralysis, cold-chill, cold, dryness, in visceral pathogenic factors such as stomach, kidney, liver-kidney, liver and in chi-blood pathogenic factors such as chi-blood deficiency, chi-blood stagnation and in extravasated blood pathogenic factors such as impact injury, an injury such as cuts, blood accumulation as well as weakness-cold and parasites. The actual amount of Radix Asari blended has ranged at a wide variety of amounts from 2.5pun(about 0.94gram) to 3don(about 11.25gram). 26.09% of the prescriptions used 1don(about 3.75gram). The function of Radix Asari is to calm down, to get the consciousness back, to bore through, to warm up the meridians and to run it smoothly, to discharge of phlegm from the combination of drugs and prescriptions.

진전 변증에 관한 문헌적 고찰 - 중국 논문 중심으로 - (Literature Review on Syndrome Differentiation of Tremor, Focusing on Chinese Journals)

  • Background : It was not enough to apply three kinds of syndrome differentiation in our oriental medical textbook to tremor's treatment according to reports of Korea and traditional medical textbook of China. Objective : To investigate syndrome differentiations by types of diseases related to tremor through Chinese journals review and to suggest adding possible syndrome differentiations. Methods : Literature search was performed using China Academic Journal (CAJ), the search engine of China National Knowledge Infrastructure (CNKI) from January 1994 to December 2009. Searching key words were Chinese characters in combination meaning tremor, paralysis agitans, and syndrome differentiation. We included all types of articles that explained or referred to definite syndrome differentiations. The symptoms and oriental medications by syndrome differentiation in selected articles were extracted and summarized. Results : 56 Chinese journals were ultimately selected. 37 kinds of syndrome differentiations about tremor were investigated, which included dual deficiency of qi and blood (氣血兩虛) quoted 31 times, liver-kidney yin deficiency (肝腎陰虧) 23 times, liver-kidney deficiency (肝腎不足) 21 times, and phlegm-heat stirring wind (痰熱動風) 20 times. 37 kinds of syndrome differentiation could by group into eight types, such as liver-kidney yin deficiency (肝腎陰虧), dual deficiency of qi and blood (氣血兩虛), phlegm-heat stirring wind (痰熱動風), heart deficiency and spirit weakness (心虛神弱), blood stasis due to qi stagnation (氣滯血瘀), sea of marrow deficiency (髓海不足), liver qi depression (肝氣鬱結), and liver yang transforming into wind (肝陽化風). Conclusion : We suggest that the syndrome differentiations of tremor, such as heart deficiency and spirit weakness (心虛神弱), blood stasis due to qi stagnation (氣滯血瘀), sea of marrow deficiency (髓海不足), liver qi depression (肝氣鬱結), and liver yang transforming into wind (肝陽化風), can be added to liver-kidney deficiency (肝腎不足), dual deficiency of qi and blood (氣血兩虛), and phlegm-heat stirring wind (痰熱動風) of the textbook. Further systematic research will be needed on them.

동의병리학에서의 병리전변이론(病理傳變理論) 고찰 및 교육의 필요성 (Investigation of the Transforming Theories of Diseases and the Need of Teaching Them in the Pathology Class)

  • In order to investigate the meanings and bases of transforming theories of diseases(病傳論), several relevant theories were collected from medical books since . The outside-to-inside changing process(pyoree jeon) was the fundamental pattern in febrile diseases regardless of slang han and wen bing after Han dynasty. But in case of numb disease the three exogenous pathogenic factors of wind, cold and dampness got into each viscera through five tissues like skin, vessesl, flesh, tendons, bones respectively. It was called corresponding changing process(sanghap jeon) here. The pathogenic changing process(byungsa jeon) had complicated details to explain pathologic processes, but could give useful informations on evaluating relative strength of pathogens and tendency ahead. The changing process of diseases of internal injuries were explained in case of emotional distress which is outbroken abruptly didn't follow regular order of viscera-emotion relations. So it was named random changing process(bulcha jeon) after 's usage. And marasmus and asthenic disease followed top-to-down or down-to-top changing process(sangha jeon) based on . There are many types of changing process of diseases between viscera and viscera or viscera and bowels like generation changing process, restriction changing process, changing process between couple, changing process of mutual transmission based on attributions of each element by the theory of 5 phases(五行論). And changing process to a neighbor doesn't have any special relations within two organs but has anatomical contiguity and physiological continuity between them. The transforming theories of diseases bring forth useful understanding on comings and goings of pathogenic factors and tendency and prognosis of disease, so they are needed to be taught in the course of pathology class.

각기(脚氣)에 대한 고찰(考察) -Beriberi와의 비교를 중심으로- (A Study on Gak-Gi(脚氣) in Comparison with Beriberi)

  • Objectives :The Korean Medical concept Gak-Gi(脚氣) is understood as being identical to Beriberi and Vitamin B1 deficiency, without the necessary investigation that is required in such identification. Therefore this study aims to systematically compare these two concepts for thorough investigation. Methods : Research was carried out in two directions. First, the cause, symptoms, treatment principles of Gak-Gi(脚氣) in the medical texts were analyzed. Second, the process of the Beriberi theory development and its identification with Gak-Gi(脚氣), medical information on Beriberi, and the basis of various diseases that have been linked to Gak-Gi(脚氣) were studied. Results of the two directions were used to compare Gak-Gi(脚氣) and Beriberi. Results : Gak-Gi(脚氣) and Beriberi overlap in many aspects. However, unlike Beriberi which is understood as nutrient deficiency essentially, Gak-Gi(脚氣) can be caused by exterior pathogens such as wind toxin, while its different manifestations and treatment principles cannot be fully explained by nutrient deficiency. Conclusions : Gak-Gi(脚氣) could be understood as a type of syndrome in which causes and symptoms are connected, where symptoms start in the legs, moving upwards. It cannot be identified with Beriberi, which has been interpreted as a single disease.

중풍 환자 변증과 기후 요소와의 상관성에 관한 연구 (The Study of Correlation between Pattern Identification of Stroke Patients and Meteorological Elements)

  • There are many reports about correlations between meteorological elements and stroke. In Oriental medicine, it is recognized that the weather affects the human body and diseases, but there are few studies about the correlation between meteorological elements and pattern identification of stroke. 105 stroke patients classified into fire-heat pattern or dampress-phlegm pattern were registered during the study period. We took the measurement of each meteorological element (atmospheric pressure, temperature, humidity, wind speed) according to pattern identification and analyzed pattern identification into two groups according to mean of each meteorological element during the study period. Mean temperature was higher with the heat-fire pattern than with the dampness-phlegm pattern. Heat-fire pattern also had higher frequency when temperature was higher than mean temperature. There was no correlation between atmospheric pressure, relative humidity, or wind speed and pattern identification.

  • In "Hwangjenaegyeong(黃帝內經)", the name of blood vessels is closely related to images of the river, and the word for blood vessel contains the following meaning, such as blood vessel, blood wind as well as blood stasis and so on. In a physiological perspective, the unobstructed blood vessel and normal operation blood are important symbols of health, for blood is the basis for human survival, physical activity and mental activity. In pathology, the external cold, excessive seven emotions and a biased diet can cause many kinds of abnormalities of the blood vessels, which can in turn become causes for a variety of diseases. The theory of blood vessels in "Hwangjenaegyeong" can provide guidance for diagnosis and treatment of cardiovascular diseases in practice.

치(齒)에 대(對)한 문헌적(文獻的) 고찰(考察) (A Literature Study of the Teeth)

  • The purpose of this study was to investigate the relationship between the teeth and Zhang-Fu(臟腑), dental diseases, and the hygiene of the mouth through the literature of oriental medicine. First, the relatonship between the teeth and Zhang-Fu is reviewed as follows: The teeth are influenced by Shen(腎) because they are the end of bone and Biao(標) of Shen. Gingiva is related to Wei(胃) and Da-Chang(大腸) because it is passed by Yangming-Channel(陽明經). The growth and nutrition of teeth depends on Shen. The pathological condition of Shen causes the gingival atrophy, the loose of teeth, dedentition due to aging, withering of teeth, and tartar: whereas the pathological condition of Wei and Da-Chang causes toothache, gingivitis, inflamed gums, bad breath, and gingival hemorrhage. Second, the causes and therapies of dental diseases through the literature can be summarized as follows: The major causes of toothache are the pathogenic condition of wind-heat and wind-cold, the heat syndrome of Wei, the damp-heat of intestine, flaring-up of fire of deficiency type, rotten tooth, etc... The principal causes of dedentition and the shaking and loose of teeth are the deficiency of Shen, and the rest of causes are the damp-heat of Yangming. Gingival atrophy is caused by the deficiency of Shen, whereas the gingival hemorrhage comes from the factors in the pathogenic factor of wind-heat of Yangming-Channel, the heat syndrome of stomach, and the deficiency of Shen. The causes of grinding of teeth during sleeping are stomach-heat, and the delayed dentition and the withering result from the deficiency of Shen-Jing.(腎精) The principal therapies of toothache are removing wind and heat, clearing away heat and prompting diuresis, clearing away the stomach-heat, replenishing vital essence to tonify the Shen, relieving superficial syndrome by wind-cold, and alleviating pain by destroying parasites. For the prescription of the principal therapies, there are Xijio Dihuang Tang, Jiajian Ganlu Yin, Qufeng Wan, Qingwei San, Tiaowei Chenggi Tang Shengong Wan, Liangge San Qingwei Tang Yunu Jian, Liuwei Dihuang Wan Zuogui Yin Bawei Wan Wanshao Dan, Xixin San Badou Wan Gianghuo Fuzi Tang, Jiuzi Tang Badou Wan, etc... The therapies of dedentition and the shaking and loose of teeth are replenishing vital essence to tonify the Shen, and warming and recuperating the Shen-Yang: as the prescription, there are Liuwei Dihuang Wana Zuogui Yin, and Bawei Wan Anshen Wan Wanshao Dan Yougui Wan etc... The therapies of gingival hemorrhage are clearing away the stomach-heat, replenishing vital essence to tonify the Shen, warming and recuperating the Shen-Yang(腎陽), and moisturing and purging intence heat with the prescription of Tiaowei Chenggi Tang Xijiao Dihuang Tang, Liuwei Dihuang Wan Zuogui Yin, Bawei Wan Anshen Wan, and Yunu Jian. The therapy of gingival atrophy is replenishing vital essence to tonify the Shen in the prescription of Liuwei Wan Bawei Wan Ziyin Dabu Wan. The therapies of grinding of teeth during sleeping are clearing away the stomach-heat and purging intense heat, and invigorating the spleen through eliminating dampness in the prescription of Qingwei San, Wumei Wan, etc... The therapy of delaed dentition is replenishing vital essence to tonify the Shen with the prescription of Liuwei Wan Buyin Jian, etc... Third, clinical treatment reports of dental diseases are reviewed as follows: The toothache due to stomach-heat was treated by medical herbs like Gypsum, Natrir Sulfas, Rehmanniae, Schizonepetal Herba, Menthae Folium, Cimicifugae Rhizoma, and Scrophulariae Radix. The therapies of toothache due to flaring-up of fire in deficiency type from deficiency of Shen provided with replenishment of vital essence to tonify the Shen and clean ministerial fire, and the prescription was the kind of Liuwei Wan, which worked very well. The therapy of dedentition and loose of teeth due to deficiency of Shen was done to stablize the teeth as tonifing the Shen with the prescription of Guchi Wan. The rate of imrovement was over 90%. The destruction of periodontal tissue due to periodonititis was cured of dispelling wind, reducing heat, and alleviating pain, It was improved by taking Zizhi Xingiong Tang, Guchi Xiaotong San, Yunii Jian, and Qingwei San about 3-7 days, and the rate of improvement was over 80%. Fourth, the prevention and regimens are reviewed as follows: As a physical and breathing exercise of the teeth, tapping teeth which stimulates the circulation of Qi(氣) and Xue(血) had been used. The tapping time of 14, 17, 36, etc... has been reported, and it should be applied based on the body condition. The medical herbs for gargling and brushing teeth have been used. Specifically, Cimicifugae Rhizoma, Gypsum, Gypsum Fibrosum, and Indigo pulrelrata Lereis have been used to reduce heat, Coptidis Rhizama and Yang Jinggu to eliminate damp-heat, Amomi Semen, Cyperi Rhizoma, Flos Caryophylli, Asari Radix, Piperis Longi Fructus, Santali Albae Lignum, Meliae Fructus, Moschus, Aquillaiae Lignum, and Borneol to promote the circulation of Qi and to relieve pain, Ligustici Radix, Angelice Radix, Rhizoma Nardostachydis, Tribuli Semen to relieve superficial syndrome by means of diaphiresis, and Cnidii Rhizoma, Angelicae sinensis Radix, and Olibanum to promote blood circulation to stop pain.

전염성 감염병에 대한 신속변증 시행을 위한 팔강복합증형 표준안 연구 (Studies on the Standard Measure of Compound Patterns of Eight Principles for Rapid Pattern Differentiation against Epidemic Contagious Diseases)

  • In order to secure practising rapid pattern(證, zheng) differentiation against acute infectious diseases like corona virus disease-19(COVID-19) showing rapid variation and contagion, a simplified classification of stages centering on the exterior-interior pattern identification with 2 step-subdivision by cold, heat, deficiency, excess pattern and pathogens is proposed. Pattern differentiation by compound patterns of 8 principles is made for the non-severe stage of general cold and the early mild stage of epidemic disease. Compound pattern's names of 8 principles about external infectious diseases are composed of three stages, that is disease site-characters-etiology. Based on early stage symptoms of fever or chilling etc., exterior, interior and half exterior and half interior patterns are determined first, and then cold, heat, deficiency, excess patterns of exterior and interior pattern respectively are determined, and then more concrete differentiation on pathogens of wind, dryness, dampness and dearth of qi, blood, yin, yang accompanied with constitutional and personal illness factors. Summarizing above descriptions, 4 patterns of exterior cold, exterior heat, exterior deficiency, exterior excess and their secondary compound patterns of exterior cold deficiency and exterior cold excess and so on are classified together with treatment method and available decoction for a standard measure of eight principle pattern differentiation.
