Real-time Interactive Control of Magnetic Resonance Imaging System Using High-speed Digital Signal Processors (고속 DSP를 이용한 실시간 자기공명영상시스템 제어)

  • A real time interactive controller (spectrometer) for magnetic resonance imaging (MRI) system has been developed using high speed digital signal processors (DSP). The controller generates radio frequency (rf) waveforms and audio frequency gradient waveforms and controls multiple receivers for data acquisition. By employing DSPs having high computational power (e.g., TMS320C670l) real time generation of complicated gradient waveforms and interactive control of selection planes are possible, which are important features in real-time imaging of moving organs, e.g., cardiac imaging. The spectrometer was successfully implemented at a 1.5 Tesla whole body MRI system for clinical application. Performance of the spectrometer is verified by various experiments including high- speed imaging such as fast spin echo (FSE) and echo planar imaging (EPI). These high-speed imaging techniques reduce measurement time, however, usually intensify artifact if there is any systematic phase error or jitter in the synchronization between the transmitter, receiver, and gradients.

  • A PET-MR system is particularly useful in diagnosing brain diseases. We have developed a prototype positron emission tomography (PET) system which can be inserted into the bore of a whole-body magnetic resonance imaging (MRI) system that enables us to obtain PET and MRI images simultaneously with a reduced cost. Silicon photomultipliers (SiPM) are appropriated as a PET detector at PET/MR system because detectors have a high gain and are insensitive to magnetic fields. Despite of its improved performance compared to that of PMT-based detectors, there is a problem of the photo-peak channel shift which is due to the increase of the temperature inside the ring detector. This problem will occur decreasing sensitivity of the PET and image distortion. In this paper, I quantitative analyze parameters of the KAIST SiPM depending on temperature by experiments. And I designed cooling methods in consideration of the degradation of sensors for correction of the temperature in the PET gantry. According to this research, we expect that distortive images and degradation of the sensitivity will not be occurred with using the above idea to reduce heat even if the PET system operates for a long time.

The New Finding on BOLD Response of Motor Acupoint KI6(照海) by fMRI (fMRI를 이용하여 수지굴신운동(手指屈伸運動)과 조해(照海)(KI6) 자침(刺鍼)에 의(依)한 대뇌운동피질(大腦運動皮質)의 활성변화(活性變化)에 관(關)한 비교(比較) 연구(硏究))

  • Introduction : Recent studies Suggested that there is a strong correlation between acupuncture stimulation and its related cortical activation. Anther study showed that either positive or negative BOLD effects could be observed depending on anatomical structure in acupuncture stimulation. In ttis study, we investigated a new acupoint $KI_6$ (照海), which was known as motor-related acupoint and obtained an evidence that the stimulation of $KI_6$ resulted in either negative or positive BOLD response to stimulation. Methods & Results : 1. Subjects and paradigms : Two separate stimulation paradigms were performed on five healthy (aged 22-23 yrs) in this study. First, the paradigm of acupuncture stimulation was that the acupuncture needle was inserted in acupoints $KI_6$, which is located in lateral side of the foot and then continuously twisted(補瀉를 除外한 捻轉法) for 70 seconds for 10 cycles of activation. During rest period (70 seconds), the needle was completed removed from acupoint. Total 60 cycles were performed and 10 images were obtained per cycle. Second, nonacupoint was randomly selected and the same paradigm was performed as acupoint stimulation. The stimulation protocol comprised 10 cycles of alternating. activation and rest (10 images per cycle). Total 60 cycles were performed and each cycle take about 1.5 sec for motor task. Subjects take an at least 15 minutes break before starting anther paradigm. 2. fMRI mapping : Multi-slice functional images were obtained on a 1.5T Magnetom Vision MRI scanner (Simens Medical, Erlangen, Germany) equipped with high performance whole-body gradients. The BOLD T2 * - weighted images were acquired with acho planar imaging sequence (TR = 1.2 sec, TE = 60 msec, and flip angle = $90_{\circ}$). The other sequence parameter are : FOV = 210 mm, matrix=$64{\times}128$ or $64{\times}64$, slice number=10 and slice thickness = 5 or 8 mm. the anatomic images were obtained with Spin-echo T1-weighted images. The resulting images were then anaiyzed with STIMULATE (CMRR, U. of Minnesota) to generate functional maps using a student T-test (p < 0.005) and cluster analysis. Both positive and negative response were evaluated. Conclusions : We have observed the activation of the motor cortex by stimulating motor-related acupoint ($KI_6$). Among five subjects, negative BOLD response was shown in four and positive response in one. All subjects showed positive response to conventional finger flexion-extension task. To understand the detailed mechanisms of correlation between acupuncture stimulation and BOLD fMRI changes and two typs of response, further study strongly required.

  • The pulsatile nature of blood flow makes artefacts in 2D Fourier transform image. Spatial presaturation is known to be effective in eliminating flow artefacts when the spin echo acquisition is employed. However. this method requires additional RF pulse and spoiling gradient for presaturation. In this paper a new flow saturation technique which does not require additional saturation-RF and gradient is proposed. The proposed technique is equivalent to the existing saturation technique but the elimination of the flow component is achieved by a pair of tailored $90^{\circ}-180^{\circ}$ RF pulses in tile spin echo sequence. By use of two tailored RF pulses with opposite phase polarity, a linear phase gradient is generated for those moving materials and consequently all the spins of moving materials become dephased thereby no signal is observable. Computer simulations and experimental results obtained using both a phantom and a human volunteer with a 2.0 T whole body system are also presented.

  • In this paper, we describe a field measurement technique using multiple gradient echo method. For this purpose, 3 differential phase map is used between two images obtained in a single scan. The phase map is further processed to unfold phase folding (2n $\pi$). The inhomogeneity map is used to correct image distortion along readout gradient direction. Some experimental results obtained at 1.0 Tesla whole body MR system are shown with the proposed technique.

자기공명영상에서 신호의 dynamic range와 quantization noise 분석

  • 목적: 고자장 자기공명영상 시스템 등으로 신호대잡음비가 향상됨에 따라 데이터 측정에서 analog-to-digital converter (ADC)의 quantization noise 가 중요한 시스템 사양으로 부각되고 있다. 특히 자기공명영상은 공간주파수 영역에서 데이터를 측정하기 때문에 dc와 ac간의 신호 차이가 매우 크며, 이러한 dynmic range는 3-D 영상에서 더욱 커진다. 본 연구에서는 다양한 자기총명 영상기법 및 실험 파라미터에 따른 신호의 dynamic range와 ADC의 bit 수에 따른 quantization noise를 살펴봄으로써, 주어진 시스템에 적합한 ADC의 bit 수를 분석하고자 한다. 대상 및 방법: 펄스 시퀀스의 종류, 파라미터, 2D/3D 등에 따른 각 신호의 크기를 수학적으로 모델링하여 신호의 크기를 예측하였다. 또한 whole body MRI 시스템에서 실험을 통하여 신호의 크기를 비교하였다. ADC의 quantization noise를 실험과 시뮬레이션을 통하여 살펴보았다. 시뮬레이션은 test 영상을 Inverse FFT 하여 spatial frequency domain data를 만든 후, 다양한 bit 수의 ADC로 quantization을 한 후 다시 영상을 재구성하였다. 재구성된 영상과 원영상 간의 error가 quantization noise가 된다. 또한 이러한 error가 주파수 영역에서의 error 값과 일치하는지를 확인하였다.

The effects of Mozart's music on metabolic response upon stress

  • Mozart's music has been suggested to affect spatio-temporal reasoning of listeners, which has been called "Mozart effect". However, the effects of Mazart's music on human metabolism have not been known. We dissected Mozart's music into its compositional elements and studied their effects on metabolism of experimental animals. Mozart music significantly reduced cortisol level induced by stress. NMR metabolomic study revealed different urine metabolic profile according to the listening to Mozart's music. In addition, each element of music exhibited different metabolic profile. Functional MRI study also showed enhanced brain activity upon listening to Mozart's music. Taken together, Mozart's music seems to be related with brain activity, stress hormone and whole body metabolism.

A Case of Gliosarcoma Associated with Neurofibromatosis Type 1 (제 1 형 신경섬유종증과 동반된 교육종)

  • A rare case of gliosarcoma with neurofibromatosis type I is presented. The patient was a 33-year-old woman who had headache and vomiting for one week. Multiple neurofibromas over her whole body with many cafeau- lait spots were present since childhood. At admission, she had no focal neurological deficit and ophthalmologic examination revealed bilateral Lisch nodules. Brain CT and MRI revealed a heterogeneously enhancing mass in the left fronto-parietal region with marked peritumoral edema and mass effect. The tumor was removed gross totally and a gliosarcoma was diagnosed histopathologically. Post operative course was uneventful with resolution of symptom, followed by radiotherapy with 60 Gy. A brief overview is given of this rare case together with the pertinent literature.

f-MRI with Three-Dimensional Visual Stimulation (삼차원 시각 자극을 이용한 f-MRI 연구)

  • Purpose : Instead of conventional two-dimensional (2-D) visual stimuli, three-dimensional (3-D) visual stimuli with stereoscopic vision were employed for the study of functional Magnetic Resonance Imaging (f-MRI). In this paper f-MRI with 3-D visual stimuli is investigated in comparison with f-MRI with 2-D visual stimuli. Materials and Methods : The anaglyph which generates stereoscopic vision by viewing color coded images with red-blue glasses is used for 3-D visual stimuli. Two-dimensional visual stimuli are also used for comparison. For healthy volunteers, f-MRI experiments were performed with 2-D and 3-D visual stimuli at 3.0 Tesla MRI system. Results : Occipital lobes were activated by the 3-D visual stimuli similarly as in the f-MRI with the conventional 2-D visual stimuli. The activated regions by the 3-D visual stimuli were, however, larger than those by the 2-D visual stimuli by $18\%$. Conclusion : Stereoscopic vision is the basis of the three-dimensional human perception. In this paper 3-D visual stimuli were applied using the anaglyph. Functional MRI was performed with 2-D and 3-D visual stimuli at 3.0 Tesla whole body MRI system. The occipital lobes activated by the 3-D visual stimuli appeared larger than those by the 2-D visual stimuli by about $18\%$. This is due to the more complex character of the 3-D human vision compared to 2-D vision. The f-MRI with 3-D visual stimuli may be useful in various fields using 3-D human vision such as virtual reality, 3-D display, and 3-D multimedia contents.

The Usefulness of F-18 FDG Whole Body PET in the Evaluation of Postoperative Recurrence of Cancer (수술 후 암 재발 판정에 있어서 전신 F-18 FDG-PET의 유용성)

  • The purpose of this study was to evaluate the usefulness of whole body F-18 FDG PET scan for detecting postoperative recurrence of cancer. One hundred four cancer patients after operation were enrolled(14 brain tumor, 15 head and neck cancer, 23 gynecologic cancer, 16 gastrointestinal cancer, 16 thyroid cancer, and 20 other cancers). Besides conventional images(CI) including CT and MRI, F-18 FDG PET scan was obtained on ECAT EXACT 47 scanner(Siemens-CTI), beginning 60 minutes after injection of 370MBq(10mCi) of F-18 FDG. Regional scan was also obtained with emission image. Transmission images using Ge-68 were carried out for attenuation correction in both whole body and regional images. Findings of PET, and CI were confirmed by pathology or clinical follow up. The sensitivity and specificity of PET for detecting recurrence were 94% and 92%, respectively. Contrarily, the sensitivity and specificity of CI were 78% and 68%. CI results were negative and PET results were positive in 11 cases. The biopsy or clinical follow-up of those cases confirmed recurrence of tumor. False negative cases of CI were frequent in patients with gynecologic cancers. Also we measured the serum concentration of tumor markers in patients with gynecologic cancer(CA125), thyroid cancer(thyroglobulin), and colorectal cancer(CEA). The sensitivity and specificity of tumor markers were 71% and 84%, respectively, We conclude that F-18 FDG PET can be used valuably in detecting recurrent foci of a wide variety of malignancy compared to conventional diagnostic methods.
