Gadolinium contrast agents (CAs) are integral components of clinical magnetic resonance imaging (MRI). However, safety concerns have arisen regarding the use of gadolinium CAs, due to their association with nephrogenic systemic fibrosis (NSF). Furthermore, recently the long-term retention of $Gd^{3+}-based$ CAs in brains patients with normal renal function raised another possible safety issue. The safety concerns of $Gd^{3+}-based$ CAs have been based on the ligand structure of $Gd^{3+}-based$ CAs, and findings that $Gd^{3+}-based$ CAs with linear ligand structures showed much higher incidences of NSF and brain retention of CAs than $Gd^{3+}-based$ CAs with macrocyclic ligand structure. In the current study, we report the in vivo biodistribution profile of a new highly stable multifunctional $Gd^{3+}-based$ CA, with macrocyclic ligand structure (HNP-2006). MR imaging using HNP-2006 demonstrated a significant contrast enhancement in many different organs. Furthermore, the contrast enhanced tumor imaging using HNP-2006 confirmed that this new macrocyclic CA can be used for detecting tumor in the central nervous system. Therefore, this new multifunctional HNP-2006 with macrocyclic ligand structure shows great promise for whole-body clinical application.
Background: Precise staging of lung cancer is usually evaluated by PET-CT and brain MRI. Recently, however, whole-body diffusion-weighted magnetic resonance imaging (WB-DWI) has be applied. The aim of this study is to determine whether the diagnostic performance of lung cancer staging by WB-DWI is superior to that of PET-CT+brain MRI. Materials and Methods: PET-CT + brain MRI and WB-DWI were used for lung cancer staging before surgery with 59 adenocarcinomas, 16 squamous cell carcinomas and 6 other carcinomas. Results: PET-CT + brain MRI correctly identified the pathologic N staging in 67 patients (82.7%), with overstaging in 5 (6.2%) and understaging in 9 (11.1%), giving a staging accuracy of 0.827. WB-DWI correctly identified the pathologic N staging in 72 patients (88.9%), with overstaging in 1 (1.2%) and understaging in 8 patients (9.9%), giving a staging accuracy of 0.889. There were no significant differences in accuracies. PET-CT + brain MRI correctly identified the pathologic stages in 56 patients (69.1%), with overstaging in 7 (8.6%) and understaging in 18 (22.2%), giving a staging accuracy of 0.691. WB-DWI correctly identified the pathologic stages in 61 patients (75.3%), with overstaging in 4 (4.9%) and understagings in16(19.7%), giving a staging accuracy of 0.753. There were no significant difference in accuracies. Conclusions: Diagnostic efficacy of WB-DWI for lung cancer staging is equivalent to that of PET-CT + brain MRI.
다발성 골수종(multiple myeloma)은 단일 클론에서 기원한 형질세포의 증식성 질환으로 비호지킨림프종에 이어 두 번째로 흔한 악성 혈액종양질환이다. 과거에는 'CRAB'이라는 임상 소견과 일반 촬영을 통한 골격계 조사(skeletal survey)에 의존하여 진단하였으나, 최근 International Myeloma Working Group의 개정된 가이드라인에 영상의학적 진단기준이 새로 제시되었다. 이에 병변의 조기 발견이 가능한 전신자기공명영상(whole-body MRI; 이하 WB MRI)의 임상적 유용성이 강조되고 있다. 또한 WB-MRI의 확산강조영상(diffusion-weighted image)과 딕슨 기법을 이용한 지방함량 정량화(fat quantification) 영상을 이용한 정확한 치료반응 평가가 가능하다. 이러한 추세에 따라 표준화된 영상 획득 및 보고를 위한 다기관, 다학제적 컨센서스인 Myeloma Response Assessment and Diagnosis System (이하 MY-RADS)가 발표되었다. 본 종설은 다발성 골수종의 전신자기공명영상 및 MY-RADS의 임상 적용에 대하여 최신 지견을 중심으로 종합적 기술하고자 한다. 이에 더하여 한계점과 추후 개선 방향에 대해서도 고찰하고자 한다.
목적 : 분화된 갑상선암 환자에서 방사성옥소(I-131)를 이용한 전신스캔은 보통 수술 후 잔여 갑상선 조직, 재발된 병변 부위 또는 전이 병소 등을 찾는데 유용한 방법이다. 최근 고선량 방사성동위원소를 이용한 치료는 증가 추세에 있으며, 동위원소 치료를 위한 입원대기는 수개월씩 지연되고 있다. 본원에서는 정화조 설비를 확장하여 주당 치료 가능 인원이 증가되었고 이로 인해 I-131 전신스캔 환자가 늘어나면서 검사시간 또한 증가되었다. 이를 개선하기 위해 본 연구에서는 기존의 검사시간을 단축하면서 병소의 위치를 정확하게 구별할 수 있도록 Transmission scan tool을 제작하여 우수한 분해능을 가진 전신영상을 얻고자 한다. 실험재료 및 방법 : 2008년 2월부터 7월까지 본과를 내원하고 ORBITER Gamma Camera를 이용하여 I-131 전신스캔을 시행하는 환자를 대상으로 하였다. 먼저 전신 스캔을 시행한 후 Transmission scan를 위해 검사 Table에 Rail을 설치하고 $^{99m}Tc$-pertechnetate 2 mCi를 희석한 Flood phantom을 올려놓을 수 있는 Tool을 자체 제작하여 Transmission image를 얻어 전신 스캔에 Fusion 하였다. 결과 : I-131 전신스캔과 Transmission scan의 Fusion된 영상은 간단한 marking을 통하여 구강이나 침샘부위, neck 부위의 병소, 전이병소의 위치를 감별하는데 우수한 분해능을 가진다. 또한 추가적인 국소 영상이 불필요하여 8~28분 정도의 검사 소요시간을 단축할 수 있었다. 결론 및 고찰 : I-131 전신스캔에서 Transmission scan은 방사능의 감약을 통하여 체표면의 윤곽을 정확히 나타낼 수 있으며 미리 시행한 I-131 전신스캔과의 Fusion 영상을 통하여 분해능을 향상시킴으로서 잔여 갑상선 조직이나 전이병소 등의 위치를 나타내는데 유용하다. 또한 추가적인 국소영상 촬영이 불필요함에 따라 검사에 소요되는 시간을 단축할 수 있고, 체표면 윤곽을 나타내어 해부학적 위치를 파악하는데 도움을 줄 수 있는 다른 임상검사에도 확대 적용할 수 있을 것이다.
MRI는 부분 영역을 자세히 영상화하는 검사임에도 인체에 위해한 RF를 전신에 주고 있다. 이러한 문제점을 개선하기 위해 저자들은 RF차단 방호복을 자제제작하여 적용한 결과 적용 전보다 적용 후가 95% 이상 RF를 차단하였다. 따라서 MRI 검사 시 본 연구의 방호복이 인체의 위해를 줄일 수 있는 최적의 대안임을 알 수 있다.
The purpose of this investigation is examining how treatment for foot reflex massage affect the changes in body temperature and blood composition. The cases of experiment are 10 male(n=2) and female(n=8) who don't have special clinical problems. This is the process of experiment. First, we photograph the anterior part and posterior of the upper, lower half of the body with an infrared imaging system. Second, among same parts, we compare the temperature of the whole body before treatment for foot reflex massage with the temperature after it. Then we draw blood from the body and observe a change: in numerical value of blood before and after treatment. Before and after treatment, we come to measure temperature of the whole body, the numerical value of WBC, RBC, platelet, HCT, Hb through analysis of blood, and the result of AST(GOT), ALT(GPT), ALP, GGT through analysis of serum enzyme activity of blood. There were no significant differences in blood composition. In terms of SPSS statistic program, when we explain each numerical value before and after foot massage, the abdominal, the buttocks, the femoral region universally take successful results about a change in body temperature(p<0.05).
고속 디지털신호처리기를 사용한 자기공명영상 실시간 대화형 제어기(스펙트로미터)를 개발하였다. 개발린 제어기는 rf 파형과 경사자계 파형을 만들고, 신호 측정을 위한 다중 측정기를 제어한다. TMS320C6701과 간은 높은 계산 능력을 가진 디지털신호처리기를 사용함으로써 복잡한 경사자계파형의 실시간 계산 및 출력이 가능해졌다. 또한 회전 행렬을 실시간으로 계산함으로써 심장과 같이 움직임이 큰 장기의 실시간 영상에서 얻고자하는 평면을 대화식으로 조절이 가능해졌다. 개발된 스펙트로미터를 1.5 테슬라 전신자기공명 영상시스템에 성공적으로 적용하였다. 개발된 스펙트로미터를 고속스핀에코나 echo planar imaging(EPI) 등과 같은 초고속자기공명영상에 적용하여 성능을 검증하였다. 이것은 이들 초고속 자기공명영상기법들이 측정 시간을 단축해주는 대신에 스펙트로미터의 송신부와 수신부 또는 경사자계부간의 동기나 위상에 에러가 있을 경우 문제점을 크게 부각시켜 시스템의 성능 평가에 적합하기 때문이다.
Postural balance of temporomandibular joint (TMJ) reflects the neuromuscular balance. TMJ posture treatment for meridian yinyang balance has been asserted to have an impact upon cervical spine alignment with an episodic good clinical result for refractory torticollis cases. A retrospective study with one-group pretest-posttest design was conducted to review cervical spine imaging data and clinical symptom changes in cervical dystonia cases to explore the effect of TMJ balance treatment. Review of clinical symptoms and cervical spine imaging data including computed tomography and simple x-ray of 9 cervical dystonia cases revealed significant improvement of cervical spine alignment and clinical symptoms. Improvement of cervical spine alignment may be an underlying mechanism of TMJ balance treatment, as a way of balancing the whole-body meridian system.

The following recommendations are presented herein: All trauma patients admitted to the resuscitation room should be constantly (or periodically) monitored for parameters such as blood pressure, heart rate, respiratory rate, oxygen saturation, body temperature, electrocardiography, Glasgow Coma Scale, and pupil reflex (1C). Chest AP and pelvic AP should be performed as the standard initial trauma series for severe trauma patients (1B). In patients with severe hemodynamically unstable trauma, it is recommended to perform extended focused assessment with sonography for trauma (eFAST) as an initial examination (1B). In hemodynamically stable trauma patients, eFAST can be considered as the initial examination (2B). For the diagnosis of suspected head trauma patients, brain computed tomography (CT) should be performed as an initial examination (1B). Cervical spine CT should be performed as an initial imaging test for patients with suspected cervical spine injury (1C). It is not necessary to perform chest CT as an initial examination in all patients with suspected chest injury, but in cases of suspected vascular injury in patients with thoracic or high-energy damage due to the mechanism of injury, chest CT can be considered for patients in a hemodynamically stable condition (2B). CT of the abdomen is recommended for patients suspected of abdominal trauma with stable vital signs (1B). CT of the abdomen should be considered for suspected pelvic trauma patients with stable vital signs (2B). Whole-body CT can be considered in patients with suspicion of severe trauma with stable vital signs (2B). Magnetic resonance imaging can be considered in hemodynamically stable trauma patients with suspected spinal cord injuries (2B).
Clinical application of positron emission tomography (PET) is rapidly increasing for the detection and staging of cancer at whole-body studios performed with 2-[fluorine-18]fluoro-2-deoxy-D-glucose (FDG). Although many cancers can be detected by FDG-PET, there has been limited clinical experience with FDG-PET for the defection of gynecological cancers including malignancies in uterus and ovary. FDG-PET can show foci of metastatic disease that may not be apparent at conventional anatomic imaging and can and in the characterization of indeterminate soft-tissue masses. Most gynecological cancers need to surgical management. FDG-PET can improve the selection of patients for surgical treatment and thereby reduce the morbidity and mortality associated with inappropriate surgery. FDG-PET is also useful for the early detection of recurrence and the monitoring of therapeutic effect. In this review, I discuss the clinical feasibility and limitations of this imaging modality in patients with gynecological cancers.
