목적 : 정상인 경수에 대한 내부 부위별, 경수 레벨별 확산텐서영상과 뇌척수액 속도에 대한 정상인 기준을 마련하고자 하였다. 대상 및 방법 : 정상인을 대상으로 3T MRI를 이용하여 영상을 획득하였다. 확산텐서영상은 척수 레벨별로 각각 미리 정의해둔 백질의 후삭, 좌, 우 측삭과 회백질에 관심영역을 정하여 FA, MD에 대한 평균값을 구하였다. PC 기법을 이용한 뇌척수액 속도 측정은 C2-3, C4-5, C5-6 척수레벨을 각각 스캔한 후, 척수 레벨별 최대 수축기 및 이완기 속도를 측정하였다. 결과 : FA 값과 MD 값에서 회백질과 백질의 내부 구조 사이에서는 통계적으로 유의한 차이 (p < 0.05)를 보였다. 경수 레벨별에서는 회백질의 FA 값에서 통계적으로 유의한 차이(p < 0.05)를 보였으며, 나머지 부분에서는 통계적으로 유의한 차이를 보이지 않았다 (p > 0.05). 뇌척수액 속도 측정 결과, 평균 최대 수축기 속도 $5.18{\pm}2.00cm/sec$, 평균 최대 이완기 속도 $-7.32{\pm}3.18cm/sec$ 이었으며, 경수 레벨별로는 통계적으로 유의한 차이를 보이지 않았다 (p > 0.05). 결론 : 정상인에 대한 경수 확산텐서영상의 정량적 수치 및 경수 레벨에 영향을 받지 않는 뇌척수액 속도에 대한 평균치를 알 수가 있었다.
Objectives The purpose of this study is to evaluate the relationship between the severity of white matter changes (WMC), risk factors and cognitive domains, including executive function profiles. Method Forty nine subjects over 55 years with subjective memory complaints were assessed with MRI and neuropsychological tests. The WMC were assessed by MRI T2-FLAIR images and divided into 3 groups of mild vs. moderate vs. severe and 2 groups of mild-moderate vs. severe by using Mantyla's criteria and Fazeka's criteria. The risk factors were examined in hypertension, heart disease history and chemistry Lab. Medical conditions which affect to cognitive dysfunction and definite dementia were also excluded. Results Comparing 3 groups, hypertension was identified as a risk factor of the WMC. Comparing 2 groups, total cholesterol and LDL were identified for as the risk factor of WMC. The severity of WMC was significantly associated with cognitive disturbances and their main effect on cognition was working memory and inhibition. Conclusion The risk factors of the WMC in the elderly were hypertension, hyperlipidemia. The severity of WMC appears to be associated with executive dysfunction in the elderly.
Background: Pulsatility of cerebral arteries and aortic stiffness have been associated with white matter hyperintensities (WMH). We explored which is better correlated with the severity of WMH in a population with acute lacunar infarct. Methods: We included patients with acute small subcortical infarcts who underwent transcranial Doppler (TCD) and brachial ankle pulse wave velocity (baPWV). Exclusion criteria were any stenosis or occlusion on major cerebral arteries on magnetic resonance angiography; poor temporal insonation windows; ankle brachial index < 0.9; and atrial fibrillation. We assessed the performance of the pulsatility index of bilateral middle cerebral arteries (PI-MCA) and baPWV for predicting moderate-to-severe WMH, defined as an Age Related White Matter Changes score > 5, and then sought to find independent predictors using binary logistic regression analysis. Results: Eighty-three patients (56 males, mean age $61.5{\pm}11.4$) participated in the study. Uni-variate analysis showed old age and high PI-MCA were significantly correlated with moderate-to-severe WMH. However, baPWV was not associated with the severity of WMH. Multivariate analysis revealed old age (odds ratio per 1-year increase, 1.068; p = 0.044) and upper tertile of PI-MCA (odds ratio, 5.138; p = 0.049) were independently associated with moderate-to-severe WMH. Receiver-operating characteristics showed PI-MCA differentiated those with and without moderate-to-severe WMH with an area under the curve of 0.719. Conclusions: PI-MCA derived from TCD was better correlated with the severity of WMH than baPWV in a population with lacunar infarction. Pulsatility of cerebral arteries may better predict cerebral small vessel disease than the aortic stiffness index.
This study was carried out to investigate the NPY-immunohistochemical characteristics of the olfactory bulb in the striped field mouse(Apodemus agrarius). The animals were anesthesized with thiopental sodium and perfused with 4% paraformaldehyde through left ventricle and aorta. Brains were removed and tranfered 10%, 20% and 30% sucrose. Sections were then cut on a cryostat into $40{\mu}m$-thick. The tissue immunostained with avidin-biotinylated complex method. The main olfactory bulb consisted of seven circumferential laminae : an olfactory nerve fiber layer, a glomerular layer with glomeruli surrounding by periglomerular cells, an external plexiform layer having granule and tufted cells, a mitral cell layer, a narrow internal plexiform layer, a granule cell layer forming several cell rows and a layer of white matter. The accessory olfactory bulb had four layers : an olfactory or vomeronasal nerve fiber layer, a glomerular layer consisting of small glomeruli, a mixed layer not distinguishing the external plexiform/mitral cell/granule cell layers and a granule cell layer. Most of NPY-immunoreactive(NPY-IR) neurons in main olfactory bulb were localized in the deeper portion of granule cell layer, white matter and anterior olfactory nucleus. In addition, some NPY-IR neurons were identified in the external plexiform layer. The shape of NPY-IR neurons of all olfactory bulb were predominant round or oval, sometime multipolar in shape. And most NPY-IR processes were parallel to long axis of white matter. In accessory olfactory bulb, NPY-IR neurons were not found in all region.
Vascular dementia (VaD) is a group of heterogeneous diseases with the common feature of cerebral hypoperfusion. To identify key factors contributing to VaD pathophysiology, we performed a detailed comparison of Wistar and Sprague-Dawley (SD) rats subjected to permanent bilateral common carotid artery occlusion (BCCAo). Eight-week old male Wistar and SD rats underwent BCCAo, followed by a reference memory test using a five-radial arm maze with tactile cues. Continuous monitoring of cerebral blood flow (CBF) was performed with a laser Doppler perfusion imaging (LDPI) system. A separate cohort of animals was sacrificed for evaluation of the brain vasculature and white matter damage after BCCAo. We found reference memory impairment in Wistar rats, but not in SD rats. Moreover, our LDPI system revealed that Wistar rats had significant hypoperfusion in the brain region supplied by the posterior cerebral artery (PCA). Furthermore, Wistar rats showed more profound CBF reduction in the forebrain region than did SD rats. Post-mortem analysis of brain vasculature demonstrated greater PCA plasticity at all time points after BCCAo in Wistar rats. Finally, we confirmed white matter rarefaction that was only observed in Wistar rats. Our studies show a comprehensive and dynamic CBF status after BCCAo in Wistar rats in addition to severe PCA dolichoectasia, which correlated well with white matter lesion and memory decline.
Objectives Although ginseng has been reported to protect neuronal cells and improve various cognitive functions, relationship between ginseng supplementation and response inhibition, one of the important cognitive domains has not been explored. In addition, effects of ginseng on in vivo human brain have not been investigated using the diffusion tensor imaging (DTI). The purpose of the current study is to investigate changes in intrusion errors and white matter microstructure after Korean Red Ginseng supplementation using standardized neuropsychological tests and DTI. Methods Fifty-one healthy participants were randomly allocated to the Korean Red Ginseng (n = 26) or placebo (n = 25) groups for 8 weeks. The California Verbal Learning Test was used to assess the number of intrusion errors. Intelligence quotient (IQ) was measured with the Korean Wechsler Adult Intelligence Scale. Depressive and anxiety symptoms were evaluated using Hamilton Depression Rating Scale, Hamilton Anxiety Rating Scale, and Hopkins Symptom Checklist-25. The fractional anisotropy (FA) was measured from the brain DTI data. Results After the 8-week intervention, Korean Red Ginseng supplementation significantly reduced intrusion errors after adjusting age, sex, IQ, and baseline score of the intrusion errors (p for interaction = 0.005). Change in FA values in the left anterior corona radiata was greater in the Korean Red Ginseng group compared to the placebo group (t = 4.29, p = 0.04). Conclusions Korean Red Ginseng supplementation may be efficacious for improving response inhibition and white matter microstructure integrity in the prefrontal cortex.
Objectives The aim of this study is to investigate the correlation between degenerative changes in brain [i.e., global cortical atrophy (GCA), medial temporal atrophy (MTA), white matter hyperintensities (WMH)] and neurocognitive dysfunction in Korean patients with Alzheimer's disease. Methods A total of 62 elderly subjects diagnosed with Alzheimer's disease were included in this study. The degenerative changes in brain MRI were rated with standardized visual rating scales (GCA or global cortical atrophy, MTA or medial temporal atrophy, and Fazekas scales) and the subjects were divided into two groups according to the degree of degeneration for each scale. Cognitive function was evaluated with Korean version of the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) and several clinical features, including apolipoprotein E ${\varepsilon}4$ status, lipid profile and thyroid hormones, were also examined. Chi-square test and Fisher's exact test were performed to analyze the relationship between the degree of cerebral degeneration and neurocognitive functions. Results Demographic and clinical features, except for the age, did not show any significant difference between the two groups divided according to the degree of cerebral degenerative changes. However, higher degree of GCA was shown to be associated with poorer performance in verbal fluency test, word list recall test, and word list recognition test. Higher degree of MTA was shown to be associated with poorer performance in Mini-Mental State Examination in the Korean Version of CERAD Assessment Packet (MMSE-KC), word list recognition test and construction praxis recall test. Higher degree of white matter hyperintensities was shown to be associated with poorer performance in MMSE-KC. Conclusions Our results suggest that severe brain degeneration shown in MRI is associated with significantly poorer performance in neurocognitive tests in patients with Alzheimer's disease. Moreover, the degree of GCA, MTA and white matter hyperintensities, represented by scores from different visual rating scales, seems to affect certain neurocognitive domains each, which would provide useful information in clinical settings.
피질하 낭종을 동반한 거대뇌성 백질뇌병증은 뇌백질의 희귀성 질환으로 1990년대에 처음 보고되었다. 이는 자기공명 영상(MRI)에서 미만성의 백질 이상 및 다양한 정도로 발견되는 피질하 낭종과 함께 뇌의 종창을 보이는 것이 전형적이며, 이 병변은 특징적으로 대뇌 측두엽의 전반부를 침범하고, 심부의 뇌백질, 대뇌 기저핵, 시상과 소뇌는 상대적으로 보존되는 경향을 보인다. 피질하 낭종을 동반한 거대뇌성 백질뇌병증이 있는 환아는 큰 머리와 함께 운동 장애, 실조증, 강직과 인지 기능 장애 등의 신경학적 결함을 보인다. 저자들은 큰 머리와 함께 경련, 발달 지연, 뇌 MRI 의 T2 강조영상에서 양측성으로 전두, 측두, 두정엽에서 고신호강도의 백질 병변을 보이는 피질하 낭종을 동반한 거대뇌성 백질뇌병증에 합당한 소견을 보이는 20개월의 남자 환아를 경험하였기에 보고하는 바이다.
연구목적 본 연구의 목적은 알츠하이머병 및 경도인지장애 환자에서 백질고강도신호 차이에 따른 신경인지 기능 및 행동심리증상(BPSD)을 비교하는 것이다. 방 법 본 연구는 후향적 연구로서, 알츠하이머병 및 경도인지장애로 진단받은 115명을 대상으로 하였다. 뇌 자기공명영상의 백질 고강도 신호(white matter hyperintensity, WMH)는 표준화된 시각 기반 척도(Fazekas scales)에 의해 평가되었으며, 참가자들은 Fazekas 척도에 따라 두 그룹으로 분류되었다. 신경인지기능은 임상치매평가척도(CERAD-K)에 의해, BPSD는 한국형 신경정신행동검사(K-NPI)로 평가되었다. WMH의 심각도에 따른 신경인지기능 및 BPSD의 차이를 분석하기 위해 독립표본 t-test를 시행하였다. 결 과 WMH의 중증도가 높은 군은 유의하게 낮은 언어 유창성을 나타내었다(p<0.05). 또한 WMH의 중증도가 높은 군은 유의하게 높은 K-NPI 점수를 보였다(p<0.01). 결 론 WMH와 실행기능과 관련된 신경인지검사 간에는 유의한 연관이 있었다. 또한 WMH는 BPSD의 중증도에 영향을 미치는 것으로 보인다. 임상실제에서 WMH를 알츠하이머 병(Alzheimer's disease, AD)및 경도인지장애(Mild cognitive impairment, MCI)환자를 치료하는데 유용한 정보를 제공할 것이다.
