PURPOSE: The purpose of this study was to investigate the effects of observed action gait training on stroke patients. METHODS: 22 subjects were randomized into two groups. The observed action gait training performed that watched a video of normal gait before gait training and the general gait training without watching it. The experimental group(n=11) performed observed action gait training and the control group(n=11) performed general gait training. Both group received gait training for 3 times per week during 8 weeks. RESULTS: The experimental group showed significant differences in the cadence, gait velocity, stride, step, single limb support, double limb support, stride length and step length(p<.05). The control group showed significant differences only in the stride(p<.05). CONCLUSION: The observed action gait training affected coordination and weight shift, as well as symmetry of the body. Plasticity of the brain was facilitated by repetitive visual and sensory stimulation. The observed action gait training promoted the normal gait by watching the normal gait pattern. In conclusion, motor learning through the sensory stimulation promotes brain plasticity that could improve motor function, and observed action gait training indirectly identified stimulated brain activities.
본 연구의 목적은 공압 트레이닝 방법의 원리와 적용 방법 등 새로운 트레이닝 개념을 소개하는데 있다. 특히 공기라고 하는 눈에 보이지 않는 무형의 저항을 이용한다는 새로운 소식도 함께 전달하면서 트레이닝 방법에 대한 시야확대에 기여하고자 시도되었다. 시대의 흐름에 따라 트레이닝법의 변천을 제시하였고, 왜 이러한 형태로 변화되는지에 대한 이론적 고찰을 하였다. 즉, 중량저항을 시작으로 유압식, 등속성 운동, 그리고 공압에 이르기까지 왜 이러한 훈련방법을 적용해야하는지에 대한 당위성을 제시하였다. 또한 공압 트레이닝의 우수성을 제시하였다. 가까운 미래에는 지금보다 더 우수한 트레이닝 방법이 생겨나게 될 것이다. 결론적으로, 첨단 트레이닝에 대한 관심과 적용이 스포츠 발전에 많은 기여를 할 것이다.
Purpose: The emphasis on gait rehabilitation after stroke depends on training support through the lower limbs, balance of body mass over the changing base of support. However, muscle weakness, lack of control of lower limb, and poor balance can interfere with training after stroke. For this case study report, a wearable robot orthosis was applied to stroke patients in order to verify its actual applicability on balance and gait ability in the clinical field. Methods: Two stroke patients participated in the training using the wearable robot orthosis. Wearable robot orthosis provides patient-initiated active assistance contraction during training. Training includes weight shift training, standing up and sitting down, ground walking, and stair up and down Training was applied a total of 20 times, five times a week for 4 weeks, for 30 minutes a day. Gait ability was determined by Stance phase symmetry profile, Swing phase symmetry profile, and velocity using the GAITRite system. Balance ability was measured using the Biodex balance system. Results: Subjects 1, 2 showed improved gait and balance ability with mean individual improvement of 72.4% for velocity, 19.4% for stance phase symmetry profile, 9.6% for swing phase symmetry profile, and 13.6% for balance ability. Conclusion: Training utilizing a wearable robot orthosis can be useful for improvement of the gait and balance ability of stroke patients.
목적 : 본 연구의 목적은 경직성 뇌성마비아동의 앉은 자세균형에 대한 집중체중 이동훈련의 효과를 알아보고자 하였다. 연구방법 : 경직성 사지마비로 진단 받은 11세 여자를 대상으로 ABA 실험설계를 이용하여 8주간 실험을 진행하였다. 기초선(A1)에서 일반 물리치료를 수행하였고, 중재 중(B1, B2)에서 일반 물리치료와 집중체중 이동훈련을 실시하였으며, 중재 후(A'1, A'2)에서 다시 일반 물리치료를 실시하였다. 체간조절능력은 한국형 체간조절 측정척도(TCMS-K)와 균형측정시스템(Biorescue, RM Ingenierie, France)를 사용하여 측정하였다. 연구결과 : 한국형 체간조절 측정척도의 모든 항목이 집중체중 이동훈련 기간 중에 증가하였다. 또한 안정성한계 면적이 집중체중 이동훈련 기간 중에 증가하였고 왼쪽/오른쪽 그리고 앞/뒤 안정성한계 면적이 집중체중 이동훈련 기간 중에 대칭적이었다. 결론 : 집중체중 이동훈련이 경직성 뇌성마비 아동의 체간조절 능력을 개선하는데 긍정적인 효과가 있었다. 본 연구는 집중체중 이동훈련이 경직성 뇌성마비 아동의 독립적 앉기, 자세조절, 일상생활을 위한 균형능력 개선에 도움이 될 것으로 사료된다.
The purpose of this study is developing a virtual bicycle system for improving the ability of postural balance control for adults in various age groups. The system consists of an exercise bicycle that allows tilt in accordance with the postural balance of the subject in the system, a visual display that shows virtual road, and a visual feedback system. The rider of the system tries to maintain balance on the bicycle with a visual feedback of a virtual road while the pedaling speed, the heading direction, and various weight distribution information are updated to the subject as visual feedbacks in the display. A series of experiments were performed with various subjects to find the factors related to postural balance control in the system. The related parameters obtained were weight shift, magnitude of the deviation from the center of the virtual road, and variables related to the movement of the center of pressure. The results found that the ability to control postural balance in the system improved with the presentation of visual feedback information of the distribution of weight. It was also found that the general performance of the subject on balance in the system improved after ten days long training. The results show that the newly developed system can be used for the diagnosis of postural balance as well as for the stimulation of various senses such as vision and somatic sense in the field of rehabilitation training.
Objective: This study analyzes the effect of angle conditions of rehabilitation equipment used for supporting hemiplegic patients on their rehabilitation training for standing action. The study was performed by adjusting the rear angle of seat inclination through a motion analysis. Background: Owing to a loss of muscle rigidity and degradation of muscle control ability, hemiplegic stroke patients suffer from asymmetrical posture, abnormal body balance, and degraded balance abilities due to poor weight-shifting capacity. The ability to shift and maintain one's weight is extremely essential for mobility, which plays an important role in our daily life. Thus, to improve patients' ability to maintain weight evenly and move normally, they need to undergo orthostatic and ambulatory training. Method: Using a motion analysis system, knee movements on both hemiplegic side and non-hemiplegic side were measured and analyzed in five angles ($0^{\circ}$, $10^{\circ}$, $30^{\circ}$, $50^{\circ}$, $70^{\circ}$) while supported by the sit-to-stand rehabilitation equipment. Results: The knee movements on both sides increased as the angle increased in angle support interval to support a hemiplegic patient's standing up position. In standing up interval, a hemiplegic patient's knee movement deviations on both sides decreased, and the movement differences between hemiplegic and non-hemiplegic legs also decreased as the angle increased. Conclusion: The results of this study showed that the rehabilitation effectiveness increases as the angle increases, leading to a balanced standing posture through the decrease of movement difference between hemiplegic and non-hemiplegic sides and an improved standing up ability through the increase of knee movement on both sides. However, angles higher than $50^{\circ}$ didn't provide a significant effect. Therefore, a support angle under $50^{\circ}$ was proposed in this study. Application: The results of this study are expected to be applicable to the design of sit-to-stand support equipment to improve the effectiveness of the rehabilitation process of hemiplegic patients.
Purpose: The study investigated the effect on chronic stroke patients' balance of a weight shift to the affected side using an insole on the less affected side during treadmill walking training. Methods: The subjects were 7 patients who had been diagnosed with stroke 6-24 months prior to the study. In each case, an insole was applied on the patient's less affected side during treadmill walking training. Each training session lasted 30 minutes and was undertaken 5 times per week for 4 weeks. Biorescue equipment that measures shifts in center of pressure was used to assess balance ability as measured by the Korea-Berg balance scale (K-BBS) before and after each training intervention. The Wilcoxon signed-rank test was used to evaluate within-group effects. Results: The results revealed statistically significant before and after differences in area, pressure, length, and mean velocity of the balance test and on K-BBS (p < 0.05). Conclusion: In chronic stroke patients, using an insole to adjust the height of the shoe on the less affected side is an effective means of increasing weight-shifting on the paralyzed side during treadmill gait training.
This study aimed to pilot test a newly developed bilateral upper limb rehabilitation training program for improving the upper limb function of individuals with chronic stroke using a visual feedback method. The double-group pretest-posttest design pilot study included 10 individuals with chronic stroke (age >50 years). The intervention (four weekly meetings) consisted of five upper limb training protocols (wrist extension; forearm supination and pronation; elbow extension and shoulder flexion; weight-bearing shift; and shoulder, elbow, and wrist complex movements). Upper limb movement function recovery was assessed with the FuglMeyer Assessment of the Upper Extremity, the Wolf Motor Function Test, the Trunk Control Test, the modified Ashworth Scale, and the visual analog scale at baseline, immediately after, and four weeks after the intervention. The Fatigue Severity Scale was also employed. The Fugl-Meyer Assessment of the Upper Extremity and Wolf Motor Function Test showed significant improvement in upper limb motor function. The Trunk Control Test results increased slightly, and the modified Ashworth Scale decreased slightly, without statistical significance. The visual analog scale scores showed a significant decrease and the Fatigue Severity Scale scores were moderate or low. The bilateral upper limb training program using the visual feedback method could result in slight upper limb function improvements in individuals with chronic stroke.
This paper describes the quantitative analysis on the improvement of equilibrium sensory using virtual bicycle system. We have used a virtual bicycle system that combines virtual reality technology with a bicycle. In this experiment, 10 subjects were tested to investigate the influencing factors on equilibrium sensory. Straight road and curved road driving at several factors including cycling time, number of times of path deviation, and center of pressure(COP) were extracted and evaluated to quantify the extent of control. Also, To improve the effect of balance training, we investigated the usefulness of virtual feedback information by weight shift. The result showed that the system could be effective for equilibrium sensory rehabilitation training device. The analysis method might also have wider applicability to the rehabilitation field.
This study is aimed to compare the effect of visuo-perceptual biofeedback sitting balance training and conventional sitting balance training using Balance Master on stroke patients with that of program in order to analyze the effect it has on dynamic postural balance. The subjects are twenty-four stroke patients who are receiving physical therapy in Ilsan Paik Hospital and can maintain sitting posture by themselves. These patients were divided to control group and experimental group randomly. In order to compare to control and experimental group before and after the balance training, they were tested with Mann-Whitney U test and in order to compared the changes before and after the balance training, they were tested with Wilcoxon signed-ranks test. The results are as follows: we measured the ability of dynamic posture balance control with limit of stability(LOS) test and rhythmic weight shift test. There was an increasing improvement in the ability of dynamic posture balance control of the experimental group that had visuo-perceptual biofeedback sitting balance control training using the Balance Master(p<0.05, p<0.01). According to the results from above, compared to conventional sitting balance training programs, visuo-perceptual biofeedback sitting balance control training using the Balance Master is considered to be a more valuable therapy in balance control improvement and physical function improvement. It is considered that if the weak points are made up, the training with Balance Master will give help to stroke patients and to patients with balance control disabilities and will further more contribute to successful rehabilitation therapy.
